구진(灸津) 약침(藥鍼)이 Carrageenan으로 유도한 관절염에 미치는 영향(影響) (Effect of moxi-tar herbal acupuncture on the carrageenan-induced arthritis in rats)

  • Obiecnves : The purpose of this study was to examine the analgesic effect of moxi-tar on a rat model of carrageenan-induced arthritis and the relations between moxi-tar-induced analgesia and endogenous NO and iNOS, cyclooxygenase-2 (COX-2), and c-Fos protein expression in the spinal cord. Methods : Carrageenan-induced arthritis rat model was used to test the effect of moxi-tar as a chronic pain model. After the induction of arthritis, rats subsequently showed a reduced stepping force of the affected limb for at least the next 4 days. The reduced stepping force of the limb was presumably due to a painful knee, since oral injection of indomethacin produced temporary improvement of weight bearing. Moxi-tar dissolved in ethyl alcohol was injected several acupoints. After the treatment, behavioral tests measuring stepping force were periodically conducted during the next 12 hours. Results : Moxi-tar produced significant improvement of stepping force of the hindlimb affected by the arthritis lasting at least 9 hours. The magnitude of this improvement was equivalent to that obtained after an oral injection of 3 mg/kg of indomethacin and this improvement of stepping force was interpreted as an analgesic effect. Moxi-tar produced the improvement of stepping force of the affected hindlimb in a dose-dependent manner. Both NO production and iNOS, COX-2 protein expression increased by arthritis were suppressed by moxi-tar. moxi-tar on combination with electroacupuncture (EA) produced more powerful and longer lasting improvement of stepping force of the hindlimb affected by the arthritis than either moxi-tar or EA did. Conclusion : The present study suggest that moxi-tar produces a potent analgesic effect on the chronic knee arthritis pain model in the rat and that moxi-tar-induced analgesia modulate endogenous NO through the suppression of iNOS/COX-2 protein expression.

섬유조직염과 관절염 환자의 피로, 통증, 및 통증 대처 (The study on Fatigue, Pain, and Coping of Pain in Fibromyalgia and Arthritis patients)

  • The propose of this study was to identify fatigue, pain and coping of pain and to compare the variables between fibromyalgia and chronic arthritis. The sample consisted of 133 patients who visited H university hospital. Data were collected by questionnaire from May 1 to September 30, 1999. Data was analyzed by descriptive statistics, $x^2$-test, pearson correlation coefficient, and ANOVA. As a results, most of all patients felt fatigue and the mean score of the fatigue was above average. The mean score of rheumatoid arthritis and fibromyalgia patients on pain was higher than Osteoarthritis patients, and there was the statistically significant difference among three groups on pain(F=10.63, p=0.00). There was also the statistical difference among three groups on coping of pain(F=4.74, p=0.01). The mean score of rheumatoid arthritis and fibromyalgia patients on coping of pain was higher than Osteoarthritis patients. Fatigue showed positive relationship with pain(r=.262, p=.002), and pain showed positive relationship with coping of pain(r=.319, p=.000). According to this finding, fibromyalgia patients and rheumatoid arthritis patients felt high fatigue and pain, therefore the development of nursing intervention for relieving fatigue and pain would be needed.

Effects of NOS Inhibitors on Arthritis and Arthritic Pain in Rats

  • Among the arthritis symptoms, chronic pain is the most serious, and it can profoundly affect the quality of human life. Unfortunately, the mechanism of development in arthritis and arthritic pain has not yet been precisely elucidated. Accumulating evidence indicates that nitric oxide (NO) plays a pivotal role in nociceptive processing in the spinal cord. However, the modulation mechanism of NO in the peripheral site of arthritis and arthritic pain has not been clarified. Therefore, I determined in the present study which nitric oxide synthase (NOS) was involved in the induction of arthritis and arthritic pain. Monoarthritis was induced by intra-articular injection of carrageenan (2%, $50{\mu}l$) into rats, and resulted in the reduction of weight load on the injected leg, increase of knee joint diameter and inflammatory response. Pre-treatment of rats with L-N6-(1-iminoethyl)-lysine (L-NIL, $500{\mu}g$, in $50{\mu}l$), an inhibitor of inducible NOS (iNOS), partially prevented the induction of pain-related behavior and partially reduced inflammatory response in the synovial membrane in the knee joint. These results suggest that iNOS in the knee joint may play an important role in the induction of pain-related behavior and inflammation, and that NO produced by iNOS may be associated with nociceptive signaling in the peripheral site.

만성관절염환자의 원인지각과 치료지시이행에 관한 연구 (A Study on the Relationship between Causual Perceptions and Compliance in Patients having Chronic Arthritis)

  • This deductive-survey study was undertaken in order to examine if there were relationship between causal perceptions, expectation for the cure and compliance. The sampling method was a non-probability, purposive sampling technique. The participants of this study was 195 volunteers 1) who have been diagnosed as having chronic arthritis and 2) who were at the rheumatis center of one university hospital in Seoul between September 18th to September 25th 1989. This instruments used for this study were the compliance scale developed by Choi and causal perception scale developed by the researcher. Analysis of data was done using pass analysis, Pearson correlation coefficient. The result of study were as follow : Hypothesis 1 : "It's correlated that causal perception, expectation for cure and compliance" was accepted. (F=4.85, p< .05) Hypothesis 2 : "It's correlated that causal perception, expectation for cure and with depression" was partially accepted. Total age group-worry and anxiety (r=.1580, p<.001) After 40-function of immunity (r=.1731, p<.05) warry and anxiety (r=.1730, p<.001). In the relationship between general characteristics and the variables, age group correlated with compliance and causal perception.

Carrageenan 유도 관절염에 대한 구진(灸津) 약침(藥鍼)의 진통작용 (Analgesic Effects of Moxi-tar Pharmacopuncture on the Carrageenan-induced Arthritic Rats)

  • Objectives: This study was produced to examine the effects of moxibustion that had been played important role to traditional oriental medical treatment on disease. Recently, it was reported that moxi-tar which is generated in the process of moxibustion as burning combustibles decreased nitric oxide(NO) and inducible NO synthase (iNOS) generation in cellular experiments. Methods: Carrageenan-induced arthritis rat model was used to test the effect of moxi-tar as a chronic pain model. Diluted moxi-tar was single injected in several acupoints or combined with electroacupuncture (l ms, 2 Hz, and 2 mA) into contralateral ST36 acupoint for 30 min to assess the synergic effects. After the treatment, behavioral tests measuring stepping force were periodically conducted during the next 12 hours. Endogenous NO and iNOS, cyclooxygenase-2 (COX-2), and c-Fos protein expression in the spinal cord were examined on a rat model of carrageenan-induced arthritis. Results : After the induction of arthritis, rats subsequently showed a reduced stepping force of the affected limb for at least the next 4 days.   • Background: Rheumatoid arthritis is an autoimmune disease characterized by a chronic inflammatory process, primarily involving the synovial membrane of peripheral j oints, where T cell activation is found. To address the superantigen stimulation in rheumatoid arthritis, T cell clonality and the expression of activation markers were analyzed. Methods: To detect TCRB V usage, inverse PCR and sequencing were done. Monoclonal antibodies were used for flow cytometric analysis of TCRBV8 or TCRBV5. As results, a restricted usage of TCRBV3 gene was detected in synovial lymphocytes from one rheumatoid arthritis patient. However, preferential usage for TCRB V8, which may be one indicator for stimulation by staphylococcal superantigen, was not obvious although general activation of T cells was found as high DR+ percentage in synovial T cells. These data show specific antigen rather than superantigen might involve the pathogenesis of rheumatoid arthritis.

관절염 환자를 위한 아로마테라피 (Aromatherapy of Patients with Arthritis)

  • In this paper, we checked health problems of patients who suffered from arthritis. After assessing each patients condition, in order to alleviate their sufferings, we introduced several methods of aromatherapy as a holistic approach to these health problems. Aromatherapy is used of essential oils, which are obtained from aromatic plants, can be defined as a nature-friendly complementary/alternative method, has its great merit in its each way of application. If the arthritis condition is localized, massage,compress or poultices, and hand or foot baths are most effective. Many of essential oils in the recipe have dual benefits, as anti-inflammative and analgesic agents. The Juniper and carrot seed aid in eliminating fluid and toxin accumulation in the joint and surrounding tissue. There are many different courses available and health professional should choose one that is relevant to clinical practice. Considering the future of nursing as a major contribution in the consumer-centered structure of medical health service, aromatherapy as a complementary/alternative method is expected to contribute not only to establishing more effective health service but also to resolving chronic health problems of arthritis.

관절염 환자의 수중운동 지속/중단 요인에 관한 연구 (Influencing Factors on Aquatic Exercise Adherence and Dropout in Patients with Arthritis)

  • The purpose of this study was to identify influencing factors adherence and dropout of aquatic exercise in patients who had been diagnosed chronic arthritis. Subjects were 54 patients with rheumatoid arthritis and osteoarthritis who had participated in the 6-week aquatic exercise program in Taejon metropolitan city. Data was collected from march 25 to April 25, 1999 through face to face and telephone interview. Result are as follows. Fourteen subjects(26.0%) were adherer who was keeping aquatic exercise. The reasons of aquatic exercise adherence were improvements of physical and affectional factors. And the major reasons of dropout was an environment factors(65.8%) and second reasons was physical factors(34.2%). Participants of friends(r=.34, p=.014) was significantly associated with the total duration of an aquatic exercise adherence. For these results, the nurse who care to patients with arthritis have to encourage to maintain aquatic exercise within six month after they begin aquatic exercise.

Effects of the Butanol Extact of Head of Panax Ginseng on Type II Collagen-induced Arthritis in DBA/1J Mice

  • In order to evaluate the improvement effects of head of Panax ginseng on chronic arthritis, we have investigated the activity of butanol fraction (BuOH fraction) in vitro and in vivo system. BuOH fraction showed significant inhibition on the elastase activity. Anti-arthritic activity of BuOH fraction was also examined on type II collagen-induced arthritis in DBA/1J mice. Mice were immunized with injection of type II collagen emulsified in Freund's complete adjuvant, followed by a booster injection 21 days later. BuOH fraction(BHPG) was administered at an oral dose of 500mg/kg for 2 weeks from the 1st day boost. The hind paw edema was significantly decreased in the group of treatment with BuOH fraction compared to control. In collagen-induced DBA/1J mice, BuOH fraction did not affected the collagen antibody titer but significantly inhibited the tumor necrosis factoralpha(TNF-${\alpha}$) activity. These results were confirmed with histological evaluation of joint tissues. This study may raise the possibility that the usage of BuOH fraction of head of Panax ginseng as alternative medicine for the relief and prevention of rheumatoid arthritis symptoms.
