  • The mandibular condyle is a region that plays a key role in the opening and closing of the mouth, and because fracture causes functional and aesthetic problems such as facial asymmetry, it is very important to perform accurate reduction. Traditionally, there has been disagreement on how to manage fracture of the mandibular condyle. This review explores the misunderstanding of mandibular condyle fracture treatment and modern-day treatment strategies.

  • Objective: The aim of this study is to introduce activities for job stress management in shipbuilding industry. Background: Recently, workers are exposed many job stress like workload, interpersonal conflict while management condition of company changes day by day. Particularly, shipbuilding industry has poor environment which aggravating the stress to the workers. So managing job stress may be more important. Method: The method of this study is to investigate preventive actions of job stress in "S" shipbuilding company. Results: This study through case of "S" shipbuilding company introduce activities of prevention of job stress including identification of level of job stress, managing of psychological clinic, training of worker and manager. Conclusion: In order to manage the job stress in the workplace, 1) The total management system based on the concern of the management in the workplace has to be constructed. 2) Computerized system for operation of total management system has to be constructed. 3) New employees need training for Job Stress. Application: Through this case study, I hope to help prevention activities of job stress in the workplace.

Effect of BHT-C extract on the infarction in cerebral ischemic rats (BHT-C의 허혈성 뇌졸중 동물에서의 뇌부종 억제효과)

  • Objectives : BHT has been commonly used to treatment of brain disorders in Oriental clinic in Korea. The purpose of this study was to determine the inhibitory effect of modified BHT-C extract on the transient forcal cerebral ischemia in rats. Method : We prepared ischemic rats by the transient middle cerebral artery occlution(MCAO; 90 min occlusion and 144 h reperfusion) in rat brains. BHT-C extract (100 and 200 mg/kg, i.p.) was administered every day after the onset of MCAO until 6 day. Result : BHT-C extract increased survival rate of ischemic rats compared with vehicle-treated rats. BHT-C extract treated rats (100 and 200 mg/kg) were shown a significant reduction in infarct volume compared with vehicle-treated rats. Conclusions : These results suggest that BHT-C extract may contribute to its protective effects in brain ischemia through the reduction of brain infarction.

A Case Report of Traumatic Tracheoesophageal Fistula (외상성 기관식도루 -수술체험 1례-)

  • Acquired, traumatic tracheoesophageal fistula [TEF] is rare and difficult problem to manage. This 55 years old man met with a roller accident of a tractor. During accident, he received a penetrating injury on the left upper sternal border. At local clinic, he received closed thoracotomy drainage [CTD]for relief of pneumothorax[left]. Three days after CTD, he complained abdominal pain and hematemesis. The endoscopy revealed large ulcer at the stomach, so he received subtotal gastrectomy. On 10th day post subtotal gastrectomy, he developed aspiration and coughing from a TEF. The esophagogram showed large TEF at the mid-trachea level. So he transfered to our hospital for operation. This patient was operated on for late TEF three weeks after injury. We have used absorble 4-0 Vicryl sutures to repair trachea. We repair all esophageal injuries with two layers of nonabsorbable silk suture. Where suture line on the esophagus, the strap muscle was interposed for reinforcement. And for feeding, the feeding jejunostomy was performed. Postoperatively the osteomyelitis of the manubrium site was developed, so on the 30th postoperative day, an ostectomy of manubrium, both clavicle and fight 1st, 2nd ribs, and the pectoralis major musculo-cutaneous flap coverage were performed.

  • Patients with severe mental challenges are often subjected to extensive dental problems. Their impaired communication and lack of cooperation unable to receive timely dental treatments, which deteriorates their adverse oral conditions. In spite of a limited time frame, well-planned comprehensive treatments can be delivered in an one-day operation setting under general anesthesia. This multidisciplinary treatment regimen primarily comprises of single-visit endodontics, direct bonded restoration, periodontal treatment, and surgical extraction. Further, one or two additional visits for prosthetic procedures are able to accomplish functional reestablishment and esthetic enhancement. This article discusses on clinical managements for severely mentally challenged patients under a single-day operation.

  • We present a case of a colonic involvement associated with necrotizing pancreatitis, with a review of the literature. A 10 year old boy had an appendectomy at the local clinic ten days ago. On admission, he complained nausea, vomiting and severe constipation. His abdomen was distended and he had tenderness on the left abdomen. Laboratory and radiologic studies revealed findings consistent with acute pancreatitis with colonic complication. He was treated conservatively for 30 days but did not improve. On hospital 30th day, abdominal pain developed and his vital sign changed. Abdominal CT suggested ischemic change of the transverse colon. At laparotomy, the left colon showed stenosis. The greatly distended transverse colon was resected and a transverse end colostomy was done. He was discharged at postoperative 45th day with improvement and colostomy closure was performed 8 months later.

Case reports of amniotic membrane derived-cell treatment for feline chronic renal failure

  • Three different cats who had chronic kidney disease (CKD) were treated for more than one month with fluid therapy in an animal clinic. Although this long-term treatment and hospitalization, there was no clinical improvement in clinical signs as well as serum biochemical indexes including blood urea nitrogen (BUN), creatinine (CREA), and phosphate (PHOS). All cases were then injected three times with allogeneic stem cells through an intravenous route for treatment on Day 0, 7, and 14 or 30. On the same day, clinical observation and blood tests for serum biochemistry were conducted together. Upon administrating stem cells to the CKD cats, clinical conditions and the indexes of BUN and CREA were clinically improved within normal ranges. Additionally, one of the cats who had the renal cysts presented clinical improvement with showing decreased cysts size than before.

Pediatric Finger Warts Treated Using Taeeumjowi-tang: a case report

  • Warts caused by the human papillomavirus (HPV) are generally treated with cryotherapy, CO2 laser ablation, interferon injections, and bleomycin injections. However, it is sometimes difficult to treat children because the treatment can be painful. In addition, recurrence may occur after treatment. In this study, warts completely disappeared following the administration of herbal medicine in two children, with warts in multiple parts of the hands and around the nails. Two pediatric patients visited the hospital for treatment of warts around their fingers and nails. Both patients received Taeeumjowi-tang (TJT) as a decoction for 60 days. TJT was performed twice per day for the 11-year-old patient and once per day for the 7-year-old patient. Patient progress was observed monthly, and the visual condition of the warts was photographed during the visits. After approximately two months of treatment, the warts disappeared from the fingers and nails of both patients. This case study suggests that the oral administration of TJT may be effective for pediatric patients with warts. Further studies are required to determine the efficacy and safety of these therapies.

The Effects of Forests Healing for Cognitive Function (산림치유프로그램이 인지기능에 미치는 영향)

  • Objectives : The purpose of this study is to examine the effects of 3 day Forests healing programs for cognitive improvement, which consists of helpful contents of Korean medicine. Methods : This study examined the effects of forests healing for mild cognitive impairment patients, who visited a Korean medicine hospital neuropsychiatry clinic for outpatients. The Forests healing programs for cognitive improvement was run with 24 participants, in which 9 of them quit in the middle of the program, so excluded in data analysis. The Forests healing programs for cognitive improvement was consisted of 3 day helpful Korean medicine contents. The primary outcome measurement was a Computerized Neurocognitive Test (CNT), which measured the cognitive functions with computer programs. Results : The results of this study showed that CNT scores were not significantly changed after 3 day forests healing. General psychological states, including depression, anxiety, and anger, were also checked. Among them, depressive mood significantly improved. Quality of life (QOL) was also enhanced after 3 day program. These results indicate that the 3 day Forests healing programs for cognitive improvement was effective for depressive mood and QOL of mild cognitive impairment patients, but not enough for cognitive functions directly. Conclusions : The 3 day Forests healing programs for cognitive improvement were shown to be an effective intervention in improving some aspects of mild cognitive impairment patients, including depressive mood, and QOL. We suggest that repeated participation of this program can enhance cognitive function of mild cognitive impairment patients.

Improvement of Pregnancy Rates by Coculture of Human Embryos with Cumulus Cells in Glucose and Phosphate Free M-TALP Media (Glucose와 Phosphate가 제거된 M-TALP 배지에서의 난구세포 공배양에 의한 임신율 향상에 관한 연구)

  • The beneficial effect of glucose and phosphate ions in culture medium on the development of human embryos in vitro has not been fully elucidated. The purpose of this study was to evaluate the influence of fertilization and culture of embryos in glucose/phosphate-free m-TALP medium on pregnancy rates in IVF-ET program. The patients in 244 IVF-ET cycles received GnRH agonist + HMG regimens. A does of 10,000 IU HCG was administered when two or more dominent follicles reached 18mm in diameter. Thirty-six hours after HCG, oocytes were recovered transvaginally using ultrasound guidance. Aspirated oocytes were matured for 4 to 6 h in TCM-199 supplemented with 10% follicular fluid (FF). Insemination was carried out with 50,000 motile spermatozoa in TCM-199 + 10% FF or m-TALP + 5% FF + 5% fetal cord serum (FCS) according to experimental design. After 6 h, oocytes were washed 3 to 4 times and cultured in each fresh medium. After 20 h, oocytes were freed from cumulus/corona cells and examined for the presence of pronuclei. Fertilized oocytes were transferred into each co-culture drops and cultured for further incubation. On day 3, embryo transfer was performed with grade 1 and 2 embryos. Monolayers for co-culture of embryos were prepared by plating $1{\times}10^5$ cumulus cells/ml in 10ul drop of TCM-199 + 10% FF or m-TALP + 5% FF + 5% FCS media 24 h prior to the onset of co-culture. Development to 4 to 16 cell stage was observed at 70x magnification following two days of incubation. Pregnancy was confirmed by detecting increasing serum ${\beta}$-hCG concentrations for 11 days following embryo transfer. Data were analyzed by ${\chi}^2$-test. Oocytes from 244 IVF-ET cycles were randomized. The number of cycles and mean age of patients were 97 and 147, 31.3 yrs and 31.2 yrs for TCM-199 (control) and m-TALP groups, respectively. The mean number of retrieved oocytes/cycle, fertilization rates, number of embryos transferred/ET and pregnancy rates were 11.1 and 10.3, 65.1% and 67.3%, 4.1 and 4.7, 28.9% and 43.8% for TCM-199 and m-TALP groups, respectively. Differences in the pregnancy rates were found between control and m-TALP groups (p<0.05). The pregnancy rate of patients divided according to maternal age groups of ${\leq}30$, 31-35, $36{\leq}$ were 44.4% and 49.0%, 26.1% and 41.3%, 29.2% and 41.2% for control and m-TALP groups, respectively. These data indicate that culture of human embryos in glucose/phosphate-free m-TALP medium improves pregnancy rates.
