Investigation of High-Sensitivity C-reactive Protein and Erythrocyte Sedimentation Rate in Low Back Pain Patients

  • Background: Chronic low back pain can be a manifestation of lumbar degenerative disease, herniation of intervertebral discs, arthritis, or lumbar stenosis. When nerve roots are compromised, low back pain, with or without lower extremity involvement, may occur. Local inflammatory processes play an important role in patients with acute lumbosciatic pain. The purpose of this study was to assess the value of erythrocyte sedimentation rate (ESR) and high sensitivity C-reactive protein (hsCRP) measurements in patients with chronic low back pain or radiculopathy. Methods: ESR and hsCRP were measured in 273 blood samples from male and female subjects with low back pain and/or radiculopathy due to herniated lumbar disc, spinal stenosis, facet syndrome, and other diseases. The hsCRP and ESR were measured prior to lumbar epidural steroid injection. Results: The mean ESR was 18.8 mm/h and mean hsCRP was 1.1 mg/L. ESR had a correlation with age. Conclusions: A significant systemic inflammatory reaction did not appear to arise in patients with chronic low back pain.

만성 요통에 대한 매선요법을 병행한 한방치료의 효과에 대한 후향적 연구 (The Effects of Korean Medical Treatment Combined with Embedding Acupuncture on Patients with Chronic Lower Back Pain: a Retrospective Study)

  • Objectives : The purpose of this study was to investigate the clinical effects of Korean medical treatments combined with Embedding acupuncture on patients with chronic lower back pain. Methods : We reviewed the medical records of 40 patients with chronic lowerback pain hospitalized at Dongshin Korean Medicine Hospital from March, 2015 to February, 2016. They were divided into two groups: the embedding acupuncture group(20 patients) and the non-embedding acupuncture group(20 patients). To evaluate the efficacy of the treatments, the 40 patients were asked to complete a Numerical Rating scale (NRS) and the Oswestry Disability Index (ODI) four times during admission. Results : The mean NRS of the embedding acupuncture group decreased more significantly than the non-embedding acupuncture group at days three and ten of admission. The ODI change and ODI rate of change of the embedding acupuncture group were significantly greater than the non-embedding acupuncture at days three and ten of admission. Conclusion : Korean medical treatment combined with embedding acupuncture might be effective in reducing pain and improving the life quality of patients with chronic lower back pain. We hope that further studies will be done to produce more clinical data and ensure effective application of these results.

말의 만성요통에 대한 전침치료 효과 (The Effect of Electro-Acupuncture Treatment for Chronic Back Pain in Horses)

  • This study was performed to investigate that electro-acupuncture might relieve chronic back pain in horses. Twenty horses with chronic back pain were chosen from referring to College of Veterinary Medicine, University of Florida. They were assigned the patients referred to 2 treatment groups and 1 control group. The treatment group I was electrostimulated to 4 acupoints. The acupuncture points of Lumbosacral point, GV-6, BL26, and BL54 were used.; The treatment group II was taken the phenylbutazone orally. Control group was administered the normal saline per oral. Back pain score system was used to evaluate the degree of back pain. The treatment group r had significant lower back pain score from two times of electroacupuncture treatment. However, the treatment group II and control group did not change back, pain scores during experimental periods. Thus, Electroacupuncture treatment might be effective therapy for chronic back pain in horses.

Narrative Review of Pathophysiology and Endoscopic Management of Basivertebral and Sinuvertebral Neuropathy for Chronic Back Pain

  • Chronic lower back pain is a leading cause of disability in musculoskeletal system. Degenerative disc disease is one of the main contributing factor of chronic back pain in the aging population in the world. It is postulated that sinuvertebral nerve and basivertebral nerve main mediator of the nociceptive response in degenerative disc disease as a result of neurotization of sinuvertebral and basivertebral nerve. A review in literature is done on the pathoanatomy, pathophysiology and pain generation pathway in degenerative disc disease and chronic back pain and management strategy is discussed in this review to aid understanding of sinuvertebral and basivertebral neuropathy treatment strategies.

진동자극 환경에서의 요부 안정화 운동이 만성요통환자의 균형능력 및 기능장애수준에 미치는 영향 (The Effects of Integrating Vibration Into Stabilization Exercise on Balance and Disability in Patients With Chronic Low Back Pain)

  • Background: Compared to healthy people, patients with chronic lower back pain have reduced balance abilities which may cause proprioception problems, patients with chronic lower back pain avoid physical activities due to pain, and reduced activity levels lead to muscle weakening, which can further exacerbate pain. Recently, there have been many studies on the use of sensory stimulation; and among these studies, interventions that use vibrational stimulation have shown functional improvements in the patients. Objects: This study examined the effects of a stabilization exercise with vibration stimulation on the balance ability and disability in patients with chronic back pain. Methods: The subjects of the study were 30 persons who were randomly assigned to the experimental group and the control group, with 15 subjects in each. The subjects were evaluated before and after intervention via a balance ability test, the Korean Oswestry disability index (KODI) test, a pain test, and a proprioceptive sensory test. Both groups received general physical therapy. The experimental group performed the stabilization exercise with vibration stimulation, and the control group performed a general stabilization exercise, three times a week for six weeks. Results: After the intervention, both groups showed significant improvements in the balance ability test, the KODI test, the pain test, and the proprioceptive sensory test. The experimental group showed statistically significant, higher improvements than the control group in the balance ability test, the KODI test, and the proprioceptive sensory test. Conclusions: The stabilization exercise with vibration stimulation for patients with chronic back pain has been reported to provide greater functional improvements than the conventional intervention method. Therefore, the stabilization exercise in a vibration stimulation environment could be a useful intervention for patients with chronic back pain.

The Effects of Spinal Decompression Therapy on Pain and Disability in Patients with Chronic Low Back Pain

  • Purpose: The purpose of this study was to examine the effects of spinal decompression therapy on pain and disability in patients with chronic low back pain. Methods: Twenty patients with chronic low back pain were divided into an experimental group (spinal decompression therapy, n=10) and a control group (conservative physical therapy, n=10). Both groups were treated three times a week over a four-week period. Results: The comparison of between-group changes post-treatment revealed statistically significant lower levels of pain and disability in the experimental group than the control group. The comparison of within each group changes before and after the treatment showed statistically significant declines in pain and disability indexes of both groups. Conclusion: Spinal decompression therapy may be an effective intervention for improving pain and disability in patients with chronic low back pain.

만성 요통환자의 건강지각과 건강증진 행위에 관한 연구 (A Study on Health Perception and Health Promoting Behavior in Chronic Back Pain Patients)

  • Purpose: The purpose of this study was to show a relationship between health perception and health promoting behaviors in chronic low back pain patients. Method: The subjects for this study were 213 persons who the visited hospital with low back pain-related problems. Results: The higher the level of the health perception in chronic back pain patients was the higher the rate of the practice of health promoting behaviors (r=0.393, p<.001). The health perception T score was $50.00{\pm}10.00$. As for health promoting behaviors, the T score was $49.99{\pm}10.00$. The subscale of the highest mean score was interpersonal support $(2.96{\pm}0.64)$ and the subscale of the lowest mean score was exercise $(2.13{\pm}0.99)$. Conclusion: This study showed that chronic low back pain patients had a lower level of perception of their health, and their practice to improve their health was not enough. Therefore, nurses should educate and encourage chronic low back pain patients in proper exercises and correct posture to strengthen and maintain lumbar extension muscle power.

요추부 불안정성을 가진 요통환자의 요추부 안정화 운동과 둔근 강화 운동이 통증, 기능장애 및 심리사회수준에 미치는 효과 비교연구 (Comparison of Lumbar Stabilization Exercises and Gluteal Strengthening Exercises on Pain, Disability and Psychosocial Factors in Low Back Pain Patients with Lumbar Instability)

  • Background: Lumbar stabilization exercise and gluteal muscle strengthening exercises are widely used to treat for lower back pain patient. The present study aimed to compare the effects of lumbar stabilization exercise and gluteal muscle strengthening exercises on chronic lower back pain with lumbar spine instability, with regard to pain intensity, disability, and psychosocial factors. Methods: Among 53 patients with chronic lower back pain, those with spine instability were selected using 5 examination tests. The selected 28 patients were randomly assigned to lumbar stabilization exercise group (LSE, n=15) and gluteal strengthening exercise group (GSE, n=13). Each group performed the corresponding exercise for 40 minutes, twice a week for 4 weeks. To analyze and compare the effects, pain intensity, the level of low back disability, and psychosocial factors were assessed before and after intervention. Results: There was significant difference in lower back pain intensity between the two groups before and after intervention. The change in low back disability was significant in the GSE group alone following intervention (p<.05), but no other significant difference was found between the groups. Among psychosocial factors, the changes in the fear-avoidance beliefs questionnaire (FABQ)-physical activity and FABQ-total were significant in the LSE group alone following intervention (p<.05). However, no significant difference were found in these factors between the two groups before and after intervention. Conclusions: LSE and GSE for lower back pain with lumbar spine instability showed no significant difference for pain intensity, physical disability, or psychosocial functioning.

다중운동 프로그램이 만성 요통 환자의 통증 강도, 몸통 근력 및 오스웨스트리 장애지수에 미치는 영향 (Effects of a Multi-modal Exercise Program on Pain Intensity, Trunk Muscle Strength, and Oswestry Disability Index in Patients with Chronic Low Back Pain)

  • Background: The purpose of this study was to examine the effects of a multi-modal exercise program for patients with chronic low back with respect to pain intensity, trunk muscle strength and Oswestry disability index. Methods: Thirty patients with chronic low back pain were recruited and divided equally into two groups. The multi-modal training program comprised a series of exercises such as warm-up, stabilization exercises, stretching, endurance exercises, and cool down whereas the control group performed only stabilization exercises. The both group spent an equal amount of time performing 60 minutes per day, three times per week, for five weeks. Results: The experimental group demonstrated statistically significant improvements in range of motion, trunk muscle strength, the visual analogue scale, and the Oswestry Disability Index (p<.05). Intergroup comparison showed a statistically significant difference in the range of motion of the lumbar spine and the degree of disability in the experimental group. Muscle strength and pain were statistically significant in both groups. Conclusion: The multi-modal exercise program is effective for patients with chronic low back pain, as it reduces lower back pain, increases trunk muscles strength, and decrease the potential for becoming disabled.

고압산소와 물리치료의 복합 적용이 노인의 만성 요통 완화에 미치는 영향 (Influence of the Application to Hyperbaric Oxygen Therapy and Physical Therapy on Chronic Low Back Pain in Elderly)

  • Purpose: To investigate the effects of hyperbaric oxygen therapy (HBOT) on chronic low back pain in elderly patients with impaired functioning in daily life. Methods: 35 subjects with chronic low back pain were randomly allocated into two groups, HBOT group and Placebo group. We applied TENS and mechanical massage to the lower back in both groups. Then 35% oxygen at 1.4 ATA was given to the HBOT group and atmospheric oxygen at 1 ATA to the placebo group. Results: Comparing the HBOT group with the placebo group after performing interventions six times in 2 weeks, a statistically significant difference in the Visual Analogue Scale (VAS) and Oswestry Disability Index (ODI) scores was observed between the two groups. There was a statistically significant difference in VAS and ODI scores (56% and 34.97% respectively) between the HBOT group and the placebo group. A statistically significant difference was ascertained in the degree of variables VAS and ODI in the HBOT group. Conclusion: Application of HBOT in chronic low back pain could be beneficial for reducing pain and could help in functional recovery.
