Podiatric Clinical Diagnosis using Decision Tree Data Mining (결정트리 데이터마이닝을 이용한 족부 임상 진단)

  • With growing concerns about healthy life recently, although the podiatry which deals with the whole area for diagnosis, treatment of foot and leg, and prevention has been widely interested, research in our country is not active. Also, because most of the previous researches in data analysis performed the quantitative approaches, the reasonable level of reliability for clinical application could not be guaranteed. Clinical data mining utilizes various data mining analysis methods for clinical data, which provides decision support for expert's diagnosis and treatment for the patients. Because the decision tree can provide good explanation and description for the analysis procedure and is easy to interpret the results, it is simple to apply for clinical problems. This study investigate rules of item of diagnosis in disease types for adapting decision tree after collecting diagnosed data patients who are 2620 feet of 1310(males:633, females:677) in shoes clinic (department of rehabilitation medicine, Chungnam National University Hospital). and we classified 15 foot diseases followed factor of 22 foot diseases, which investigated diagnosis of 64 rules. Also, we analyzed and compared correlation relationship of characteristic of disease and factor in types through made decision tree from 5 class types(infants, child, adolescent, adult, total). Investigated results can be used qualitative and useful knowledge for clinical expert`s, also can be used tool for taking effective and accurate diagnosis.

A Study on the Variables Affecting Self-Directed Learning of Workers: Focusing on Hope and Growth Mindset (직장인들의 자기주도적 학습에 영향을 미치는 변인: 희망과 성장 마인드셋을 중심으로)

  • The purpose of this study was to investigate factors affecting self-directed learning of workers. For this purpose, 335 workers were surveyed in two cities in the northwestern part of Chungcheongnamdo. The research was carried out by difference test to identify the diffrence of self-directed learning according to general characteristics and by correlation analysis of main variables. Also in order to identify the effect of hope and growth mindset on self-directed learning, hierarchical regression analysis was conducted by diving the group into employee-level and higher employee-level. The main results are as follows. First, Self-directed learning differed by gender. Second, correlation analysis revealed that hope, growth mindset and self-directed learning were positively correlated. Third, regression analysis showed that the pathway thinking of hope and the intelligence of the growth mindset influenced self-directed learning of employee-level workers. And it showed that the pathway thinking of hope influenced self-directed learning of higher employee-level workers. Finally, we discussed policies to enhance workers' self-directed learning based on positions.

Polymorphisms of XRCC1 and ADPRT Genes and Risk of Noncardia Gastric Cancer in a Chinese Population: a Case-control Study

  • Objective: Gastric cancer (GC) is one of the most common malignancies and its mortality ranks third among all cancers in China. We previously noted that XRCC1 Arg194Trp was associated with GC risk in Western China in a study on XRCC1 Arg194Trp and ADPRT Val762Ala. We aimed to further explore the association of these polymorphisms with risk of the noncardia subtype. Methods: We enrolled 176 noncardia GC patients and 308 controls from four hospitals and a community between October 2010 and August 2011. Genotyping was performed in a 384-well plate format on the Sequenom MassARRAY platform. A self-designed questionnaire was utilized to collect epidemiological data from the subjects regarding demographic factors and potential risk factors. Results: Subjects were aged $56.8{\pm}11.8$ (mean ${\pm}$ standard deviation) and $57.6{\pm}11.1$ years in the case and control groups, respectively. Individuals carrying the XRCC1 Trp/Trp or Arg/Trp variant genotype were at significantly increased risk of noncardia GC (adjusted OR, 1.48; 95% CI, 1.00-2.17), after adjustment for family history of cancer, drinking, and smoking. The increased risk of XRCC1 Arg194Trp variant genotype was more pronounced among subjects below 60 years old (adjusted OR, 1.78; 95% CI, 1.07-2.96), compared to older individuals. ADPRT Val762Ala variants (Ala/Ala or Val/Ala) were not associated with noncardia GC (adjusted OR, 1.03; 95% CI, 0.69-1.54). Conclusions: Our study suggests that XRCC1 Arg194Trp is a genetic susceptibility factor for developing noncardia GC in Han Chinese in Western China. In particular, individuals with the XRCC1 Arg194Trp variant genotype are at increased risk for GC below 60 years old.

Association between Atopic Dermatitis and Attention Deficit Hyperactivity Disorder Symptoms in Korean Children (소아.청소년에서 아토피 피부염과 주의력결핍과잉행동장애와의 관계)

  • Objective : The aim of this study was to examine the relationship between IgE-mediated atopic dermatitis and attention deficit hyperactivity disorder(ADHD) in Korean children. Method : This case-control study included 69 patients with atopic dermatitis and 42 normal students. Diagnoses of atopic dermatitis and assessments of its severity were made by the dermatologist. The MAST tests were administered to atopic dermatitis patients. The parents of all of the subjects completed the parent versions of the ADHD Rating Scale-IV(ARS). Results: Compared to the normal control group, the atopic dermatitis group scored significantly higher in the total, inattention, and hyperactivity/impulsivity profiles of the ARS(t=3.35, p<0.01 : t=6.41, p<0.01 ; t=3.93, p<0.01, respectively), The atopic dermatitis patients with ADHD hyperactivity/impulsivity subtype showed more severe symptoms of atopic dermatitis than those with ADHD inattentive subtype. The MAST-positive atopic dermatitis patients showed significantly increased incidence of ADHD symptoms than the MAST-negative counterparts. Conclusion : The results of this study suggest the possibility of an association between IgE-mediated atopic responsiveness and ADHD symptomatology.

  • An eruption failure can be observed for child and adolescent periods when the primary dentition is changed to the permanent dentition through the mixed dentition frequently. The eruption failure can lead to miss erupting times of the tooth, then it will cause a lot of problems including root resorption, esthetic problem, transposition of adjacent tooth, malocclusoin and etc. Especially, the maxillary first molar is importantly concerned with occlusion and growth and is an essential tooth for development and maintenance of occlusion. So, it is a momentous part of more proper occlusal management to find these abnormal cases at the early stage and solve the problems. The sorts of eruption failures of the maxillary first molars can be divided into delayed eruption, impaction and the primary retention and the secondary retention. When physical obstacles cause impaction, first of all they must be removed then we can treat the impaction with observation after removal, surgical exposure or orthodontic traction. If the source of impaction is an ectopic eruption, the treatment can be a brasswire, a pendulum appliance, a space maintainer or space regainer after the extraction of the second deciduous tooth and etc. These cases are made a diagnosis of eruption failures of the maxillary first molars in mixed dentition period and have good prognosises after my treatments. So I reported them.

Association of depression with chewing problems in Koreans : A cross-sectional study using the Korea National Health and Nutrition Examination Survey 2016 (한국인에서 씹기 불편감과 우울증의 연관성: 2016 국민건강영양조사를 이용한 단면 연구)

  • Purpose: This study was designed to analyze the relationship between the presence and severity of depression and chewing problems (CPs) in a representative sample of the general population. Materials and Methods: Health surveys and examinations were conducted on a nationally representative sample (n = 8150) of Korean was conducted. CPs was determined by a simple survey response concerning "Do you feel uncomfortable about chewing your food because of problems with your mouth such as teeth, dentures and gums?" Depression was defined as individuals with a total score ≥ 10 on the Patient Health Questionnaire (PHQ)-9 survey. Data regarding demographics, socioeconomic history and comorbid health conditions were used to analyze adjusted logistic regression models. Results: In the Korean population, the prevalence of depression was significantly greater in individuals with CP (17.2%) than in those without CP (10.2%). On multivariate logistic regression analysis, the presence of depression was significantly associated with CPs (adjusted odd ratio [aOR]: 1.90, P < 0.001). The risk of CPs increased with increasing severity of depression as follows: severe depression (OR: 2.62, P < 0.001), moderately severe depression (OR: 2.19, P < 0.001). Conclusion: The presence of depression was significantly associated with CPs, especially in severely depressed individuals. Depression screening should be considered in treating CP patients.

The Effect of Sensory Integration Program(Sensory Integration Therapy) after School on Functional Task Performance in School (방과 후 감각통합프로그램(감각통합치료)이 학교에서의 기능적 과제 수행에 미치는 효과)

  • Objective : This study is designed to identify effect of a sensory integration(SI) program provided to elementary school students as an after-school activity. The study is looking at both occupational performance components and school-task performance which is an area of occupational performance. Methods : SI program specially designed as an after-school activity was provided to three boys who are first- or second grade students of Y elementary school in Dae-Jeon city. The SI intervention was organized with 25 sessions and carried out for two months. One group pretest-posttest design was used to evaluate the effectiveness of the SI program. Measurements used are Short Sensory Profile(SSP), K-DTVP-2, and BOTMP for occupational performance components, and School Function Assessment for functional schooltask performance. Results : It is found that posttest scores were enhanced in all assessments SSP; BOTMP; K-DTVP-2; and SFA. Conclusion : Results of this study simply propose that sensory integration program designed as after-school activity may have positive effects on not only occupational performance components but also on school-task performance of elementary school student. Since sample size is too small, however, there is limitation to find statistical significance. Further researches testing large number of subjects are required to boost the finding of this study.

Trajectories of Marital Satisfaction of Parent: Relatedness to Behavior Problems of Children (부모의 결혼만족도 변화 유형에 따른 자녀의 문제행동 차이)

  • This study examined the trajectories of the marital satisfaction of parents to classify its latent classes and how marital satisfaction influences the behavioral problems of their children between the identified latent classes. The 1st to 8th and 10th data from the Korea Child-Adolescent Panel Survey were analyzed using the latent class growth analysis and BCH method. First, based on the mother's trajectory of marital satisfaction, five latent classes were identified: 'low constant', 'intermediate constant', 'temporary increment-constantly decrement', 'high constant, and 'highest constant'. At the same time, based on the father's trajectory of marital satisfaction, four latent classes were identified: 'increment', 'intermediate-slightly decrement', 'high-slightly decrement', and 'highest constant'. Second, mothers with low marital satisfaction had more children with behavioral problems, and their influence had more problems with internalized behavioral problems. These problems progressed to externalized behavioral problems as they grew. Both internalized and externalized behavioral problems were also found between the identified latent classes of the father's marital satisfaction. Children of fathers with low marital satisfaction showed more behavioral problems. These findings suggest that the marital satisfaction of parents is an important variable that can influence the behavioral problems of their children.

Renal involvement in children and adolescents with inflammatory bowel disease

  • Purpose: The incidence of inflammatory bowel disease (IBD) is rapidly increasing, and several reports have described the renal complications of IBD. We sought to evaluate the clinical manifestations of renal complications in children with IBD in order to enable early detection and prompt treatment of the complications. Methods: We retrospectively reviewed the medical records of 456 children and adolescents aged <20 years who had been diagnosed with IBD since 2000. We analyzed patient age, sex, medication use, IBD disease activity, and clinical manifestations of renal symptoms. Results: Our study comprising 456 children with IBD included 299 boys (65.6%) and 157 girls (34.4%). The study included 346 children with Crohn disease and 110 children with ulcerative colitis. The incidence of kidney-related symptoms was 14.7%, which was significantly higher than that in normal children. We observed 26 children (38.8%) with isolated hematuria, 30 children (44.8%) with isolated proteinuria, and 11 children (16.4%) with hematuria and concomitant proteinuria. A renal biopsy was performed in 7 children. Histopathological examination revealed immunoglobulin A nephropathy in 5 children (71.4%). All children presented with mild disease and well-controlled disease activity of IBD. Conclusion: Children with IBD are more likely to show kidney-related symptoms than healthy children and adolescents are. Therefore, regular screening of urine and evaluation of renal function in such children are necessary for early detection of renal complications.

Correlation of Executive Function and Quantitative Electroencephalography in Children and Adolescents with Attention-Deficit/Hyperactivity Disorder (주의력결핍 과잉행동장애 소아청소년의 실행기능과 정량화 뇌파의 상관성 연구)

  • Objectives : Attention-deficit hyperactivity disorder(ADHD) is characterized by significant impairments in executive functions, with a prevalence of approximately 3-5% of all children worldwide. The goal of this study was to examine the relationship between executive functions and electrophysiological activities in children and adolescents with ADHD. Methods : In 31 patients with ADHD, resting-state EEG was recorded, and Comprehensive Attention Test(CAT), Stroop Color-Word Inference Test(Stroop CWIT), Trail Making Test(TMT), and Wisconsin Card Sorting Test(CST) were administered. Korean version of the ADHD Rating Scale(K-ARS) was assessed. Results : Alpha and beta power positively correlated with the Attention Quotient(AQ), while delta power negatively correlated with AQ from CAT. In the Stroop CWIT, decreased delta power and increased beta power were related to higher performance. Power of the alpha band increased with higher TMT performance. Moreover, delta power negatively correlated with good performance on the CST, while alpha and high gamma band showed a positive correlation. Correlation with the parent-rating of ADHD symptoms showed a negative correlation between alpha power and higher scores on the K-ARS. Conclusions : These findings indicate that relative power in higher frequency bands of EEG is related to the higher executive function in children and adolescents with ADHD, while the association with the relative power in lower frequency bands of EEG seem to be vice versa. Furthermore, the findings suggest that QEEG may be a useful adjunctive tool in assessing patients with ADHD.
