ASSESSMENT OF THE ANATOMIC VARIATION OF MANDIBULAR INCISIVE CANAL IN CHIN BONE HARVESTING (하악정중부에서 자가골 채취시 절치관의 해부학적인 변이에 대한 평가)

  • The interforaminal region is usually considered as a safe region in the chin bone graft without important vital anatomical structures to be damaged. But the accurate anatomy of the interforaminal region, with its potential clinical relationships, is controversial. Moreover some complications suggesting damage of incisive terminal branches after chin bone harvesting are reported such as sensory discomfort and pain etc. In order to verify incisive innervation of symphyseal area, we examined the cross-sectional CT scan images taken for preoperative planning of implant placement with chin bone graft and some parameters were measured; (1) visuality rating of incisive canal (2) vertical and horizontal diameter of canal (3) distance from lower border of the incisive canal to the lower border of the mandible (4) shortest distance from anterior border of the incisive canal to the anterior border of the mandible. We report the positive outcome that decrease the complications related with the damages of incisive branch during bone harvesting from the chin.

  • 해양음향 토모그래피 수행을 위해 필수적인 Forward 모의 알고리즘을 연구를 통해 Forward 문제에서의 선결문제(음파 전달경로의 안정성, 식별능력 및 분해능)를 고잘하였다. 이를 위해 동해의 기존 관측 자료를 이용하여 평균해양에 대한 음속구조 및 EOF 분석을 수행, 재현된 음속구조를 이용, 음선 모델에 의한 고유음선 정보를 파악하여 고유음선의 도달경로 및 도달시간의 비교를 시도하였다.

  • The author has evaluated the panoramic image clarity of the midfacial anatomic structures in dry skull according to the skull position. The radiopaque markers were attached to the anatomic structures: infraorbial rim, upper and lower borders of zygomatic arch, pterygomaxillary fissure, lateral pterygoid plate, pyriform aperture of nasal cavity, lateral wall of maxilla, orbital floor, infraorbital foramen, and nasal floor. Position of the skull were divided into four groups. standard, 25mm forward, chin-down, chin-up position. The results were as follows: 1. The pyriform aperture of nasal cavity, lateral wall of the maxilla, orbital floor, infraorbital foramen and nasal floor did net cast any discernible image. 2. Nearly all images of midfacial structures were blurred in the chin-up position. 3. The forward position provided good visualization of the maxillary sinus. 4. The chin-down position provided good visualization of the zygomatic arch, pterygomaxillary fissue, and lateral pterygoid plate.

  • The purpose of this study was to investigate the changes of the shape of the panoramic innominate line and also to examine the relation of the panoramic innominate line to maxillary posterior wall according to positional changes in panoramic radiographs of the patients who had no pathoses in the maxillary sinus and adjacent bony structures. Thus these panoramic radiographs were obtained and evaluated in standard, forward 10mm, backward 10mm, chin down 10°, and chin up 10° positions. And the following results were obtained : 1. Panoramic innominate lines were not changed in shape according to various positional changes in panoramic radiography. 2. The panoramic innominate line was observed to be more clear images and decreased distance to the posterior wall of maxillary sinus at forward 10mm and chin down 10° positions. And the panoramic innominate line was observed more blurred images and increased distance to the posterior wall of maxillary sinus at backward 10mm and chin up 10° positions.

  • Piezosurgery uses modulated ultrasonic vibration to allow controlled cuting of bony structures. delicate bony structures can be cut easily and with great precision without destruction of soft tissue. Piezosurgery can be used to harvest the chin bone for intra-oral augmentation. Maxillary sinus membrane perforation is the most common complication that occurs with the sinus elevation augmentation procedure. This complication can occur during the osteotomy, which is performed with burs, or during the elevation of the membrane using manual elevators. The purpose of this study is to introduce new surgical technique that simplifies chin bone graft and maxillary sinus surgery, thus avoiding perforating the membrane.

Genioplasty is getting popular in order to improve the facial impression as facial beauty is considered as improvable factor. Through the geniplasty, chin can be moved to wanted location 3 dimensionally Genioplasty is relative simple but precise diagnosis and accurate surgical technique is very important for accurate and satisfying results. The form and shape of chin itself can be analysized and also must be evalulated in relation to the nose and lip and face. Author introduces the analysis of the chin, various surgical techniques of genioplasty and presents some cases.

The predictability of dentoskeletal factors for soft-tissue chin strain during lip closure

  • Objective: To investigate the dentoskeletal factors which may predict soft-tissue chin strain during lip closure. Methods: The pretreatment frontal and lateral facial photographs and lateral cephalograms of 209 women (aged 18-30 years) with Angle's Class I or II malocclusion were examined. The subjects were categorized by three examiners into the no-strain and strain groups according to the soft-tissue chin tension or deformation during lip closure. Relationships of the cephalometric measurements with the group classification were analyzed by logistic regression analysis, and a classification and regression tree (CART) model was used to define the predictive variables for the group classification. Results: The lower the value of the overbite depth indicator (ODI) and the higher the values of upper incisor to Nasion-Pogonion (U1-NPog, mm), overjet, and upper incisor to upper lip (U1-upper lip, mm), the more likely was the subject to be classified into the strain group. The CART showed that U1-NPog was the most prominent predictor of soft-tissue chin strain (cut-off value of 14.2 mm), followed by overjet. Conclusions: To minimize strain of the soft-tissue chin, orthodontic treatment should be oriented toward increasing the ODI value while decreasing the U1-NPog, overjet, and U1 upper lip values.

Alveolar Cleft Reconstruction Using Chin Bone and Autogenous Tooth Bone Graft Material: Reports of 5 Cases

  • Purpose: To report the successful results of using chin bone graft and autogenous tooth bone graft material (AutoBT) in alveolar cleft patients. Materials and Methods: Five patients with alveolar cleft defects underwent alveolar bone grafting. Three patients were treated using chin bone graft, and the other two patients underwent AutoBT graft. After implant site development using chin bone graft in the fi rst three cases, endosseous implant restorations were placed. In case #4 and 5, AutoBT graft material was placed to guide the normal eruption of partially impacted maxillary right canine and to the upper docking site after distraction osteogenesis. Result: Successful implant restorations with closure of the oronasal fistula were achieved in alveolar cleft defect reconstruction using either chin bone graft (Case #1, 2, 3) or AutoBT graft material (Case #4, 5). Case #4 showed enlarged follicle of the right maxillary canine, indicating a normal eruption guide pattern. Conclusion: Both chin bone graft and AutoBT graft showed favorable outcomes in reconstructing alveolar cleft defects. Autogenous tooth bone graft opens up the possibility of avoiding harvesting autogenous bone graft with complications and morbidities.

A STUDY OF RELATIVE PROPORTIONS OF SKELETAL AND SOFT TISSUES OF CHIN IN NORMAL KOREAN ADULTS (한국 정상성인에서 이부의 골격 및 연조직의 상대적 비율에 관한 연구)

  • The esthetics of chin contour is very important in facial deformities, and the quantity of soft tissue overlying the symphysis can be helpful to the correct diagnosis and surgical treatment for optimal facial harmony. The objectives of this study is to measure the different thicknesses of skeletal and soft tissues of chin on lateral cephalometric radiographs, to obtain the mean values of normal Korean adults, and to investigate the relative proportions of skeletal and soft tissues of chin. The results were as follows ; 1. In normal Korean adults, the relative proportions of the distances of soft tissue toward skeletal tissue were 1.5 in B point and 0.95 in Pg, and there were significant difference between male and female in the relative proportion of Pg (p<0.05). The relative proportion of the distances of vertical soft tissue toward horizontal soft tissue of Me was 1.91, and there were not significant difference between male and female. 2. In normal Korean adults, the distances of skeletal and soft tissues of chin showed no correlation, but the distances of vertical and horizontal soft tissues of Me showed high correlation (p<0.01). 3. In normal Korean adults, the correlations among the distances of each skeletal and soft tissues of chin were significantly high in both of male and female (p<0.01). Among the relative proportions of the distances of skeletal and soft tissues, the correlations of the proportions of B point and Mp and the proportions of Mp and Pg were significantly high (p<0.01).

Posteroanterior cephalometric study of frontal ramal inclination in chin-deviated individuals (이부편위자의 하악골상행지 경사에 관한 정모두부방사선학적 연구)

  • Objective: The purpose of this study was to compare the right and left differences of frontal ramal inclination in chin-deviated individuals. Methods: Thirty adult patients with clinically apparent chin deviation were selected as the chin-deviated group, and 30 adult patients with symmetric faces were selected as the control group. On the posteroanterior cephalograms, the frontal ramal inclination was measured, and the right and left differences were compared and analyzed. Results: While the control group did not show right and left differences of frontal ramal inclinations, the chin-deviated individuals showed right and left differences of frontal ramal inclination. In chin-deviated individuals, the frontal ramal inclinations of the non-deviated side were statistically greater than those of the deviated side. The right and left differences of frontal ramal inclinations, with menton deviation and the right and left differences of maxillary heights showed statistically significant correlation. Conclusion: We conclude that the frontal ramal inclinations should be considered in the diagnosis of facial asymmetry.
