Vaniching Lung: Report of One Case (거대기종성 폐낭포증 1례 보고)

  • A case of Vanishing lung of right entire lung field in a man of 36 years of age was encountered at Dept. of Chest Surgery of Chonnam University Hospital. His chief complaints were cough, severe dyspnea and chest pain for about 14 years. Right pneumonectomy was done and gross finding was multiple chambered cysts of the right lung with thin epithelium. The review of the literatures was also done.

Surgical Experiences of Boerhaave`s Syndrome -10 Cases analysis- (Boerhaave syndrome의 외과적 치험)

  • All 10 cases of spontaneous rupture of esophagus had violent vomiting as precursor. 9 patients were male, 1 case was female. Chief complaints were chest pain and dyspnea. Chest P \ulcornerA and esophagogram were mainly used as confirm diagnostic tool. Perforation sites of all cases were at distal esophagus near the G-E junction. 6 cases were received primary repair within 24 hrs, other cases were managed with surgical drainage after exclusion and diversion of esophagus. Empyema was the most frequent complication. Other complications were sepsis, pneumonia, leaking etc. Overall mortality rate was about 70.0%.

  • Authors experienced a case of malignant melanoma of the mediastinum, At the time of first visit to the hospital, the female patient, aged 48, and had chief complaints of right shoulder pain that was radiated to its forearm and right Horner`s syndrome. In chest X-ray and CT, extrapulmonary circumscribed mass in the right apicomedial aspect of mediastinum was found. She had no nevus and no melanoma on entire skin area. We excised the tumor and confirmed it as malignant melanoma by histopathology.

  • 음성은 일상생활에서 표현과 대화의 중요한 수단으로 쓰이는 것으로, 자기 표현이 어려운 영유아나 소아에서는 통증이나 배고픔, 불편함 등을 울음소리로서 표현하기도 한다. 이러한 음성을 이용한 말은 성장과정에서 주변으로부터 듣고 이를 흉내냄으로써 자기 나름대로의 독특한 음성양상을 갖게되는 것이고, 어린 시절 잘못된 발성습관은 후일 성인에서의 잘못된 발성양상으로 지속적인 음성질환을 나타낼 수 있을 것이다. 그러나 이러한 소아에서는 후두 검사가 어렵고, 또 음성변화가 왔을 때 후두의 악성질환이 드물며 보통 저절로 잘 치유되리라는 생각에 진단과 치료가 늦어지는 경우가 흔히 있게된다.   • One nine-minth-old female crossbred dog of Maltese and Shih-tzu with chief complaints of left hindlimb lameness and pain was diagnosed as Legg-Calve-Perthes disease(LCPD) in radiographic examination. Femoral head and neck ostectomy for the treatment of LCPD obtained effective result.

  • A case of 35-year-man with a rhabdomyosarcoma in the mandible has been presented. Chief complaints are dull sensation and growing mass at the area of the right mandible. In order to make diagnosis, roentgenograms were taken which revealed some sort of radiopaque and punched appearane at the area from right body to angle of the mandible, and biopsy was made, by which rhabdomyosarcoma was observed. The author treated this patient in terms of hemimandiblectomy, neck dissection and resin-made mandible implantation, and has observed this patient for two and half years after operation. The author satisfy that this patient has been recovered.

  • One six-year-old male Cocker spaniel with chief complaints of mandibular swelling and anorexia was referred. This dog was diagnosed into lymphosarcoma by the results of the examination with peripheral blood and fluid of the lymph nodes and bone marrow aspirated. This patient was multicentric form clinically and was cytomorphologically identified into lymphoblastic form of lymphosarcoma.

  • BACKGROUND : The purpose of this study was to investigate the new effective oriental medical treatments in pediatric diseases and its clinical applicability. METHOD : The study was composed of 1725 new patients who had been treated at the pediatric unit in the Dongguk Bun-Dang Oriental Medicine Hospital for 1 year, from 1 January 1997 to 31 December 1997, and had ages between 0 and 18 years. The chief complaint was mainly categorized by oral examination on patients and their care-givers. RESULT 1. The ratio of male to female was 1.35:1 and the number of each age group and its percentage distribution was 228 patients (13.2%) for the age group between 0 and 1 year, 746 patients (43.2%) between 2 and 6 years, and 751 patients (43.6%) for the age group over 7 years. 2. The chief complaint of the new outpatients was weakness, asthma, common cold, sinustis, skeletal disease, atopic dermatitis, stomach-ache, short stature, diarrhea, nasal blooding, night terror, allergic rhinitis, gastric disorder, enuresis, dermatitis, strabismus in the order of majority.3. The number of patients and its percentage for the most three complaints was 494 patients (28.6%) for weakness, 647 patients (37.4%) for respiratory disease, 144 patients (8.3%) for digestive disease. 4. The digestive disease tended to increase in summer, and appeared mostly in the age group between 0 and 2 years. The respiratory disease increased In inter-season and winter, but decreased significantly in summer. The age group between 1 and 5 years was the largest group of respiratory disease. CONCLUSION 1. The chief complaint in pediatric diseases that needed an oriental medical treatment was mainly the disease that tends to take long time and the weakness, and appeared frequently in digestive and respiratory diseases. 2. The oriental medical treatment was still preferred as a way to improve the weakness by patients, rather than a way to overcome their disease. In particular, the study shows that the oriental medical treatment should be emphasized in terms of preventing the disease. 3. The new diseases, which were developed with the change of human life and environment, (atopic dermatitis, enuresis, short stature, obesity, dysthymic disorder, strabismus), should be investigated as a new field of oriental medical treatment.
