Clinical Analysis of Recurrence in Inflammatory Laryngeal Disease after Laryngomicrosurgery (술 후 재발한 염증성 성대 병변의 임상 양상)

  • Microscopic surgery of the larynx normally have a low recurrence rate. However, once they do recur, it is uneasy to manage recurrent diseases. Recurrence is often the result of inappropriate postoperative care and voice management. Out of the 764 patients operated for benign inflammatory laryngeal diseases, we have conducted a clinical analysis of 54 cases who have had at least one episode of recurrence after the initial treatment with laryngomicrosurgery (7.1% of recurrence rate). High risk groups for recurrence were determined, and they were related to cigarette smoking, alcohol intake, laryngopharyngeal reflux. From our experience and literature review, we conclude that pre- and postoperative patient education concerning these risk factors and voice usage Is of critical need to prevent recurrence of inflammatory laryngeal lesions after laryngomicrosurgery.

  • DNA damage is an essential step in the pathogenesis of cancer and probably of other chronic degenerative conditions related to cigarette smoke (CS), such as atherosclerosis, cardiomyopathies, etc. Although the major goal of primary prevention is to refrain from smoking, chemoprevention by means of dietary and pharmacological agents provides a complementary preventive strategy. In spite of its overwhelming epidemiological importance, experimental studies evaluating CS as a complex mixture are relatively scanty.(omitted)

Recent clinical study of stroke patients (뇌졸중 환자의 최근 임상적 고찰)

  • The purpose of this study is to present the tendency of stroke data on patients with stroke admitted to the hospital and to investigate the risk factors of stroke. We reviewed of 104 patients with stroke admitted to the hospital of from July 2001 to August 2001. The highest incidence of the stroke was noted in the group of 60 years of age with the rate of male(63.5%) to female(36.5%). The occurrence rate of ischemic stroke(51.9%) was higher than that of hemorrhagic stroke(48.1%). Middle cerebral arterial territory was the most commonly involved site cerebral arterial causes. The possible contributing factors of stroke were hypertension(52%), cigarette smoking(36.5%) and diabetes mellitus(18.3%). cardiac patients(16.3%). The seasonal preference was winter and autumn followed by summer and spring.

  • This paper describes the generation and decay phenomena of gas, vapor and particulate phase components of environmental tobacco smoke in 18 m3 controlled experimental atmosphere chamber. Real time-weighted average concentration ratios of markers were determinated at no ventilation rates and sampling durations of starting to smoking 45 min. Average concentration of major ETS markers was no significant on the mainstream smoke contents of commercial cigarette and decay ratios were dependent on first order kinetic. RSP/nicotine, solanesol and 3-EP were good predictors of ETS concentration in the public indoor field. The concentration ratio of vapor phase and particulate phase components is highly variable to assessment of indoor air quality with ETS. Key words : ETS, chamber study, ETS markers.

  • Objectives: To present the bases for health education on electronic cigarettes, we analyze the present state of electronic cigarettes. Methods: We applied descriptive study, reviewing the literature from domestic and foreign electronic databases such as National Discovery for Science Leaders and PubMed. Results: Electronic cigarette has been the subject of much controversy but very little study. I reviewed and summed up the definitions, device safety, health risk, smoking cessation effects, addiction, second hand smoke, and regulations of electronic cigarettes. Conclusion: Finally,I presented the evidence-based contents and desirable direction for health education about electronic cigarettes. The results suggested that the health educators focus on the potential addiction to electronic cigarettes, instead of the cessation effects.

Serum Antioxidant Vitamins and Erythrocyte Lipid Peroxide Levels in Female Adolescent Smokers

  • The purpose of this study was to investigate the association between adolescent smoking and antioxidant vitamins. Subjects were 87 non-smokers and 90 smokers. who were female high school students. Smokers were divided into tow groups by smoking status, 35 light smokers (packyear<1) and 53 heavy smokers(packyear$\geq$1). Dietary intakes were examined through questionnaires and nutrient intakes of vitamin C and A were analyzed by smoking status using Computer Aided Nutritional analysis program for professional (CAN-PRO). Serum vitamin C level was measured by 2,4-dinitrophenylhydrazine method and serum levels of vitamin A and E were measured by HPLC. Erythrocyte lipid peroxide levels was measured by thiobarbituric acid reactive substance (TBARS) method. All data were statistically analyzed by SAS PC package program. The mean vitamin C intakes of non-smokers, light smokers and heavy smokers were 58.2mg/day, 50.1 mg/day 58.1mg/day, respectively. The mean vitamin A intakes of non-smokers, light and heavy smokers were 28.1$\mu\textrm{g}$R.E./day, 278.7$\mu\textrm{g}$R.E./day and 289.6$\mu\textrm{g}$R.E./day, respectively. There was no significant difference in dietary intakes of antioxidant vitamins by smoking status. However, the serum vitamin C level, 11.40mg/l in heavy smokers was 12% lower than that of than that of 12.70mg/l in non-smokers. The serum vitamin A level was not significantly different among the groups. Vitamin E level, 8.79mg/l in heavy smokers was 8% lower than that of 9.53mg/l in non-smokers. There was no significant correlation between the dietary intakes and serum levels of vitamin A and C. The erythrocyte TBARS level, 1.90nmol/ml in light smokers was significantly lower (p<0.05) than 2.71 nmol/ml in heavy smokers or non-smokers. The correlation data showed that the cerythrocyte TBARS level had a significant positive correlation with packyear. Overall results might suggest that cigarette smokers with a longer smoking history need more dietary intakes of vitamin C and E than do non-smokers to reach the same serum level.

Dynamic Developmental Factors and their Problem Solving of Patients that Abuse Marihuana (마리화나 남용환자의 역동적인 발달요인과 문제해결)

  • This study was made on patients who were Hospitalized due to sickness caused by the abuse of marihuana at the Psychiatric Section of a hospital during the period from Feb. to Oct., 1974. The following conclusion was obtained by reviewing the literature with respect to the nursing, and problem solving of those patients. 1. It appears that marihuana is a comparatively mild intoxicant in ordinary preparation without causing physical dependence or tolerance. 2. According to the status of men who are marihuana smokers, approximately 20% of them were college students, those preparing to retake college entrance examinations, non-employed and pharmacists. The men belong mostly to the middle foreigner′s mistress of higher, income bracket, and in the case of women, most of the smokers were US. servicemen entertainers. 3. Dynamic developmental factors: Case 1. : The patient had a characteristic, hysterical and emotionally unitable character, and was of low intelligence, In addition, to this already existing problem, the added uses of marihuana caused a mental illness to develope. Case 2 : The character, was reserved and introspective, her creative power and sentiment was fading and his ability of self-control was weakened. She used the smoking of marihuana to get rid of her own feeling of inferiority complex and tensions coming from interpersonal relationships. Case 3 : The patient was unconditionally resistive to the authoritativeness of superiors and irresponsible in his relationship with women, in his attitude concerning sex in general. He smoked marihuana because he felt become peace-loving and get enchanted experience through smoking it. 4. The points of issue appearing from the above case; (1) Movement of anti-social feelings against the "established system" by the youngsters. (2) Family problem. (3) Shamelessness, loss of motivation, disorderly attitude toward the sex, (4) Worries concerning the future. (5) Lack of knowledge concerning smoking of cigarette and marihuana. Chronic use of marihuana made, those youngman who had originally been ambitions to achieve something in life, lazy, inefficient, unable to make long-term plans, are such weak mined persons that they did not try to overcome problems when encountered. This will pose a great and important question in the mental health of the society, 5. Treatment and Problem sieving According to the literature, we will have to place importance upon hospitalized treatment The phases of treatment were divided into five parts. (1) Prehospital phase (2) Withdrawal Phase (3) Rehabilitation phase (4) Transitional phase (5) After-care phase The experiments have proved that there was much progress in the recovery of patients through environment therapy, supportive therapy and group psychotherapy. This was the above mentioned 5 phases of treating process in accordance with the weekly schedule of the hospital. It was thought that the patients would require prolonged after care management even after they were released from the hospital and that they will also require periodic visit, to the hospital and doctor′s interview with their family. In conclusion, the question of the young generation and marihuana smoking is becoming a great social problem in which their resistances to the "established system" and society is growing in the from of antiestablishment movements. In our country, the smoking of marihuana is gradually developing, therefore, I think, that it would be a very fortunate thing for us, if this report could be helpful for the motivation of further study on the questions of the young generation and its problems.
