Correction of High Fold without Skin Excision (피부 제거 없이 시행한 높은 쌍꺼풀의 교정)

  • Purpose: Double - eyelid operation is one of the most common cosmetic operations among Korean patients. In such operations, however, the complexity of and individual differences in the patients' anatomical structures may cause various complications, such as asymmetry, retraction of the eyelid, and the occurrence of a high fold. A high fold occurs frequently, and its correction is not very simple. Many methods have been developed to correct it, and among these, the operation involving the excision of the skin between the previous double - eyelid line and the new double - eyelid line is usually selected by plastic surgeons. In many cases, however, patients have insufficient eyelid skins for this operation. In this study, the authors introduce an operation procedure for high - fold correction that does not involve skin excision Methods: From June 2005 to June 2009, 246 cases were treated with this procedure. After the incision of the new double - eyelid line, dissection was done between the previous scar tissue and the levator aponeurosis. Then the orbital septum, orbital fat or the retro - orbicularis oculi fat was slid down and sutured with a tarsal plate. Such sutures were repeated at four to five points, including the lateral and medial limbus, to prevent the reattachment of the previous scar and to create a new double - eyelid line at the end of the orbital septum. Results: Most of the high - fold patients were satisfied with the procedure described above. Their previous scar was hidden under the new double - eyelid line after the operation. In the six cases, the scar was visible in the patients who had a very high and deep inner line. As such, scar revision was undertaken three months after the operation. It is known that scar revision is also required after an operation involving skin excision in the case of a very high inner - eyelid line. Conclusion: This method is an appropriative procedure for high - fold correction for patients who have insufficient upper - eyelid skin.

Development of an Equine Antitoxin by Immunizing the Halla Horse with the Receptor-Binding Domain of Botulinum Neurotoxin Type A1

  • Botulinum neurotoxins (BoNTs), produced by Clostridium botulinum, are the most toxic substances known. However, the number of currently approved medical countermeasures for these toxins is very limited. Therefore, studies on therapeutic antitoxins are essential to prepare for toxin-related emergencies. Currently, more than 10,000 Halla horses, a crossbreed between the native Jeju and Thoroughbred horses, are being raised in Jeju Island of Korea. They can be used for equine antitoxin experiments and production of hyperimmune serum against BoNT/A1. Instead of the inactivated BoNT/A1 toxoid, Halla horse was immunized with the receptor-binding domain present in the C-terminus of heavy chain of BoNT/A1 (BoNT/A1-HCR) expressed in Escherichia coli. The anti-BoNT/A1-HCR antibody titer increased rapidly by week 4, and this level was maintained for several weeks after boosting immunization. Notably, $20{\mu}l$ of the week-24 BoNT/A1-HCR(-immunized) equine serum showed an in vitro neutralizing activity of over 8 international units (IU) of a reference equine antitoxin. Furthermore, $20{\mu}l$ of equine serum and $100{\mu}g$ of purified equine $F(ab^{\prime})_2$ showed 100% neutralization of 10,000 $LD_{50}$ in vivo. The results of this study shall contribute towards optimizing antitoxin production for BoNT/A1, which is essential for emergency preparedness and response.

Accelerated Hyperfractionated Radiotherapy for Locally Advanced Uterine Cervix Cancers (국소진행된 자궁경부암에서의 가속과분할 방사선치료)

  • Purpose: To assess the efficacy of the use of accelerated hyperfractionated radiotherapy(AHRT) for locally advanced uterine cervix cancers. Materials and Methods: Between May 2000 and September 2002, 179 patients were identified with FIGO stage IIB, IIIB, and IVA cancers. Of the 179 patients, 45 patients were treated with AHRT(AHRT group) and 134 patients were treated with conventional radiotherapy(CRT group), respectively. Patients undergoing the AHRT regimen received a dose of 30 Gy in 20 fractions(1.5 $Gy{\times}2$ fractions/day) to the whole pelvis. Subsequently, with a midline block, we administered a parametrial boost with a dose of 20 Gy using 2 Gy fractions. Patients also received two courses of low-dose-rate brachytherapy, up to a total dose of 85{\sim}90 Gy to point A. In the CRT group of patients, the total dose to point A was $85{\sim}90$ Gy. The overall treatment duration was a median of 37 and 66 days for patients that received AHRT and CRT, respectively. Statistical analysis was calculated by use of the Kaplan-Meier method, the log-rank test, and Chi-squared test. Results: For patients that received cisplatin-based concurrent chemotherapy and radiotherapy, the local control rate at 5 years was 100% and 79.2% for the AHRT and CRT group of patients, respectively(p=0.028). The 5-year survival rate for patients with a stage IIB bulky tumor was 82.6% and 62.1% for the AHRT group and CRT group, respectively(p=0.040). There was no statistically significant difference for severe late toxicity between the two groups(p=0.561). Conclusion: In this study, we observed that treatment with AHRT with concurrent chemotherapy allows a significant advantage of local control and survival for locally advanced uterine cervix cancers.

  • Purpose : This study was designed to evaluate the perception of cancer in the students of Korea University. Methods : 1,000 students of Korea University were surveyed by questionnaire from March 2001 to September 2002. Nine hundred ninety two complete questionnaires were returned. The data were analyzed by descriptive statistics, and Chi-square analysis using the SPSS program. Results : 701 students (76.1%) replied that cancer is a curable disease. However, the degree of satisfaction for cancer treatment with modem clinical medicine was low (37.5%). Moreover, they thought that there was no difference between the efficacy of treatment by chemotherapy, alternative medicine, or dietary modification. But, the positive evaluation for the effectiveness of chemotherapy in the students who had cancer patients in their families was higher than that in the students without the experience of cancer patients in their families. Six hundred ninety four students (75.3%) replied positively for euthanasia in terminal cancer patients. And they thought that the dignity and the right of the patients were the most important point that should be considered in making the decision of euthanasia. Conclusions : The credibility to modem clinical medicine for cancer treatment was relatively low in the students of Korea University who had no cancer patients in their families. So, further studies and trials will be warranted to evaluate the causes of these results and improve the credibility of modem clinical medicine for cancer treatment in the general population.

  • Hyang-Yack-Ku-Keup-Bang(鄕藥救急方) is our own, medical work written about the middle of the time of Korea Dynasty. I restored and researched this book because it needed to be illuminated about its medico-historic value and then I came to some conclusions as follows. 1. Hyang-Yack-Ku-Keup-Bang was published in Dae-jang-do-kam(大藏都監) of Kanghaw island(江華島) about the middle of Korea Dynasty. Choi Ja-ha(崔自河) republished it on original publication ground in Euiheung(義興) of Kyungsang-Province(慶尙道) in July, Taejong's(太宗) 17th year of Chosen Dynasty (A.D.1417) and this book was published again in Chungcheng Province(忠淸道) in Sejong's(世宗) 9th year(A.D.1427). The book published in Taejong's days was in the possession of books department of Kung-nae-cheng(宮內廳) in Japan and was the oldest medical book of existing ones. 2. Bang-Jung-Hyang-Yack-Mock-Cho-Bu(方中鄕藥目草部) of this book was originally intended to be adjusted in each division with the title of Bang-Jung-Hyang-Yack-Mock(方中鄕藥目). But Herb part(草部) only followed editing progress of Jeung-Lew-Bon-Cho(證類本草), the rest is not divided into each part and is together arranged at the below of Herb part with the title of Bang-Jung-Hyang-Yack-Mock-Cho-Bu. The Korean inscriptions on some drugstuffs in this book are different between Native Name(鄕名) of three volumes of provisions and general-spoken(俗云) of Bang-Jung-Hyang-Yack-Mock-Cho-Bu. In this, it is estimated that the publishing time and editor of tile volume of provisions and Bang-Jung-Hyang-Yack-Mock-Cho-Bu are different. I think Choi Ja-ha compiled this behind three volumes of provisions when he published. 3. This book picked some prescriptions which consisted of obtainable drugs with ease in Korea in the books of Chell-Keum-Yo-Bang(千金要方), Oi-Dae-Bi-Yo(外臺秘要), Tae-Peong-Sung-Hye-Bang(太平聖惠方), Ju-Hu-Bang(?後方), Kyung-Hum-Yang- Bang(經驗良方) Bo-Je-Bon-Sa-Bang(普濟本事方) Bi-Ye-Baik-Yo-Bang(備預百要方) and so on and got together our own prescriptions. On the whole Bi-Ye-Baik-Yo-Bang was a chief referrence book, On this, other books referred to and corrected. 4. In provisions quoted from Hyang-Yack-Jip-Sung-Bang(鄕藥集成方), there are seven provisions; leg-paralysis part, coughing part, headache part, obstetrics part, etc. don't show in this book. This is why Choi Ja-ha published only certain texts on Dae-jang-do-kam edition his own posession. So we can think the existing edition has a little misses compared with original edition. 5. This book recorded only names of drugstuffs in animal drug department like fowls, crab, goldbug, earthworm, etc. and didn't tell us ways of taking those. This is effect of Buddhist culture on medicine. This is efforts to practice 'Don't murder';one of Five Prohibition of Buddhism. 6. Beacause this book was published at the time, when our originative medicine would be set forth. This followed the Chinese ways in Theory, Treatment, Prescription and used 'Hyang Yack' in Medication out of theory of Korean medicine, which was a transitional form. So this is all important material which tell us aspects of development of 'Hyang Yack' the middle of Korea Dynasty.and this is also the beginning of originative, medical works like Dong-Eui-Bo-Kam(東醫寶鑑), Dong-Eui-Su-Bo-Won(東醫壽世保元). 7. There are few contents based on 'Byen-Jeung-Lon-Chi(辨證論治)'in this book. So we can see this book is not for doctors who study medical thoughts but for general public who suffer from diseases resulted from war. Because this book was written for a first-aid treatmeant, this is an index of medical service for the people those days. And this is also an useful datum for first-aid medicine or military medicine in these modern days. 8. Nowadays, parts of learned world of Korean medicine disregard essential theories and want to explain Korean medicine only by the theories or the methods of Western medicine. Moreover they don't adopt Chinese and Japanese theorys & thoughts about Oriental medicine in our own style and just view in there level. What was worse, there is a growing tendency for them to indulge in a trimming policy of scholarship and to take others' ideas. I think these trends to ignore our own medical thoughts involving growth of 'Hyang Yack' in the middle of Korea Dynasty, Dong-Eui-Bo-Kam and Dong-Eui-Su-Se-Bo-Won. So we, as researchers of Korean medicine, must get out of this tendency, and take over brilliant tradition and try to develop originative Korean medicine.

Oral Administration of Gintonin Attenuates Cholinergic Impairments by Scopolamine, Amyloid-β Protein, and Mouse Model of Alzheimer's Disease

  • Gintonin is a novel ginseng-derived lysophosphatidic acid (LPA) receptor ligand. Oral administration of gintonin ameliorates learning and memory dysfunctions in Alzheimer's disease (AD) animal models. The brain cholinergic system plays a key role in cognitive functions. The brains of AD patients show a reduction in acetylcholine concentration caused by cholinergic system impairments. However, little is known about the role of LPA in the cholinergic system. In this study, we used gintonin to investigate the effect of LPA receptor activation on the cholinergic system in vitro and in vivo using wild-type and AD animal models. Gintonin induced $[Ca^{2+}]_i $ transient in cultured mouse hippocampal neural progenitor cells (NPCs). Gintonin-mediated $[Ca^{2+}]_i $ transients were linked to stimulation of acetylcholine release through LPA receptor activation. Oral administration of gintonin-enriched fraction (25, 50, or 100 mg/kg, 3 weeks) significantly attenuated scopolamine-induced memory impairment. Oral administration of gintonin (25 or 50 mg/kg, 1 2 weeks) also significantly attenuated amyloid-${\beta}$ protein ($A{\beta}$)-induced cholinergic dysfunctions, such as decreased acetylcholine concentration, decreased choline acetyltransferase (ChAT) activity and immunoreactivity, and increased acetylcholine esterase (AChE) activity. In a transgenic AD mouse model, long-term oral administration of gintonin (25 or 50 mg/kg, 3 months) also attenuated AD-related cholinergic impairments. In this study, we showed that activation of G protein-coupled LPA receptors by gintonin is coupled to the regulation of cholinergic functions. Furthermore, this study showed that gintonin could be a novel agent for the restoration of cholinergic system damages due to $A{\beta}$ and could be utilized for AD prevention or therapy.

Effect of Foliar Application of GA3 on the Flower Bud Formation and Fruit Quality of Satsuma Mandarine (C. unshiu Marc. cv. Miyagawa) (지베렐린 엽면살포가 '궁천조생' 감귤의 착화와 과실품질에 미치는 영향)

  • BACKGROUND: Control of alternate bearing satsuma mandarin in Jeju is very important to maintain the optimum price of fruit and get the sustainable income of farmers. Unlike orange, Satsuma mandarin is well known to sensitive on alternate bearing. We carried out the experiment to know the effect of foliar application of $GA_3$ on the flowering and fruit quality of satsuma mandarin (C. unshiu Marc. cv. Miyagawa). METHODS AND RESULTS: To experiment, the treatments consist of control, different concentration of $GA_3$ (25, 50 and 100 mg/L), machine oil emulsion 100 times and mixture of various concentration of $GA_3$ (25 and 50 mg/L) with machine oil emulsion 100 times which it was applied on 15 year-old Miyagawa satsuma mandarin at December 29, 2011. Foliar application of $GA_3$ in winter reduced the flowering of satsuma mandarin. Flower-leaf ratio was significantly reduced at 100 mg/L $GA_3$, while no differences observed in low concentration of $GA_3$ (25 and 50 mg/L). However, it was significantly decreased to 0.19 in application of $GA_3$ 25 and 50 mg/L with machine oil emulsion 100 times mixture. Number of leaves per fruit was significantly increased as foliar application of $GA_3$ also it reduced the fruits remarkably. Soluble solid contents and Hunter's a of peel color ratio showed no difference among $GA_3$ single treatments, but it was reduced in $GA_3$ 25 and 50 mg/L with machine oil emulsion 100 times mixtures significantly. From the results, it has been found that higher $GA_3$ concentration can reduce the number of flowers on the alternate bearing of satsuma mandarin. However, it was found that lower concentration of $GA_3$ with machine oil emulsion mixture 100 times can reduce flowering. CONCLUSION(S): The foliar application of $GA_3$ (100 mg/L) can alleviate alternate beraring. Also, mixture of lower concentration of $GA_3$ with machine oil emulsion 100 times can retard flowering more significantly while it needs further confirmation.

Effects of Supplemental Asterias amurensis Extract in the Experimental Diets on Growth, Blood Chemistry and Superoxide Production of Kidney Phagocytes of Sebastes schlegeli (불가사리(Asterias amurensis) 추출물을 첨가한 사료의 급이가 조피볼락(Sebastes schlegeli)의 성장, 혈액성상 및 식세포 활성산소 생산에 미치는 효과)

  • This study was conducted to examine the effects of Asterias amurensis prethanol extract on growth performance, serum traits, and superoxide production of phagocytes in Sebastes schlegeli. The effects of Asterias amurensis extract on growth performance, specific growth rate (SGR), feed concentration ratio (FCR), coefficient of fatness (CF), and survival rate (SR) of fish fed diets containing various concentrations of Asterias amurensis extract were measured. There were no significant differences in SGR, FCR, CF, and SR among the experimental groups. This result was produced because experimental diets were coated to prevent repellent action of fish. To investigate the effects of Asterias amurensis extract on the metabolism, the contents of glucose, glutamic oxaloacetic transaminase (GOT), and total cholesterol in serum were measured. The contents of glucose and total cholesterol in serum increased dose-dependently and serum GOT content showed no significant difference among the experimental groups, suggesting that Asterias amurensis extract was non-toxic material. To confirm the effects of Asterias amurensis extract on the immune system of fish, superoxide production of phagocytes was measured. Asterias amurensis extract caused a dose-dependent increase of superoxide production of phagocytes. When considering these results, Asterias amurensis extract could be utilized as an additive to augment immune function in diets.

Relationship between NVOCs Concentration and Korean Red Pine (Pinus densiflora S. et Z.) Forest Environment in Spring Season (봄철 소나무숲의 NVOC 농도와 숲환경과의 상관관계)

  • This study aimed at a scientific examination of the relationship between NVOCs concentration and the physical environment of red pine forest in spring season. Atmospheric NVOC samples in red pine forest was collected through five trials conducted from March to May 2013, using Tanax-Ta-charged disposable tubes and mini-pumps. At each trial, measurements were taken at three different points in daytime (sunrise, southing and sunset). For maximum accuracy, two tubes were used for each measurement at the same location, and the mean value was used for analysis. Compound analysis on the NVOC samples was done using the HS-SPME method and GC-MS. Analysis of the relationship between NVOC and the physical forest environment found higher concentrations of most substances, including ${\alpha}$-pinene and ${\beta}$-pinene, with higher temperature, dew point, and lower concentrations with higher wind velocity. The findings of this study offer scientific evidence which can inform the creation of 'healing forests' and 'recreational forests' as well as forest environment in general, helping to promote public health and recreational activities.

Gold-Silver Mineralization of the Au-Ag Deposits at Yeongdong District, Chung-cheongbuk-Do (충청북도(忠淸北道) 영동지역(永同地域) 금은광상(金銀鑛床)의 금은광화작용(金銀鑛化作用)에 관한 연구(硏究))

  • Most of the gold (-silver) vein deposits at Yeongdong District are mainly distributed in the precambrian metamorphic rocks. Based on the Ag/Au total production and ore grade ratios, the chemical composition of electrum and the associated sulfides, the gold(-silver) deposits at Yeongdong District may be classified into 4 classes: pyrrhotite - type gold deposits( I), pyrite - type gold deposits (IT A; massive vein), pyrite - type gold deposits (II B; nonmassive vein) and argentite - type gold - silver deposits(III). The chemical study on electrum(including native gold) revealed that Au content (2.8 to 92.4 atomic%) of electrums varies very widely for different classes of deposits. The Au content of electrum associated with pyrrhotite (Class I), ranging from 47.1 to 92.4 atomic% Au, is clearly higher than that associated with pyrite (Classes IIA, IIB and III). In contrast, classes I, II, and III deposits do not show clear differences in Au content of electrum. In general, pyrrhotite - type gold deposits(I) are characterized by features such as simply massive vein morphology, low values in the Ag/Au total production and ore grade ratios, the absence or rarity of silver - bearing minerals except electrum, and distinctively simple mineralogy. Although the geological and mineralogical features and vein morphology of pyrite - type gold deposits(IIA)are very similar to those of pyrrhotite - type gold deposits (I), Class II A deposits reveal significant differences in the associated iron sulfide (i. e. pyrite) with electrum and Au content of electrum. The Ag/Au total production and ore grade ratios from Class II A deposits are relatively slightly higher than those from Class I deposits. Pyrite - type gold deposits(II B) and argentite - type gold - silver deposits (III) have many common features; complex vein morphology, medium to high values in the Ag/Au total production and ore grade ratios and the associated iron sulfide (i. e. pyrite). In contrast to Class II B deposits, Class III deposits have significantly high Ag/Au total production and ore grade ratios. It indicates distinct difference in the abundance of silver minerals (i. e. native silver and argentite). The fluid inclusion analyses and mineralogical data of electrum tarnish method indicate that the gold mineralization of Classes I and II A deposits was deposited at temperatures between $230^{\circ}$ and $370^{\circ}C$, whereas the gold (-silver) mineralization of Classes ITB and ill formed from the temperature range of $150^{\circ}-290^{\circ}C$. Therefore, Classes I and IT A deposits have been formed at higher temperature condition and/or deeper positions than Classes IIB and III.
