치료사에 의해 적용된 경피신경전기자극과 가정에서 시행된 경피신경전기자극이 경부통 환자의 통증과 장애에 미치는 영향 (The Effect of TENS by Physiotherapist versus Home based TENS Intervention to reduce Pain and Improve Disability in Patients with Mechanical Neck Disorder)

  • Purpose: The purpose of this study was to determine the effect of TENS by physiotherapist versus home based TENS intervention to reduce pain and improve disability in patients with mechanical neck disorder. Methods: The subjects of the study were 30 selected patients who had been diagnosed with subacute or chronic neck disorders without neurological damage, during the period of four weeks, three times a week, and thirty minutes for one session, 15 patients received TENS by physiotherapist. 15 patients received home based TENS intervention. The primary outcome was pain intensity measured in using the Visual Analog Scale(VAS). The second outcome was Neck Disability Index to patient's disability. Results: The change in the pain perception degree were statistically significant in both group(p<0.05). TENS by physiotherapist group showed significantly improvement in disability, but, Home based TENS intervention group is not. TENS by physiotherapist group showed significantly greater improvement in pain intensity and patient's disability than the home based TENS intervention group. Conclusion: This study shows that received TENS by physiotherapist was effective in reducing pain, improving disability for mechanical neck disorder patient, physiotherapist' knowledge need to improve patient's pain and disability.

Treatment of Head and Neck Area Pain by Multidisciplinary Approach with Template

  • Purpose: The headache is a symptom that various somatic or non-somatic disorders gives an effect to head and neck system. The neck and the shoulder pain is a common muscle pain that can not control and bothers the patient after chronic state. The headache and the neck and the shoulder muscle pain are treated with various conventional treatment methods. But, there are cases that symptoms did not resolve or increased in some clinical cases. And generally, the result of temporomandibular disorders (TMD) treatment is good. But, despite of a normal treatment was performed for TMD, there are cases that TMD symptoms did not resolved in clinical cases. In template clinic of Soonchunhyang University Bucheon Hospital, co-operative neurophysiologic treatment of Department of Neurosurgery and Dentistry are done for patients, who had head and neck pain or atypical symptoms that did not treated with various conventional treatment method such as surgery or medication etc. Materials and Methods: Four hundred fifty one patients who have treated in the template clinic, Soonchunhyang University Bucheon Hospital, from January of 2006 to December of 2008 were subjected in this study. Result: Overall average age was 31.9 years old. Ratio of numbers is 74.3% in female and 25.7% in male. The success rate of treatment in TMD symptom was 89.9%, in headache was 88.8%, in muscle pain was 81.6%. Statistically significance of differences visual analogue scale evaluation between before and after had been treated patients who have over average grade headache was calculated by paired t-test. P<0.05 was considered significant. Conclusion: We suggest the template appliance can be attempted for cases whose headache, the neck and the shoulder muscle pain and TMD are not resolved with various conventional treatment methods.

  • Purpose : This study aimed to evaluate the effects of sitting questionmark exercise (SQE) and brugger's relief exercise (BRE) on pain, ROM, proprioception, NDI in 60 Patients with Chronic Cervical Pain (CCP). Methods : In this study the VAS and NDI were used to investigate changes in pain and disability with SQE and BRE. The pre and post intervention intra group differences were analysed with a paired t-test for mean values, and the inter group differences were analyzed with an independent t-test for mean values. Results : The pain of both of groups was lowered with statistical significance. Pain of SQE group is lower than that of BRE group with statistical significance. ROM in both groups was improved, but there is no significant difference between two groups. NDI in both of groups were significantly decreased after the intervention, but there was no significant difference between groups. there was NDI ratio (%) significant difference between each groups. Conclusion : SQE intervention may be considered a more effective clinical approach for reducing pain and restoring proprioception in patients with CCP.

Proposal of Applying the Exercise Program for the Prevention of Work-related Chronic Low Back Pain

  • Objective: The purpose of this research is to provide exercise programs for the prevention of work related chronic back pain. Background: In order to prevent musculoskeletal disease, including proper medical care health promotion programs are needed. Method: This is a research of musculoskeletal disease looking at 618 workers working at a car engine manufacturing factory from April to July of 2008. Through questionnaire specific areas of musculoskeletal diseases experienced by the workers were identified and preventative exercise program for chronic low back pain was recommended. Result: Research showed that of the musculoskeletal disease experienced by the workers, 197 presented with low back pain, 171 presented with shoulder pain, 64 presented with neck pain and 44 presented with knee pain. The symptoms of low back pain included stiffness(143), twinge and burning sensation(24) and absence of sensation(19). Using this result 4 types of exercise programs were recommended for prevention of chronic low back pain. Conclusion: Preventative exercise programs recommended for the workers in this research is easily accessible for the workers. Use of the suggested exercise programs will inevitably decrease work related low back pain. Also 2 other recommendations were made: 1) Internal structural change may be necessary using ergonomics. 2) More exercise programs to be used to increase adaptation and tolerance of joints and muscles that are constantly used for repetitive work. Application: This study can be used to provide for the prevention of work-related Chronic Low Back pain.

무통성 결절 양상의 갑상선에 발생한 만성 육아종성 염증 1예 (A Case of Chronic Granulomatous Inflammation of Thyroid Gland Presenting as a Painless Thyroid Nodule)

  • Some clinical diseases, such as granulomatous thyroiditis, tuberculosis, and sarcoidosis can cause granulomatous inflammation in thyroid, and theses have various clinical presentations. Granulomatous thyroiditis is an inflammation of thyroid gland, and may be painful and tender in case of infection, radiation, or trauma. Otherwise, autoimmune conditions, medications, or an idiopathic fibrosis may cause to be a painless thyroididtis. It is self-limited, possibly viral, inflammatory thyroid disorder usually presented with thyroid pain and systemic symptoms. Tuberculosis of the thyroid occurs only rarely and the patient may be asymptomatic. In thyroid sarcoidosis, most common findings are painless, progressive enlargement of the thyroid with normal function. We have experienced a case of chronic granulomatous inflammation of thyroid gland presenting as a painless thyroid nodule and report it with a review of literature.

경락마사지가 만성 통증 노인에게 미치는 효과 (The Effects of Kyongrak Massage in the Elderly with Chronic Pain)

  • Purpose: this study was implemented to develop new nursing intervention, Kyongrak massage, and to examine its effect on chronic pain in elderly women. Method: Kyongrak massage is a manipulation massaging on Kyonghyul site, and Duboo, Kyongchoo, Chucksoo, Hajee, Sangjee Kyongrak massages were used in this study. The subjects(11persons) took Kyongrak massage on head, neck, spinal cord, upper extremities and back lower extremities for 25minutes daily during 5days. The dependent variables were subjective health state, paid, blood pressure, pulse and body temperature. This study was carried out, at 10-12 o'clock, from August 7th, to 22th, 2000. Data were analyzed using frequency, percentage, mean, standard deviation, paired t-test by SPSS PC. Results: subjective health state((t=-3.560, p=.005), body temperature(t=-2.557, p=.029) were increased, and pain level(t=7.884, p=0.000), systolic blood pressure(t=2.923, p=.015) were decreased significantly by Kyongrak massage. Conclusions: The above results have informed us that this Kyongrak massage program(Duboo, Kyongchoo, Chucksoo, Hajee, sangjee) for 25minutes is a useful nursing intervention to decrease chronic pain in each life styles.

이마주름제거술 후 발생한 만성통증의 치료경험 -증례보고- (Wound Infiltration in Patients with Chronic Pain after Forehead Lift Surgery -A case report-)

  • Local infiltration in surgical wounds can reduce postoperative pain with very few side effects. We experienced two cases of chronic pain after forehead lift surgery. A 41-year-old male patient was suffering from a headache at the crown of the head and pain from the posterior neck. A 54-year-old female patient had pain around the forehead, worsened by psychological stress. They underwent forehead lift surgery 10 years and 8 months ago, respectively. Conservative treatments such as medication and physical therapy were not effective. After wound infiltration with 1% lidocaine more than ten times, pain significantly resolved in both patients.

Effects of Myofascial Release and Posture Correction Exercise on the Neck Movement and the Quality of Sleep in Patients with Chronic Tension-Type Headaches

  • Background: Tension-type headaches, which make up the highest proportion of headaches, are prone to develop into chronic tension-type headaches (CTTH). The characteristic of CTTH in patients is that the active myofascial trigger point (ATrP) which causes pain in the muscles of the back of the head is increased, compared to the normal headache and moves the head position forward. Objective: The aim of this study was to investigate the effects of myofascial release (MFR) and posture correction in effectively improving neck function and sleep quality in the symptoms of CTTH patients. Design: Observer-blind study Methods: To reduce ATrP, MFR was applied and exercise was also applied to correct posture. The subjects of this study were 48 individuals randomly divided into three groups; The MFR group using the MFR technique; The MFR with exercise group subject to both the MFR technique and forward head position correction exercises (MFREx), and the control group. MFR and MFREx groups were given the relevant interventions twice a week for four consecutive weeks, and went through the number ATrPs, range of motion (ROM) of neck, Neck Disability Index (NDI) and the Pittsburgh Sleep Quality Index (PSQI) before and after the intervention. A physical therapist, who was fully familiar with the measuring methods of the equipment, was the measurer and not aware of the target's condition was blinded to take measurements only before and after intervention. Results: There was a significant improvement in the ATrP, Neck ROM, NDI and PSQI in the group of patients to whom the MFR technique and MFREx were applied. MFREx was more effective in increasing neck mobility. Conclusions: According to this study, the application of MFR is effective in improving neck movement and sleep quality in chronic tension headache patients.

추나요법을 병행한 한방치료로 호전된 만성 긴장성 두통 치험 1례 (A Case Report on Chronic Tension-type Headache Improved by Korean Medicine with Chuna Manual Therapy)

  • Objectives : The purpose of this study is to evaluate the Korean medical treatment for chronic tension-type headache, especially chuna manual therapy on cervical vertebrae. Methods : One patient was treated with acupuncture, herbal medication and chuna manual therapy on cervical vertebrae. To evaluate the pain of head and neck, visual analog scale(VAS) was measured. Results : After treatment for 1 week, the pain of head and neck was declined from VAS 10 to VAS 3. Conclusions : Korean medical treatment including acupuncture, herbal medication and cervical chuna manual therapy is effective for chronic tension-type headache. But further studies are required to prove the effectiveness of chuna manual therapy on cevical vertebrae.
