본 연구에서는 뉴로피드백을 이용한 로봇 발명 교육 프로그램이 ADHD 아동의 주의집중력에 미치는 영향을 연구하였다. 이를 위하여 12차시로 구성된 로봇 발명 교육 프로그램을 개발하였다. 이 프로그램에서 학생들은 스스로 로봇을 만든 다음 뇌파 측정 장치를 이용하여 로봇을 움직이는 활동을 한다. 뇌파의 측정은 뉴로스카이의 마인드셋을 이용하였으며, 퍼즐박스를 이용하여 뇌파 신호를 로봇에 전달하였다. 실험을 위하여 천안 지역 ADHD 아동 8명을 대상으로 선정하였다. 연구는 총 10회에 걸쳐 진행되었다. 주의집중력 변화를 살펴보기 위한 도구로 행동 관찰, 부모용 아동 문제 행동 체크리스트, 교사용 주의력 결핍 평정 척도, ADS 등을 이용하여 사전 사후 검사를 실시하였다. 행동관찰 결과 평균 45%이상의 주의산만 행동을 보이던 아동들이 32%까지 감소하였다. 부모용 아동문제 행동 체크리스트의 경우 큰 변화를 보이지 않았다. 교사용 주의력 결핍 척도인 CTRS-R의 경우 대상 아동 전체적으로 주의집중력이 향상되는 것으로 나타났다. ADS 검사 결과에서는 사전 측정시 정상 범위를 넘어선 수치가 프로그램이 진행됨에 따라 정상 수치로 나타나는 것을 볼 수 있었다.

Background : The objective of this study was to evaluate the diagnostic efficiency of the Comprehensive Attention Test (CAT) by comparing the results of the continuous performance test (CPT) and the CAT in children and adolescents with attention-deficit hyperactivity disorder (ADHD). Method : A total of 110 children and adolescents with ADHD (mean age : 11.2${\pm}$2.9 years, 76 boys) and 36 children and adolescents without ADHD (mean age : 12.0${\pm}$2.7 years, 25 boys) completed the CAT. We compared the specificity and sensitivity of the CPT and CAT at two different cut-off points via the McNemar test. The areas under the receiver operating characteristic curves (AUC) between the two groups were compared using Medcalc software. Results : The sensitivity and specificity of the CPT were .419 and .806, respectively. The sensitivity of the CAT was .827, which was significantly higher than that of the CPT (p<.001), and the specificity of CAT is .444, which was significantly lower than that of CPT (p<.001). The AUC of the CPT and CAT was .664 and .692, respectively, and there were no significant difference between the two groups on the paired comparison (p=.513). Conclusions : The results of this study suggest that the CAT has moderate specificity and high sensitivity. The CAT can be used as a useful tool to evaluate the neuropsychological function of children and adolescents with ADHD.
This research explored links between mother/child personality characteristics based on their diagnostic and clinical pathology. The 232 participants, selected from mother-child pairs receiving psychological care in a hospital setting, were tested and interviewed by clinical practitioners. Correlations were found between mothers' Psychopathic Deviate, Paranoia, Psychasthenia, and Schizophrenia and sons' Psychasthenia, aggression, alienation, depression, and anger. Mothers classified with Hysteria were likely to have aggressive children. Mothers with low self-esteem, repression, frustration, and strong levels of extroversion had daughters with anger/rage issues. Sons of mothers diagnosed with Hypochondriasis, Depression, Hysteria, Psychopathic Deviate, Paranoia, and Psychasthenia had high rates of Oppositional Defiant Disorder and ADHD. Children of mothers diagnosed with depression had high rates of ADHD. Specifically, mothers with Hypochondriasis, Hysteria, and Psychopathic Deviate had daughters with high rates of ADHD.
정상적인 어린이와 ADHD 질환군 어린이와의 뇌혈류 상태를 비교하기 위하여 SPECT 영상을 획득하고, 정량적 분석 방법으로 유의수준별 혈류의 증가 또는 감소가 나타나는 클러스터(cluster)의 분포변화와 증가 또는 감소율의 변화를 조사하고자 한다. 64명의 어린이 ADHD환자군과 12명의 정상군에 대하여, Multi SPECT 3 (Simens) camera를 이용하여 방사성의약품 $^{99m}Tc-ECD$를 정맥 주사후 30분에 영상을 획득하고, 뇌혈류 상태를 통계적 파라미터 지도작성법(SPM99)으로 분석하였다 정상 어린이군에 대한 ADHD환자군의 뇌혈류 증가와 감소부위의 분포와 비교하기 위하여 통계적 유의수준 P<0.001부터 P<0.05범위에서 나타내었다. ADHD환자군의 뇌혈류 증가부위로 유의수준 P<0.003에서 P<0.01까지 대상회전에서 나타나기 시작하였으며, p<0.03에서 우측의 대뇌후엽, p<0.05에서는 소뇌 좌측상부 등 3개의 클러스터가 나타났다. 대상회전에서, 정상군에 비하여 ADHD 질환군이 평균 19.83%의 혈류증가율을 나타내었으며, 32명의 ADHD 환자는 19.83%이상 증가되었다. 대뇌 우측후엽에서는 정상군보다 평균 19.61%가 증가되었고, 환자 수는 36명이었다. 그리고 소뇌의 좌측상부에서는 평균18.72%의 증가율을 나타냈으며, 평균 증가율보다 더 많은 증가율을 나타내고 있는 ADHD 환자는 34명이었다. 각각의 클러스터에서 혈류 증가율과 환자 수는 유의수준의 변화에 영향을 받지 않았다. 뇌혈류감소의 경우, 유의수준 P<0.002에서 좌측의 대뇌전엽에서 감소를 나타내는 클러스터가 나타나기 시작하여, 유의수준 P<0.01까지 좌, 우의 대뇌전엽, 좌측의 대뇌 전장(claustrum). 우측의 대뇌측엽 등에서 감소된 클러스터가 뚜렷하게 나타났다. 좌측의 대뇌전엽에서 정상군에 비하여 18.79%가 감소되었고, ADHD환자중 36명은 18.7%이상 감소되었다. 좌측의 대뇌 전장에서는 18.41%가 감소되었고, 33명의 ADHD환자는 18.41%이상 감소되었다. 혈류의 감소율과 환자수는 유의수준 값의 변화에 영향을 받지 않았다.

Objectives : The aim of this study is to investigate the effectiveness of treatment with osmotic-release oral system methylphenidate (OROS-MPH) on quality of life (QOL) in children with attention-deficit hyperactivity disorder (ADHD). Another aim is to assess the relationship between change in QOL and other factors including children's symptoms and academic performance or parents' depression and parenting stress. Methods : A total of 111 medication-naive children with ADHD in a multicenter, open-label, 12-week trial of OROS-MPH completed an evaluation using diverse rating scales at two time points; at baseline and after 12 weeks of treatment. Scales for investigation of children included the Parent Report Form-Children's Health and Illness Profile-Children's Edition (PRF-CHIP-CE) on QOL, the ADHD Rating Scale-IV on symptoms, and the Academic Performance Rating Scale (APRS). The Beck Depression Inventory and Parenting Stress Index were used for assessment of their parents. Results : Total scores for mean PRF-CHIP-CE increased from $207.9{\pm}26.7$ at baseline to $226.3{\pm}25.9$ after 12 weeks of treatment (p<.001). The change of APRS showed the strongest correlation with the increment of PRF-CHIP-CE scores (Pearson coefficient=0.561, p<.001), even after controlling for other factors (partial correlation coefficient=0.420, p<.001). Conclusion : Treatment with MPH-OROS results in improvement of QOL in children with ADHD in Korea. The advance in academic performance plays a key role in this change of QOL.
Learning disabilities (LD) are heterogeneous group of disorders with evidences of genetic or familial trait, intrinsic to the individual and presume to be due to central nervous dysfunction. Learning disabilities and attention deficit hyperactivity disorder (ADHD) are the two of the most common disorders in the population of school-age children. Typically academic achievements in children with learning disabilities are significantly lower than expected by their normal or above normal range of IQ. Although academic and cognitive deficits are hallmarks of children with LD, those children are also at risk for a broad range of behavioral and emotional problems. Almost all cases meet criteria for at least one additional diagnosis such as ADHD, developmental coordination disorder, depression, anxiety, obsessive compulsive disorder, tic disorder, among which ADHD is particularly predominant. Because of the response to the therapeutic intervention program is promising and positive when applied early, it is critical to recognize patients as early as possible. Pediatricians often are the first to hear from parents worried about a childs academic progress. It is not the responsibility of pediatrician to make a diagnosis, referring children for a diagnostic evaluation of LD is a reasonable first step. Pediatricians can make early referral of suspicious children by asking some serial short questions about basic and processing skills. With a basic knowledge about the clinical characteristics, diagnostic and therapeutic procedures of LD, pediatricians also can provide primary counseling and education for parents at their outpatient clinical settings.
The effectiveness of behavior group counseling using the games of children with Attention Deficit Hyperactivity Disorder(ADHD) was studied with 24 primary school children (12 each in the experimental and in the control groups). The subjects were selected on the basis of the Abbreviated Conners Teacher Rating Scale-Revised. The counseling program consisted of 12 sessions over a 6-week period. Effectiveness of the training was evaluated with the Conners Teacher Rating Scale-Revised, the ADHD teacher's rating scale, and the Korea-Child Behavior Checklist. Findings were that the group receiving counseling exhibited improved attention, social relationships, and self control. They also exhibited decreased carelessness-passivity, hyperactivity, hostility and fewer behavior problems.

Objectives : The current review aimed to describe the major findings of the NIMH Collaborative Multisite Multimodal Treatment Study of Children with Attention-Deficit Hyperactivity Disorder (MTA) with regard to the treatment of children with attention-deficit hyperactivity disorder (ADHD). Methods : We performed a general review of the literature regarding the efficacy of the MTA's proposed treatments. Results : There is a large and still increasing body of evidence regarding the MTA's treatment outcomes. We present and discuss details of the findings at each follow-up point. Conclusion : Currently, findings regarding the MTA's treatments suggest children with combined-type ADHD exhibit significant impairment in adolescence, despite their initial symptom improvement. Further studies, using innovative treatment approaches and targeting specific areas of adolescent impairment, are needed in order to enhance ADHD treatment outcomes.

목 적:주의력결핍 과잉운동 장애의 진단과 치료 효과를 평가하기 위해 전산화된 한국판 주의력 장애 진단 시스템(ADHD Diagnostic System:ADS)을 개발한 후, 연령별 규준을 산출하였다. 방 법:서울, 경기도, 강원도 지역의 유치원, 초등학교, 중학교에 재학중인 5세에서 15세 연령범위의 총 847명(남자 429명, 여자 418명)의 정상 아동 및 청소년이 규준집단으로 연구에 참여하였다. 또한 ADS의 신뢰도와 변별 타당도를 검증하기 위해 서울대학병원 소아정신과에서 DSM-IV 기준에 의해 ADHD로 진단받은 아동 30명이 본 연구에 포함되었다. 정상 집단의 아동들에게는 그들이 다니는 학교의 컴퓨터실에서 ADS를 실시하였고, ADHD 아동들에게는 소아정신과 외래 임상심리실에서 ADS를 실시한 후, 정상 아동의 자료에 입각해서 연령별 규준을 산출하였고, 신뢰도와 타당도 분석을 하였다. 결 과:ADS의 내적 일치도 계수(Cronbach's ${\alpha}$)는 .85이었다. 정상 집단과 ADHD 집단간에 오경보 오류를 제외한 모든 측정치에서 유의미한 차이가 있었다. 요인분석 결과, ADS 시각, 청각 검사에서 모두 3개의 요인이 산출되었다. 요인 1은 '주의력 요인'으로 누락오류, 오경보 오류, 민감도(d')가 포함되었고, 요인 2는 '정보처리 속도 요인'으로 정반응시간, 정반응시간의 표준편차가 여기에 해당되며, 요인 3은'충동성 요인'으로 반응기준(${\beta}$)이 포함되었다. 요인 1, 2, 3은 ADHD의 핵심적 증상과 정확하게 일치하므로, 이상과 같은 요인분석 결과는 ADS가 주의집중력 장애를 도구로써 구성 타당도가 있음을 입증해주었다. 판별분석 결과, 시각, 청각 ADS가 정상 아동과 ADHD 아동의 96.7%를 정확하게 변별해 주는 것으로 나타났다. 논 의:ADS가 주의력 결핍-과잉 운동 장애를 일관성이 있게 평가하는 신뢰롭고, 타당한 검사로 입증되었고, ADS를 통해 정상 집단과 ADHD 집단을 정확하게 분류할 수 있음이 시사되었다.
ADHD 아동은 발달과정에서 학업 성취를 비롯한 많은 문제들을 야기할 수 있으며, 지적인 능력에서도 부주의성과 연관된 특정적인 검사패턴을 보여주고 있다. 저자들은 1999년 3월 1일부터 1999년 12월 31일까지 DSM-IV의 ADHD 기준을 만족시키는 환자들과 대구 시내의 모 초등학교에서 정상대조군을 선발하여, K-ABC를 이용하여 아동의 지능과 습득도를 평가하였다. ADHD 아동의 경우에는 4-6주간의 약물치료를 시행한 후 동일한 검사들을 재시행하여 약물치료 전후의 차이를 비교하였다. 약물치료전의 ADHD 집단은 정상대조군에 비해 지적인 능력아 유의하게 낮은 것으로 나타났다. 약물치료 전후에서 ADHD 집단의 전체 지능은 유의한 변화가 없었으나 약물치료후의 동시처리척도는 정상대조군과 차이가 없을 정도로 호전이 되었으며, 이는 주의집중력의 증가, 특히 분산집중력의 증가를 의미하며 산만성이 감소되는 것을 의미한다. K-ABC의 하위 영역에서는 약물치료를 받지 않은 ADHD 잡단은 전반적인 하위 영역들이 유의하게 낮은 수행을 보이고 있지만 시각적 세부단서에 대한 주의력은 약물치료에 의해 유의하게 변화되어 정상 대조군과 차이가 없을 정도로 향상되었다. 습득도 하위 영역에서는 어휘력을 제외한 영역에서 ADHD 집단이 정상 대조군에 비해 유의하게 낮은 수행을 보였으나, 결정성 지능의 경우에서 약물치료 후 정상 대조군과 차이가 없었다. methylphenidate를 이용한 약물치료는 분산 집중력, 분석과 조직화와 같은 실행능력을 향상시키며 산만성을 유의하게 감소시켜주는 것으로 나타났다. ADHD 아동에서 약물치료와 함께 다른 형태의 치료가 결합될 경우, 학업수행과 사회성 등 전반적인 영역에서의 향상이 나타날 수 있을 것으로 기대된다.
