A unhealthy 58-year-old male patient required extraction of left upper second molar due to advanced periodontitis. Lidocaine contained 1 : 100000 epinephrine for left posterior superior alveolar nerve block was administered in the mucobuccal fold above the second molar to be treated at the local private dental clinic. After four hours of posterior superior alveolar block anesthesia, patient feeled double vision and discomfort of eyeball movement. At next day, he complained difficulty of left eyeball movement, vertigo and diplopia. He was referred to our department via local clinic and department of ophthalomology of our hospital. He was treated by medication and eyeball exercise, and then follow up check. The double vision and medial rectus muscle palsy disappeared patially after 2 months of block anesthesia. We described herein an ocular complication of diplopia and inferior rectus muscle palsy after posterior superior alveolar nerve block for extraction of left upper second molar, and review the cause or origin of this case. The autonomic nervous system is presented as the logical basis for the untoward systems of ophthalmologic sign likely to diplopia and inferior rectus muscle palsy, rather then simple circulation of anesthetic solution in the vascular network.

Objective: To quantify the effects of midline-related landmark identification on midline deviation measurements in posteroanterior (PA) cephalograms using a cascaded convolutional neural network (CNN). Methods: A total of 2,903 PA cephalogram images obtained from 9 university hospitals were divided into training, internal validation, and test sets (n = 2,150, 376, and 377). As the gold standard, 2 orthodontic professors marked the bilateral landmarks, including the frontozygomatic suture point and latero-orbitale (LO), and the midline landmarks, including the crista galli, anterior nasal spine (ANS), upper dental midpoint (UDM), lower dental midpoint (LDM), and menton (Me). For the test, Examiner-1 and Examiner-2 (3-year and 1-year orthodontic residents) and the Cascaded-CNN models marked the landmarks. After point-to-point errors of landmark identification, the successful detection rate (SDR) and distance and direction of the midline landmark deviation from the midsagittal line (ANS-mid, UDM-mid, LDM-mid, and Me-mid) were measured, and statistical analysis was performed. Results: The cascaded-CNN algorithm showed a clinically acceptable level of point-to-point error (1.26 mm vs. 1.57 mm in Examiner-1 and 1.75 mm in Examiner-2). The average SDR within the 2 mm range was 83.2%, with high accuracy at the LO (right, 96.9%; left, 97.1%), and UDM (96.9%). The absolute measurement errors were less than 1 mm for ANS-mid, UDM-mid, and LDM-mid compared with the gold standard. Conclusions: The cascaded-CNN model may be considered an effective tool for the auto-identification of midline landmarks and quantification of midline deviation in PA cephalograms of adult patients, regardless of variations in the image acquisition method.

The purpose of this study is to evaluate the tissue response in applying of various bone substitutes included toothash-plaster mixture, resorbable hydroxylapatite (HA) and demineralized freeze-dried bone and to show the clinical usefulness of toothash-plaster mixture for the repair of craniomaxillofacial bone defect. For this experiment, 100 Sprague-Dawley rats weighing 200gm or more were used. There were four experimental groups: group I, toothash-plaster mixture; group II, demineralized freeze-dried bone; group III, resorbable HA; and group IV, control group. A full thickness, round bone defect measuring 10mm in diameter was created in the midcranium, and the substitutes cited above were embedded in the experimental rats based on their group assignment. Blood clot was filled in the rats assigned to the control group. Experimental rats were sacrificed on the 1st, 3rd, 5th, 8th, 12th and 24th week after implantation and stained with the hematoxylineosin, Masson's Trichrome, using Van Gieson's stain method, and were examined under light microscope. The results were as follows: 1. In all the groups, prominent inflammatory reaction and the infiltration of multinucleated giant cells were noted during the early stage. Gradual healing decreased this reaction. 2. Among the rats in the experimental group II, which were given demineralized freeze-dried bone implants, active formation of new bone traveculae manifested. Chondroid tissues appeared, and it was suggested that the defect was filled with newly formed bone by virtue of osteoinductive activity. On the 12th week after the experiments, most of the defect was filled with newly formed bone trabeculae. 3. In experimental groups I and III, it was noted that HA manifested a healing process similar to that characterized by the toothash-plaster mixture, but inflammatory reaction was more prominent in experimental group I. Active osteoblasts were observed along the periphery of osteoid tissues, while newly formed bone trabeculae appeared adjacent to the implanted materials three weeks later. Formation increased to the extent that newly formed bone trabeculae fused directly with the host bone. Increase in new bone ingrowth into the filling materials was revealed by both experimental groups. 4. In the control group, new bone formation adjacent to the host bone was observed, but most of the defect was filled with mature connective tissue 24 weeks after the experiments.
Purpose: This study attempts to compare the stability of hydroxy-apatite coating implant with that of sandblasted, large-grit and acid-etched surface implant at an early state of installation. Methods: 35 implants were installed in 18 patients, who had visited hospital for implant installation. The early stability at operation, 6 weeks and 12 weeks after operation using Osstell$^{TM}$ mentor (Integration Diagnostics, Savedalen, Sweden) and Periotest$^{(R)}$ (Siemens AG, Benssheim, Germany) were measured, and subsequently analyzed statistically. Results: OsstellTM mentor value of hydroxy-apatite coated implant (HAPTITE) was measured as $70.14{\pm}9.07$ at the stage of installation, $76.98{\pm}5.25$ at 6 weeks and $80.28{\pm}4.23$ at 12 weeks after installation. A statistically significant increase in measurement value was observed after 6 weeks and 12 weeks than when implants were placed. In case of IMPLANTIUM (DENTIUM Co. Ltd., Seoul, Korea), the measurement value was $74.68{\pm}7.42$ at installation, $79.03{\pm}4.39$ at 6 weeks and $80.59{\pm}3.59$ at 12 weeks after installation. In addition, a statistically significant increase in the value was observed when comparative analysis of the value at after installation and 12 weeks after installation was carried out. However, no significant difference between HAPTITE and IMPLANTIUM was observed. The average measurement value of periotest$^{(R)}$ was $-1.94{\pm}3.90$ at installation of HAPTITE, $-4.03{\pm}1.48$ at 6 weeks and $-5.00{\pm}1.71$ at 12 weeks after installation. Moreover, whilst comparing the value at after installation and 12 weeks after installation, statistically significant decrease in the value was observed. In case of IMPLANTIUM, the average measurement value was measured as $-4.25{\pm}1.76$ at installation, $-4.76{\pm}0.97$ at 6 weeks and $-5.18{\pm}0.91$ at 12 weeks after installation and no statistically significant difference was observed. Furthermore, no statistically significant difference was observed between HAPTITE and IMPLANTIUM. Conclusion: In this study, both the implants demonstrated favorable early stability at the time of measurement using Osstell$^{TM}$ mentor and Periotest$^{(R)}$. Moreover, based on the observed results, both HAPTITE and IMPLANTIUM are considered as potent to exhibit clinically stable and prognostic results.
To compare the effect of hyaluronate and dexamethasone on the temporomandibular joint arthrocentesis the author investigated 22 temporomandibular joint disorder(TMD) patients with pain and limitation of mouth opening who visited at the Department of Oral and Maxillofacial Surgery. Chosun Dental Hospital and were made a diagnosis as Wilkes stage III or IV of TMJ internal derangement clinically and radiographically. The two groups consisted of 10 patients with injection of sodium hyaluronate 10mg$(Artz^{(R)})$(hyaluronate group) on the upper joint space of the affected temporomandibular joint 5 times at intervals of a week after arthrocentesis, and 12 patients with injection of dexamethasone$(Oradexon^{(R)})$ at a time(dexamethasone group). Maximum mouth opening, pain value and satisfaction value during mastication were assessed on a visual analog scale before arthrocentesis and after 6 months. Then the within-group and between-group differences were evaluated in the obtained data and the clinical success rate of each group was calculated according to our success criteria. The results were as follows. 1. the mean of maximum mouth opening before arthrocentesis and after 6 months in the hyaluronate group were 24.9mm and 39.0mm respectively, and those before arthrocentesis and after 6 months in the dexamethasone group were 25.7mm and 41.3mm respectively. 2. The mean of pain value on a visual analog scale in the hyaluronate group before arthrocentesis and after 6 months were 6.7 and 1.8 respectively, and those in the dexamethasone group before arthrocentesis and after 6 months were 7.0 and 1.8 respectively. 3. The mean of satisfaction value during mastication on a visual analog scale in the hyaluronate group before arthrocentesis and after 6 months were 2.8 and 7.7 respectively, and those in the dexamethasone group before arthrocentesis and after 6 months were 3.1 and 7.8 respectively. 4. There were statistically significant differences between all measurements before arthrocentesis and after 6 months(P<0.001), but no difference between all measurements in the hyaluronate group and those in the dexamethasone group. 5. The over all success rate of the hyaluronate group and the dexamethasone group were 60.0% and 63.6% respectively. In summary, there was significant difference between the effect of hyaluronate and dexamethasone on the temporomandibular joint arthrocentesis but hyaluronate is better than corticosteroid as the injection drug in consideration of the side effect related with repeated injection.
The aim of this study was to recommend the optimal age for prevention of periodontal disease and to investigate the trend of treatment modality according to different period. From the chart recordings of the patients who had been treated periodontally from Jan. 1981 to Dec. 1995 in the dept .of periodontics, Chosun University Dental Hospital, those of the periodontally treated patients on more than 4 sixtants were selected for the present study. The distribution of the patients was counted according to the age group and the gender. And they were divided into 3 groups(group 1: 1987-1958, group 2: 1985-1990, group 3: 1991-1995) by 5 year interval according to the treated year. The periodontal treatment modalities were classified into non-surgical therapy, pocket elimination surgery, regenerative periodontal surgery, mucoginigival surgery, clinical crown lengthening, and others. The results were as follows; 1.In the distribution of the periodontally treated patients according to the age group, 40's age group was the highest, and 30's, 40's, and 50's age groups occupied more than two thirds(73%). 2.The sexual distribution of the periodontally treated patients showed that males(53.4%) were a little more than females(46.6%). Within 20's group female was higher, but within 40's male was higher. 3.Regardless of the age group and the gender, pocket elimination surgery was the most frequent treatment modality. 4.In group 1 and 2(1987-1990) the main treatment modality was pocket elimination surgery and non-surgical therapy, but in group 3(1991-1995) it was regenerative periodontal surgery and pocket elimination surgery. The above results suggest that the prevention of periodontal disease should be initiated from early twenties, and the most frequent treatment modality may be closely related with development of new material and method.

A 13-year-old, 4.2 kg female poodle was referred for failure of first bilateral mandibular surgery at a local animal hospital after pathologic fracture. Surgery was performed with 2.0-mm miniplates/screws and porcine cancellous bone grafts. In addition, because of the large size of the right segmental defect, a barrier absorbable membrane was employed for guide bone regeneration on right mandible. After surgery, follow-ups performed at 1 day, 1, 4, 8, and 12 weeks; there were no signs of dental malocclusion, nonunion or soft tissue infection. However, a 1-year long-term follow-up showed nonunion in the left mandibular fracture site for which a collagen membrane had not been used. It is considered that use of porcine bone graft with barrier absorbable membrane may be effective for the repair of mandibular nonunion in a geriatric dog.

Objective: To investigate demographic and skeletodental characteristics of one-jaw (1J-OGS) and two-jaw orthognathic surgery (2J-OGS) in patients with skeletal Class III malocclusion. Methods: 750 skeletal Class III patients who underwent OGS at 10 university hospitals in Korea between 2015 and 2019 were investigated; after dividing them into the 1J-OGS (n = 186) and 2J-OGS groups (n = 564), demographic and skeletodental characteristics were statistically analyzed. Results: 2J-OGS was more frequently performed than 1J-OGS (75.2 vs. 24.8%), despite regional differences (capital area vs. provinces, 86.6 vs. 30.7%, p < 0.001). Males outnumbered females, and their mean operation age was older in both groups. Regarding dental patterns, the most frequent maxillary arch length discrepancy (ALD) was crowding in the 1J-OGS group (52.7%, p < 0.001) and spacing in the 2J-OGS group (40.4%, p < 0.001). However, the distribution of skeletal pattern was not significantly different between the two groups (all p > 0.05). The most prevalent skeletal patterns in both groups were hyper-divergent pattern (50.0 and 54.4%, respectively) and left-side chin point deviation (both 49.5%). Maxillary spacing (odds ratio [OR], 3.645; p < 0.001) increased the probability of 2J-OGS, while maxillary crowding (OR, 0.672; p < 0.05) and normo-divergent pattern (OR, 0.615; p < 0.05) decreased the probability of 2J-OGS. Conclusions: In both groups, males outnumbered females, and their mean operation age was older. The most frequent ALD was crowding in the 1J-OGS group, and spacing in the 2J-OGS group, while skeletal characteristics were not significantly different between the two groups.

The aim of this study was conducted to investigate the awareness and practice of personal hygiene and clinic hygiene of infection among physical therapist and to analyze the factors affecting it to provide basic data for the establishment of nosocomial infection management programs and policies in the physical therapist unit. In this study, 320 physical therapists were collected and analyzed. The study tool used a self-administered questionnaire to investigate general characteristics and awareness and practice of nosocomial infections. Responses were determined as 5-Likert scales and data were analysed using percentage, independent t-test, ANOVA. As a result of this study, 17.8% of infectious disease management departments were not found, and 41.6% of physical therapists were not educated about nosocomial infection. In addition, physical therapists with sufficient protective equipment for treatment were very low at 25.3%. Thus, in order to increase awareness and practice of nosocomial infection in the future, it is necessary to provide enough protective equipment for the treatment within the hospital, and it is considered that the nosocomial infection education of the physical therapist should be carried out regularly in the hospital itself.

The purpose of this study was to find a method for improving the children's behavior during dental treatment in relation with dentist's attire. For this study 150 children, visiting a dental clinic, were asked to participate in a survey on the attire of the dentist. The results of the survey were as follows: 1. In the survey for the preference between gown and ordinary clothes according to sex(P<0.05), age(P<0.05), residence (P<0.05) and number of visits(P<0.05), there was more preference for gown(65.3%). 2. The preference for the type of gown and attire under the gown was a long gown and white shirt (30.7%), followed by a suit-like gown (18.7%) and white shirt, and long gown and polo shirt(17.3%). 3. The preference for the color of gown was white(34.7%), followed by pink (18.7%), green(18.0%), blue(15.3%) and yellow(13.3%). 4. The preference for the pattern on the gown was cartoon characters(49.3%), followed by hospital logo(28.7%) and no pattern(14.7%). 5. The preference for ordinary clothes of female dentists was polo shirt(37.3%), followed by striped shirt(28.7%), round T-shirt(18.7%), and everyday Korean traditional dress(15.3%). 6. The preference for ordinary clothes of male dentists was pattern necktie and no pattern Y-shirt(28.0%), followed by no pattern necktie and Y-shirt(21.3%) or pattern shirt with no necktie, and no pattern Y-shirt with no necktie(14.7%). 7. The preference on the protective equipment worn was mask and glove(28.7%), followed by no protective equipment(26.7%), mask, glove and protective glasses(22.7%) at)d only mask(22.0%). Based on the above results, it seems that parting with the traditional white gown and wearing multi-color and pattern attire will provide psychological stability and help improve children's behavior during treatment.

- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
