CARE OF RECURRENT TEMPOROMANDIBULAR JOINT DISLOCATION IN CEREBROVASCULAR ACCIDENT PATIENT : REPORT OF A CASE (뇌졸중 환자에서 재발성 턱관절 탈구의 관리 : 증례보고)

  • Temporomandibular joint dislocation causes considerable pain, discomfort, and swelling. The anatomic construction of the articular fossa and the eminentia articularis may predispose to dislocation, and weakness of the connective tissue forming the capsule is believed to be a predisposing factor. The capsule may be stretched and, more rarely, torn. Dislocation may be unilateral or bilateral and may occur spontaneously after stretching of the mouth to its extreme open position, such as during a yawn or during a routine dental operation. Manual reduction with the patient under muscle-relaxing condition or anesthesia is recommended method. After the reduction of an acute dislocation, immobilization of the jaw is recommended to allow the stretched and sometimes torn capsule to heal, thus preventing recurrence. A Barton's bandage may be applied for 2 to 3 weeks to prevent the patient from opening the jaw too wide. But, it results in recurrent dislocation in the neurologically disabled patient, because of loose intermaxillary fixation. This is a case report about management of recurrent temporomandibular joint dislocation by multiple loop wirings and intermaxillary elastics in cerebrovascular accident patient.

  • The hazard ratio may be useful as a descriptive measure to compare the hazard experience of a treatment group with that of a control group. In this paper, we propose a kernel estimator of hazard ratio with censored survival data. The uniform consistency and asymptotic normality of the proposed estimator are proved by using counting process approach. In order to assess the performance of the proposed estimator, we compare the kernel estimator with Cox estimator and the generalized rank estimators of hazard ratio in terms of MSE by Monte Carlo simulation.

  • Some mathematical constants have been used in evaluating series involving the Zeta function, the origin of which can be traced back to an over two centries old theorem of Christian Goldbach. We show some of the series involving the Zeta function can be evaluated in terms of the Catalan's constant by using the theory of the double Gamma function.

  • In this paper we consider the problem of whether certain homogeneous or non-homogeneous differential polynomials in f(z) necessarily have infinitely many zeros. Particularly, this extends a result of Gopalakrishna and Bhoosnurmath [3, Theorem 2] for a general differential polynomial of degree $\bar{d}$(P) and lower degree $\underline{d}$(P).

  • 본 논문에서는 실용음악학과를 지원하는 수험생들의 입시준비교육기관인 실용음악학원의 교육과정과 실태에 대하여 기술한다. 또한 영리 목적의 대형 실용음악학원들의 입시 프로그램과 다양한 세부전공 수험생들의 보편적 전략을 비교함으로써 입학 후 실기 곡 위주의 입시교육으로 인해 발생할 수 있는 신입생들의 전공실기 이수능력문제와 대학의 실용음악교육과정 운영상의 문제점을 분석한다. 아울러 일관성 있는 심화교육을 위하여 실용음악학원과 대학 상호간의 교육프로그램 연계와 실용음악학원의 단계별 입시프로그램을 제안하고자 한다.

Patterns of Antimicrobial Resistance and Detection of mecA Gene from Methicillin Resistant Staphylococcus aureus Isolated from Healthcare Facilities and U.S. Military Hospital in Korea

  • A total of 108 strains of MRSA (Methicillin-resistant Staphylococcus aureus) clinical isolates was collected from $121^{st}$ general hospital (U.S. military hospital), Korean healthcare facility from January to March in 2005 and Wonju Christian hospital in 1999. Antimicrobial susceptibility test by Vitek System and MIC test using oxacillin and cephalothin stripes by E-test were executed. PCR based detection of mecA gene was performed on the all of MRSA clinical isolates, too. MRSA clinical isolates were characterized with antimicrobial resistance patterns, PCR based detection of mecA gene and validation of the multiplex PCR strategy of SCCmec among clinical isolates.

  • Penetrating cerebral injuries caused by foreign bodies occur rarely due to the substantial mechanical protection offered by the skull. Throughout most of history, the brain, residing in a "closed box" of bone, has not been vulnerable to external aggression. Recently, we encountered a serious penetrating craniocerebral injury caused by a nail gun. Total excision of the offending nail via emergency craniotomy was performed, but the patient's neurologic status was not improved in spite of aggressive rehabilitative treatment. Here, we report on this troublesome case in light of a review of the relevant literature.

Chronic Hereditary Tyrosinemia Type I with Novel Mutation in FAH Gene (FAH gene novel mutation을 가진 만성형 Hereditary tyrosinemia 1형)

  • A 22-month-old girl who had taken iron supplements due to iron deficiency anemia, presented bloody mucoid stool for one month. She had a bruise at the right periorbital area due to minor trauma and hepatosplenomegaly. Laboratory studies showed anemia, thrombocytopenia, elevated alkaline phosphatase (ALP), hypophosphatemia, decreased haptoglobin, hypocomplementemia, negative direct/indirect Coomb's test, normal vitamin D3 level and high PTHi. Wrist x-ray showed no signs of rickets. The abdominal ultrasound showed only accessory spleen. Tandem mass spectrometry was normal. During follow up, bloody stool regressed after seven days of withdrawal of iron supplement and cow milk, and the total CO2 level had been within 15-20 mEq/L with normal anion gap. NGS (next generation sequencing) panel test for evaluation of renal tubular acidosis showed negative results. After low dose steroid and vitamin D supplements under the impression of hypocomplementemic vasculitis, thrombocytopenia, C3/C4, decreased haptoglobin, and elevated ALP level became normal. At 57 months of age, laboratory findings showed elevated liver enzyme, ALP and gamma-glutamyl transferase again. And liver cirrhosis with splenomegaly and diffuse renal disease were reported with abdomen CT scan. Liver biopsy reported macro- and micronodular cirrhosis. Urine organic acid profile showed elevated succinylacetone level. Whole exome sequencing revealed novel compound heterozygous mutations (NM_00137.2:c.107T>C, NM_00137, 2:c.614T>C) in FAH gene and confirmed by Sanger sequencing. Consequently, the patient was diagnosed as chronic hereditary tyrosinemia type I. She started low phenylalanine/tyrosine diet and nitisinone treatment. Our case had presented symptoms very slowly, which is the first case of chronic tyrosinemia type I in South Korea.

Reye syndrome after acute enteritis during the neonatal period (신생아기 장염에 의하여 발병된 Reye 증후군)

  • Purpose : The prevalence of Reye syndrome has decreased since late 1980's. But we report that recently there were concentrative attacks of Reye syndrome after acute enteritis during the neonatal period. Methods : Clinical symptoms and laboratory results(quantitative organic acid analysis, routine chemistry, arterial blood gas analysis, serum ammonia) of seven patients admitted at the Samsung Medical Center, Sanggye Paik Hospital, Wonju Christian Hospital and Chuncheon Sacred Heart Hospital, referred from Jan. 2005 to Apr. 2005, were analysed retrospectively. The major clinical symptoms were derived from the patients' clinical records sended with urine samples and quantification of organic acids were done with gas chromatography and mass spectrometry. Results : The mean age of seven cases is 18 days and the major preceding symptoms were gastrointestinal symptoms(vomiting, diarrhea, refusal to feeding). The major clinical symptoms were clouded conciousness, repiratory difficulty, vomiting, seizures, and diarrhea. One patient died; that patient's serum ammonia was twenty times higher than normal. Conclusion : The seven patients were neonates. Reye syndrome has been known to be closely related with upper respiratory infections as a preceding disease and to internal use of aspirin, but in our study, the major preceding disease of the seven cases was gastrointestinal infection and none of these used aspirin.
