학령기 비만아동을 위한 지역사회 참여형 비만관리 프로그램의 효과 (The effect of community based participatory obesity management program in obese school children)

  • 본 연구는 학령기 비만아동을 위한 지역사회 참여형 비만관리 프로그램을 개발하여 비만지수와 운동습관, 식습관, 생활습관에 미치는 영향을 검정한 비동등성 대조군 사전 사후설계이다. 연구대상자는 초등학교 4학년 학생 중 체질량지수가 2007년 소아 및 청소년 표준 성장 도표를 이용하여 85백분위수 이상인 아동으로 실험군 24명과 대조군 27명이었다. 비만관리 프로그램은 실내활동과 실외활동으로 이루어졌으며, 10주간 진행되었다. 실외활동은 줄넘기와 걷기운동을 매주 2회, 전래놀이를 매주 1회 실시하였다. 수집된 자료의 분석은 Kolmogorov-Smirnov test, chi-square test, independent t-test, repeated measure ANOVA를 사용하였다. 연구결과 실험군에서 체질량지수는 $23.64kg/m^2$에서 $22.96kg/m^2$로 감소하였고(p<.001) 복부지방률은 0.889에서 0.887로 유의하게 감소하였다(p<.036). 그러나 체지방률은 유의한 변화가 없었다. 중재 후 운동습관(p=.004), 식습관(p=.003), 생활습관(p=.001)은 모두 향상되었다. 이러한 결과를 통해 본 연구에서 개발된 지역사회 참여형 비만관리 프로그램은 학령기 비만아동들의 비만지수와 비만을 감소시키는 생활습관의 개선에 효과적인 간호 중재임이 확인되었다.

  • 비만성 사구체경화증(obesity-associated focal segmental glomerulosclerosis)은 비만과 부종이 없는 신증후군 범위의 단백뇨, 사구체 비대 및 경화 등의 임상상을 보이는 질환으로, 다수의 환자에서 신부전으로 진행되는 것으로 알려져 있다. 연구자들은 부종 없이 심한 단백뇨와 저알부민혈증을 보인 14세의 Prader-Willi 증후군 여아에서 신생검을 통하여 사구체비후와 메산지움 증식이 동반된 국소성 분절성 사구체경화 소견을 관찰하였다 이로써 소아의 Prader-Willi 증후군에서도 비만성 사구체경화증에 의한 신부전으로의 진행 위험이 있음을 알리는 바이다.

체질량 지수로 판정된 학령전 남아의 비만에 영향을 미치는 요인 (Risk Factors of Obesity by Body Mass Index in Preschool Boys)

  • This study was carried out to identify dietary risk factors for childhood obesity. The study subjects were male children, aged 2-6years, who were enrolled in nursery schools and kindergartens. We obtained anthropometric measurements from 949 boys and dietary questionnaires from 253 boys. The subjects were classified into two groups using the BMI criteria, normal (n=200) group with the BMI below 85th percentile and the obese group with the BMI at 85th percentile and above (n=53). The logistic regression analysis showed that the odds ratios (OR) for obesity were, birth weight greater than 4.0㎏ (3.27) , increased meal speed (4.98) , the frequency of overeating more than twice a week (2.63), the consumption of cooked rice more than one bowl per meal (2.77), the consumption of milk and dairy products less than 5 times a week (2.81), and increased maternal meal speed (1.98). Stepwise multiple logistic regression analyses showed that the variables which contribute to obesity significantly were birth weight greater than 4.0 ㎏ (OR 5.19), the frequency of overeating more than twice a week (OR 2.51) , the consumption of cooked rice more than one bowl per meal (OR 2.74), and the consumption of milk and dairy products less than 5 times a week (OR 4.43) (p<0.0001) . The results of this study indicate that dietary habits of both children and of their mothers as well as child's birth weight are important variables that contribute to childhood obesity. 최근 임상연구를 중심으로 한 소아비만에 관한 방법론적 연구 (Methodological research of obese children focused on the recent clinical researches)

  • Objectives : The purpose of this study is to investigate the recent tendency of clinical researches of obese children for finding better oriental medicine treatments. Methods : This study was focused on diagnoses, treatments, prognoses and cures of obese children based on 37 of other papers which are J Korean Oriental Med, J Korean Oriental Pediatrics, J Korean Acd Fam Med, J Korean society for the study of obesity, Korea Sport Research, J Korean Academy of Pediatric Allergy and Respiratory Disease, J Korean Society for Health Education and Promotion, Korean J Pediatric, and Korean J Oriental Physiology and Pathology. Results : The rate of obese children has been increased continuously. There are several reasons for increased rate : For example, inadequate eating habit, lack of exercise, and genetic factors such as inherited diseases, and the disorder of the endocrine system. The obesity in childhood or adolescent can cause not only the physical problems but also the mental problems. It is necessary for children to diet, change life style, exercise continuously, and being active in order to prevent child obesity and keep healthy. Obesity can be treated through therapy diet, exercise, behavior modification, drug therapy and operation. Conclusion : It is important to recognize the children obesity, and make better treatments for that in the way of oriental cure. Moreover, additional reports should be keeping up based on continuing clinical researches.

학령전기 아동의 비만에 대한 아버지의 인식 유형 (The Typology of Childhood Obesity of Fathers with Preschool Children)

  • Traditionally, fathers have been a bread earner and responsible for external matters as the head of household. However, the interest of today's fathers in their children has changed from that of the traditional fathers due to the increased employment of female, decreased birth rate and the trend of shared role in parenting. The purpose of this study was to identify the perception type of fathers of preschool children regarding obesity and the characteristics of the type by applying Q-methodology, and to create the basic data for the development of education programs for fathers or policy-making to prevent child obesity. Data were collected by applying 38 Q statements to 24 fathers of preschool children. Collected data were analyzed with the PC QUANL program. Results indicate that fathers' perception of obesity was divided into three types: 'management of physical activity and diet,' 'generous acknowledgment of reality' and 'compromised emphasis on homemade meal.' The 'compromised emphasis on homemade meal' type is those who consider homemade food important, although they sometimes suggest conditions to make their children cooperative for food. It was found that the subject of the study uniformly recognized that processed foods, fast foods or chocolates are harmful to health. The type of fathers' perception of obesity in preschool children was discussed, focusing on the perception type identified in this study.

포토보이스를 이용한 아동 비만에 영향을 주는 환경 요인에 관한 탐색적 연구 (Environment Factors Affecting Childhood Obesity: Voices from Students, Parents, and Teachers with Photograph)

  • Purpose: This study aimed to explore the environmental factors affecting childhood obesity using photovoice from the perspectives of students, parents, and teachers in the community. Methods: Six school students, seven parents, and seven school teachers completed an assignment requiring them to take 24 pictures and participate in group discussions. After training session, the participants were asked to take pictures associated with food and physical activity environments related to childhood obesity at home, school, and within their communities for two weeks and to submit the pictures with records. Each group had four sessions for discussion. Results: School cafeteria, convenience stores near schools, instant food and fast food joints, food delivery, and high-calorie snacks comprised the food environmental factors. Lack of physical activity classes at school, commuting by car, barriers to physical activity, and use of smart-phone were environmental factors that inhibited physical activity. Conclusion: To reduce childhood obesity, the creation of a supportive environment for encouraging the consumption of healthy foods and enhancing physical activity should be considered. Modifications of and improvement to the obesogenic environment might be a good strategy to prevent and reduce childhood obesity.

대전지역 일부 초등학생의 비만수준과 관련 요인 (Prevalence and Correlates of Obesity in Elementary School Students of Daejeon City)

  • Objectives: The objective of this study is to investigate the relation between the obesity prevalence of elementary school students and some factors such as socio-demographic factors, parent's obesity, diet habit and physical activity. Methods: The students of the fourth(443), fifth(405) and sixth grade(417) at six elementary schools in the school district of Daejeon metropolitan city were sample-surveyed and classified into a normal group, a non-intensive obesity group, and a highly intensive obesity group. Results: 1. The obesity rate of the elementary school students in the school district of Daejeon metropolitan city was total 14.2%. 2. The obesity rate of students with parents who had high BMI was notably higher than the other groups. 3. Diet factors such as eating fast, always eating one more, liking fatty food and the frequency of snacks were more critical than physical activity factors in relation to school children's obesity. Conclusions: From the above evidences, it is apparent that the obesity of elementary school students have strong relations with eating habits. It is needed that obesity prevention program such as effective meal guidance in addition to intense physical activities is included in the curriculums of health education for elementary school students.

  • Purpose: This study was done to identify perceived body image, intentional caloric restriction and physical manifestations of unbalanced nutrition according to the Body Mass Index (BMI) in elementary school students. Methods: The research design was a cross-sectional descriptive study. Data collection was done by using a structured questionnaire that consisted of 34 items. Participants were 151 students in 5th and 6th grades of elementary schools in Gangwon province. Frequency and percentage, one-way ANOVA, and $\chi^2$-test with SPSS WIN 12.0 were used to analyze the data. Results: In elementary school students, over-weight/obesity and normal weight groups reported that they would be satisfied with a thinner body image compared to the present image. The score for intentional caloric restriction was significantly higher in the over-weight/obesity group than in the under-weight group. In physical manifestations of unbalanced nutrition, students in the under-weight group frequently were pale and had dry skin, whereas the over-weight/obesity group reported frequent upper respiratory symptoms. Conclusion: The results suggest that elementary school students need to develop the perception of an adequate body image and to avoid inadequate caloric intake if promotion of the child's growth and development is to be without physical manifestations of unbalanced nutrition.

Nutrition and health challenges among low-income families of young children in the post COVID-19 era: a qualitative study

  • BACKGROUND/OBJECTIVES: In the United States, one in every 5 children is obese with greater likelihood in low-income households. The coronavirus disease 2019 (COVID-19) pandemic may have accelerated disparities in child obesity risk factors, such as poor dietary intake and increased sedentary behaviors, among low-income families because of financial difficulties, social isolation and other struggles. This study reveals insights into nutrition and health challenges among low-income families of young children in West Texas to better understand needs and develop interventions. SUBJECTS/METHODS: In-depth individual interviews were performed via Zoom among 11 families of children under the age of 3. A semi-structured interview guide was developed to explore 3 areas: changes in (1) dietary intake and (2) sedentary behaviors and (3) families' preferences regarding a parent nutrition education program. Each interview was audiorecorded, transcribed, and coded using MaxQDA software. RESULTS: Eating together as a family become challenging because of irregular work schedules during the COVID-19 pandemic. Most parents stated that their children's dietary habits shifted with an increased consumption of processed foods. Many parents are unable to afford healthful foods and have utilized food and nutrition assistance programs to help feed their families. All families reported that their children's screen time substantially increased compared to the pre-pandemic times. Moreover, the majority of parents did not associate child screen time with an obesity risk, so this area could be of particular interest for future interventions. Meal preparation ideas, remote modality, and early timing were identified as key intervention strategies. CONCLUSIONS: Online nutrition interventions that emphasize the guidelines for child screen time and regular meal routines will be effective and promising tools to reach low-income parents for early childhood health promotion and obesity prevention.
