요통 환자의 심박변이도 특성에 대한 임상적 연구 (Clinical Study for Characteristics of Heart Rate Variability in Low Back Pain Patients)

  • Objectives : To study autonomic nervous system dysfunction of Low Back Pain(LBP) patients, using spectral analysis of Heart Rate Variability(HRV). Methods : HRV of 190 patients was measured and seperated into two groups, those with LBP(n=95) and healthy controls(n=95). HRV was measured by SA-6000(Medicore, Korea) for 5 minutes after 5 minutes' resting. Results : 1. Mean heart rate(MHRT) of the experimental group was slightly higher than that of the control group, but did not show significant difference(P=0.428). The square root of the mean squared differences of successive normal-to-normal intervals(RMSSD), logarithmic very low frequency power(Ln VLF) and low frequency power/high frequency power ratio(LH/HF ratio) were not significantly low between experimental group and control group(P=0.16, 0.130, 0.537). 2. The standard deviation of all the normal-to-normal intervals(SDNN), logarithmic total power(Ln TP), logarithmic low frequency power(Ln LF) and logarithmic high frequency power(Ln HF) were significantly low between experimental group and control group(P=0.03, 0.005, 0.001, 0.007). 3. Ln LF of acute group was significantly low compared with those of chronic group(P= 0.039). Conclusions : This study suggests the activity and imbalance of autonomic nervous system in LBP is low. Also sympathetic nervous system of acute LBP is lower than that of chronic LBP. Further study of HRV related to LBP is needed in the clinical medicine.

가정 운동 프로그램이 만성요통환자에 미치는 영향 (The Effect of Home Exercise Program on an Chronic Low Back Pain)

  • Background: To study the effect of the home exercise program on pain, flexibility, endurance of extensor in chronic back pain patients, and suggest optimal method for home exercise program. Methods: I divided into two groups who has chronic back pain; one is control group who was given a treatment at the hospital only and the other is experimental group who did another exercise after treatment at the hospital, and there were 10 people in each group. The manual therapy were given to all the patients in each group after applying a stupe and an electric treatment, but the experimental group conducted another exercise program at homes. All the exercise programs were applied to patients 12 times for 4 weeks totally. Result: SPSS for win version 12 was used for statistic analysis and independent t-test was used to find changes between two groups. VAS scale was used to show changes in pain between each group. The grade of pain was decreased between pre&post test to -5.60 in control group and -4.80 in experimental group but there wasn't significant difference between each group. Finger tip-to-floor test was used for the flexibility changes and it was increased between pre&post test in both groups but the change of flexibility between each groups didn't show statistical difference. Biering-Sorensen test was used to measure the endurance of extensor and it was increased between pre&post test in both groups but there wasn't significant difference between each group. Conclusion: As you read the results above, for a chronic low back pain patient, application of the manual therapy showed that it has effect on decrease of low back pain, increase of flexibility and endurance of extensor. However, the effect of home exercise treatment was not sure about improvements for chronic low back pain patient. So I think there should need further study about the effect of home exercise treatment except the treatments at hospital and the thorough education for the exercise of lumbar should be done before the study for the accurate experiment.

턱관절 자세 음양 교정술을 병행한 한방치료가 만성요통에 미치는 영향 (The Effect of Korean Medical Treatment with Postural Yinyang Correction of Temporomandibular Joint on Chronic Low Back Pain)

  • Objectives : The purpose of this study was to examine whether or not Postural Yinyang Correction of TMJ(Temporomandibular joint) using Standard intraoral appliance could decrease chronic low back pain. Methods : We reviewed the medical records of 40 patients with TMJ disorder aged from 30 to 70 years from 124 patients who were treated in chronic low back pain at Department of Acupuncture & Moxibustion of Korean Medicine Hospital of Dong-Eui University from November $1^{st}$, 2015 to June $5^{th}$, 2016. The patients were divided into 2 groups: Group A and Group B. In Group A, we treated the patients with acupuncture, sweet bee venom treatment and FCST(Functional Cerebrospinal Therapy) through Standard intraoral appliance from ABA(Accurate Balancing Appliance), Full body exercise therapy, and Upper cervical alignment balance therapy. In Group B, we treated the patients with acupuncture and sweet bee venom treatment. To estimate the effectiveness in controlling pain, we analyzed the Visual analog scale(VAS). To evaluate functional change of patients, we analyzed the Oswestry low back disability questionnaire(ODI). Results : In both groups, VAS and ODI decreased significantly. In the VAS, the result of Group A is significantly more effective than that of Group B. In the ODI, the result of Group A is statistically less effective compared to that of Group B, but not significantly. Conclusions : According to the results, Postural Yinyang Correction of TMJ using Standard intraoral appliance may be effective treatment for low back pain, but the further researches are needed.

The Effect of Lumbar Medial Branch Block on Low Back Pain

  • Objective : The goal of this study was to establish the benefit and prognostic factors of lumbar medial branch block[MBB] for low back pain. Methods : A retrospective analysis was based on the data obtained from 281 patients with low back pain, who visited our hospital between January 2001 and November 2004. Pain relief was evaluated at 2 weeks, 1 month and 3 months. The authors analyzed the results of MBB according to the patient's age, sex, symptom duration, pathologic condition, and presence of radiating pain. Results : Two hundred eighty one patients had sprain [151]. lumbar fracture [27], spinal stenosis [50], herniated lumbar disc [24] acute post-operative pain [8], and chronic post-operative pain [21] with success rate of 63.6%, 59.3%, 26.0%, 25.0%, 87.5% and 42.9%, respectively. The effects of MBB in sprain, lumbar fracture, and acute post-operative pain were significantly better than those in stenosis, herniated lumbar disc and chronic post-operative pain patients. The patients in young age group [<60 years], with short symptom duration [<6 months] and without radiating pain showed good response to lumbar MBB. Conclusion : The lumbar MBB appears to be safe and effective for low back pain in certain selected patients. Good prognostic factors were low back pain without surgical conditions and radiating pain, with short symptom duration [<6 months], and in relatively young age [<60 years] group.

요통으로 한방병원에 입원한 환자의 병력기간별 호전도에 관한 연구 (The Study on History Period and Treatment Outcomes of Patients Admitted to a Korean Medicine Hospital for Low Back Pain)

  • Objectives Low back pain causes disability and incurs large socioeconomic burden. Many cases of acute low back pain progress into chronic conditions. As the treatment period extends, burden of socioeconomic expenses also rises. Thus this study is to investigate whether history period influences improvement rate of pain and disability in the patients who receive conservative Korean medicine treatment. Methods 447 inpatients who received conservative Korean Medicine treatment for the management of low back pain have been grouped into acute, sub-acute and chronic according to the length of history period. NRS, ODI and improvement rate of each group were investigated. Treatment every patient has received as follow; acupuncture, herbal medicine, Chuna manipulation, bee venom, and physical treatment. Results Acute patients showed improvement with NRS of $3.98{\pm}2.50$ and ODI of $28.04{\pm}21.54$. The NRS of sub-acute patients showed improvement with NRS of $2.81{\pm}2.07$, ODI also showed improvement with $11.64{\pm}13.94$. Statistically significant difference was observed in NRS, ODI and improvement for acute patients. Improvement in pain and disability was observed to be larger. Conclusions It demonstrated that receiving valid conservative Korean medicine treatment is beneficial for low back pain patients in improvement of their pain and disability.

국내 비특이적 만성 요통 환자에게 적용한 비약물적 중재의 효과: 체계적 문헌고찰 및 메타분석 (The Effectiveness of Non-pharmacologic Interventions for Chronic Nonspecific Low Back Pain: A Systematic Review and Meta-analysis)

  • Purpose: The purpose of this study was to evaluate the effectiveness of non-pharmacologic interventions for chronic nonspecific low back pain (CLBP) in adults aged 18-64 years. Methods: We searched for potentially relevant randomized controlled trials and non-randomized controlled trials through five Korean electronic databases (i.e., Korean Studies Information Service System, Research Information Sharing Service, Korean Medical Database, KoreaMed, and National Assembly Library) published from January 2010 to May 2019. Two investigators independently selected the studies based on the criteria and assessed risk of bias in the included studies. We estimated the effect size of interventions using Comprehensive Meta Analysis 3.3. Results: Of 10,151 studies, 26 studies met the inclusion criteria and 15 studies were included in the meta-analysis. Exercise reduced low back pain (Hedges's g=-1.53, 95% CI: -2.22 to -0.85) and pain-related disabilities (Hedges's g=-0.92, 95% CI: -1.40 to -0.45). We found that taping was effective in decreasing low back pain (Hedges's g=-1.12, 95% CI: -1.51 to -0.73) and pain-related disabilities (Hedges's g=-0.50, 95% CI: -0.93 to -0.07). Manual therapy yielded a marginally significant reduction in low back pain (Hedges's g=-2.32, 95% CI: -4.64 to 0.00), the therapy was not effective in decreasing pain-related disabilities. Conclusion: Although there was little evidence for the effectiveness of manual therapy in adults with CLBP, exercise and taping were effective to relieve pain and pain-related disabilities. Based on these findings, we suggest the development of non-pharmacologic interventions or a nursing intervention protocol for the CLBP management. Also, nurses should consider implementation of effective non-pharmacologic interventions for CLBP.

만성요통을 가진 중년 여성의 운동요법에 대한 국내 연구논문 분석 (Analyses of Studies on Exercise Therapy for Middle-Age Women with Chronic Low Back Pain in Korea)

  • 본 연구의 목적은 최근 10년간(2005-2014) 만성 요통을 가진 중년 여성을 대상으로 국내에서 실시된 운동중재 연구의 현황을 파악하고, 운동 중재 연구의 효과를 알아보기 위해 시도된 것이다. 문헌검색은 한국교육학술정보원(http://www.riss.kr), 국회도서관, 한국학술정보(http://kiss.kstudy.com)와 DBpia(http://www.dbpia.co.kr)를 통해 검색하였다. PRISMA(preferred reporting items for systematic reviews and meta-analysis, 2008) 가이드라인에 따라 자료를 수집한 후 연구자가 원본을 확인하고, 선별하여 총 12편의 논문을 최종 분석하였다. 분석한 논문 중 간호학 전공 논문은 1편이었으며, 11편은 타과 논문이었다. Scottish Intercollegiate Guideline Network의 체크리스트를 참고하여 논문의 질적 평가를 실시하였다. 논문의 질적 평가내용을 종합한 결과, 10점 만점에 8점인 연구가 1편이었으며, 5-6점인 연구는 8편 이었고, 3-4점인 연구가 3편이었다. 운동 중재내용을 분석한 결과 평균 55분을 시행하였으며, 1주일에 평균 3.1회의 빈도로 총 시행횟수는 평균 29.7회로 진행되었다. 연구 결과 요부신경근 조절운동, 요가운동, 수중운동이 중년여성의 요통에 효과가 있는 것으로 나타났으며, 향후 이를 간호학 중재로 활용하여 근거기반 간호실무의 토대를 마련할 수 있을 것이다.

요가운동이 만성요통 완화에 미치는 영향 (The Effect of Yoga Exercise on the Relieve of Chronic Low Back Pain)

  • The purpose of this study was to test whether Yoga exercise, one of muscle relaxation, helps to relieve chronic low back paln. Sample were selected from nurses who were worked at K medical center in the period from March 18 to April 23, 1996. The sample size was fifteen. The research design was one-group pretest-posttest design. To test the major research question, this study had the following procedure. The pretest included measuring individuals' degree of pain and discomfort depending on the scope and types of an range of motion, and their pain by their activity of daily living(ADL). The treatment was conducted twice every other week. The reason why this study had twice observations was to control history effect and maturation which treat internal validity in the research design of this study. In this research design, a treatment was to expose Yoga exercise to samples. The exercise was taken in 30 minutes per day for four days in a week (Two consecutive days and twice every other day). The posttest included re-measuring the individuals' the degree of pain and discomfort, and their pain by their ADL. Several hypotheses concerning effect of Yoga exercise was analyzed by the paired t-test, comparing the difference scores between pre and post tests. The results of this study was as follows. The first hypothesis that the post-treatment group taking the Yoga exercise had the pain score lower than the pre-treatment group was supported(t=3.31, p=.005). The second hypothesis that the discomfort score of the post-treatment group had lower than does that of the pre-treatment group was supported(t=2.75, p=.016). The third hypothesis that the post-treatment group had the pain score by ADL lower than does the pre-treatment group was supported(t=5.52, p=.000). In summary, this study examined the effect of a yoga exercise, one of muscle relaxation, on those who were suffered from chronic low back pain. The effect measured by the degree of pain and discomfort with a visual analog scale was statistically significant. The degree of pain according to postures in ADL also showed statistical significance. These findings showed that a yoga exercise was effective to alleviation of chronic low back pain. A pretest-posttest control group design, however, needs to get more accurate results since the design satisfies Internal validity. Consequently, yoga as an exercise cure improves an interaction between muscles and articulations, and performance of ADL for those suffered from chronic low back pain. Furthermore, this positive impact may be an effective method as nursing intervention for their physical, mental, emotional, social, and psychological recovery.

메이트란드 도수치료가 만성 요통환자의 즉각적인 동적 균형능력에 미치는 영향 (Effect of Immediate Dynamic Balance Ability of Chronic Low Back Pain on Maitland's Manual Therapy)

  • 만성 요통 환자들에 대한 다양한 치료적 중재 방법이 있다. 그러나 대부분의 치료들이 통증에 대한 치료들이다. 만성 요통 환자들은 요추관절들의 구조적 고착들을 가지고 있다, 이러한 구조적 고착들은 관절가동범위을 저하시키고 나아가 동적 균형능력의 저하를 발생시킨다. 동적 균형능력의 저하는 손상을 가중시키고 위험상황에서의 대처능력을 감소시킨다. 따라서 본 연구에서는 만성요통환자들에 대한 즉각적인 동적 균형능력의 회복을 위하여 메이트란드의 도수치료의 방법들을 이용하였다. 만성 요통 환자 64명을 대상으로 메이트란드의 도수치료를 적용한 실험군 32명, 이학적 도구(온열팩, 저주파치료기, 초음파)를 사용한 대조군 32명을 대상으로 하였다. 각 군의 치료 전, 후의 동적 균형능력을 비교하기위하여 신뢰도가 높은 스타 익스커션 밸런스 테스트(0.81-0.96)를 이용하였다. 실험군에서 치료 후 동적 균형능력이 전방, 오른쪽, 왼쪽, 후방모두에서 증가하였고 대조군에서는 변화가 없었다. 따라서 메이트란드의 도수치료가 만성요통환자들의 요추 관절을 유연하게 해서 즉각적인 동적 균형능력을 증가시키는데 영향을 주었다고 생각된다.

허리의 분절적 가동기법이 만성 허리통증 환자의 근육 특성과 안정성 한계에 미치는 영향 (The Effect of the Lumbar Segmental Mobilization Technique on Chronic Low Back Pain Patients' the Characteristics of the Muscles, and Limited of Stability)

  • Purpose : The purpose of this study is to examine the effect of the segmental mobilization technique of the lower back on the characteristics of the muscles and limited of stability of chronic backache patients. Methods : The subjects of the study were 30 chronic back pain patients who were divided into groups of 15, a manual therapy group (Group I) and a spinal decompression therapy group (Group II), via random assignation. The subjects had 15 minutes of superficial heat therapy, 15 minutes of interference wave therapy, and 5 minutes of ultrasound therapy for conservative physical therapy. Additionally, manual therapy and spinal decompression therapy were administered to each group for 30 minutes, 5 times a week for 8 weeks. Before intervention, the characteristics of the muscles and limited of stability of the muscles were analyzed. After 8 weeks of intervention, the above items were re-measured in the same manner and analyzed between groups. Results : The results of comparative analysis of the characteristics of the muscles and limited of stability between groups showed that there were statistically significant differences. The manual therapy group (Group I) showed significant differences in characteristics of the muscles compared to the spinal decompression therapy group (Group II). The manual therapy group (Group I) showed significant differences in limited of stability compared to the spinal decompression therapy group (Group II). Conclusion : The result confirmed that manual therapy was more effective in the characteristics of the muscles and limited of stability. Based on this study, additional studies are necessary on the effect of various techniques of manual therapy on muscle activity and muscle thickness in chronic back pain patients. In order to develop an effective manual therapy program, studies using a variety of evaluations are needed.
