간호대학생의 자기성찰 및 통찰력, 학업적 자기효능감이 임상추론역량에 미치는 영향 (Influence of Self-reflection and Insight, and Academic Self-efficacy on Clinical Reasoning Competence among Nursing Students)

  • Purpose: The purpose of this study was to investigate the influence of nursing students' self-reflection and insight, and academic selfefficacy on their clinical reasoning competence. Methods: Data were collected from 147 third- and fourth-year nursing students who had more than 6 months of clinical training experience. Clinical reasoning competence, self-reflection and insight, and academic self-efficacy were measured using self-reported questionnaires. Data analysis was conducted with IBM SPSS 25.0 using one-way ANOVA, independent t-test, Pearson correlation coefficient, and multiple regression analysis. Results: Clinical reasoning competence was positively correlated with self-reflection and insight (r=.24, p=.003) and academic self-efficacy (r=.30, p<.001). Academic self-efficacy (β=.23, p=.011), dissatisfaction with major (β=-.17, p=.034), and 5~8 case studies (β=-.39, p=.027) were identified as factors influencing clinical reasoning competence (adjusted R2=.15). Conclusion: To improve the clinical reasoning competence of nursing students, it is necessary to develop the academic self-efficacy of complex cases and provide critical inquiries, debriefing tailored to clinical reasoning, and sufficient reflection during clinical and simulation practice.

간호학 실습교육에 대한 국내 연구현황 분석 (An Analysis of Research on Nursing Practice Education in Korea)

  • Purpose: This study was done to propose the developmental direction of study related to nursing practice education by grasping the trend of study in fundamental and clinical practice. Method: A total of 48 research articles which were published on nursing practice education in Korea from 2002 to 2009 were analyzed with structured analysis forms. Result: Most research was related to clinical practice (n=40). A high percentage of non-experimental research design was related to fundamental practice (75%) and clinical practice (65%). Qualitative research was only used in clinical practice (n=8). Nursing students were predominantly selected as an object of research in fundamental practice (n=6) and clinical practice (n=32). In addition, many of the areas in clinical practice were a general clinical setting without any classification of the specific area. The concepts of research in fundamental practice were related to competency in basic nursing skill and most concepts of research in clinical practice were associated with satisfaction, stress, experience, critical thinking and problem solving ability of the nursing student. Conclusion: There's something to be desired in nursing research related to instructor methods, teaching-learning methods and nursing education programs. Therefore, more specific and continuous research focused on these topics to improve clinical nursing competence of the nursing student is needed.

치기공과 학생들의 임상실습 만족도, 전문직 자아개념, 진로정체감과의 관계 (Relationship between clinical practice satisfaction, professional self-concept, and career identity among students of dental laboratory technology)

  • Purpose: The purpose of this study is to investigate the relationship between clinical practice satisfaction, professional self-concept, and career identity among students of dental laboratory technology. Methods: From September 1 to September 15, 2021, data were collected through self-questionnaires completed by 180 students with clinical practice experience. The collected data were analyzed using the IBM SPSS Statistics ver. 22.0 statistical program. Frequency and percentage, mean, standard deviation, t-test, ANOVA, correlation analysis, and linear regression analysis were performed to analyze the data. Results: The average scores for clinical practice satisfaction, professional self-concept, and career identity were 3.17, 3.03, and 2.46, respectively. There were significant correlations between students' clinical practice satisfaction, professional self-concept, and career identity. There were positive correlations between clinical practice satisfaction and professional self-concept and between professional self-concept and career identity. Conclusion: Clinical practice satisfaction was found to play an important role in the formation of a positive professional self-concept as a dental technician. Therefore, a clinical practice program should be developed and implemented that can increase the satisfaction of clinical practice.

DACUM 기법에 의한 병원 임상영양사의 직무기술서 개발 (Development of Job Description of Clinical Dietitians in Hospitals by the DACUM Method)

  • The present study was conducted to develop a standardized job description for clinical dietitians working in hospitals. A developing curriculum (DACUM) method was used for the job analysis of clinical dietitians. Based on DACUM analysis with 14 members, including clinical dietitians and professors majoring in clinical nutrition and job analysis, information on the duties, tasks, and task elements of clinical dietitians was determined. To verify the job descriptions derived from DACUM analysis, a total of 46 tertiary and general hospitals with over 500 beds were recruited for the survey. The final developed job description for clinical dietitians included 7 duties, 27 tasks, and 93 task elements. The duties consisted of nutritional assessment, nutrition diagnosis, nutrition intervention, nutrition monitoring evaluation, consultation cooperation, nutrition research, and self-development. The mean scores of perceived importance, performance, and difficulty on the clinical dietitian's task elements (out of a maximum score of 5.0) were 4.5, 3.7, and 3.5, respectively, with significant differences between the items (P<0.001). The perceived importance and performance grid of clinical dietitian's tasks showed that "construction and maintenance of collaboration" (E2) and "activity of quality improvement" (F1) received relatively low scores for performance despite their high importance scores; thus the performance of these tasks requires significant improvement. In conclusion, the job descriptions of clinical dietitians developed from this study are useful for the qualitative improvement of clinical nutrition services in hospitals.

  • Adaptive Design은 임상시험 도중에 얻어진 자료에 대해 중간분석을 실시한 결과에 따라 임상시험의 중요한 요소들을 변경할 수 있도록 하는 디자인이다. 이러한 시험디자인이 치료적 확증 임상시험에 사용되는 경우, 중간분석은 임상시험에 참여하지 않은 전문가들로 구성된 독립적인 기구에 의하여 수행되어야 하며, 그렇지 않은 경우 bias(삐뚤림)를 유발시킬 수 있음을 여러 국제 가이드라인을 인용하여 지적할 것이다. 국제 가이드라인들은 중간분석의 독립적인 수행을 위하여 Independent Data Monitoring Committee(독립적 자료모니터링 위원회)를 둘 것을 권고하고 있다.

자연 유산 수태산물의 세포유전학적 분석 (A Cytogenetic Analysis of Abortus with Spontaneous Abortion)

  • Chromosomal abnormalities of abortus have also been used to investigate the most common etiology of spontaneous abortion, but the frequency and the types of spontaneous abortions have also demonstrated considerable variation among in different countries and races. A cytogenetic analysis of 75 abortuses was performed at the GenDix, Inc. from January, 2006 to December, 2007. The frequency of chromosome abnormalities in abortus was 32.0% (24/75 cases). Among the chromosomal abnormalities, trisomy was 62.5% (15/24 cases) and the most frequent trisomy was trisomy 21 with 26.6% (4/15 cases). The average maternal age of normal and abnormal karyotypes was $34.1{\pm}3.3$ and $34.3{\pm}3.3$. Cytogenetic analysis of abortus is important for diagnosis and genetic counseling for parents with spontaneous abortion.

임상 물리치료사의 경험에 따른 시각적 보행 분석의 신뢰도 연구 (Reliability of Visual Gait Analysis according to Clinical Experience Level of Physical Therapists)

  • Purpose: Visual gait analysis plays a pivotal role in determining the important gait problem of patients. A few studies have been published and have received little attention regarding visual gait analysis on patients with orthopedic problems. The purposes of this study were to investigate the difference of reliability levels according to experience of clinical physical therapists. Methods: Thirty-five clinical physical therapists, 5 high experienced, 15 experienced, and 15 inexperienced, were recruited and individually evaluated these videotaped gait patterns of the participants, and filled up the structured gait analysis form. The gait of nine participants was videotaped. Reliability levels were calculated by the Intraclass Correlation Coefficients (ICC). Results: The inter-rater reliability of high experienced group (ICC=0.56; 95% CI: 0.50-0.62) was comparable to that of the experienced raters (ICC=0.48; 95% CI: 0.43-0.53) and inexperienced group (ICC=0.42; 95% CI: 0.38-0.46). High experienced group reached a higher inter-rater reliability level. The average intra-rater reliability of the high experienced group was 0.70 (ICCs ranging from 0.54 to 0.82). The experienced group reached an average intra-rater reliability of 0.61 (ICCs ranging from 0.47 to 0.81). The inexperienced group attained average ICC values of 0.53 (ICCs ranging from 0.30 to 0.74). Conclusion: Use of a structured gait analysis form as described in this study was found to be moderately reliable. Clinical experience appears to increase the reliability of visual gait analysis.

Prenatal Diagnosis of Mucolipidosis Type II: Comparison of Biochemical and Molecular Analyses

  • Purpose: Mucolipidosis type II (ML II), also known as I-cell disease is an autosomal recessive inherited disorder of lysosomal enzyme transport caused by a deficiency of the uridine diphosphate (UDP)-N-acetylglucosamine:lysosomal enzyme N-acetylglucosamine-1-phosphotransferase (GlcNAc-phosphotransferase). Clinical manifestations are skeletal abnormalities, mental retardation, cardiac disease, and respiratory complications. A severely and rapidity progressive clinical course leads to death before 10 years of age. Methods/Results: In this study we diagnosed three cases of prenatal ML II in two different at-risk families. We compared two procedures -biochemical analysis and molecular analysis - for the prenatal diagnosis of ML II. Both methods require an invasive procedure to obtain specimens for the diagnosis. Biochemical analysis requires obtaining cell cultures from amniotic fluid for more than two weeks, and would result in a late diagnosis at 19 to 22 weeks of gestation. Molecular genetic testing by direct sequence analysis is usually possible when mutations are confirmed in the proband. Molecular analysis has an advantage in that it can be performed during the first-trimester. Conclusion: Molecular diagnosis is a preferable method when a prompt decision is necessary.

COVID-19 소아 환자의 임상 양상 및 실험실적 특징: 체계적문헌고찰 및 메타분석 (Clinical and Laboratory Features of Pediatric Patients with COVID-19: Systematic Review and Meta-analysis)

  • Background: Although the identification of clinical and laboratory features in pediatric COVID-19 patients is essential in establishing an appropriate treatment plan, a systematic review and meta-analysis on the topic has yet to be reported. Methods: We searched MEDLINE, Embase, and Web of Science to access clinical and laboratory characteristics as well as clinical outcomes of children with COVID-19 infection. A meta-analysis using random-effect model was performed to estimate pooled prevalence and 95% confidence intervals. Results: Among the 532 studies initially collected, 12 articles were finally included in the meta-analysis. Among the investigated 320 pediatric patients with COVID-19, fever (48.2%) and cough (39.3%) were the most common symptoms. Almost one third of patients (30.4%) were asymptomatic. In laboratory findings, only 11.4% of pediatric patients experienced lymphocytopenia. Increased inflammatory markers including c-reactive protein (18.6%) and procalcitonin (32.4%) were observed. Only a few patients needed mechanical ventilation and intensive care support, and only one death was reported. Conclusion: Pediatric patients with COVID-19 infection exhibited milder symptoms and more favorable outcomes compared to adults. However, considering the high rate of asymptomatic pediatric patients, close monitoring is required to prevent community infection in asymptomatic conditions and hidden disease progression.

간호대학 임상실습강사의 역할 경험 (Clinical Instructors' Role Experience in College of Nursing)

  • Purpose: The purpose of this study was to understand the clinical instructors' role experience in college of nursing. Method: Two focus group interviews were held with a total of 12 clinical instructors. All interviews were recorded and transcribed as they were spoken, and the collected data were analyzed using content analysis of Downe-Wamboldt(1992). Results: Three themes and six subthemes were extracted from the analysis: 1) Getting recognition of special roles as a clinical instructor: 'Recognizing the role of clinical instructor for guiding student's individual practice goals', 'Recognizing helping and supporting roles for student's successful clinical practice', 2) Having difficulty in performing the role of clinical practice instructor: 'Difficulty from the lack of knowledge and experience as a clinical practice instructor', 'Difficulty from the gap between current clinical practice and purpose of clinical nursing practice', 3) Making efforts to overcome the difficulties as a clinical instructor and accomplishing personal development: 'Making efforts to acquire personal knowledge and experience', 'Making a chance to reflect on and grow up oneself'. Conclusion: Clinical instructors take an important part of clinical nursing education. To improve the quality of clinical nursing education, it is needed role reestablishment and institutional support for clinical instructors on the basis of the understanding of instructors' experience.
