Development of Time Varying Kalman Smoother for Extracting Fetal ECG using Independent Component Analysis : Preliminary Study (독립요소분석을 이용한 태아심전도 추출을 위한 시변 칼만 평활기의 개발 : 예비연구)

  • Fetal heart rate monitoring is important information to assess fetal well-being. Non-invasive fetal ECG (electrocardiography) can be derived from maternal abdominal signal. And various promising signal processing methods have been introduced to extract fetal ECG from mother's composite abdominal signal. However, non-invasive fetal ECG monitoring still has not been widely used in clinical practice due to insufficient reliable measurement and difficulty of signal processing. In application of signal processing method to extract fetal ECG, it might be lower signal to noise ratio due to time varying white Gaussian noise. In this paper, time varying Kalman smoother is proposed to remove white noise in fetal ECG and its feasibility is confirmed. Wiener process was set as Kalman system model and covariance matrix was modified according to white Gaussian noise level. Modified error covariance matrix changed Kalman gain and degree of smoothness. Optimal covariance matrix according to various amplitude in Gaussian white noise was extracted by 5 channel fetal ECG model, and feasibility of proposed method could be confirmed.

Clinical Value of Cardiovascular Calcifications on Non-Enhanced, Non-ECG-Gated Chest CT (비 조영증강 비 심전도동기 흉부 CT에서 발견되는 심혈관계 석회화의 임상적 가치)

  • Cardiovascular calcifications can occur in various cardiovascular diseases and can serve as a biomarker for cardiovascular event prediction. Advances in CT have enabled evaluation of calcifications in cardiovascular structures not only on ECG-gated CT but also on non-ECG-gated CT. Therefore, many studies have been conducted on the clinical relevance of cardiovascular calcifications in patients. In this study, we divided cardiovascular calcifications into three classes, i.e., coronary artery, thoracic aorta, and cardiac valve calcifications, which are closely associated with cardiovascular events. Further, we briefly described pericardial calcifications, which can be found incidentally. Since the start of lung cancer screening in Korea in the second half of 2019, the number of non-enhanced, non-ECG-gated, low-dose chest CT has been increasing, and the number of incidentally found cardiovascular calcifications has also been increasing. Therefore, understanding the relevance of cardiovascular calcifications on non-enhanced, non-ECG-gated, low-dose chest CT and their proper reporting are important for radiologists.

Atypical Clinical Course of after Glyphosate Herbicide Intoxication (Glyphosate 음독의 임상양상)

  • Purpose: Although glyphosate-surfactant herbicide intoxication is relatively mild toxic in humans, we encountered an atypical clinical presentation. We performed this study to understand the atypical clinical course and factors associated with severe intoxication after glyphosate ingestion. Methods: We conducted a retrospective study of 61 patients (male 43, mean age $54{\pm}18.8$ years) who were presented with glyphosate ingestion between March 1997 and March 2011. The severe intoxication group was defined as patients with systolic blood pressure less than 90 mmHg, respiratory distress needing intubation, or altered mental state. Results: Of the 61 patients, 22 patients (36.1%) had a severe clinical course, 1 patient (1.6%) had died and 1 patient (1.6%) had moribund discharge. The most common symptoms were nausea with or without vomiting which occurred in 30 patients (49.2%). Twenty-seven patients had metabolic acidosis that was the second most common medical complication. Advanced age, pH, base excess, $HCO_3$, Sat, creatinine, X-ray abnormalities and ECG abnormalities were significant factors. Hemoglobin, platelet, $pO_2$, $pCO_2$, BUN, sodium, potassium and AST levels were not different comparing the laboratory characteristics between the severe and mild intoxication groups. Conclusion: The results of this study showed that severe intoxication occurred in 22 patients (36.1%) after glyphosate intoxication. Advanced age, pH, base excess, $HCO_3$, Sat, creatinine, X-ray abnormality and ECG abnormalities were significant predictive factors for severe intoxication in patients with glyphosate surfactant herbicide poisoning.

  • Polysomnography is used to diagnose many types of sleep disorders including sleep apnea, periodic limb movement disorder, REM sleep behavior disorder, parasomnias, and narcolepsy. It is a comprehensive recording of the biophysiological changes that occur during sleep. The polysomnography monitors many body functions parameters including EEG, EOG, EMG, ECG, respiratory airflow, respiratory effort, and pulse oximetry during sleep. Multiple Sleep Latency Test (MSLT) is performed for diagnosing narcolepsy and excessive daytime sleepiness. It is usually to be done after an overnight polysomnography. The test consists of four or five 20-minute nap opportunities that are scheduled two hours apart.

  • Holter monitoring has been recognized as an useful noninvasive instrument for monitoring the cardiac electrical activity over 24 to 48 hours. Because the surface electrocardiogram (ECG) is recorded only for several seconds to minutes, it often misses or underestimates the underlying arrhythmia. The surface ECG is also easily influenced by depolarization potentials from skeletal muscle by the movement of patient (especially muscle tremor). However, holter monitoring is less affected by such factors. There has been no precedential report in veterinary medicine applying digital holter monitor with unipolar precordial chest lead using 4 electrodes. This article describes its clinical indications, equipment and technical application method in dogs.

  • 현대인의 질병 중 심혈관계 질환의 증가로 인한 건강관리의 중요성이 더해감에 따라 소형 심전계의 필요성이 대두되어지며 운동과 같은 동잡음에 취약한 환경에서도 안정적인 신호를 얻기 위한 연구가 진행되어지고 있다. 본 논문에서는 측정 리드의 거리 최소화에 따른 연구로서 여러 개의 소형 심전도 모듈을 부착하였을 때의 각각의 신호가 임상심전도와 얼마나 유사하게 측정되어질 수 있는지에 대해 연구하였으며 심전도(ECG) 신호의 측정 원리를 위하여 심장 전기쌍극자 모델에 기반하여 전극간 거리를 최소화한 패치형 전극 측정 시스템을 개발하고, 2개 이상의 다중 패치전극 측정 시스템으로부터 측정된 심장전기활동 신호를 합성하여 임상적 심전도와 유사성이 높은 전극신호 유도법을 개발하였다. 또한 이 유도법의 검증을 위하여 임상 심전도 측정장치의 결과와 상관성을 분석하였으며 최대 r 값은 0.859로 얻어졌다.

  • Blood pressure may be measured using physiological signals, such as ECG, PTT, and $SpO_2$. In this study, we designed and implemented a portable real-time blood pressure measuring system, which can access the Internet simultaneously using the IPv4 / IPv6 protocol. In addition, we presented a model for the calculation of systolic and diastolic blood pressure in a real-time environment. Through clinical experiments on 28 subjects, the blood pressure measuring system was shown to satisfy the requirements of Grade B level of the BHS (British Hypertension Society) certification.

  • In this paper, an ECG-NIBP patient monitor is designed. This is an essential equipment to measure and monitor patient's physical condition - electrocardiogram(ECG) wave, heart rate(HR), and noninvasive blood pressure(NIBP) - in ICU, CCU, and operating room. The ECG is an electrical waveform produced by relaxation and contraction of the cardiac muscle. Most physicians diagnose patient's cardiac states from ECG pattern. A blood pressure is one of the clinical indexes measured in a emergency room or operating room. In this paper, the blood pressure is measured in artery by using the nonivasive oscillometric method. The developed patient monitor was inspected and compared with other instruments in operating rooms. The results were 1bpm of maximum difference in the heart rate, 15mmHg in the systolic pressure, 16mmHg in the diastolic pressure, and 25mmHg in the mean blood pressure. But the total results were 0.15bpm of the mean difference in the heart rate, 5mmHg in the systolic pressure, 10mmHg in the diastolic pressure, and 9mmHg in the mean blood pressure. The designed ECG-NIBP patient monitor can measure the ECG wave, HR, and BP. And the multi-tasking module of pulse oximetry . respiration . temperature monitor will be added in the near future.

Using the X-ray Image, Augmented Reality based electrocardiogram measurement system Development (X-ray 이미지를 활용한 증강현실 기반 심전도 측정시스템 개발)

  • Chronic diseases are increasing nowadays as daily habits changed due to economic growth. Among chronic diseases, heart cerebrovascular disease is one of the major causes of death in South Korea that accounts for approximately 20% of mortality. Tests to measure anomaly of the heart is ECG tests, which measures and analyzes the electrical heart activity. Any mistakes in lead attachment location critically affects ECG testings, and statistical facts showed that only 2.8% of the nurses properly located leads to patients. As a solution, this paper proposes a system based on a projection-based augmented reality technology to generate X-ray images to the patient's chest to point out exact attachment locations of ECG leads. Evaluation comparison results showed a 2.6 cm difference between the conventional system and the proposed system. ECG test results also showed significant signal differences between the systems in leads V1, V2, and V3. The ECG measured accurately by the proposed system would help greatly in patient management and clinical decisions of clinicians.
