Policy Effects of Secondhand Smoke Exposure in Public Places in the Republic of Korea: Evidence from PM2.5 levels and Air Nicotine Concentrations

  • Objective: The purpose of this study was to evaluate secondhand smoke (SHS) exposure inside selected public places to provide basic data for the development and promotion of smoke-free policies. Methods: Between March and May 2009, an SHS exposure survey was conducted. $PM_{2.5}$ levels and air nicotine concentrations were measured in hospitals (n=5), government buildings (4), restaurants (10) and entertainment venues (10) in Seoul, Republic of Korea, using a common protocol. Field researchers completed an observational questionnaire to document evidence of active smoking (the smell of cigarette smoke, presence of cigarette butts and witnessing people smoking) and administered a questionnaire regarding building characteristics and smoking policy. Results: Indoor $PM_{2.5}$ levels and air nicotine concentrations were relatively higher in monitoring sites where smoking is not prohibited by law. Entertainment venues had the highest values of $PM_{2.5}$(${\mu}g/m^3$) and air nicotine concentration(${\mu}g/m^3$), which were 7.6 and 67.9 fold higher than those of hospitals, respectively, where the values were the lowest. When evidence of active smoking was present, the mean $PM_{2.5}$ level was 104.9 ${\mu}g/m^3$, i.e., more than 4-fold the level determined by the World Health Organization for 24-hr exposure (25 ${\mu}g/m^3$). Mean indoor air nicotine concentration at monitoring sites with evidence of active smoking was 59-fold higher than at sites without this evidence (2.94 ${\mu}g/m^3$ vs. 0.05 ${\mu}g/m^3$). The results were similar at all specific monitoring sites except restaurants, where mean indoor $PM_{2.5}$ levels did not differ at sites with and without active smoking evidence and indoor air nicotine concentrations were higher in sites without evidence of smoking. Conclusion: Nicotine was detected in most of our monitoring sites, including those where smoking is prohibited by law, such as hospitals, demonstrating that enforcement and compliance with current smoke-free policies in Korea is not adequate to protect against SHS exposure.

The Phosphodiesterase 4 Inhibitor Roflumilast Protects against Cigarette Smoke Extract-Induced Mitophagy-Dependent Cell Death in Epithelial Cells

  • Background: Recent studies show that mitophagy, the autophagy-dependent turnover of mitochondria, mediates pulmonary epithelial cell death in response to cigarette smoke extract (CSE) exposure and contributes to the development of emphysema in vivo during chronic cigarette smoke (CS) exposure, although the underlying mechanisms remain unclear. Methods: In this study, we investigated the role of mitophagy in the regulation of CSE-exposed lung bronchial epithelial cell (Beas-2B) death. We also investigated the role of a phosphodiesterase 4 inhibitor, roflumilast, in CSE-induced mitophagy-dependent cell death. Results: Our results demonstrated that CSE induces mitophagy in Beas-2B cells through mitochondrial dysfunction and increased the expression levels of the mitophagy regulator protein, PTEN-induced putative kinase-1 (PINK1), and the mitochondrial fission protein, dynamin-1-like protein (DRP1). CSE-induced epithelial cell death was significantly increased in Beas-2B cells exposed to CSE but was decreased by small interfering RNA-dependent knockdown of DRP1. Treatment with roflumilast in Beas-2B cells inhibited CSE-induced mitochondrial dysfunction and mitophagy by inhibiting the expression of phospho-DRP1 and -PINK1. Roflumilast protected against cell death and increased cell viability, as determined by the lactate dehydrogenase release test and the MTT assay, respectively, in Beas-2B cells exposed to CSE. Conclusion: These findings suggest that roflumilast plays a protective role in CS-induced mitophagy-dependent cell death.

HemoHIM, A herbal preparation, alleviates airway inflammation caused by cigarette smoke and lipopolysaccharide

  • HemoHIM, herbal preparation has designed for immune system recovery. We investigated the anti-inflammatory effect of HemoHIM on cigarette smoke (CS) and lipopolysaccharide (LPS) induced chronic obstructive pulmonary disease (COPD) mouse model. To induce COPD, C57BL/6 mice were exposed to CS for 1 h per day (eight cigarettes per day) for 4 weeks and intranasally received LPS on day 26. HemoHIM was administrated to mice at a dose of 50 or 100 mg/kg 1h before CS exposure. HemoHIM reduced the inflammatory cell count and levels of tumor necrosis factor receptor (TNF)-${\alpha}$, interleukin (IL)-6 and IL-$1{\beta}$ in the broncho-alveolar lavage fluid (BALF) induced by CS+LPS exposure. HemoHIM decreased the inflammatory cell infiltration in the airway and inhibited the expression of iNOS and MMP-9 and phosphorylation of Erk in lung tissue exposed to CS+LPS. In summary, our results indicate that HemoHIM inhibited a reduction in the lung inflammatory response on CS and LPS induced lung inflammation via the Erk pathway. Therefore, we suggest that HemoHIM has the potential to treat pulmonary inflammatory disease such as COPD.

Factors Related to Increasing Trends in Cigarette Smoking of Adolescent Males in Rural Areas of Korea

  • Objectives: Cigarette smoking prevalence among adolescent males in rural areas of Korea has increased in recent years. The aim of this study was to explore the factors related to increasing trends in cigarette smoking among adolescent males living in rural areas. Methods: The raw data from the Korea Youth Risk Behavior Web-based Survey from 2006 to 2009 were used. Data were analyzed by using the method of complex survey data analysis considering complex sampling design. Multiple logistic regression models were used to explore the factors affecting cigarette smoking. We evaluated the linear time trends in the prevalence of factors that were related to current smoking status and the linear time trends in cigarette smoking in groups stratified by the exposure to each factor using logistic regression models. Finally, we examined the contributions of the factors to the time trends in cigarette smoking by adjusting for each of those factors in the baseline regression models and changes in the adjusted odds ratio by survey year. Results: A statistically significant increasing trend in smoking was observed after adjusting for the factors affecting cigarette smoking. Significant factors related to cigarette use were perceived stress, experience with depression, current alcohol drinking, exposure to secondhand smoke, and academic performance. The factor related to increasing trends in cigarette smoking was academic performance. Conclusions: Stress about academic performance is an important factor affecting the increase in cigarette smoking among adolescent males in a rural area of Korea.

한국 성인의 3차 흡연으로 인한 아이의 건강 유해성 인식과 관련 요인 (Factors Associated with Beliefs among Adults in Korea about the Health Effects of Thirdhand Smoke on Children)

  • Objectives: Thirdhand smoke (THS) refers to residual tobacco smoke pollutants that remain after cigarette smoking. The purpose of this study was to determine predictors of the belief among adults in Korea that THS is harmful to children. Methods: A population-based sample of 3,000 adults (${\geq}19$ years) among a web-based panel in Korea was obtained in November 2016. The respondents filled out questionnaires, including THS beliefs, smoking status, presence of voluntary smoke-free rules at home, and support for the implementation of smoke-free regulations at home. THS beliefs were assessed with a statement that staying in a room today where people smoked yesterday can harm the health of children. Results: Overall, 92.2 % of adults believed that THS exposure could harm the health of children. Respondents who were nonsmokers, believed that SHS exposure were associated with pneumonia of children, lived in home with complete smoke-free rule at home, and supported the implementation of smoke-free regulation at home were more likely to believe that THS could harm for health of children. Conclusions: The majority of Korean adults believed that THS exposure could harm the health of children. Predictors identified in the study can be useful to address the risk of THS beliefs and promote implementation of tobacco control policies.

가열담배 사용과 연소담배 흡연의 급성 심혈관 효과 검증 (Comparison of Acute Cardiovascular Effects of Using Heated Tobacco Productsand Cigarette Smoking)

  • 본 연구는 연소담배와 비교해 서로 다른 가열담배(heated tobacco products, HTPs) 사용 시 혈역학적 지표들의 급성 반응을 비교하고, 급성 HTPs 에어로졸 노출이 심장 자율신경조절(cardiac autonomic regulation, CAR)에 미치는 영향을 탐색하려는 목적으로 수행되었다. 또한, 서로 다른 연초 스틱 간 급성 심혈관 효과의 비교 검증을 목표로 하위연구를 설계했다. 외관상 건강한 남성 흡연자 16명은 개방표지, 무작위 교차시험으로, 최소 48시간 세척기간과 함께 비-흡연(non-smoking, NS), 담배 흡연 및 서로 다른 두 가지 HTPs 사용 세션을 각각 수행했다. 하위연구는 NS와 함께 서로 다른 두 가지 연초 스틱 간 급성 심혈관 효과를 비교하기 위해, 참여자 중 8명을 대상으로 실시되었다. 수축기와 확장기 혈압 및 CAR을 반영하는 심박변이도(heart rate variability, HRV)가 각 세션 시작 전, 제품 사용 중 그리고 사용 후 30분까지 평가되었다. NS 동안 혈역학적 변화는 통계적 차가 없었으나, HTPs 에어로졸에 급성 노출은 담배 연기와 상응하는 수축기와 확장기 혈압의 유의한 증가를 초래했으며, 이들 지표에서 HTPs 기기 간 통계적 차는 없었다. 연소담배와 HTPs 사용 후 통계적으로 유의한 심장 교감신경활동(cardiac sympathetic nerve activity, CSNA)의 급성 증가와 심장 미주신경 긴장도(cardiac vagal tone, CVT)의 급격한 저하가 나타났다. 사용 기기와 독립적으로 HTPs 사용에 따른 CSNA와 CVT 반응은 연소담배와 통계적 차가 없었다. 서로 다른 두 가지 연초 스틱은 NS와 비교해 혈역학적 지표들의 유의한 증가를 유도했으나, 연초 스틱 간 통계적 차는 없었다. CVT는 두 가지 연초 스틱 사용에 따라 유의하게 감소했으며, 연초 스틱의 종류에 따라 CVT 저하에 통계적 차가 있었다. 이상의 결과들은 HTPs 에어로졸 노출이 사용 기기와 독립적으로 연소담배와 동등한 급성 심혈관 효과를 유도할 수 있으며, 연초 스틱의 종류에 따라 급성 CAR 저하가 강화될 수 있음을 시사한다.

Characterization of Respirable Suspended Particles and Polycyclic Aromatic Hydrocarbons associated with Environmental Tobacco Smoke

  • In this study, the concentrations of particulate organic constituents of environmental tobacco smoke(ETS) were determined using an environmental smog chamber, where ETS is the sole source of target compounds. ETS was generated in a 30 ㎥ environmental chamber by a number of different cigarettes, including the Kentucky 1R4F reference cigarette and eight commercial brands. A total of 12 experimental runs was conducted, and target analytes included a group of ETS markers both in vapor and particulate phase and a class of polycylic aromatic hydrocarbos(PAHs) associated with ETS particles. The mass concentrations of PAH in ETS particles were also determined. The average contents of benzo(a) pyrene and benzo(a) anthracene in ETS particles for the commercial brands were 12.8 and 21.5$\mu\textrm{g}$/g, respectively, There values are all somewhat higher than those determined previously by other studies. Results form the chamber study are further used to estimate the average and variability of cigarette yields for target compounds associated with ETS. Finally, ratios of RSP to the surrogate standards of UVPM, FPM and solanesol were calculated for each sample. The average conversion factors factors for the eight commercial brands were 7.3, 38, and 41 for UVPM, EPM, and solanesol, respectively. The UVPM and FPM factors are in good agreement with the recently published values. Whereas there might be a substantial difference in the solanesol content among cigarettes produced in different countries, the variability is somewhat greater than those of UVPM and FPM, Unfortunately, comparison of the PAH yield data from this study with literature values was complicated by a lack of consistency in cigarette smoke generating methodology. Validation of the PAH yields was also difficult due to a lack of information on the ETS related PAH in the literature. From and engineering viewpoint , however, these data on the cigarette yields of ETS components may still provide useful information to studies on the mathematical modeling of indoor air quality management regarding tobacco smoke as a source of interest, or to studies on the assessment of human exposure to ETS.

The Relation between Exposure to Environmental Tobacco Smoke and the Quantity of Cotinine in the Urine of School Children in Taif City, Saudi Arabia

  • Environmental tobacco smoke (ETS) is a major public health problem for all ages. Despite the high prevalence of smoking among the Saudi population, there is limited information about levels of urinary cotinine in Saudi children exposed to SHS. The aim of the study was to assess the exposure of schoolchildren to ETS, and measure their urinary cotinine levels. Multistage cluster sampling was carried out, where schoolchildren from 4 schools were randomly chosen from primary schools in Taif city. A questionnaire including questions on SHS exposure and smoking rules in the residence were sent to students parents/guardians. Urine samples were taken and analyzed for total cotinine using chemiluminescent immunoassay. Of the studied children, 38.4% had a smoking father, 61.8%, 41.2% and 49.3% of them were exposed to ETS indoors, outdoors and both indoors and outdoors respectively. The mean urinary cotinine was significantly higher among children exposed to ETS compared to unexposed children. Urinary cotinine levels in children with both indoor and outdoor exposure was significantly higher compared with its level in children with single exposure. A significant positive correlation was found between urinary cotinine concentrations and the number of cigarette packs smoked by parents, and the number of smokers in the residence. The mean urinary cotinine level was significantly higher in children who reported no smoking rules at the residence.. The study revealed a high exposure of Saudi children to ETS. An antismoking media awareness campaign on the harmful effects of ETS should be carried out, in addition to family counseling programs targeted to parents to protect their children from ETS.
