A case of cutaneous leishmaniasis developed in a 48 year old Korean male who returned from middle east was studied by light and electron microscopic observations. Light microscopically, the lesion consisted of heavy chronic ill-defined granulomatous inflammation involving entire thickness of the dermis, composed of mainly histiocytic and small mononuclear cell infiltrations without evidence of necrosis or giant cell formation. Giemsa staining revealed numerous intracellular micro-organisms within histiocytes, showing dark stained central dot surrounded by light stained cytoplasm. Electron microscopically, the organisms were observed mostly ovoid in shape and frequently binary mitotic features within the host cells. follicle consisted of double unit membranes and microtubules, which are immediately below these membrnae. A long kinetoplast was noted within a very elongated mitochondrion at the center of the organisms and a flagella rose in front of the kineoplast but ended within the cytoplasm. Large numbers of free ribosomes, occasional Golgi complex and SER were also noted, but RER was seldom found. These ultrastructural features corresponded to promastigote stage of Leishmania tropica. In principle, leishmaniasis is a tropical disease and can not be found in temperate zone. However, travel to mideast by many Koreans may contract this disease while they are in endemic regions.

Background: Miescher's cheilitis granulomatosa (MCG) is a rare chronic inflammatory disease and is known as the monosymptomatic clinical form of Melkersson-Rosenthal syndrome (MRS). It is characterised by swelling of one or both lips and more frequently affects the upper lip. Histopathological findings show the presence of numerous inflammatory infiltrates and granuloma formations. Pharmacological treatments and surgery have provided results that are positive yet insufficiently stable in the long term. The clinical case described is of a 68-year-old female patient with a diagnosis of MCG of the upper lip. Case presentation: The patient was diagnosed and treated at the Oral Medicine and Oral Pathology outpatient clinic of Maxillofacial and Odontostomatology Unit, Fondazione Cà Granda IRCCS Ospedale Maggiore Policlinico. The patient was recommended localised treatments of photobiomodulation (PBM) using a diode laser with a 635 nm and 980 nm dual-wavelength (λ) approach, a 600-micron fibre, and a handpiece with a 1-cm-diameter lens at 300 mW. Three treatments a week were administered for four weeks for a total of 12 treatment sessions (T1-T12). After that, the patient had a long follow-up period of about 2 years. The therapeutic results were clear from the initial stages of treatment. There was an immediate, gradual, and consistent reduction in labial swelling. A reduction in the size of the lip by about 35% at T10-T12 was observed, returning the size and volume of the upper lip within the normal clinical range. The painful symptoms subsided after the seventh treatment (T7). The histopathological check at 3 months and the follow-up in particular confirmed the disease was in remission with satisfactorily stable treatment results. Moreover, the patient did not use any other treatments on the area from the early laser treatments through to the end of the follow-up period. Conclusions: Our experience describes a clinical case of MCG treated with PBM and effectively resolved with a reduction of the lip swelling. The real success of the treatment emerged over time, showing that the tissue healing was stable. In absence of any collateral phenomena, this confirms the effective and documented therapeutic potential of PBM for chronic inflammatory infiltrates.
This paper was attempted to analyze 31 cases of fiberoptic bronchoscopy during the period of 8 months from October 1978 till February 1979 in the Chung-Ang University Hospital. The results were as follows; 1) Among 31 cases. 20 cases were male, 11 cases female. Sex ratio was 2 : 1. 2) In age distribution, 22 cases (70.1%) were in 4th-7th decade, and the other age groups showed relatively even distribution. 3) The chief complaints were hemoptysis (11 cases, 35.5%), coughing (9 cases, 29.0%), chest pain (6 cases, 19.4%), dyspnea (3 cases, 9.7%) and others (2cases, 6.5%). 4) In Gram staining of bronchial secretion, Gram (-) diplococci were in 12 cases (38.7%), Gram (+) cocci 10 cases (32.3%), Gram (+) rods 6 cases (19.4%)and Gram (-) rods 3 cases (9.7%). In culture of bronchial secretion, no growth were in 17 cases (54.8%). Neisseria group 6 cases (19.4%), Proteus and Klebsiella group 1 case (3.2%) and mixed group 3 cases (9.7%). In histopathological study, 11 cases (35.5%) revealed chronic bronchitis, 9 cases (29.0%) bronchogenic carcinoma, 3 cases (9.7%) chronic granulomatous disease and 2 cases (6.5%) no specific findings. In 6 cases biopsy specimens were too small to be examined histopathologically. 5) In diagnosis by bronchoscopic appearance and by laboratory examination of bronchoscopically removed specimens, 9 cases(29.0%) were primary carcinoma of bronchus, 6 cases (19.4%) chronic bronchitis, 4 cases (12.9%) pneumonia in the order of freguency
Purpose: Mycobacterium abscessus belongs to the group of rapid-growing atypical mycobacterium. The organism is ubiquitous and is found in soil, dust, and water. Although it rarely causes disease in humans, Mycobacterium abscessus has been associated with soft tissue infection. To the best of our knowledge, this is the first case report of facial soft tissue Mycobacterium abscessus infection in a healthy child in Korea. Methods: A 12-year-old girl presented with an erythematous skin lesion with serous discharge on her chin, which had been present for 3 weeks. On her history, she had a laceration wound on her chin at public bath and the lesion was repaired at emergency department immediately. Although conventional soft tissue infecton treatment, her lesion remains unhealed state and had serous discharge for 2 months. Moreover, we found a 1 cm sized nodular mass on her chin. Therefore we performed excision operation and referred the specimen to the laboratory for microbial and histopathologic study. Results: Pathology report confirmed the mass was enlarged lymph node with chronic necrotizing granulomatous inflammation with central microabscess. Non-Tuberculous mycobacterium identification test through tissue specimen resulted Mycobacterium abscessus. We prescribed clarithromycin for three weeks by oral administration as well as performed wound debridement and mass excision via previous wound. This way, her lesion appeared to be complete healing with minimal scarring. There were no evidence of inflammation sign or palpable mass. Conclusion: Although the prevalence is rare, Mycobacterium abscessus infections of soft tissue should be considered even in a healthy child with a lesion caused by trauma or which fails to respond to conventional treatment.
Post-weaning multisystemic wasting syndrome (PMWS) is a new emerging disease affecting nursery and growing pigs in worldwide. Porcine circovirus type 2 (PCV-2) is a most important pathogen associated with PMWS. This study was carried out to investigate the pathological changes in the pancreas of pigs diagnosed as PMWS. To detect the PCV-2 antigen and nucleic acid in the tissue, immunohistochemistry and polymerase chain reaction (PCR) was conducted, respectively. 24 pigs of 4-10 weeks old showed clinical signs of PMWS such as chronic wasting, respiratory distress and diarrhea were examined. Histopathologically, interstitial and periductular mononuclear cells infiltration were observed in pancreas. Multifocal to diffuse necrosis of acinar tissues or necrotizing to granulomatous pancreastitis with numerous syncytial cells infiltration were examined in severe cases. PCV-2 nucleic acid was detected from all tested pancreas using PCR. The PCV-2 antigen in 12 pancreas sections was detected by immunohistochemical staining. PCV-2 has a tropism for vascular endothelial cells and infiltrated macrophages. Although gross lesions are uncommon in the pancreas of pigs with PMWS, histopathological changes and the presence of PCV-2 in this tissue may be related to clinical signs associated with digestive disorders.
Tuberculosis, a chronic infectious granulomatous disease, is presumed to be quite a rare entity. Because of the development of chemotherapy and the improvement of nutritional conditions, incidence of tuberculosis have reduced. For these reasons, tuberculosis may be overlooked in the differential diagnosis whenever dealing with a submandibular swelling. Diagnosis of tuberculous lymphadenitis consists of historical data, physical findings, laboratory tests and histologic examination. The treatment of choice seems to be surgical excision and long term antituberculosis chemotherapy. Surgery provides a rapid tissue diagnosis, because the histological examination of the excisional biopsy is the most reliable diagnostic test. This is the report of a case of tuberculous cervical lymphadenitis on left submandibular area with no evidence of the involvement of lung.

Human alveolar echinococcosis (AE), a hepatic disorder that resembles liver cancer, is a highly aggressive and lethal zoonotic infection caused by the larval stage of the fox tapeworm, Echinococcus multilocularis. E. multilocularis is widely distributed in the northern hemisphere; the disease-endemic area stretches from north America through Europe to central and east Asia, including northern parts of Japan, but it has not been reported in Korea. Herein, we represent a first case of AE in Korea. A 41-year-old woman was found to have a large liver mass on routine medical examination. The excised mass showed multinodular, necrotic, and spongiform appearance with small irregular pseudocystic spaces. Microscopically, the mass was composed of chronic granulomatous inflammation with extensive coagulation necrosis and parasite-like structure, which was revealed as parasitic vesicles and laminated layer delineated by periodic acid-Schiff (PAS) stain. Clinical and histologic features were consistent with AE. After 8 years, a new liver mass and multiple metastatic pulmonary nodules were found and the recurred mass showed similar histologic features to the initial mass. She had never visited endemic areas of AE, and thus the exact infection route is unclear.

Dirofilariasis is a rare disease in humans. We report here a case of a 48-year-old male who was diagnosed with pulmonary dirofilariasis in Korea. On chest radiographs, a coin lesion of 1 cm in diameter was shown. Although it looked like a benign inflammatory nodule, malignancy could not be excluded. So, the nodule was resected by video-assisted thoracic surgery. Pathologically, chronic granulomatous inflammation composed of coagulation necrosis with rim of fibrous tissues and granulations was seen. In the center of the necrotic nodules, a degenerating parasitic organism was found. The parasite had prominent internal cuticular ridges and thick cuticle, a well-developed muscle layer, an intestinal tube, and uterine tubules. The parasite was diagnosed as an immature female worm of Dirofilaria immitis. This is the second reported case of human pulmonary dirofilariasis in Korea.

Background: Mycobacterium avium subspecies paratuberculosis (MAP) causes a chronic and progressive granulomatous enteritis and economic losses in dairy cattle in subclinical stages. Subclinical infection in cattle can be detected using serum MAP antibody enzyme-linked immunosorbent assay (ELISA) and fecal polymerase chain reaction (PCR) tests. Objectives: To investigate the differences in blood parameters, according to the detection of MAP using serum antibody ELISA and fecal PCR tests. Methods: We divided 33 subclinically infected adult cattle into three groups: seronegative and fecal-positive (SNFP, n = 5), seropositive and fecal-negative (SPFN, n = 10), and seropositive and fecal-positive (SPFP, n = 18). Hematological and serum biochemical analyses were performed. Results: Although the cows were clinically healthy without any manifestations, the SNFP and SPFP groups had higher platelet counts, mean platelet volumes, plateletcrit, lactate dehydrogenase levels, lactate levels, and calcium levels but lower mean corpuscular volume concentration than the SPFN group (p < 0.017). The red blood cell count, hematocrit, monocyte count, glucose level, and calprotectin level were different according to the detection method (p < 0.05). The SNFP and SPFP groups had higher red blood cell counts, hematocrit and calprotectin levels, but lower monocyte counts and glucose levels than the SPFN group, although there were no significant differences (p > 0.017). Conclusions: The cows with fecal-positive MAP status had different blood parameters from those with fecal-negative MAP status, although they were subclinically infected. These findings provide new insights into understanding the mechanism of MAP infection in subclinically infected cattle.

Sarcoidosis is a granulomatous disease that can involve any organ, although it primarily involves the lungs, intrathoracic lymph nodes, skin, and eyes. We present a case of sarcoidosis with pancytopenia, resulting from bone marrow involvement. A 35-year-old man was admitted to hospital for chronic cough and blurred vision. On chest computed tomography, there were multiple pulmonary nodules and mediastinal lymph nodes enlargement. As the patient also showed pancytopenia, we performed a bone marrow biopsy, as well as a transbronchial lung biopsy. Both biopsies showed non-caseating granulomas. We diagnosed the patient with sarcoidosis with pulmonary, bone marrow, uvea, liver and spleen involvement. After oral steroid therapy, the patient's symptoms as well as his pancytopenia improved. We present this case to demonstrate the significance of bone marrow biopsy in cases of sarcoidosis with pancytopenia, as well the possibility of clinical improvement with steroid treatment.
