Clavicle Hook Plate를 이용한 견봉 쇄골 관절 손상의 치료 (Treatment of Acromioclavicular Joint Injuries Using Clavicle Hook Plates)

  • 목적: 견봉 쇄골 관절 손상의 치료 방법으로서 Clavicle hook plate를 이용한 고정술의 결과를 분석하였다. 대상 및 방법: 2008년 2월부터 2010년 4월까지 본원에서 견봉 쇄골 관절 탈구로 Clavicle hook plate를 이용한 고정술을 시행하였던 18예를 대상으로 하였다. 제 3형의 견봉 쇄골 관절 손상이 7예, 제 5형이 11예였으며, Clavicle hook plate는 수술 후 3개월 이후에 제거하였다. 치료 결과의 판정은 스트레스 부하 방사선 촬영을 하여 건측과 비교하였고 Korean shoulder scoring system 및 American shoulder elbow society score를 이용하여 견관절의 기능과 환자의 만족도를 평가하였다. 평균 추시 기간은 9개월이었다. 결과: 스트레스 부하 방사선 사진에서 오구 쇄골 간격이 건측에 비해 수술 전 평균 114% 증가되어 있었으나 수술 후 최종 추시 시 23%로 감소하였으며, Korean shoulder scoring system은 평균 80점이었고 American shoulder elbow society score는 평균 74점이었다. 결론: 견봉 쇄골 관절 손상의 Clavicle hook plate를 이용한 고정 시 충분한 관절 안정성으로 조기 재활이 가능하며, 방사선학적 및 기능적으로도 비교적 만족할만한 결과를 얻을 수 있어 수술적 치료에 있어 유용한 방법 중 하나라고 사료된다.

Coracoclavicular Ligament Suture Augmentation with Anatomical Locking Plate Fixation for Distal Clavicle Fracture

  • Background: For Neer type IIB fracture of distal clavicle with coracoclavicular ligament injury, various surgical treatments have been used in literatures. However, there was no consensus on the optimal treatment. The aim of this study is to report the clinical and radiological results of open reduction and internal fixation of unstable distal clavicle fracture and suture augmentation of disrupted coracoclavicular ligament. Methods: A prospective study was performed in 23 patients with Neer type IIB distal clavicle fracture in Seoul Medical Center, Eulji Hospital, and National Medical Center. Firstly, suture anchors are inserted in the base of coracoid process and preliminary reduction was achieved by tie-off of three suture limbs around the clavicle. Then, the final fixation was completed with anatomical locking plate. Bony union and the distance between coracoclavicular ligaments were evaluated. Clinical results and complications including stiffness and secondary procedures were evaluated. Results: Bony union was achieved in all cases except one (22 of 23). At mean 14.9 months, no significant difference in the mean coracoclavicular distance was observed compared to uninjured shoulder ($8.2{\pm}7.9mm$ versus $7.3{\pm}3.4mm$, p=0.14). Pain visual analogue scale, American Shoulder and Elbow Surgeons score, Constant score, and Disabilities of the Arm, Shoulder and Hand score were 0.5, 83.4, 78.5, and 6.2, respectively. Revision surgery was performed in one case of nonunion. Four patients who complained of skin irritation underwent implant removal. Conclusions: In cases of an unstable distal clavicle fracture with coracoclavicular ligament disruption, satisfactory clinical results were obtained by locking plate fixation and coracoclavicular ligament suture augmentation concurrently.

  • Purpose: The causes of twelve cases of the mid-shaft clavicle nonunion and the results of internal fixation with plate and bone graft were investigated. Materials and method: From August 1997 to March 2003, twelve cases of the mid-shaft clavicle nonunion were operated with internal fixation with plate and bone graft. The duration of follow-up was average 13 months. Results: The causes of the mid-shaft clavicle nonunion included severe associated injury, severe initial displacement of the fracture fragments and insecure fixation. All cases were operated with internal fixation with plate and bone graft. According to the factor for evaluations of results, using a rating scale of excellent (no apparent factors), good (one factor), fair (two factors), poor (more than three factors), the results showed 10 excellent, 1 good and 1 poor. Screw loosening was developed in only one case . Conclusion: The internal fixation with plate and bone graft of the mid-shaft clavicle nonunion after failed conservative treatment achieved excellent results and seemed to be the procedure of choice for mid-shaft clavicle nonunion.

The Radiologic Comparison of Operative Treatment Using a Hook Plate versus a Distal Clavicle Locking Plate of Distal Clavicle Fracture

  • Background: The purpose of this study was to compare the radiologic results of patients who underwent surgery with a hook plate and a locking plate in distal clavicle fractures. Methods: Sixty patients underwent surgical treatment for Neer type IIa, IIb, III, and V distal clavicle fracture. Twenty-eight patients underwent fracture fixation with a hook plate and 32 with a locking plate. Coracoclavicular distance was measured on standard anteroposterior radiographs before and after the surgery, and union was confirmed by radiograph or computed tomography taken at 6 months postoperatively. Other radiologic complications like osteolysis was also checked. Results: Bony union was confirmed in 59 patients out of 60 patients, and 1 patient in the hook plate group showed delayed union. Coracoclavicular distance was decreased more in the hook plate group after surgery (p<0.01). After 6 weeks of the hook plate removal, the coracoclavicular distance was increased a little compared to before metal removal, but there was no difference compared to the contralateral shoulder. Eleven out of 28 patients (39.3%) showed osteolysis on the acromial undersurface in the hook plate group. Conclusions: Both the hook plate group and the locking plate group showed satisfactory radiologic results in distal clavicle fractures. Both hook plate and locking plate could be a good treatment option if it is used in proper indication in distal clavicle fracture with acromioclavicular subluxation or dislocation.

Plate prebending using a three-dimensional-printed model affords effective anatomical reduction in clavicular shaft fractures

  • Background: A precontoured plate rarely fits properly within the patient's clavicle and must be bent intraoperatively. This study aimed to determine whether anatomical reduction could be achieved using a plate bent before surgery. Methods: This study included 87 consecutive patients with displaced mid-shaft clavicle fractures who underwent plate fixation and were followed-up for a minimum of 1 year. After exclusions, 39 consecutive patients underwent fixation with a precontoured plate bent intraoperatively (intraoperative bending group), and 28 underwent fixation with the plate bent preoperatively (preoperative bending group). Using free software and a three-dimensional (3D) printer, ipsilateral clavicle 3D-printed models were constructed. Using plain radiographs, the distance between the edge of the lateral inferior cortex and the medial inferior cortex was measured. The angle between the line connecting the inferior cortex edge and the line passing through the flat portion of the superior cortex of the distal clavicle was measured. Results: Mean length differences between the ipsilateral and contralateral clavicle were smaller on both anteroposterior (AP; P=0.032) and axial images (P=0.029) in the preoperative bending group. The mean angular differences on both AP (P=0.045) and axial images (P=0.008) were smaller in the preoperative bending group. No significant differences were observed between the two groups in functional scores at the last follow-up. Conclusions: Smaller differences in length and angle between the ipsilateral and contralateral clavicle, indicative of reduction, were observed in the preoperative bending group. Using the precontoured technique with low expense, the operation was performed more effectively as reflected by a shorter operation time. Level of evidence: III.

견봉 쇄골 탈구와 쇄골 외측단 골절에서 일시적 오구 쇄골간 나사못 고정술 (Coracoclavicular Screw Fixation for AC Dislocation and Unstable Distal Clavicle Fracture)

  • Purpose: There are so many different methods with the their own advantages and disadvantages for acute AC dislocation and distal unstable clavicle fracture. We evaluated the usefulness of temporary Bosworth's coracoclavicular fixations using the cannulated screw and reviewed our clinical results. Materials and Methods: We evaluated 34 cases of temporary coracoclavicular fixations with cannulated screw and washer for acute AC dislocation(25cases in the Type V, Ⅲ) and distal unstable clavicle fracture(9cases in the Type ⅡB, Ⅱ+Ⅲ). All cases were fixed by the same techniques, guided cannulated screw under C-arm after the repair of the CC ligament within 1 week. Bases on the regular check, screws were removed at the 6-12 weeks under local anesthesia. The final clinical and radiological results(average 14 months follow-up) were rated with the UCLA scale. Results : All shoulders were gained nearly full ROM passively at the 3-4 weeks. The overall satisfactory resuIts(excellent or good) were achieved 88%(22/25) in the acute AC dislocation and all in the distal unstable clavicle fracture. There were no definite complications except three initial fixations loosening. Conclusion: Temporary Bosworth's coracoclavicular fixation using the cannulated screw with guided pin was simple precedure and provided enough initial stability for acute AC dislocation and distal unstable clavicle fracture.

  • 상완골 및 쇄골에서 발생한 만곡 및 가관절증에 대한 보고들은 드물다. 일반적으로 선천성 쇄골 가관절증은 우측 및 쇄골 중심부에서 호발하고 선천성 경골 가관절증과 비교 시 치유가 쉬운 것으로 알려져 있다. 본 저자들은 발생 위치 및 가관절증의 형태가 기존의 선천성 쇄골 가관절증과 다르고, 신경섬유종증과 연관되어 발생한 선천성 쇄골 가관절증을 경험하여 문헌 고찰과 함께 보고하는 바이다.

  • Displaced fracture of the distal third of the clavicle usually occurs after direct trauma to the shoulder and typically results in superior displacement of the proximal fragment. We report a previously undescribed case of downward displacement of the clavicle caused by a fall on an outstretched hand, and we suggest the mechanism of injury.

  • A large parosteal osteoma arising on the surface of the right clavicle of a 39-year-old male patient was suspected preoperatively as a parosteal osteosarcoma. The lesion was treated with wide resection and allograft reconstruction. In this case report, we discuss the accurate diagnosis and appropriate surgical treatment for unusual clavicular tumors.

흉쇄유돌근 아시혈(阿是穴)의 침치료와 스케나 치료로 호전된 이충만감 치험 2예 (Two Cases Report on the Patients with Ear Fullness by Acupuncture Therapy and SCENAR Therapy on Sternocleidomastoid Muscle)

  • We researched two patients with ear fullness after traffic accident. We diagnosed the patients' symptom as referred pain caused by clavicle branch of sternocleidomastoid (SCM) muscle. So, acupunture therapy and SCENAR therapy were used to treat the patients. We measured their discomfort by visual anlog scale (VAS) before and after treatment. As a result, the patients improved remarkably and VAS score was declined sharply when we applied acupuncture therapy and SCENAR therapy on clavicle branch of SCM. Therefore, we are certain that ear fullness can be caused by clavicle branch of SCM after traffic accident. It shows dramatical and satisfactory progress by acupuncture therapy and SCENAR therapy to relax shortening of clavicle branch of SCM.
