Arthroscopic Treatment of Chronic Calcific Tendinitis of the Shoulder (견관절 만성 석회화 건염의 관절경적 치료)

  • Shoulder is a common site for calcific deposit and is frequently asymptomatic. There is a general agreement that calcific tendinitis should be initially treated nonoperatively and excision reserved for cases unresponsive to the conservative measures. There are several reports that arthroscopic excision of symptomatic calcific deposit is proved to be efficient in the calcific tendinitis refractory to nonoperative management. The results of arthroscopic treatment of chronic resistant calcific tendinitis of the shoulder in eleven patients were evaluated. Each patient had shoulder pain for more than one year prior to the arthroscopic surgery. The average age of the patients was 48 years(range 35-70). Arthroscopic calcium removal and subacromial bursectomy was performed in all patients. Arthroscopic acromioplasty was additionally done in four patients. The results turned out to be good in nine patients with full range of motion and complete pain relief. One patient with full motion and occcasional episodes of pain was satisfactory. One patient with persistent pain was unsatisfactory which converted to satisfactory six months later after subacromial injection. So we conclude that the arthroscopic treatment is a reasonable alternative in treatment of the chronic calcific tendinitis resistant to conservative treatment.

A Clinical Case Study of Guillain-Barré Syndrome with Chronic Progression (만성적 경과관찰을 보이는 Guillain-Barré Syndrome 환자 1례)

  • Objectives: This case study reports on one patient presenting atypical and chronic disorders after being diagnosed with $Guillain-Barr{\acute{e}}$ Syndrome (GBS). Methods: One patient was treated by Yeongseonjetong-eum-gami, Taklisodok-eum-gami, electroacupuncture, and physical therapy. Any improvement in symptoms was assessed by measuring changes in the motor grade of upper limb weakness, the numerical rating scale (NRS) of lower limb numbness, and GBS scores. Results: After 24 days of treatment, the patient's symptoms of GBS showed little improvement. Conclusions: Traditional Korean medical treatment appeared to be effective in reducing GBS symptoms, but more research is required to confirm these results. When treating GBS patients, the clinician should put effort into distinguishing GBS from other nervous disorders, such as acute onset Chronic Inflammatory Demyelinating Polyneuropathy (CIDP).

Arthroscopic Meniscal Repair in a Young Patient with a Chronic Radial Tear of the Incomplete Discoid Lateral Meniscus (젊은 환자의 외측 불완전 원판형 연골판의 만성 방사상 파열의 관절경적 봉합술)

  • The objective of this case report was to evaluate meniscal suturing for a young patient with a chronic radial tear of the incomplete discoid lateral meniscus. The patient underwent saucerization in conjunction with repair of the displaced radial tear of the discoid meniscus. Six-months after surgery, arthroscopic examination showed the repaired meniscus to be healed well with good continuity. Repair of radial tears, even chronic tears, should be considered for young patients with torn discoid lateral menisci.

  • A 9-month-old, intact female mixed dog was referred to Veterinary Medical Teaching Hospital of Gyeongsang National University with symmetrically enlarged and protruded upper jaw. The patient was diagnosed as acute renal failure due to ethylene glycol poisoning and was treated for 1 month in a local animal hospital. In spite of treatment, the patient proceeded to chronic renal failure. Also, the patient's upper jaw begun to enlarge continuously. To evaluate this upper jaw, radiographic examination was performed. Skull radiographs revealed thickening of maxilla, decreased bone opacity, cortical thinning, loss of lamina dura and periodontal space in the maxilla. Diagnosis of rubber jaw syndrome is based on clinicial signs, radiographic findings and laboratory evidence of chronic renal failure due to ethylene glycol poisoning.

  • Purpose: The purpose of this study was to evaluate the prognostic effect of patient compliance with supportive periodontal treatment (PC-SPT). Chronic periodontitis patients were classified based on their compliance level, and factors affecting PC-SPT and the prognosis of PC-SPT were investigated. Methods: This study selected 206 patients who started SPT after receiving periodontal treatment between 2010 and 2012. Patients who continued SPT through February 2016 were included. The patients were classified according to whether they exhibited complete compliance (100% of visits), excellent compliance (${\geq}70%$ of visits), incomplete compliance (<70% of visits), or non-compliance (only 2 visits). Patient characteristics that could affect PC-SPT, such as age, sex, distance of the clinic from their residence, implantation, and periodontal treatment, were investigated. The number of newly decayed and extracted teeth, alveolar bone level changes around the teeth and implants, and implant removal were examined to evaluate the prognosis of PC-SPT. Results: Sex and the presence of an implant significantly affected PC-SPT. Additionally, the number of newly decayed and extracted teeth and changes in alveolar bone levels around the teeth and implants were significant prognostic factors related to PC-SPT. Conclusions: PC-SPT in chronic periodontitis patients will help maintain periodontal health and prevent further periodontal disease.

  • Postural orthostatic tachycardia syndrome (POTS) is characterized by increased heart rate with preserved blood pressure on orthostatic stress. Many patients with postural orthostatic tachycardia syndrome can be misdiagnosed as neurosis, chronic fatigue or anxiety disorder. We report a patient with POTS who presented chronic dizziness and fatigue. In approaching to a patient with orthostatic or nonspecific chronic dizziness, the postural heart rate as well as blood pressure should be checked not to miss the diagnosis.

  • Pulmonary strongyloidiasis is an uncommon presentation of Strongyloides infection, usually seen in immunocompromised hosts. The manifestations are similar to that of acute exacerbation of chronic obstructive pulmonary disease (COPD). Therefore, the diagnosis of pulmonary strongyloidiasis could be challenging in a COPD patient, unless a high index of suspicion is maintained. Here, we present a case of Strongyloides hyperinfection in a COPD patient mimicking acute exacerbation, who was on chronic steroid therapy.

  • Objective : The purpose of this study is to report the effect of Korean medicine on Bell's palsy with Chronic suppurative otitis media. Methods : We used herbal medication, acupuncture, pharmacopuncture and cupping therapy for treatment. To evaluate the results of the treatment, we observed the patient's condition. Results : The patient's objective and subjective symptoms showed noticeable improvement after treatment. Conclusions : We suggest that Bell's palsy with chronic suppurative otitis media can be treated by Korean medicine effectively.

Qi therapy as a complementary therapy in chronic myeloid leukemia

  • We describe the successful treatment of a case of chronic myeloid leukemia with Qi therapy. The patient's disease was managed with conventional medical treatment and Qi therapy as a complementary therapy. Before Qi therapy, 95% of the patient's bone marrow showed evidence of disease. A second bone marrow sample five months after Qi therapy revealed that 38% of the bone marrow was normal; one year after Qi therapy the bone marrow was no longer producing any cancer cells. Although these results were obtained for a single case only and may not constitute conclusive evidence, the data suggest that Qi therapy given as a complementary therapy during conventional medical treatment may have beneficial effects on chronic myeloid leukemia.

A Case Report of Decreased Chronic Coccygodynia from Coccygeal Subluxation after Chuna Manipulation (만성 미골통을 호소하는 미골 후방 아탈구 환자의 추나 치료 치험 1례)

  • Coccygodynia is a common problem that is characterized by pain in the tailbone that radiates to the lower sacral and perineal areas. The purpose of this study is to report the pain decrease after Chuna manipulation on a chronic coccygodynia patient. We diagnosed a patient suffering from chronic coccygodynia as subluxation of Co2 below Co1. And we treated for correcting the subluxation with Chuna manipulation. The effectiveness of the Chuna manipulation was evaluated with visual analogue scale(VAS) score and the angle between Co1 and Co2. It was decreased that both VAS score and angle between Co1 and Co2. The effect was maintained for a period of at least 6 months. We concluded that Chuna manipulation has effectiveness on chronic coccygodynia from coccygeal subluxation.
