모래여과 및 오존처리에 의한 하천수 수질개선 효과 연구(1) - BOD, COD, SS 및 색도 제거 경향 고찰 (Improvement of River Water Quality By Combined Treatment of Sand Filtration and Ozonation(1) - Focusing on Reduction of BOD, COD, SS and Color)

  • While various aspects affect river water quality, reduction of water flow rate during dry seasons is one of the most significant factors causing severe water pollution in river water environment. The aim of this study is to investigate the feasibility of applying a physicochemical method (sand filtration + ozonation) for improving river water quality within a short period. The parameters analyzed and assessed were $COD_{cr}$, BOD, SS and color. The source river water had a severe pollution level showing COD 8.8~17.2 mg/L (ave. 11.9 mg/L), BOD 4.8~13.3 mg/L (ave. 8.3 mg/L), SS 9.0~22.1 mg/L (ave. 12.8 mg/L) and color 34.4~77.1 degree (ave. 56.5 degree) during the experimental periods. The variation trends showed a relatively low correlation between BOD and COD and between color and COD, while SS showed very low correlation with other parameters. The combined process of sand filtration and ozonation showed averaged removal efficiency of COD 37.2%, BOD 48.4%, SS 60.1% and colority 45.1%, respectively. The marked change of BOD level from 8.3 mg/L to 4.3 mg/L under the experimental conditions in this study implied the improvement of class V to class III set by the river water quality standard in Korea.

Implant-supported overdenture manufactured using CAD/CAM techniques to achieve horizontal path insertion between the primary and secondary structure: A clinical case report

  • This report describes the case of an edentulous patient with an atrophic maxilla and severe class III malocclusion. Prosthetic rehabilitation was performed using CAD/CAM techniques for manufacturing an implant-supported overdenture with horizontal insertion. A vestibulo-lingual insertion overdenture is a precision prosthesis with a fixation system affording a good fit between the primary and secondary structure. Both structures exhibit passive horizontal adjustment. This treatment option requires the same number of implants as implant-supported fixed dentures. The horizontal assembly system prevents the prosthesis from loosening or moving in response to axial and non-axial forces. The technique was used to rehabilitate a patient presenting an atrophic upper maxilla, with the insertion of 8 implants. No complications were reported at follow-up 3, 6 and 12 months after fitting of the prosthesis. This system offers solutions to the clinical and laboratory complications associated with hybrid prostheses, concealing emergence of the chimneys and improving implant-prosthesis hygiene.

아크릴 섬유의 아미도옥심화에 관한 연구(I) -아미도옥심 반응과 천이금속의 흡착능- (Studies on Amidoximated Acrylic Fiber(I) -Amidoximation and Adsorption Capacity to Transition Metals -)

  • In order to investigate a practical application of fibrous absorbents to transition metals such as copper, nickel, cobalt, chrome, and iron, amidoximated fiber as a particular class of solid chelate agents were prepared from acrylic fibers treatment with hydroxylamine. The adsorption mechanisms of metal ions onto amidoximated acrylic fibers and their complexes were studied. Amidoximation of acrylic fiber with hydroxylamine is found to be first-order reaction, followed by the disappearance of infrared adsorption peaks due to nitrile groups of acrylic fibers. The uptake of metal ions onto amidoximated acrylic fiber is increased with temperature raising and the adsorption is also depended on pH of the soiutions. About 70% of metal ions can be recovered from aqueous solutions of Ni(II), Co(II), Cr(III), and Fe(II) on the concentration below 5x 10$^$^{-4}$ in the range of pH 2.1~10.0. Transition metals are adsorbed to form complex with amidoxime group by the ligand sites such as C=N, NH, NO, NHOH.OH.

Brain Metastases from Solid Tumors: an Institutional Study from South India

  • Background: Brain metastases are the most common intra-cranial neoplasms. The incidence is on a rise due to advanced imaging techniques. Aims: The objective of the study was to analyse the clinical and demographic profile of patients with brain metastases from primary solid tumors. Materials and Methods: This is a retrospective single institutional study covering 130 consecutive patients with brain metastases from January 2007 to August 2014. Results: Some 64.6% of the patients were females. The majority were in the sixth decade of life. The site of the primary tumor was the lungs in 50.8% of the cases. The overall median time from the diagnosis of the primary malignancy to detection of brain metastases was 21.4 months. Survival was found to be significantly improved in patients with solitary brain lesions when compared to patients with multiple brain metastases, and in patients undergoing surgical excision with or without cranial irradiation when compared to whole brain irradiation alone. The majority of the cases belonged to the recursive partitioning analysis class II group. Whole brain radiation therapy was delivered to 79% of the patients. Conclusions: Most of the patients with brain metastases in the study belonged to recursive partitioning analysis classes II or III, and hence had poor prognosis. Most of the patients in the Indian context either do not satisfy the indications for surgical excision or are incapable of bearing the high cost associated with stereotactic radiosurgery. Treatment should be tailored on an individual basis to all these patients.

교정 진단을 위한 피개교합심도지수 (Overbite Depth Indicator, ODI) 의 새로운 평가 (A NEW APPROACH TO USE OVERBITE DEPTH INDICATOR IN ORTHODONTIC DIAGNOSIS)

  • Since 1984, many patients have been treated with Multiloop Edgewise Archwire (MEAW) Technique and diagnosed with ODI (Overbite Depth Indicator) and APDI (Anteroposterior Dysplasia Indicator) by the authors. 234 samples of them were selected randomly for the statistical analysis (age, sex, Angle's classification, treatment period, extraction, ODI etc.). Especially, ODI was analysed statistically and its application methods were reviewed. The results and conclusions were as follows: 1. On the 150 patients with normal overbite, the mean values of Class I, II, III malocclusion were $67.5^{\circ}$, $72.2^{\circ}$ and $59.0^{\circ}$. They were significantly different on the level of p < 0.01. 2. In normal overbite samples, ODI decreased with the increase of APDI and the correlation coefficient was -0.54. It seems that this result reflects the characteristics of AB to mandibular plane angle. 3. The regression equation was Y = - 0.57X + 114.64, where X is APDI and Y is ODI. In cases of small or large APDI, it seems to be absurd that the patient's ODI is compared with the mean ODI to differentiate diagnostically the open bite or deep bite tendency from the normal.

원발성 심장종양에 대한 외과적 치험 (Surgical Treatment of Primary Cardiac Tumor)

  • We experienced 6 cases of primary cardiac tumor, all received operation for removal of tumor. Mean age was 43.8 years-old ranging from 17 years-old to 66 years-old. Five cases were female, one case was male. Five cases were benign, myxoma, all located within left atrium. One case was malignant, angiosarcoma within right atrium. All patient showed cardiac manifestations. One case was in NYHA functional class II, two were in III, three were in IV. Four patients showed constitutional symptoms, but no one showed evidence of embolic phenomenon. All case of myxoma showed cardiomegaly except one malignancy. Only one case was regular sinus rhythm, three were sinus tachycardia 8z two were atrial fibrillation. The most common site of tumor origin was fossa ovalis limbus[four of all]. Two of five myxomas received emergency operation, one patient died postoperatively. Lived four patients showed no evidence of recurrence[mean follow-up, 3,5 years], but one patient has Grade II /IV mitral regurgitation & in OPD follow-up now, One malignant case, 17 years-old cerebral palsy female, was angiosarcoma occupied most of right atrial chamber originated from anterior wall of right atrium, received emergency operation which was removal of mass & reconstruction of right atrium with artificial pericardial patch. This patient died on postoperative 36th day due to persistent LCOS[low cardiac output syndrome] with combined sepsis.

Root coverage with a modified laterally positioned flap combined with a subepithelial connective tissue graft in advanced recession

  • Purpose: A laterally positioned flap (LPF) combined with a subepithelial connective tissue graft (SCTG) is one of the conventional approaches for resolving gingival recession defects, with the advantages of flap flexibility and extended coverage of the tissue graft. However, thus far, evidence is lacking for the use of this technique for the treatment of advanced gingival recession defects. This report discusses three Miller class III cases with interproximal bone loss and wide and deep defects treated with a combination procedure of a modified laterally positioned flap (mLPF) and SCTG. Methods: mLPF combined with SCTG was performed for each case. The defect size and the degree of hypersensitivity at baseline and the final appointment in each case were documented. Results: The three cases had a mean initial defect of $7.7{\pm}1.5mm$ and a mean residual defect of $1.7{\pm}1mm$ at the 6-, 3-, and 36-month follow-up, respectively, after the root coverage surgery. The symptom of hypersensitivity was improved, and the patients were satisfied with the clinical outcomes. Conclusions: The results demonstrated that the combination of mLPF with SCTG is promising for treating these advanced cases with respect to obtaining the expected root coverage with the gingival tissue.

Distribution and phenotypes of hemifacial microsomia and its association with other anomalies

  • Objective: To investigate the distribution and phenotypes of hemifacial microsomia (HFM) and its association with other anomalies. Methods: This study included 249 Korean patients with HFM, whose charts, photographs, radiographs, and/or computed tomography scans acquired during 1998-2018 were available from Seoul National University Hospital and Dental Hospital. Prevalence according to sex, side involvement, degree of mandibular deformity, compensatory growth of the mandibular body, and Angle's classification, and its association with other anomalies were statistically analyzed. Results: Prevalence was not different between male and female patients (55.0% vs. 45.0%, p > 0.05). Unilateral HFM (UHFM) was more prevalent than bilateral HFM (BHFM) (86.3% vs. 13.7%, p < 0.001). Although distribution of the Pruzansky-Kaban types differed significantly in patients with UHFM (I, 53.0%; IIa, 18.6%; IIb, 24.7%; III, 3.7%; p < 0.001), no difference was observed in occurrence between the right and left sides (52.6% vs. 47.4%, p > 0.05). Among patients with BHFM, prevalence of different Pruzansky-Kaban types on the right and left sides was greater than that of the same type on both sides (67.6% vs. 32.4%, p < 0.05). Despite hypoplasia of the condyle/ramus complex, compensatory growth of the mandibular body on the ipsilateral side occurred in 35 patients (14.1%). Class I and II molar relationships were more prevalent than Class III molar relationships (93.2% vs. 6.8%, p < 0.001). Forty-eight patients (19.3%) had other anomalies, with 50.0% and 14.4% in the BHFM and UHFM groups (p < 0.001). Conclusions: Patients with HFM require individualized diagnosis and treatment planning because of diverse phenotypes and associations with other anomalies.

성인의 동맥관 개존증 결찰술 시행후 심흉비의 변화 -30례 보고- (The Change of Cardiothoracic Ratio after Ligation of Patent Ductus Arteriosus in Adult -Report of 30 cases-)

  • 배경: 성인의 동맥관개존증치료에는 폐고혈압, 동맥벽 석회화, 동맥관의 동맥류화와 같은 어려움이 있다. 저자들은 동맥관 결찰후에 심장의 크기가 어떻게 변화하는지 알아보고자 하였다. 대상 및 방법: 마산 삼성 병원에서는 1987년에서부터 1997년 까지 30명의 성인 동맥관 개존 환자를 결찰법으로 수술 했다. 결과: 이들은 남자가 9명 이었으며 여자가 21명이었다. 나이는 16세 에서 44세 였고 평균 26.1세였다. 이들 중 폐동맥 고혈압은 15례에서 있었고 9명의 환자에서 NYHA class III 이상의 운동시 호흡곤란이 있었다. 수술 방법은 모두 테프론을 덧댄 결찰법을 시행 하였고, 수술후 합병증은 3례의 창상 감염과 1례의 일시적인 애성이 있었다. 수술전 심흉 비는 54.7%에서 수술후 51.9%로 변화 되었다. 심흉비의 변화는 울혈성 심부전을 가진 환자들에서 더욱 특징적 이었는데 수술전 64.8%에서 수술후 58.5%로 변화 되었다. 수술과 연관된 사망은 없었다. 결론: 이상의 연구에서 성인에서 동맥관을 결찰한 뒤에 심흉비가 감안하는 것을 알 수 있었다.

골격성 제3급 부정교합환자의 하악지 시상분할 골절단술후 하안면 폭경 및 고경의 변화에 대한 두부계측 방사선학적 연구 (A CEPHALOMETRIC STUDY ON CHANGES OF FACIAL MORPHOLOGY IN THE FRONTAL VIEW FOLLOWING MANDIBLE SETBACK SURGERY ( BSSRO ) IN PATIENTS WITH SKELETAL CLASS III DENTOFACIAL DEFORMITIES)

  • Purpose : The purpose of this study was to analyze the lower third facial changes in frontal view after mandibular setback surgery. Materials and Methods : In this study, fifteen subjects(6 males and 9 females) with class III dental and skeletal malocclusions who were treated with BSSRO(Bilateral Sagittal Split Ramus Ostetomy) were used. Frontal cephalometric radiographs were taken preoperatively and more than 6 months postoperatively, and hard tissue(H2-Hl) and soft tissue changes (S2-S1) were measured on vertical and horizontal reference lines. In 15 cases, changes which developed more than 6 months after surgery were studied. Results : The results were as follows. 1. In the facial height, hard tissue $decreased(2.46{\pm}2.76mm)$ with statistical significance(P<0.01), and soft tissue also $decreased(1.64{\pm}3.66mm)$. As a result, the facial height generally becomes shorter after sagittal split ramus osteotomy. 2. In the mandibular width, hard tissue $decreased(2.08{\pm}3.59mm)$ with statistical sgnificance(P<0.05), but soft tissue $increased (2.14{\pm}5.73mm)$ without statistically significant difference(P>0.05) postoperatively. 3. In the facial index, hard tissue $decreased(0.23{\pm}2.21%)$, but soft tissue $increased(2.41{\pm}3.46%)$ with statistical significance. Conclusion : One of the main purpose of orthognathic surgery is to achieve facial esthetics and harmony. In order to fullfill this purpose, it is important to carry out a precise presurgical treatment planning by estimating the changes of frontal profile after surgery.
