재가 중증 뇌병변 장애인의 기능상태 및 케어요구 목록 평가 (Analysis of Functional Status and Care Needs among the Community-Dwelling Disabled Persons with Cerebral Impairment)

  • Purpose: Using comprehensive and valid instrument, MDS-HC 2.0, this study aimed to analyze the functional status and to evaluate the care needs of the community-dwelling disabled with cerebral impairment. Method: With a convenient sample of 88 disabled with cerebral impairment, the data were collected at a community health center located in rural area in Choongchung providence in August 2005. Subject's functional status and care needs were evaluated using Minimum Data Set-Home Care version 2.0. Result: Significant proportion of subjects were totally dependent for locomotion-outdoor (26.1%), personal hygiene (24.1%), bathing (24.1%). For IADLs, over 40% of subjects were totally dependent for ordinary house work, managing finances, or shopping. Top five ranked care needs were preventive health care measures (100%), communication disorders (71.6%), visual function (55.7%), health promotion (52.3%), and pressure ulcers (48.9%). The proportion of triggered clinical assessment protocols were significantly higher in disability level I group for the risk of institutionalization (p=<.001), communication disorders (p=.004), cognitive problems (p=.001), pressure ulcers (p=<.001), skin and foot conditions (p=.010), and urinary incontinence and indwelling catheters (p=<.001). Conclusions: It is necessary to provide community based rehabilitation services that are individualized for their service needs thus enhance optimal level of functioning.

학교급식 시설 현황 및 위생교육 실태 분석 - 서울, 경기, 강원, 충청지역을 중심으로 - (Analysis of the School Foodservice Facilities & Sanitary Education (Seoul, Gyeonggi, Kangwon and Choongchung Areas in Korea))

  • The purpose of this study was to analyze the status of school foodservice facilities, utilities and equipment & sanitary education in provinces, This study was carried out from September 2008 until December 2008 and is targeted towards schools' dietitians that work at schools with school foodservice. 1,623 schools participated in this research and Kangwondo, Choongchungdo including the metropolitan area with frequent occurrence of food bone disease were the targets of this research. The regional distribution of this research is as follows; 377 schools in Seoul, 648 schools in Gyeonggido, 160 schools in Kangwondo, 438 schools in Choongchungdo equaling 1,623 schools in total. And out of the total 1,623 schools, 815 (50.2%) were elementary schools and 808 (48.8%) were middle and high schools (p < 0.001). Most of the elementary schools, excluding 4 schools, were self-operated. In the case with middle and high schools, 81.5% (513 schools) were self-operated and 18.4% (295 schools) were contracted. When dealing with the sanitation management of school foodservice in Kangwondo and Seoul, elementary schools were less equipped with equipment and facilities than middle and high schools which proved that they were in need of improvements (p < 0.01). Schools with self-operated foodservice, in particular, were in need of preparation zone improvements. 52.3%~88.0% of stainless equipment such as utensils, spoons/chopsticks, subsidiary food tray, and food trays were sterilized by dryers. Work tables, vegetable slicers, and mincers were chemically sterilized and plastic materials were sterilized by ultraviolet rays. Data from newspapers, Internet and TV are collected beforehand and then an annual hygiene educations plan for employees are put together. Hygiene education for employees are carried out monthly through oral method.

구연산/칼슘/인산염 용액을 이용한 토양 중금속 안정화: 토양 미생물이 미치는 영향 (Stabilization of Heavy Metals using Ca-Citrate-Phosphate Solution: Effect of Soil Microorganisms)

  • 거풍광산 주변 밭의 오염된 중금속을 토양오염공정시험법과 TCLP를 이용하여 분석한 결과 비소, 납, 카드뮴, 구리, 아연과 같은 중금속의 농도가 전국 토양 평균치를 초과하는 것으로 나타났다. 본 연구에서는 중금속 오염된 토양의 안정화를 위해서 구연산칼슘인산염 용액을 이용하는 방안에 대한 기초연구를 실험실 조건과 현장 조건에서 수행하였다. 실험실 조건에서 구연산칼슘인산염 용액을 투여한 실험 결과, 고압멸균처리한 반응조에 비해 토착미생물 군집이 존재하는 반응조에서 용존 인산염의 농도가 현격히 감소하였으며, 중금속 안정화율도 높다는 것을 확인하였다. 현장 실험에서도 구연산칼슘인산염 용액 투여가 용존 인산염 감소와 중금속 안정화 결과를 가져왔다. 이는 미생물의 인산염 사용이 중금속 안정화 향상에 도움이 되었음을 의미한다. 현장 실험에서 미생물군집을 분석한 결과, 구연산칼슘인산염 용액 투여로 다양성이 증대되었으며, Anaerofilum과 Treponema 같은 혐기성 미생물 개체가 우점종으로 발현되는 결과를 얻었다. 이러한 결과들을 살펴볼 때, 구연산칼슘인산염의 투여로 인한 토착미생물 신진대사 활성화를 통해서 토양 내 혐기성 환원 조건을 제공하거나 토양미생물이 인산염을 중금속 안정화에 이용되기 쉽게 도와주는 미지의 기작을 통해서 해당 현장의 중금속 안정화에 기여한 것으로 해석된다.
