Objectives : The purpose of this study was to evaluate the scientific literature demonstrating the effectiveness of Chuna manual therapy (CMT) in the treatment of spinal scoliosis. Methods : A literature search was conducted using eight electronic databases to identify all randomized controlled clinical trials (RCTs) that investigated CMT as a treatment for spinal scoliosis. The Cochrane risk of bias tool was used to assess the methodological quality of each RCT. Results : Five RCTs met our inclusion criteria and were included in the analysis. These studies demonstrated positive results of CMT with respect to the reduction of Cobb's angle, vertebral rotation angle score, bending test score, and efficacy rate compared with brace treatment. Positive results were also assured, in terms of the reduction of Cobb's angle, pulmonary function, and efficacy rate when comparing CMT combined with other therapy with brace treatment, gymnastic training, or traction therapy. Conclusions : This review has identified encouraging and limited evidence of CMT for the treatment of spinal scoliosis. However, to obtain stronger evidence, without the disadvantages of study design and quality, we recommend that treatment effectiveness of CMT for spinal scoliosis is investigated further using a well-designed RCT.
This study was performed to compare manual liquid-based preparation with conventional Papanicolaou tests in view of the cytologic diagnoses and specimen adequacy. The specimens of 5,979 women from 33 local clinics and 1 general hospital were prepared by both manual liquid-based preparation and conventional Papanicolaou test. The cytologic diagnoses and specimen adequacy were evaluated in Department of Fathology in Kyoungpook National University School of Medicine. A conventional Papanicolaou test was always prepared first, after that residual material on the sampling device was rinsed into a liquid preservative, and then thin-layer slides were prepared using manual method of liquid-based cervicovaginal cytology. Conventional and liquid-based slides were read independently, and cytologic diagnoses and specimen adequacy were classified using the Bethesda System. Of the cases, 5,763 (96.3%,) had the same interpretation, and there was no significant diagnostic difference in 5,853 (97.8%) cases. When evaluating cases with more than one diagnostic class difference, the manual liquid-based preparation demonstrated a statistically significant overall improvement (2.1%) in the detection of squamous intraepithelial lesion and invasive cancer. Using manual method of liquid-based preparation, there was 14.1%, reduction in unsatisfactory slides through excellent cellular presentation. In conclusion, the manual liquid-based preparation produces standardized quality, superior sensitivity and improved adequacy as compared to the conventional method.

Objectives : This study aimed to systematically and comprehensively review controlled clinical trials on the effectiveness and safety of Chuna Manual Therapy for stroke. Methods : By October 7, 2019, three core databases and three domestic databases were searched. Seven major academic journals on the related field were also hand-searched. Methodological quality of the included studies was evaluated using the Cochrane risk of bias tool. Meta-analysis was conducted and the quality of its evidence was assessed using the GRADE methodology. Results : Five articles met the eligibility criteria. The results of most of included studies were in favor of Chuna Manual Therapy. No study described any adverse events during or after the clinical trials. Meta-analysis of three eligible studies showed that the pre- and post-treatment scores on the Modified Barthel Index (mean difference 8.00, 95% confidence interval 0.26 to 15.74) and Berg Balance Scale (mean difference 3.57, 95% confidence interval 0.64 to 6.51) of the Chuna-treated group were significantly different, but only marginally higher than those of the non-treatment group. Thus, the level of evidence gathered from these studies was assessed to be low. Conclusions : Based on current available evidence, any confirmative conclusions cannot be made on the effectiveness and safety of Chuna Manual Therapy for stroke because of the small sample size, low methodological quality, presence of statistical heterogeneity, and missing safety information. More rigorously designed large-scale multi-center studies are needed to establish more specific and credible evidence to support or oppose the use of Chuna Manual Therapy for stroke.

Objectives A case report is a detailed medical description of previously unreported observations or complications of medical conditions, unique use cases of tests, or new side effects. The purpose of this study was to evaluate the quality of case reports published in the Journal of Korea CHUNA Manual Medicine for Spine & Nerves (JKCMSN). Methods Case reports published in the JKCMSN from January 2013 to December 2020 were selected using the Oriental Medicine Advanced Searching Integrated System (OASIS). The quality of the reports was assessed based on the Consensus-based Clinical Case Reporting Guideline Development (CARE) guidelines. Results Sixty case reports that met the inclusion criteria were included in the assessment. Our analysis revealed that 71.42% of the case reports included all the necessary information and their level of reporting was good. However, their scores in the qualitative subdivisions of the reporting level parameters showed that the quality level was uneven. More than 65% of the papers did not report in the following categories: "Keywords," "Patient information," "Timeline," "Diagnostic assessment," "Therapeutic interventions," "Follow-up and outcomes," "Patient perspective," and "Informed consent." Conclusions Our analysis shows that CARE guidelines need to be followed more rigorously to improve the quality of reporting in the JKCMSN. Further efforts are necessary to develop reporting guidelines and evaluation tools relevant to the Korean clinical context.

Objectives The purpose of this study is to analyze the trend and indication of clinical research about Visceral Chuna Manipulation and to suggest the direction of further study. Methods We searched 9 electronic databases (MEDLINE Pubmed, EMBASE, CENTRAL, CiNii, CNKI, KMbase, OASIS, NDSL, KISS) using the search terms "visceral manipulation", "visceral manual therapy" and identified relevant literature that investigated visceral manipulation as a intervention for various subjects. Results Among 142 studies were searched and screened, 16 met designated criteria. Visceral Chuna Manipulation showed the effectiveness in musculoskeletal disease, digestive disease, psychiatry, obstetrics, circulatory and oncology. Conclusions This study shows possibility of Visceral Chuna Manipulation as a treatment for many diseases. Better designed further study is necessary to establish the evidence of clinical efficacy of visceral manipulation and to suggest standard of techniques and treatment protocols for diverse patients.

Objectives : The present study introduces the clinical application of Hamstring Chuna Manual Therapy to a patient who underwent the pain after spinal fusion. Methods : A patient who had the pain after spinal fusion was hospitalized at Bucheon Jaseng hospital of Oriental Medicine for 35 days. During the hospital treatment, the pain was relieved by Hamstring Chuna Manual Therapy. The improvement of the patient was measured by VAS(Visual Analogue Scale) score, SLR(Straight-Leg Raising) test, and walking distance. Results : The values of the patient's VAS score, SLR test, and walking distance measured before and after Hamstring Chuna Manual Therapy presented that the pain was relieved after Hamstring Chuna Manual Therapy. Conclusions : Therefore, this case demonstrates that Hamstring Chuna Manual Therapy is effective treatment method for relieving pain in the spine.

Objectives: To evaluate the evidence supporting the effectiveness of Chuna manual therapy for rheumatoid arthritis. Methods: We conducted a search across 9 electronic databases to find all randomized controlled clinical trials (RCTs) that used Chuna manual therapy as a treatment for rheumatoid arthritis. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results: Our inclusion criteria were met by 5 RCTs. The meta-analysis showed positive results for the use of Chuna manual therapy combined with oriental usual care (UC) in terms of the efficacy rate, pain, and duration of morning stiffness when compared to western UC. Positive results were also obtained in terms of the efficacy rate, when Chuna manual therapy combined with medication was compared to medication treatments alone. Conclusions: Our systematic review found encouraging, but limited evidence of Chuna manual therapy for rheumatoid arthritis. However, to obtain stronger evidence without the drawbacks of trial design and the quality of studies, we recommend a comparative research to test the effectiveness of Chuna manual therapy.

Objectives This study aimed to evaluate the effectiveness of manual therapy on constipation in patients with Parkinson's disease. Methods We searched electronic databases (PubMed, EMBASE, Cochrane Library, CAJ, KISS, KISTI, KMBASE, NDSL, OASIS, and RISS) for randomized controlled trials for manual therapy on constipation in patients with Parkinson's disease. Results Three randomized controlled trials were selected from the online database. One study demonstrated that manual therapy had positive results compared to routine nursing care with western medication. However, two studies demonstrated that manual therapy had no positive results compared to routine nursing care with western medication or western medication when used alone. Conclusions Our systematic review did not verify the clinical effect of manual therapy on constipation in patients with Parkinson's disease. Thus, more high-quality studies are needed to verify the effectiveness of manual therapy for constipation in patients with Parkinson's disease.
Objectives This study investigated the trends in domestic and international clinical research in craniosacral therapy, classified as a type of Chuna manual therapy, and suggested further directions in Korean medicine. Methods This scoping review was performed using the Arksey and O'Malley methodological framework and preferred reporting items as per the preferred reporting items for systematic reviews and meta-analyses extension for scoping reviews checklist. Eight electronic databases (PubMed, EMBASE, Cochrane Library, Koreanstudies Information Service System [KISS], KMBASE, Oriental Medicine Advanced Searching Integrated System [OASIS], Research Information Sharing Service [RISS], ScienceON) were searched to identify articles with the search terms "craniosacral therapy" and "cranial osteopathy" until December 2021. Results Forty-five studies were eligible as per our inclusion criteria. Most research studies (n=44) were conducted in the field of medicine and pharmacy, especially in rehabilitation medicine (n=16). As a result of the study design, randomized controlled trials (n=20) were the most common, and chronic pain (n=9) was the most frequently targeted disease, followed by headache (n=7). Thirty-two studies suggested interventions and 20 studies used Upledger's 10-step protocol. The average duration of craniosacral therapy was 41 min per session, administered 1.4 times per week. Outcome measurements were analyzed and categorized with the examination procedure for the patient. Conclusions This is the first scoping review of craniosacral therapy in Korea, and we believe that our findings could support its utility as Chuna. In the future, more studies should be conducted to establish the evidence of clinical efficacy of craniosacral therapy and develop standard techniques in Korean medicine.

Objectives This systematic review aimed to provide evidence for the effectiveness and safety of Chuna manual therapy (CMT) for carpal tunnel syndrome (CTS). Methods We searched 12 electronic databases (PubMed, Ovid-MEDLINE, EMBASE, The Cochrane Library, CNKI, CiNii, ScienceON, KMbase, KISS, OASIS, KMTANK, and RISS) up to the end of October 2021. We only included randomized controlled trials (RCTs) investigating CMT for the treatment of CTS. The methodological quality of the included RCTs was evaluated using the Cochrane Risk of Bias Tool 1.0. Results Six RCTs were eligible for inclusion in our study. A meta-analysis of four studies demonstrated positive results for the use of CMT when used in conjunction with Korean medicine treatment compared to those for Western conservative treatment for CTS. Conjunction treatment significantly improved the total efficacy rate compared to conservative treatment alone (P=0.0007, n=4). Conclusions There is reliable evidence for the use of CMT in treating CTS based on a published meta-analysis. To measure only the effect of Chuna, an experimental group should be designed with Chuna alone to evaluate its effectiveness. However, it should be noted that the studies included in this systematic review were heterogeneous and of low quality, thus warranting further investigation using well-designed RCTs.
