  • Objectives: To evaluate the evidence supporting the effectiveness and safety of Chinese medicine, including Chuna manual therapy, for the treatment ofosteoarthritis. Methods: We conducted a search of 8 electronic databases(Pubmed, Web of Science, EMBASE, Cochrane Library, CAJ, KISTI, NDSL, and RISS) to find all randomized controlled clinical trials that discuss Chuna manual therapy forosteoarthritis till May 2018. Results: Our inclusion criteria selected 7eligible RCTs involving 633 participants. The meta-analysis of 6 studies showed positive results for the useof Chinese medicine, including Chuna manual therapy, for treating knee osteoarthritis. There was no evidence of worsening symptoms or any adverse events. Conclusions: Our systematic review found favorable evidence regardingthe effectiveness of Chinese medicine, including Chuna manual therapy, for treating knee osteoarthritis. However, our systematic review had several limitations owingto the low quality of original articles andadditionalclinical trials would be necessary for stronger evidence.

  • Objectives The purpose of this study was to report the effect of Korean medicine treatment, including the application of joint mobilizing chuna, by reporting two cases after tibial plateau fracture surgery. Methods Two patients with tibial plateau fractures were treated using joint mobilizing chuna, myofascial chuna, acupuncture, and herbal medication. The effect of the treatments was evaluated using the range of motion, manual muscle test, numeric rating scale, and Korean Knee Injury and Osteoarthritis Outcome Score. Results In both cases, the range of motion, muscle strength, and pain were significantly improved. In particular, the range of motion for knee joint flexion increased by 47° in case 1 and 30° (right) and 42° (left) in case 2. Conclusions Korean medicine treatment, especially joint mobilizing chuna, may be an effective intervention for rehabilitation after tibial plateau fracture surgery.

  • Objectives This study was conducted to analyze the therapeutic effect of Chuna therapy for lateral epicondylitis. Methods Key words such as lateral epicondylitis and Chuna were searched in 10 databases (National Digital Science Library [NDSL], Oriental Medicine Advanced Searching Integrated System [OASIS], Research Information Sharing Service [RISS], Koreanstudies Information Service [KISS], KMBASE, Cochrane Library, PubMed, EMBASE, J-STAGE, China Academic Jurnals [CAJ]), and only suitable randomized controlled trials (RCTs) were selected. Results 8 RCTs were finally selected, and Chuna therapy showed positive results for lateral epicondylitis compared to western medicine, acupuncture, warm-acupuncture and Chinese medicine iontophoresis. Conclusions In this study, we finally reviewed 8 studies about Chuna therpay used for lateral epicondylitis. The studies showed that Chuna therapy is effective in treating lateral epicondylitis. However, because all of selected studies were found in Chinese database, more high-quality clinical trials about Chuna therapy for lateral epicondylitis are needed in Korea.

  • Objectives This study aimed to investigate the status of Chuna manual therapy(CMT) usage before and after COVID-19 social distancing measures. Methods This study utilized Health Insurance data from April 2019 to May 2023. Quarterly data were collected for Simple Chuna (code 40710), Complex Chuna (code 40720;50% patient expense), Complex Chuna (code 40721;80% patient expense), and Special Chuna (code 40730). Results During the period covered by health insurance, the number of CMT patients was 8,315,838, with 19,332,786 instances of CMT, and a total cost of 544,651,407 won. Due to COVID-19, the application of CMT decreased in the first quarter of 2020, but recovered from 2021, showing an increase in usage by the first quarter of 2023. The decline in the use of all medical institutions in the fourth quarter is believed to be due to restrictions on health insurance coverage for CMT. Conclusions Further research on CMT health insurance is necessary. This research should inform discussions on policies aimed at expanding health insurance coverage for CMT.

  • Objectives : To evaluate the evidence supporting the effectiveness of Chuna manual therapy for functional dyspepsia. Methods : We conducted search across the 3 electronic databases (Pubmed, CAJ and Oasis) to find all of randomized controlled clinical trials(RCTs) that used Chuna manual therapy as a treatment for functional dyspepsia. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results : Thirteen RCTs met our inclusion criteria. The meta-analysis showed positive results for the use of Chuna manual therapy combined with medication treatments in terms of the efficacy rate when compared to medication treatments alone. Positive results were also obtained, in terms of the efficacy rate, when comparing Chuna manual therapy combined with medication plus specific electromagnetic therapy to medication treatments alone. Conclusions : Our systematic review found encouraging but limited evidence of Chuna manual therapy for functional dyspepsia. However, to obtain stronger evidence without the drawbacks of trial design and the quality of studies, we recommend sham-controlled RCTs or comparative effectiveness research to test the effectiveness of Chuna manual therapy.

In Chuna group, we treated patients with Distraction & Translation technique. For evaluating Function of Temporomandibular joint of Patients, we use the Modified Craniomandibular Index(mCMI) before treatment and after 6th treatment. Results : In Mandibular Movement(MM) of patients, the patients of Acupuncture Group were more effective than Chuna group. In TMJ noise(TN) of patients, the patients of Chuna Group were more effective than Acupuncture group. • Objective : The purpose of this study is to review the randomized clinical trials of Chuna manual therapy for cervicogenic headache and provide a evidence for the efficacy of Chuna manual therapy. Methods : We searched randomized clinical trials that performed Chuna manual therapy for cervical headache up to Feb. 2017 in 6 databases. Randomized clinical trials were selected according to the inclusion criteria and the data were extracted and analyzed. The risk of bias was assessed using the Cochrane Risk of Bias Criteria. Results : 16 RCTs met the inclusion criteria. The meta-analysis of 13 RCTs showed favorable results for the use of chuna manual therapy compared to drug, physical treatment. Conclusions : In 16 RCTs, we found that Chuna manual therapy was effective in cervicogenic headache. However, all RCTs are exposed to a number of bias risks. Therefore, well designed clinical trial would be needed to raise the evidence level of Chuna manual therapy.

  • Background: The purpose of this study was to analyze the effect of national health insurance coverage of Chuna therapy in April 2019 on the costs and service uses in automobile insurance. Methods: This study used the claim data from Health Insurance Review and Assessment Service. A total of 189,912 inpatients and 1,550,497 outpatients who received Chuna therapy covered by automobile insurance in oriental medical institutions were included. The analysis period was from July 2018 to December 2019, and a total of 18 months before and after April 2019, when Chuna therapy was covered by national health insurance. Interrupted time series analysis was applied to analyze the impact on the costs and service uses of Chuna therapy in automobile insurance before and after April 2019. Results: From July 2018 to December 2019, for 189,912 inpatients the cost and the number of times for Chuna therapy per capita were increased by 22.0% and decreased by 7.3% respectively right after the implementation of the policy. In the case of 1,550,497 outpatients, the cost of Chuna therapy per capita tends to be increased by 0.4% in overall study periods and increased 28.4% immediately after the implementation of the policy. Meanwhile, the number of times and visits for Chuna therapy per capita tends to be increased by 0.4% in overall study periods but decreased by 0.4% after the implementation of the policy. Conclusion: Results suggest that if the national health insurance coverage of oriental medicine services increases according to the policy stance for benefit expansion in national health insurance, the criteria for providing national health insurance benefits should be considered with the comprehensive impacts on the costs and service uses of automobile insurance.
