하수오(何首烏)와 백하수오(白何首烏)의 기원과 명칭에 대한 연구 (Studies of Name and Herbal Origins of Ha-Soo-Oh)

  • Polygoni multiflori Radix has been used as a tonic medicine. In Korea, Cynanchi wilfordii Radix have been used too. Their names are resembled, but their plant origines are different. Polygoni multiflori Radix is called 何首烏 or 赤何首烏, and Cynanchi wilfordii Radix is 白何首烏 or 白首烏. They are suggested that they had been confused using at the early days in drug history. Polygoni multiflori Radix is enclosed in the pharmacopoeias of Korea, North Korea, Chinese and Japan. The nomina of pharmacopoeias are 赤何首烏 at North Korea and 何首烏 at other countries; Korea, Chinese and Japan. Cynanchi wilfordii Radix is just enclosed in Korea and North Korea. It means that Cynanchi wilfordii Radix has been commonly prescribed in Korea and North Korea than other countries. The nomina of pharmacopoeias are 白首烏 in Korea and 白何首烏 in North Korea. The characteristics of 何首烏 in ancient herbal records are confused of Polygoni multiflori Radix and Cynanchi wilfordii Radix. But Polygoni multiflori Radix is fixed at 何首烏 later. In Korea (south and north) Cynanchi wilfordii Radix has been recorded to using in 東醫寶鑑 that was Korea traditional Medicinal book and wrote at 1613. The 白首烏 is named in chinese about 20 century, but 白何首烏 is in korea about 19 century. In these consequences, prescription of Cynanchi wilfordii Radix in Korea is earlier than Chinese and Japan, and more common consumption too. So the nomen of 白何首烏 is better properly than 白首烏 in Korean Herbal pharmacopoeia.

코로나-19 환자의 후유증 한약 치료에 대한 고찰 (Review on Herbal Medicine Treatment for Late Complications of COVID-19 Patients)

  • Objective: The purpose of this study was to collect and evaluate evidence of herbal medicine treatments for the residual effects of COVID-19. Methods: Nine electronic databases, namely PubMed, Science Direct, CINAHL, CNKI, Wanfang, J-STAGE, CiNii, OASIS, and Science ON, were searched with the following keywords: 'COVID-19'; 'sequelae'; 'Convalescence' for 'P (Population)' and 'Medicine, Chinese Traditional'; 'Korean medicine'; 'Herbal medicine' for 'I (Intervention)'. For the selected studies, various clinical data were extracted including patient symptoms, pattern identification, herbal medicines prescribed, and treatment results. Results: A total of nine clinical studies were identified (two case control trials; one case series; and six case reports). Various residual symptoms had been detected in the recovery stage of COVID-19 with the most commonly used pattern identifications being deficiency in both lung and spleen and in both qi and yin. Any herbal medicine prescriptions were written using these patterns. We found several reports of the positive effect of herbal medicines in that it improved symptoms, lung function, and lung inflammation. No adverse events were reported. Conclusions: The results indicate that treatment with herbal medicines has a possible therapeutic effect on the residual symptoms of COVID-19. Large-scale studies are necessary to confirm the effectiveness of these approaches.

기면증 치료에 대한 중의학 임상연구 동향 (A Review on Clinical Research Trends in the Treatment on Narcolepsy in Traditional Chinese Medicine)

  • Objectives: The purpose of this study was to review the research trends in the treatment on narcolepsy in traditional Chinese medicine. Methods: We searched articles in the China National Knowledge Infrastructure (CNKI) October 2009-September 2019. Keywords were 发作性睡病, 嗜睡病, and 嗜睡症. Results: Among a total of 81 articles, 12 articles were selected. The International Classification of Sleep Disorders was most frequently used as a diagnostic criteria. Feng Chi (GB20) and Baek Hoi (GV20) are the most commonly used acupoints in acupuncture treatment. Glycyrrhizae Radix (甘草), Atractylodis Rhizoma Alba (白朮), and Poria (Hoelen) (茯苓) are the most commonly used preparations in herbal medicine. The effective rate is most commonly used as an outcome measurement. Conclusions: Acupuncture and herbal medicine could be considered to improve the symptoms of narcolepsy. In the future, this study could be primary data for the development of more clinical research on the treatment on narcolepsy in Korean medicine.

수근관 증후군의 최근 6년간의 연구 동향 분석: China National Knowledge Ingrastructure (CNKI) 보고 연구를 중심으로 (A Narrative Review of Carpal Tunnel Syndrome in Traditional Chinese Medicine: Using China National Knowledge Infrastructure Database)

  • Objective The purpose of this study was to investigate clinical studies between 2015 and 2020 on the traditional Chinese medicine treatment for carpal tunnel syndrome (CTS) using the China National Knowledge Infrastructure (CNKI) database. Methods We searched clinical studies on the Chinese medicine treatment published in traditional Chinese medical journals on CTS through the CNKI search. Fifteen published studies between 2015 and 2020 were chosen and examined for the year of publication, sample size, study design type, diagnosis and evaluation of diverse criteria, treatment period, purpose of the study, and classification based on the treatment technique. Results Out of 73 studies, 15 had practiced various Chinese medical treatments, such as acupuncture, moxibustion, manipulation, herbal medicine application therapy, and external herbal treatment. Conclusions Various studies have investigated the Chinese medicine treatment for CTS in China. To envelope the treatment of CTS in Korean medicine, steady and valuable therapies for high-level clinical research are required. In the future, this study could be helpful in the development of the treatment of CTS with Korean medicine.

편두통 변증과 처방에 관한 문헌적 고찰 - 중국 논문 중심으로 - (Literature Review on syndrome differentiation and herbal medicine of Migraine - focusing on chinese journals -)

  • Objective : To investigate the syndrome differentiation's types and herbal medicine of migraine through Chinese journals review Methods : Journal search was performed using the searching engine of China Academic Journal(CAJ) in China National Knowledge Infrastructure(CNKI) from January 2000 to November 2010. Searching key words were "migraine", "chinese traditional medicine" and "syndrome differentiation". We included all kinds type of journals that explained or referred definite syndrome differentiations. The methods of treatment and Herbal medications by syndrome differentiation in contents of finally selected journals were extracted and summarized. Results : Eighteen chinese journals were selected finally. Fifteen kinds of syndrome differentiations about migraine were investigated, which included blood stasis due to qi stagnation (氣滯血瘀) quoted 15 times, middle obstruction of phlegm-dampness (痰濕中阻) 11 times, liver yang transforming into wind (肝陽化風) 10 times, deficiency of qi and blood (氣血虧虛) 6 times, wind-cold invading 風寒侵襲 淸陽鬱遏 4 times, cold invading reverting yin (寒犯厥阴) 4 times, liver-kidney deficiency (肝腎虧虛) 3 times, liver qi depression and qi stagnation (肝鬱氣滯) 2 times, liver depression transforming into fires (肝鬱化火) 2 times, wind-fire of liver-gallbladder (肝膽風火) 3 times, intense stomach fire and heat (胃火熱盛) 2 times, insufficiency of blood deficiency (血虛不榮) 2 times, insufficiency of qi deficiency (氣虛不充) 2 times, insufficiency of kidney qi and sea of marrow deficiency (腎氣不足, 髓海空虛) 2 times, and qi depression due to wind invading (風邪侵襲, 氣鬱不宣) 2 times. Conclusion : We suggests the first choice of oriental treatment for migraine can be considered among syndrome differentiation's types of blood stasis due to qi stagnation, middle obstruction of phlegm-dampness, liver yang transforming into wind, deficiency of qi and blood, and cold invading reverting yin. further systematic study will be needed.

  • The objective of this study was to investigate the diagnosis and treatment of Sjogren's syndrome(SS) such as syndrome differentiation and herbal medicine by reviewing Chinese traditional medicine's journals. The journal search was carried out using China National Knowledge Infrastructure(CNKI) and PubMed from January 2007 to July 2012. Searching key words were the various combination of "Sjogren's syndrome", "Traditional Chinese Medicine", "herbal medicine", and "syndrome differentiation". The final selection of 57 studies were extracted and summarized by two researchers independently. The syndrome differentiation was classified as yin deficiency with fluid depletion, yin deficiency with dryness heat, dryness toxin with yin damage, internal obstruction of static blood, dual deficiency of qi and yang, dampness-heat obstructing, wind with external contraction, liver qi depression, blood deficiency and wind-dryness, dual deficiency of yin and yang, and internal obstruction of phlegm-blood stasis. Liriope platyphylla(麥門冬), Rehmania glutinosa(生地黃), and Scrophularia buergeriana(玄蔘) were primarily prescribed to tonify yin, engender fluid and moisten dryness.

Rapid molecular authentication of three medicinal plant species, Cynanchum wilfordii, Cynanchum auriculatum, and Polygonum multiflorum (Fallopia multiflorum), by the development of RAPD-derived SCAR markers and multiplex-PCR

  • Definitive identification of original plant species is important for standardizing herbal medicine. The herbal medicines Cynanchi Wilfordii Radix (Baekshuoh in Korean and Beishuwu in Chinese) and Polygoni Multiflori Radix (Hashuoh in Korean and Heshuwu in Chinese) are often misidentified in the Korean herbal market due to morphological similarities and similar names. Therefore, we developed a reliable molecular marker for the identification of Cynanchi Wilfordii Radix and Polygoni Multiflori Radix. We used random amplified polymorphic DNA (RAPD) analysis of three plant species, Polygoni multiflorum, Cynanchum wilfordii, and Cynanchum auriculatum, to obtain several species-specific RAPD amplicons. From nucleotide sequences of these RAPD amplicons, we developed six sequence characterized amplification region (SCAR) markers for distinguishing Polygoni Multiflori Radix and Cynanchi Wilfordii Radix. Furthermore, we established SCAR markers for the simultaneous discrimination of the three species within a single reaction by using multiplex-PCR. These SCAR markers can be used for efficient and rapid authentication of these closely related species, and will be useful for preventing the distribution of adulterants.

소아 변비의 한약 치료에 대한 최신 임상연구 동향 - 중국 RCT 연구를 중심으로 - (Recent Trends in Clinical Research of Herbal Medicine Treatment for Pediatric Constipation - Focused on Chinese Randomized Controlled Trials -)

  • Objectives The purpose of this study is to analyze the trends of recent clinical studies on the effect of herbal medicine for pediatric constipation in China, and to evaluate the efficacy and safety of herbal medication treatment for pediatric constipation. Methods The authors searched the clinical studies using China National Knowledge Infrastructure (CNKI) published from January 2016 to June 2020. We analyzed the literature in regards to focusing on the treatment methods and results. Results A total of 29 randomized controlled trials were selected and analyzed. In all the those studies, the total effective rate of herbal medicine treatment group in pediatric constipation was significantly higher than those of the control group. The most commonly used herbs for pediatric constipation were Atractylodis Rhizoma Alba (白朮) followed by Aurantii Immaturus Fructus (枳實), Glycyrrhizae Radix (甘草), Semen Cannabis (火麻仁), Raphani Semen (萊菔子). The recurrence rate of the herbal medicine treatment group was significantly lower than those of control group. In most studies, the adverse events from the herbal medicine treatment group were significantly less than those of the control group. Conclusions Based on the results of the analyzed clinical studies, herbal medicine treatment has been shown to be safe and effective in the treatment of pediatric constipation. Additional clinical studies are needed to solidify these findings.

아토피 피부염의 한약 치료에 대한 무작위대조군연구 중심의 연구 동향 - CNKI를 중심으로 (Research Trends of Randomized Controlled Trials on Herbal Medicine Treatment for Atopic Dermatitis-Focusing on CNKI)

  • The purpose of this study was to analyze the research trends of randomized controlled trials on herbal medicine treatment for atopic dermatitis in China for the last 5 years. We searched for randomized controlled trials with the intervention of herbal medicine for the treatment of atopic dermatitis in the CNKI (China National Knowledge Infrastructure) from January 2014 to December 2018. For the screening of the paper, we used '特应性皮炎' and '异位性皮炎' which mean atopic dermatitis and search was limited to three areas within Medicine & Public Health: Traditional Chinese Medicine, Traditional Chinese Medicinal Herbs, Combination of Traditional Chinese Medicine with Western Medicine. Among the 136 searched studies, we selected a total of 34 studies and analyzed a year of publication, subject characteristics, study design and intervention, prescribed herbal medicine and herbs, pattern identification, evaluation criteria, and outcomes. Longmu decoction (龍牡湯) and Polia Sclerotium (茯笭) was the most frequently prescribed medicine and herb. The most commonly used pattern identification was Blood deficiency and Wind-dryness (血虛風燥), and among them, the most frequently prescribed herb is Rehmanniae Radix (生地黃). In most studies using the total effectiveness and SCORAD index as an outcome measure, the herbal medicine treatment group showed statistically better results than the control group. As a result of the safety assessment, the herbal medicine treatment group was reported having significantly fewer side effects compared to the control group. Hence, it was confirmed that the intervention including herbal medicine had a significant effect on atopic dermatitis. This study would be able to provide the basis of clinical research on atopic dermatitis and applied to the treatment of atopic dermatitis.

화병 진료지침 개발을 위한 한약 임상시험 방법론 연구 (The Systematic Review of Clincal Trial(s) Intervening Traditional Korean Herbal Medicine: Extension of Developing Clinical Guideline for Hwa-byung)

  • Objectives : To address most probable and suitable method for designing clinical trial intervening Traditional Korean Herbal Medicine on hwa-byung. Study Design : A systematic review of research studies of complementary and/or alternative medical(CAM) treatment of depression, and of domestic clinical trials of Traditional Korean Medicine, and of Chinese clinical trials of Traditional Chinese Medicine. Methods : Randomized, controlled trials(RCTs) of treatment of depression intervening herbal medicine were searched through MEDLINE, Cochrane Library, and CNKI databases. Also, domestic RCTs intervening Traditional Korean Herbal Medicine were searched through Korean Traditional Knowledge Portal and Korean studies Information Service System(KISS). Studies were evaluated using Iadad scale and self-designed tool for this study. Results : Thirty four RCT studies( 10 from MEDLINE, 16 from CNKI, 8 domestic studies) of herbal medicine met inclusion criteria. Mean Jadad score of studies published in English was 2.8${\pm}$0.79, in Chinese 1.94${\pm}$0.77, and in Korean 2.75${\pm}$0.71. Twenty one percent of studies included pattern differentiation in their inclusion criteria. Twenty nine percent of studies used combined treatment of herbal and conventional medicine. Among studies on depression, 9% included Complementary Medical assessing tools. Conclusions : There is shortage of domestic clinical trial involving herbal medicine. In China, studies tend to focus on investigating effect of Combined treatment of herbal and conventional medicine on depression. Clinical trial(s) of hwa-byung should provide good internal validity by describing methodology for randomization, double-blinding, and attrition. Also, specific guideline for clinical trial, including Traditional Korean Medical aspects across inclusion criteria, and assessing tools is needed.
