The Effects of Education of Chronic Diseases Management for the Elderly Group in Parts of Seoul (서울지역 일부 노인집단에 대한 만성질환관리 교육의 효과)

  • This study was conducted to evaluate the effects of health-behavioral change for the elderly group after community based education of chronic diseases management. We measured self recognition of health status, medication administration of hypertension and diabetes, regular check for blood pressure and blood sugar level, recognition of body indicators (weight, hight, blood pressure, blood sugar etc), knowledge level for chronic diseases management and smoking and alcohol habitation before and after education of chronic diseases management for participants. The subjects of this study consist of 432 people with community-dwelling Seoul citizen being active churches. Education programs designed essential parts of fundamental chronic diseases management, physical exercises for health promotion, diet and nutrition etc. All data collection completed for 5 months from Aug. 2008 to Dec. 2008 by trained surveyors via interview survey. The data obtained were analyzed using descriptive statistics, Wilcoxon Singed Rank test, McNemar test and Paired t-test. The results showed that self recognition of health status, knowledge level for chronic diseases management, recognition of body indicators were statistically significantly increased after the education of chronic diseases management. Also, blood pressure were statistically significantly decreased in elderly with hypertension and blood sugar were statistically significantly decreased in elderly of high-risk group. Based on these results, it was suggested that preventive education policy of chronic diseases management should be considered with priority coming true for successful aging society.

A Panel Study on the Effect of Obesity and the Chronic Diseases on the Health Care Expenditures (비만과 만성질환이 의료비에 미치는 효과에 대한 패널분석)

  • We analyze the determinants of obesity and the chronic diseases using the Korea Health Panel data. Also we analyze the effect of obesity and the chronic diseases on the health care expenditures. Through this study, to reduce the health care expenditures, we suggest the policy implication that might curb the obesity and the chronic diseases. We estimate the determinants of obesity, the chronic diseases, and the health care expenditures using 2SLS (two stage least squares) estimation method under the simultaneous equations framework. Result says that obesity and chronic diseases significantly have positive effects on the health care expenditures. Also the determinants of the health care expenditures that have positive effects are age, income and health care utilization variables.

  • Chronic diseases management can be effectively achieved through early detection, continuous treatment, observation, and self-management, rather than a radar approach where patients are treated only when they visit a medical facility. However, previous studies have not been able to provide integrated chronic disease management services by considering generalized services such as hypertension and diabetes management, and difficult to expand and link to other services using only specific sensors or services. This paper proposes clinical rule flow model based on medical data analysis to provide personalized care for chronic disease management. Also, we implemented that as Rule-based Smart Healthcare System (RSHS). The proposed system executes chronic diseases management rules, manages events and delivers individualized knowledge information by user's request. The proposed system can be expanded into a variety of applications such as diet and exercise service in the future.

The Use of Traditional Korean Medicine and its Affecting Factors among Patients with Chronic Disease in Jeju Province, Korea (만성질환자들의 한의약의료서비스 이용과 결정요인 -제주도 보건소이용 환자를 중심으로-)

  • Background : Traditional Korean medicine(TKM) has gained in popularity among chronic patients in recent years. The use of TKM among patients with chronic diseases is common, with about two thirds of patients using some form of TKM in Korea. Objectives : The purpose of this study is to analyze the use of TKM and determine what factors affect to use TKM among patients with chronic diseases. The study also aims to provide information on TKM therapies and assist therapy selection among various TKM therapies for patients with chronic diseases. Methods : The design of the study was descriptive cross-sectional, and data were collected using a 21-item questionnaire. This study was conducted with subjects with confirmed diagnosis of chronic diseases, who live in Jeju Province, Korea. Results : Among the participants, past or current TKM use was 66.7%, with a statistically significant difference in gender and level of health status groups(p<0.05), but no difference in age, marital status, education, occupation, and income groups. The most common therapies of TKM used by the patients included acupuncture(51.1%), physiotherapy(16.8%), cupping(13.5%), and herbal medicine(4.8%). The main benefits from TKM perceived by the patients were chronic diseases management and health promotion. In a logistic regression analysis, significant influencing factors related to TKM use were gender, family income level, the extent of recognition of efficacy, heath status, and health security program. Conclusions : In this study the socio-demographic and health status, recognition of TKM efficacy factors associated with TKM use among patients with chronic diseases were similar to those found in the general population. The findings suggest that due to the relatively high use of TKM among patients with chronic diseases in Korea, this topic should be taken into account in the development of a holistic approach for patients with chronic diseases and an efficient chronic disease management system. Additionally proactive and consistent management of TKM is necessary in the health care system in Korea.

Indicators for Chronic Disease Management of Older Persons (노년기 건강을 위한 만성질환 관리지표 개발)

  • Objectives: This study was performed to develop the indicators for national surveillance of chronic disease, which is a governmental concern to manage the chronic disease for older persons. It is necessary that chronic disease surveillance system needs to be made in Korea for effective management of chronic diseases. With the system, we know the prevalence and incidence of chronic diseases, observe the trend of utilization for caring the chronic diseases, and analyze the behavior change for prevention of chronic diseases. Methods: This study was carried out by analyzing the data by which the indicators was produced, by reviewing how the United States made the indicators. By benchmarking the United States, the sources of data of the national surveillance indicators for chronic diseases in Korea were compared. Results: In this study, the most significant indicators were identified and proposed to improve the surveillance indicators by changing the sources of data. These findings warrant further development of the health policy for the chronic disease prevention and establishment of the chronic disease surveillance system. Conclusions: The results of this study can be used to develop national surveillance indicators to manage the chronic diseases and can be used as basic data to develop community health programs.

Effects of the Mother-Medical Staff Partnership on Mothers' Condition Management Ability for Children with Chronic Allergic Diseases (만성 알레르기 질환 아동의 어머니-의료진 간 파트너십이 어머니의 아동 상태 관리능력에 미치는 영향)

  • Purpose: This study aimed to identify the effects of the mother-medical staff partnership on mothers' condition management ability for children with chronic allergic diseases. Methods: A total of 109 Korean mothers caring for a child with a chronic allergic diseases, identified according to the allergic march, such as food allergy, atopic dermatitis, allergic rhinitis, and allergic asthma, were recruited from the pediatric department of a general hospital in Seoul through convenience sampling. Data were collected using structured self-reported questionnaires from August 1, 2017 to October 31, 2017. and analyzed by descriptive statistics and multiple regression using SPSS version 22.0. Results: The Mother-medical staff partnership had a statistically significant effect on mothers' condition management ability for children with chronic allergic diseases (p<.05). Among the general characteristics, satisfaction with nursing services had a statistically significant effect on mothers' condition management ability (p<.05). Conclusion: In conclusion, strategies to support children with chronic allergic diseases, as well as their caregivers, should consider the mother-medical staff partnership as part of a family-centered approach.

Concept Analysis of the Self Management in Children with Chronic Disease (만성질환아의 자기관리에 대한 개념 분석)

  • Purpose: The goal of this study was to define and clarify the concept of "self management" of children with chronic diseases. Methods: For analyzing the concept of self management, this study used the hybrid model. This study involved in-depth interviewing nine children with chronic disease. Results: The concept of self management of children with chronic disease can be categorized with three dimensions: internal, environmental, and behavioral aspects. Besides, five attributes and thirteen indicators were analyzed. The concept of self management of children with chronic disease can be expressed as a kind of process; a) re-established yourself (internal dimension), b) assistance by the surrounded (environmental dimension), and c) positive health management behavior with self control of daily life (behavioral dimension). Conclusion: The self management is important for children with chronic disease to control their disease for long time. The attributes and indicators drawn by this study could be used for tool development and useful resources of self management competence in children with chronic disease.

  • Objectives: To provide basic data needed to develop national physical activity policy, this study was examined to identify the relationship among physical inactivity, physical activity, and chronic diseases. Methods: I have reviewed articles and research reports in relation to physical activity, health, and chronic disease published in national and international since 2005 through PubMed, RISS, and KISS. Results: physical activity should be the priority of public health for the prevention and management of chronic diseases as following reasons. 1) Prevalence of physical inactivity has been continuously increased. 2) There were strong evidences that sufficient physical activity could prevent and treat dozens of chronic diseases. Conclusions: Both central and local governments need to make the amount of physical activity to be increased by providing information and establishing a physical activity-friendly environment. Also, the local public health centers and the primary medical institutions are institutionally needed to counsel and prescribe every patient's exercise program at every visit.

  • The number of people with chronic diseases has been increasing steadily but the indicators for the management of chronic diseases have not improved significantly. To improve the existing chronic disease management system, a new policy will be introduced, which includes the establishment of care plans for hypertension and diabetes patients by primary care physicians and the provision of care coordination services based on these plans. Care coordination refers to a series of activities to assist patients and their families and it has been known to be effective in reducing medical costs and avoiding the unnecessary use of the hospital system by individuals. To offer well-coordinated and high-quality care services, it is necessary to develop a service quality assurance plan, track and manage patients, provide patient support, agree on patient referral and transition, and develop an effective information system. Local governance should be established for chronic disease management, and long-term plans and continuous quality improvement are necessary.

Factors Affecting Regular Medical Services Utilization of Chronic Disease Patients - Focusing on the Hypertension, Diabetes Mellitus, Hyperlipidemia - (만성질환자의 정기적 의료이용에 영향을 미치는 요인 - 고혈압, 당뇨병, 고지혈증을 중심으로 -)

  • Objectives: This study aims to identify the factors associated with regular medical services utilization of chronic disease patients. Methods: The research selected 4,489 adults aged over 30, diagnosed with hypertension, diabetes, hyperlipidemia, hypercholesterolemia, from the Korea health panel. We analyzed states of regular medical service utilization using descriptive statistics. Multiple regression analysis was used to examine the main factors associated with regular medical services utilization in chronic disease patients. Results: In terms of socio-demographic factors, gender, age, marital status, education level, employment, household income and disability were significantly different between hypertension, diabetes, hyperlipidemia and hypercholesterolemia. Among health status and behavioral factors, number of chronic diseases, subjective health status, smoking, high risk drinking, regular meals, physical activity, obesity were significantly different. From the multiple logistic regression analysis, age, number of chronic diseases, obesity, type of chronic diseases were associated with regular medical services utilization. Conclusions: It is necessary to develop effective health education programs and individualized approach to improve continuous management in chronic diseases patients.
