Malassezia pachydermatis에 의한 개의 피부염 치료 증례 (A case of dermatitis associated with Malassezia pachydermatis in a dog)

  • A 4-year-old female American cocker spaniel was hospitalized with 6 months prolonged dermal problems in The Veterinary Medical Teaching Hospital of Seoul National University. Initial noticed signs by owner of this dog were rashes and papules on ear part and they were spreaded whole body. On initial physical examinations, papules, pustules, crusts, and erythema were presented on whole body. Diameters of focal scaly and erythematous alopetic lesions were varied ranging 1 to 4cm. Skin scraping for detecting ectoparasites and wood's light examinations for dermatophyte were negative. Any bacteria were not detected but Malassezia pachydermatis was detected on cytological examination and fungal culture. The dog was treated with Itraconazole (5mg/kg, q12h) and Cephalexin (20mg/kg, q12h) orally and then bathed with 2% chlorhexidine shampoo twice per week for 5 weeks. From 12 weeks after starting therapy, all dermal problems were disappeared although some mild pyoderma were once relapsed. Regular chlorhexidine bathing treatment so far has been continued for preventing recurrence.

의치장착자의 구취발생에 관한 연구 (A study of oral malodor of denture wearers)

  • In order to investigate the amount of oral malodor of denture wearers, concentration of volatile sulfur compounds of 20 denture wearers(10 male, 10 female) was measured by Halimeter(RH-17, Interscan, CA, USA) at following four conditions ; condition 1(with denture) : male - $415.0{\pm}90.1$, female - $430.9{\pm}101.3$ condition 2(immediately after denture removal) : male - $291.4{\pm}35.5$, female - $259.8{\pm}20.4$ condition 3(with denture after 30 minutes immersing in chlorhexidine) :male - $210.1{\pm}25.7$, 여자 - $197.4{\pm}22.2$ condition 4(with denture after 24 hours immersing in chlorhexidine) : male - $120.1{\pm}35.2$, 여자 - $109.2{\pm}20.0$ There was no sexual difference, but, value at the condition 1 showed statistically significant difference from the others(p<0.05). It is dentists' responsibility and privilege to understand and diagnose the strong association between mouth and oral malodor and treat it.

Effect of three different irrigation solutions applied by passive ultrasonic irrigation

  • Objectives: This study evaluated the maximum depth and percentage of irrigant penetration into dentinal tubules by passive ultrasonic irrigation (PUI). Materials and Methods: Thirty extracted human teeth were instrumented and divided into three groups. According to final irrigation regimen, 5.25% sodium hypochlorite (Group A, NaOCl), 2% chlorhexidine (Group B, CHX) and saline solution (Group C, control group) were applied with Irrisafe 20 tips (Acteon) and PUI. Irrigant was mixed with 0.1% rhodamine B. Sections at 2 mm, 5 mm, and 8 mm from the apex were examined with confocal laser scanning microscopy (CLSM). The percentage and maximum depth of irrigant penetration were measured. Kruskal-Wallis test and Mann-Whitney test were performed for overall comparison between groups at each level and for pairwise comparison, respectively. Within a group, Wilcoxon test was performed among different levels. p values less than 0.05 were considered significant. Results: In all groups, highest penetration depth and percentage of penetration were observed at the 8 mm level. At 2 mm level, Groups A and B had significantly greater depths and percentages in penetration than Group C (p < 0.05), but there were no significant differences between Groups A and B. At 5 mm level, penetration depths and percentage of penetration was not significantly different among the groups. Conclusions: NaOCl and CHX applied by PUI showed similar depth and percentage of penetration at all evaluated levels.

Central line-associated bloodstream infections in neonates

  • Newborn infants, including premature infants, are high-risk patients susceptible to various microorganisms. Catheter-related bloodstream infections are the most common type of nosocomial infections in this population. Regular education and training of medical staffs are most important as a preventive strategy for central line-associated bloodstream infections (CLABSIs). Bundle approaches and the use of checklists during the insertion and maintenance of central catheters are effective measures to reduce the incidence of CLABSIs. Chlorhexidine, commonly used as a skin disinfectant before catheter insertion and dressing replacement, is not approved for infants <2 months of age, but is usually used in many neonatal intensive care units due to the lack of alternatives. Chlorhexidine-impregnated dressing and bathing, recommended for adults, cannot be applied to newborns. Appropriate replacement intervals for dressing and care sets are similar to those recommended for adults. Umbilical catheters should not be used longer than 5 days for the umbilical arterial catheter and 14 days for the umbilical venous catheter. It is most important to regularly educate, train and give feedback to the medical staffs about the various preventive measures required at each stage from before insertion to removal of the catheter. Continuous efforts are needed to develop effective and safe infection control strategies for neonates and young infants.

치근관 내 편성 혐기성 세균에 대한 서양산 고추냉이 뿌리 추출물의 항균효과 (THE ANTIMICROBIAL EFFECT OF HORSERADISH (ARMORACIA RUSTICANA) ROOT EXTRACTS AGAINST OBLIGATE ANAEROBES IN ROOT CANAL)

  • 치근단 감염 시 물리적인 작용으로 증상이 완화되지 않을 때, 근관 내 세균을 제거하기 위해 항균성을 지닌 소독제나 세척제가 필요하다. 천연항균제에 대한 관심이 증가하면서 여러 연구들에 의해 고추냉이 추출물의 구성 성분 중 하나인 allyl isothiocyanate에 대한 항균성이 밝혀졌다. 이에 본 연구에서는 근관내 감염시 자주 발견되는 세균들 중 편성혐기성 세균인 Fusobacterium nucleatum, Prevotella nigrescence, 그리고 항생제에 내성이 커서 항균 작용실험 시 대조균으로 자주 사용되는 Clostidial perfringens에 대한 서양산 고추냉이 뿌리 추출물의 항균효과를 알아보고, 대표적인 항균제인 클로르헥시딘과 비교하여 다음과 같은 결과를 얻었다. 1. 본 연구에 사용된 3종의 편성 혐기성 세균에 대한 서양산 고추냉이 뿌리 추출물의 최소 억제 농도는 평균 87-470 ppm, 최소 살균 농도는 평균 156-625 ppm으로 F. nucleatum에 대해 가장 강한 항균효과를 나타내었으며, 항균제에 저항성 이 큰 C. perfringens에도 항균효과를 나타내었다. 2. 본 연구에 사용된 3종의 편성 혐기성 세균에 대한 클로르헥시딘의 최소억제 농도는 평균 3.12-6.25 ppm, 최소 살균 농도는 평균 10.94 ppm이었다. 3. 서양산 고추냉이 뿌리 추출물은 87-470 ppm의 농도에서 3.12-6.25 ppm의 클로르헥시딘과 비슷한 세균성장 억제 효과를 가지며 156-625 ppm의 서양산 고추냉이 뿌리 추출물은 10.94 ppm의 클로르헥시딘과 비슷한 살균 효과를 보였다.

  • 서양산 고추냉이(Horseradish, Armoracia rusticana)의 주성분인 allylisothiocyanate(AIT)는 각종 세균에 대한 살균 효과로 주목받고 있다. 이전 연구들을 통해 구강내 다양한 세균 및 진균류에 대한 항균성이 입증되었으나 근단내 병소를 유발 하는 혐기성 세균에 대한 연구는 미진한 실정이다. 본 연구에서는 사람의 근단 병소에서 직접 분리한 E. faecalis와 F. nucleatum에 대한 서양산 고추냉이 뿌리 추출물의 항균활성을 표준균주와 비교해보고 이를 대표적인 항균제인 클로르헥시딘과 비교하여 다음과 같은 결론을 얻었다. 1. 서양산 고추냉이 뿌리 추출물은 E. faecalis 임상 분리균주에 대하여 표준균주와 같거나 약간 높은 농도에서 항균효과를 나타냈고, $625.0\sim1,250.0{\mu}g/ml$의 농도에서 클로르헥시딘($7.8\sim15.6{\mu}g/ml$)과 대등한 항균효과를 가지는 것으로 나타났다. 2. 서양산 고추냉이 뿌리 추출물은 F. nucleatum 임상 분리균주에 대하여 표준균주와 같거나 약간 높은 농도에서 항균효과를 나타냈고, $78.1\sim312.5{\mu}g/ml$의 농도에서 클로르헥시딘($7.8\sim15.6{\mu}g/ml$)과 대등한 항균효과를 가지는 것으로 나타났다. 이번 연구로 표준균주 뿐만 아니라 구강에서 직접 분리한 혐기성 세균에 대한 서양산 고추냉이 뿌리 추출물의 항균성을 확인할 수 있었고, 추후 근관 세척제나 소독제로 서양산 고추냉이 뿌리 추출물을 이용할 수 있는 가능성을 보여주었다.

항진균제를 첨가한 수종의 점막 조정제가 Candida albicans 성장 억제에 미치는 효과 (THE INHIBITING EFFECT OF TISSUE CONDITIONER COMBINED WITH ANTIFUNGAL AGENTS ON THE GROWTH OF CANDIDA ALBICANS)

  • Tissue conditioners have been used for treatment of denture stomatitis caused by wearing of dentures. Early studies pointed out Candide albicans (C. albicans) as main etiologic factor, and antifugal agents were added for control of the species. But there is a little information about broad comparison on the effect of tissue conditioners and antifungal agents added. The purpose of the present study was to compare the inhibiting effect of four tissue conditioners and one temporary soft liner on the growth of C. albicans for treatment of denture stomatitis using gel diffusion method by measuring diameter of the zone of growth inhibition. Three antifungal agents were added to each material for evaluation of the effect of added agents. Finally, observation was made to evaluate the effect of the loss of antifungal elements by aging of the specimen. The results of this study were obtained as follows : 1. Tempo had remarkable antifungal effect showing the zone of growth inhibition as 2.35 mm at 1st day, and was most effective on End: 4th and 7th day from incubation (p<0.05). But Coecomfort, Dura conditioner, Visco-gel, Coe-soft had little antifungal effect from the 1st day of incubation. 2. Nystatin was most effective showing 9.60-12.04 mm of zone of inhibition at the 1st day from incubation. The antifungal properties were reduced to amphotericin B, chlorhexidine and materials without agent (p<0.05), and the effect was diminished by time. 3. As pretreatment with amphotericin B, nystatin, chlorhexidine, Tempo was very effective at the 1st day from incubation showing zone of inhibition as 3.65, 12.04, 4.78 mm with addition of each agent. Dura conditioner had strongest antifungal effect at the next day as 2.86, 5.33, 1.29 mm of zone of inhibition, and yielding results of Coe-comfort, Tempo, Coe-soft was shown at 4th and 7th day from incubation (p<0.05). Taken all together, tissue conditioners have little antifungal effect except Tempo. Formation of the zone of growth inhibition was due to agents amphotericin B, nystatin, chlorhexidine and nystatin was most effective. Conclusively, it is advisable to select material which is effective on the growth of C. albican and consider addition of antifungal agents for treatment of denture stomatitis.

A periodontitis-associated multispecies model of an oral biofilm

  • Purpose: While single-species biofilms have been studied extensively, we know notably little regarding multispecies biofilms and their interactions. The purpose of this study was to develop and evaluate an in vitro multispecies dental biofilm model that aimed to mimic the environment of chronic periodontitis. Methods: Streptococcus gordonii KN1, Fusobacterium nucleatum ATCC23726, Aggregatibacter actinomycetemcomitans ATCC33384, and Porphyromonas gingivalis ATCC33277 were used for this experiment. The biofilms were grown on 12-well plates with a round glass slip (12 mm in diameter) with a supply of fresh medium. Four different single-species biofilms and multispecies biofilms with the four bacterial strains listed above were prepared. The biofilms were examined with a confocal laser scanning microscope (CLSM) and scanning electron microscopy (SEM). The minimum inhibitory concentrations (MIC) for four different planktonic single-species and multispecies bacteria were determined. The MICs of doxycycline and chlorhexidine for four different single-species biofilms and a multispecies biofilm were also determined. Results: The CLSM and SEM examination revealed that the growth pattern of the multispecies biofilm was similar to those of single-species biofilms. However, the multispecies biofilm became thicker than the single-species biofilms, and networks between bacteria were formed. The MICs of doxycycline and chlorhexidine were higher in the biofilm state than in the planktonic bacteria. The MIC of doxycycline for the multispecies biofilm was higher than were those for the single-species biofilms of P. gingivalis, F. nucleatum, or A. actinomycetemcomitans. The MIC of chlorhexidine for the multispecies biofilm was higher than were those for the single-species biofilms of P. gingivalis or F. nucleatum. Conclusions: To mimic the natural dental biofilm, a multispecies biofilm composed of four bacterial species was grown. The 24-hour multispecies biofilm may be useful as a laboratory dental biofilm model system.

  • The purpose of this study is to compare effects of denture cleansers and disinfectants on the color. surface hardness, and surface roughness of reinforced acrylic resin using polyhedral oligosilsesquioxane molecules(POSS resin) to those of common resins. According to manufacturer's instructions, 45 specimens were made from three denture resins(Luciton 199, Paladent 20, POSS resin), and polished. Five denture cleansers(distilled water, glutaraldehyde, alkaline hypochlorites, chlorhexidine, alkaline peroxides) in combination with three denture resins were evaluated before and after immersion for 7 days. Color data in $L^*a^*b^*$ system were measured with a colorimeter. Surface hardness data were measured with a microhardness tester. Surface roughness data were measured with a 3-dimensional surface analyzer. Data were analyzed with two-way ANOVA, one-way ANOVA, and t-test. The results were as follows : 1. All resins(Luciton 199, Paladent 20, POSS resin) showed significant differences in color after immersion in hypochlorites(p<0.05). 2. POSS resin showed significant differences in color compared with Paladent 20 in all denture cleansers, but no statistically significant differences with Luciton 199(p<0.05). 3. Luciton 199 showed significant differences of surface hardness in chlorhexidine, Paladent 20 showed significant differences in glutaraldehyde and chlorhexidine. POSS resin showed a little change of surface in all denture cleanser, but no statistically significant differences(p<0.05). 4. Luciton 199 and Paladent 20 showed significant differences of surface roughness in hypochlorites and glutaraldehyde, and POSS resin showed no statistically significant differences in all denture cleansers(p<0.05).
