Balcony genioplasty: a novel technique for better esthetic results in patients with deep mentolabial fold

  • Background: To introduce a novel technique for advancement genioplasty helping surgeons to avoid soft tissue difficulties especially in short-faced patients with deep mentolabial fold and everted lower lip. Case presentation: In a trapezius-shaped, osteotomy was performed in the chin region. The mobilized segment was advanced, and the existing gap was grafted using interpositional allograft materials. Each side had been fixated by three-hole plates and two screws. The outcomes revealed no change in lower anterior teeth vitality. The patients did not report any changes of sensation in lower lip and chin either. The measurements indicated no increase in depth of mentolabial fold in patients undergoing this surgical technique. The postoperative evaluation showed a successful esthetic outcome for the patient and the surgeon concurrently. Conclusion: Based on our experience, the authors concluded that the Balcony technique is a simple and reliable procedure for patients with a deep mentolabial fold.

하안면과 경부의 심미성 증진을 위한 지방흡입술을 동반한 Submentoplasty 술식: 증례보고 (Submentoplasty for esthetic improvement of the neck-lower facial region : Two cases report)

  • Purpose : The purpose of this study is to evaluate the clinical availability of submentoplasty for esthethic improvement of the cervico-facial region of patients with obtuse chin-neck angle. Materials and methods : Case 1. We evaluate the changes of submental line length and chin-neck angle of 35-year-old woman with skeletal Class III and mandibular excess with excessive submental fat before and after surgery: Bilateral sagittal split ramus osteotomy(BSSRO) setback(5mm), Mandibular Angle Reduction, Reduction Malarplasty and Submentoplasty. In this case, It was done simultaneously with orthognathic surgery. Case 2. The changes of submental line length and chin-neck angle of 20-year-old man with skeletal class III and maxillary defiency were evaluated before surgery, at first surgery : Lefort I osteotomy(6mm posterior Impaction), BSSRO setback(9mm), Paranasal Augmentation and at second surgery: genioplasty(6mm advanced) with submentoplasty. In this case, submentoplasty and advancement genioplasty were done after 2 months post-operative periods. Results : Case 1. In case of the Skeletal Class III mandibular excess with submental fat deposit, It showed the improvement of submental angle and length of submental line after simultaneous submentoplasty. Submental angle is changed from $177^{\circ}$ (pre-op) to $151^{\circ}$ (post-op) and submental line length is changed from 8mm(pre-op) to 36mm(post-op). Case 2. The improvement of submental angle and length of submental line after delayed submentoplasty was aquired in case of the skeletal class III maxillary defiency. Submental angle is changed from $154^{\circ}$ (pre-op) to $161^{\circ}$ (first surgery) and to $153^{\circ}$ (second surgery) and submental line length is changed from 25mm(pre-op) to 19mm(first surgery) and to 23mm(second surgery). Conclusion : The results suggest that Submentoplasty is useful surgical procedure for esthethic improvement of the cervicofacial region of patients with obtuse chin-neck angle.

골격성 III급 부정교합 환자에서 양악 수술후 연조직 변화에 대한 연구 (SOFT TISSUE CHANGES FOLLOWING BIMAXILLARY SURGERY IN SKELETAL CLASS III MALOCCLUSION PATIENTS)

  • The purpose of this study was to assess the soft tissue changes using twenty skeletal class III malocclusion patients who treated with bimaxillary surgery for the correction of dentofacial deformities. Patients were divided into two groups. One was impaction and advancement of maxilla with mandibular set-back (Group 1), the other was downward and advancement of maxilla with mandibular set-back (Group 2). Preoperative and postoperative one year cephalometric data were analyzed and compared. Results obtained were as follows: 1. The ratio of horizontal changes of soft tissue to hard tissue at Nt to ANS, Ls to UI, Li to LI, sPog to Pog were 1:0.60, 1:0.79, 1:0.47, 1:0.63 in group 1 respectively, and 1:0.59, 1:0.48, 1:0.83, 1:1.09 in group 2 respectively. Soft tissue changes were highly predictable at the upper lip, lower lip, and chin area. 2. The ratio of vertical changes of soft tissue to hard tissue at Nt to ANS, Li to LI were 1:0.72, 1:0.06 in group 1, and others showed no statistically significant difference. 3. The ratio of horizontal changes of Ls to hard tissue movements at LI(h) was 1:-0.82 in group 1 and at UI(h), LI(h) were 1:0.48, 1:0.01 in group 2. These ratios of group 1 were greater than those of group 2. 4. The direction of horizontal change of Li was the same as that of hard tissue change. The ratio of horizontal changes of Li to LI was 1:0.47 in group 1 and others showed no statistically significant difference. 5. The changes of upper lip thickness and length were -1.6mm, -1.4mm in group 1, and -1mm, -2.7mm in group 2. 6. The ratios of thickness of upper lip to ANS, UI, LI were 1:-0.83, 1:-0.37, 1:0.11 in group 1. There was similar trend in group 2, and there were no statistically significant difference. These results suggest that prediction of changes in soft tissue of upper lip, lower lip, and chin were 79%, 47%, and 63% in group 1, and 48%, 83%, and 109% in group 2. There was a tendency to decrease in thickness and increase in length of the upper lip.

구순 및 구개열을 가진 상악 후퇴증 환자의 교정-외과적 치험 1례 (A Case of Orthognathic Surgery in Congenital Alveolar-Palatal cleft patient)

  • 유아기에 구순성형술 및 구개성형술 등에 의한 중 안면부의 성장장애를 동반한 성인환자에서 만기 이차성 골이식술(Late secondary bone graft)을 동반한 Le Fort I osteotomy를 시행한 결과 기능적, 심미적으로 양호한 개선 효과를 얻었다. 1. 증례에서는 골지지가 거의 없는 우측 상악 중절치 및 측절치 부위의 치아를 발거하고, 술전 교정치료로 변위된 치아의 배열과 소실된 공간을 회복한 후 장골이식을 동반한 상악골 전진술을 시행하였다. 2. 파열 변연부위에 골점막 절개를 시행한 후 순측 구개측 봉합 및 비점막을 거상하여 구비강 누공을 폐쇄한 후 장골능에서 얻은 골수-망상골을 이식하였다. 3. 수술후 구비강 폐쇄로 비음이 개선되었고, 술후 8개월된 방사선 사진상 파열부위의 골 재생을 확인한 후 보철치료를 시행하였다. 4. 표준 두부방사선 계측상 상악골의 전진에 의한 측모의 현저한 개선을 관찰할 수 있었다.

  • 본 교실에서는 이전에 구순 및 구개성형술을 받은 2명의 상악골 발육부전 환자에서 각각 상하악 동시 이동술과 상악 단독 이동술을 시행하여 다음과 같은 결과를 얻었다. 1. 술후 상악골은 상방으로 이동한 후 다시 하방으로 이동하였으며 후방으로의 재발은 비교적 작았다. 2. 두 증례 모두 술후 하악골의 전방 재발이 많았다. 3. 순악구개열환자의 외과적 교정술에 있어서 술후 안정성을 높일 방법들이 강구되어야 할것으로 사료된다.

Research on Correlation between Innovative Activities & Development Strategies of New Product and Development Performance in Taiwan's High-tech Companies

  • Due to the advancement of technology and rapidly changing environment of the market, the life cycle of high-tech products is becoming shorter. The enterprise must constantly innovate and select correct development strategy of new product in order to respond to customers' demands for upgrading operational performance of industry. Development of new product is the critical activity for enterprise's survival and growth. This research focuses on the effects of Taiwan high-tech companies' introduction of innovative activity and development strategy of new product on development performance of new product for analysis and exploration. The result findings reveal that: (1) High execution degree of innovative activity has positive effect on the implementation of development strategy of new product; (2) The companies with better implementation of development strategy of new product reveal better development performance; (3) The companies with higher degree of execution of innovative activity and better execution of development strategy of new product reveal better development performance of new product.

부족교정된 일측성 관상봉합 조기유합증 환자의 수술 교정예 (A Case of Surgical Correction of Undercorrected Unicoronal Synostosis)

  • Purpose: Unicoronal synostosis is the craniofacial anomaly caused by premature fusion of unilateral coronal suture. Ipsilateral flattening of the frontal and parietal bones, temporal retrusion with elevation and recession of the supraorbital rim are main clinical features. Compensatory contralateral frontal bossing and deviation of the nasal root and/or chin can also occur. There is a controversy about techniques for surgical correction, however, bilateral approach technique is more effective for correction of deformity. Methods: A 4-year-old patient with unicoronal synostosis had undergone unilateral suturectomy at 28-month-old but fronto-facial deformity had remained and aggravated as she grew older. She had both fronto-facial and endocranial asymmetry. We performed coronal cranial approach and fully exposed affected cranium including supraorbital rim. Anterior 2/3 calvarial reconstruction with bilateral frontal bone osteotomy and fronto-orbital bandeau advancement was performed. Results: Fronto-facial symmetry including fronto-orbital contour, nasal devation was improved. Endocranial twisting was also improved from $158^{\circ}$ to $162^{\circ}$ in CSO(crista gallisella turcica-opisthion) degree. There was no postoperative complications and no need for revision, and facial asymmetry improved at the period of 2 years of follow-up. Conclusion: Bilateral approach with fronto-orbital bandeau remodeling in surgery of unicoronal synostosis looked superior to unilateral approach in achieving better symmetry and preventing recurrence of asymmetry. Remodeling surgery should be tried in patients even at an older age to correct fronto-facial asymmetry.

  • Facial feminization surgery (FFS) incorporates aesthetic and craniofacial surgical principles and techniques to feminize masculine facial features and facilitate gender transitioning. A detailed understanding of the defining male and female facial characteristics is essential for success. In this first part of a two-part series, we discuss key aspects of the general preoperative consultation that should be considered when evaluating the prospective facial feminization patient. Assessment of the forehead, orbits, hairline, eyebrows, eyes, and nose and the associated procedures, including scalp advancement, supraorbital rim reduction, setback of the anterior table of the frontal sinus, rhinoplasty, and soft tissue modifications of the upper and midface are discussed. In the second part of this series, bony manipulation of the midface, mandible, and chin, as well as soft tissue modification of the nasolabial complex and chondrolaryngoplasty are discussed. Finally, a review of the literature on patient-reported outcomes in this population following FFS is provided.

3D프린팅을 활용한 주얼리 디자인 연구 - 곡선 형태를 중심으로 (A Study on Jewelry Design Using 3D-Printing - Focusing on Curved Form)

  • 본 논문은 산업전반에 빠르게 도입되어가고 있는 3D프린팅 기술을 이용하여 주얼리 디자인에 적용하기 위해 시작되었다. 선행연구를 살펴본 결과 디자인 분야에서는 3D출력 방식중 FMD방식이 가장 많이 사용되고 있었다. 하지만 주얼리 디자인에서는 3D프린팅은 제작과정 중 주조 과정에 사용되고 있어 출력방식이 FMD가 아닌 DLP방식을 사용하고 있다. 따라서 연구자는 DLP방식의 3D프린팅을 활용하여 주얼리 디자인에 응용함으로서 주얼리 디자인의 재료적 기능과, 활용 가능성에 대해 연구 작품을 통해 알아보았다. 그 결과 3D프린팅 출력 후 주조과정 없이 주얼리 디자인에 응용하고 에나멜, 정은 등을 함께 활용하여 브로치를 완성함으로서 가볍고 단단하며 다양한 색상의 완성품을 제작하여 적용 가능한 재료임을 알 수 있었다. 또한 기하학 구조가 아닌 주얼리 디자인에서 주로 사용되는 곡선적 디자인을 Rhino CAD로 크기와 형태를 정확히 예측할 수 있어 향후 주얼리 디자인 조형 형태 전개에 쉽게 활용할 수 있는 다양한 발전 가능성을 엿 볼 수 있었다.

자연스러운 목턱각 성형을 위한 안면부 조직확장술 (The facial tissue expansion to achieve the natural cervicomental angle)

  • Purposes: Wide scars occurring on the lower face and neck are a source of both functional and esthetic problems. Consequently, we can use skin grafts, pedicled flaps, free flaps, and tissue expansion for the reconstruction of this area. Compared with other reconstruction techniques, tissue expansion is advantageous in that it enables the maintenance of a color and texture similar to that of the adjacent tissue. However, the conventional method of tissue expansion has been reported to lead to an unnatural cervicomental angle and to the deformity of adjacent structures. We have therefore made efforts to prevent these problems through the use of several operative procedures. Methods: Forty-one patients with lower facial and cervical scars underwent tissue expansion. The tissue expansion was performed using a rectangular-shaped Nagosil$^{(R)}$ tissue expansion device. On insertion of the tissue expander, the intermediate area of superficial fat layer was dissected and then the tissue expander was inserted to make a flap that was as thin as possible. In advancement of the flap, a capsule-formed by the tissue expander-was used for the interrupted fixed suture of the flap to the fascia of the platysma muscle of the neck. This procedure was performed multiple times and also performed between the flap and the periosteum of the mandible, such that the tension was removed during the suture of the flap margin. Finally, the patients were fitted with a Jobst$^{(R)}$ facial garment in order to stabilize the operation site at least twelve months. Results: The most prevalent location of the scar was the cheek (15 cases), followed by the chin in 14 cases and the neck in 12 cases. The mean size of scar was $55.7{\pm}39.4cm^2$. Conclusions: Using our procedures, we have experienced no significant deformities and have also achieved a more natural cervicomental angle in the patients.
