Unusual Presentation of a Rib Osteochondroma as Hard Breast Lump in a Young Male: A Case Report (젊은 남성에서 딱딱한 유방 덩이로 만져진 갈비뼈 골연골종의 드문 증상: 증례 보고)

  • Osteochondroma arising from the rib is rare. They arise as bony outgrowths from the rib and extend either extrathoracically into the subcutaneous plane or intrathoracically compressing the lung or mediastinal structures. A 23-year-old male patient presented with complaints of breast lump since last year. On clinical examination, a hard bony projection with lobulated contour was palpable. Chest radiograph and contrast-enhanced CT showed a bony outgrowth arising from the anterior aspect of costochondral junction of the right fourth rib with displacement of pectoralis major muscle anteriorly. Osteochondroma should be considered as a differential diagnosis in the presentation of hard lump in the breast along with other chest wall tumors.

Imaging Feature of Radiation Induced Lung Disease (방사선 폐손상의 방사선학적 소견)

  • Background and Purpose: Radioopaque lesions are commonly seen in patients who received thoracic radiotherapy for various kinds of thoracic neoplasm, But therir exact diagnos are sometimes uncertain. Patients and Methods: We examined simple chest radiograph and computed tomogram(CT) of 69 patients who received thoracic radiotherapy for lung cancer and were follow up at least 6 months in Yeungnam University Medical Center. Results: Of the 69 patients. thirty-eight patients showed radioopaque lesions in their chest radiographs except radiation fibrosis; radiation pneumonitis was witnessed in 24 patients. infectious pneumonia in 8 patients, and recurrence in 6 patients. In radiation pneumonitis patients, the pneumonitis occurred usually between 50 to 130 days after receiving radiation therapy, and interval between pneumonitis and fibrosis is 21 to 104 days. Simple chest radiographs of radiation pneumonitis(24 patients) represented ground glass opacities or consolidation in 4 cases(type I, 17%), reticular of reticulonodular opacities in 10 cases(type II, 42%), irregular patchy consolidations in 2 cases( type III, 8%), and consolidation with fibrosis in 8 cases(type IV, 33%), CT represent ground glass opacities or consolidation in 5 cases(type I, 29%), irregular nodular opacities in 3 cases(type II, 19%), irregular opacity beyond radiation fields in 3 cases(type III, 18%), and consolidation with fibrosis in 6 cased(type IV, 35%). The CT of four patients who represented type II on simple chest radiographs reveal type I and III, and CT of two patients with clinical symptoms who had no abnormal finding on simple radiograph revealed type I. Conclusions: In conclusion, computed tomogram is superior to the simple radiograph when trying to understand the pathologic process of radiation pneumonitis and provide confidence in the diagnosis of radiation induced lung disease.

Korean Clinical Imaging Guidelines for Justification of Diagnostic Imaging Study for COVID-19 (한국형 COVID-19 흉부영상 진단 시행 가이드라인)

  • To develop Korean coronavirus disease (COVID-19) chest imaging justification guidelines, eight key questions were selected and the following recommendations were made with the evidence-based clinical imaging guideline adaptation methodology. It is appropriate not to use chest imaging tests (chest radiograph or CT) for the diagnosis of COVID-19 in asymptomatic patients. If reverse transcription-polymerase chain reaction testing is not available or if results are delayed or are initially negative in the presence of symptoms suggestive of COVID-19, chest imaging tests may be considered. In addition to clinical evaluations and laboratory tests, chest imaging may be contemplated to determine hospital admission for asymptomatic or mildly symptomatic un-hospitalized patients with confirmed COVID-19. In hospitalized patients with confirmed COVID-19, chest imaging may be advised to determine or modify treatment alternatives. CT angiography may be considered if hemoptysis or pulmonary embolism is clinically suspected in a patient with confirmed COVID-19. For COVID-19 patients with improved symptoms, chest imaging is not recommended to make decisions regarding hospital discharge. For patients with functional impairment after recovery from COVID-19, chest imaging may be considered to distinguish a potentially treatable disease.

  • 간질성 폐질환(Interstitial Lung Disease) 컴퓨터지원진단(Computer-Aided Diagnosis: CAD)시스템은 방사선의사들이 흉부 X-ray영상에서 석회화와 섬유화를 탐지하고자 적용하였다. 진단 중에 발생할 수 있는 오진율을 줄이고 간질성 폐질환이 존재하는 폐야에서 이상유무를 판단하여 검출을 표시하도록 하였다. 본 논문에서는 디지털 흉부영상에서의 간질성 폐질환을 검출하기 위해 폐 텍스처(texture)의 물리적 척도를 측정하기 위한 방법을 제안한다. 2차원의 푸리에 변환으로부터 얻어지는 파워스펙트럼(power spectrum) 분석에 기반을 두는 방법으로 각각의 ROI(Region Of Interest)에서 구한 평균제곱자승오차(Root Mean Sguare: RMS)와 파워스펙트럼의 첫 번갠 모멘트(Moment)는 폐 텍스처의 밀도변동의 크기(magnitude)와 섬세함(fineness)을 나타낸다. 실험결과 다양한 간질성폐질환을 가진 비정상 폐 텍스처의 RMS와 첫 번째 모멘트와는 차이가 있었다. 디지텔 흉부영상으로부터 계산되어진 정량화된 텍스처의 척도는 방사선의사의 간질성 폐 질환을 진단함에 효율적인 질환 탐지를 가능하게 하였으며 진단율을 향상시킬 수 있었다.

Radiologic Findings of Bronchial Asthma (기관지 천식의 영상 소견)

  • Asthma is the most common disease of the lungs, and one that poses specific challenges for the physicians including radiologist. This article reviews for the clinical diagnosis, Radiologic features, and differential diagnosis of asthma, and outlines the radiologic features of the complications of asthma. Bronchial wall thickening and hyperinflation characterize the chest radiograph of the patients with asthma. On CT scan one may see airway wall thickening, thickened centrilobular structures, and focal or diffuse hyperlucency. Apparent bronchial dilatation may be seen, but the diagnosis of bronchiectasis should be made with caution. Quantification of changes in the airway wall and lung parenchyma may be valuable in understanding the mechanisms of asthma and in evaluating the effects of treatment. The challenge for the physician evaluating the images of a patient with asthma is to find complications.

  • Tuberculosis (TB) remains a major global health problem, and the incidence of TB cases has not significantly decreased over the past decade in Korea. The standard short course regimen is highly effective against TB, but requires multiple TB-specific drugs and a long treatment duration. Recent studies using late-generation fluoroquinolones and/or high-dose rifapentine-containing regimens to shorten the duration of TB treatment showed negative results. Extending the treatment duration may be considered in patients with cavitation on the initial chest radiograph and positivity in sputum culture at 2 months of treatment for preventing TB relapse. Current evidence does not support the use of fixed-dose combinations compared to separate drugs for the purpose of improving treatment outcomes. All patients receiving TB treatment should be monitored regularly for response to therapy, facilitation of treatment completion, and management of adverse drug reactions. Mild adverse effects can be managed with symptomatic therapy and changing the timing of the drug administration, but severe adverse effects require a discontinuation of the offending drugs.

  • A previously healthy 57-year-old woman with dyspnea and wheezing presented to the emergency department a few minutes after exposure to unknown gas from mixing bleach (sodium hypochlorite) and cleaning agent (hydrochloric acid) at work place. Initial physical examination revealed severe wheezing on both whole lung fields, but the chest radiograph was normal. Arterial blood gas analysis showed only moderate hypoxemia. The patient was treated with oxygen, $\beta$adrenergic bronchodilators, antihistamines and corticosteroids, after then symptoms were improved. And the patient discharged against medical advice. We report a rare case of reactive airways dysfuntion syndrome from chlorine gas exposure.

  • Tension pneumothorax during one-lung ventilation (OLV) is a rare but life-threatening complication. A 79-year-old male patient who was diagnosed with lung cancer underwent $Univent^{(R)}$ Tube (Fuji Systems Corporation, Tokyo) intubation for left upper lobectomy. Two hours after the initiation of OLV, the patient could not tolerate it. Thus, one-and two-lung ventilation were alternatively applied to continue the operation. After the operation, an emergent chest radiograph was taken, and pneumothorax was found at the right (dependent) lung field.

Accumulation of Ga-67 in Metastatic Pulmonary Nodules from a Moderately Differentiated Adenocarcinoma of the Rectum (직장의 중등도 분화성 선암으로부터 전이된 폐결절에서의 Ga-67 섭취)

  • A 67-year-old woman who had undergone anterior resection for a moderately differentiated adenocarcinoma of the rectum and wedge resection for liver metastasis presented with hematogenous lung metastasis. Metastatic pulmonary nodules in both lung fields were shown on plain chest radiograph and CT. Ga-67 SPECT images revealed accumulation of radioactivity corresponding to the pulmonary nodules. The authors present an unusual case of accumulation of Ga-67 in metastatic pulmonary nodules in a patient with a moderately differentiated adenocarcinoma of the rectum.

The medical management of mitral stenosis in a Bull Terrier

  • An eight-year-old, 28-kg male bull terrier who showed signs of lethargy and cough was referred for further evaluation of congestive heart failure. On presentation, physical examination revealed a systolic murmur at the left apex of the heart. Moreover, chest radiograph evaluation confirmed the mild alveolar and interstitial patterns in the caudal lung lobes and a grossly enlarged left atrium and left ventricle. Electrocardiography showed atrial fibrillation with a wide QRS complex, and transthoracic echocardiography revealed marked enlargement of the left atrium with abnormal morphology of the mitral valve. The thickened, hammer-like appearance and abnormal diastolic motion of the mitral valve leaflets were characterized by decreased leaflet separation and doming of the valve. The diagnosis was mitral stenosis with congestive heart failure and atrial fibrillation. The owner declined interventional valvuloplasty. Medical treatment included furosemide, pimobendan and diltiazem. Regular health check-ups have shown that vitality and clinical signs have improved considerably, and the dog have remained stable for 6 months after the presentation.
