천식 증상을 가진 변형 협심증 환자에 대한 한방치료 증례보고 (A Case Report of Korean Medicine Treatment for A Patient with Variant Angina and Asthma Symptoms)

  • Objective: The purpose of this case study was to report the symptom improvement effect of integrative Korean medicine treatment based on Pyongsim Solution: Cheonwangbosim-dan in a patient with variant angina and asthma symptoms. Methods: This 68-year-old male patient was treated with Pyongsim Solution, Gamihyangsayukgunja-tang, Gwakhyangjunggi-san, acupuncture, pharmacopuncture, and cupping therapy for 24 days. The European Quality of Life-5 Dimensions-5 Levels profile, Numerical Rating Scale score, and pulse rate per minute were measured on admission, at two weeks of hospitalization, and upon discharge to assess the clinical outcomes. Results: After treatment, the "mobility" dimension profile of the European Quality of Life-5 Dimensions-5 Levels changed from 1 to 1, "self-care" changed from 1 to 1, "usual activity" changed from 3 to 2, "pain/discomfort" changed from 4 to 3, and "anxiety/depression" changed from 3 to 2. The Numerical Rating Scale score for chest pain decreased from 7 to 5, chest tightness decreased from 6 to 4, sleep disturbance decreased from 6 to 3, polydipsia decreased from 5 to 3, and stomach discomfort decreased from 5 to 3. All the profiles of these five dimensions were unchanged for 11 days in the period between the two weeks of hospitalization and discharge. There were also no changes in the profiles of either "mobility" or "self-care" throughout the entire 24-day hospitalization period. Conclusion: This case report shows that integrative Korean medicine treatment based on Pyongsim Solution can improve the symptoms of patients with variant angina and mild intermittent asthma, suggesting that Pyongsim Solution could be an important herbal medication for treatment.

동작성 향상을 위한 비행복의 패턴개선 및 평가 (The Improvement and Evaluation of the Flight Suit Pattern to Enhance Movement)

  • The purpose of this study is to evaluate a new flight suit in bilateral ways; subjectively and objectively in the improvements of comfort and mobility. Seven healthy males at the age of twenties volunteered in this wearing test. They were over 177 cm in height and 92-95 cm in chest circumference, which satisfies the M95 Special' size described in the Korean Military Specifications and Standards. The subjective satisfaction was evaluated according to the criteria of ease of donning and doffing, fitness of wear, mobility, space between cloth and body, convenience in using restroom. Subjective satisfaction evaluation says that the best improvement of movement are neck circumference, chest breadth, armscye circumference, waist back length, crotch length, crotch height and body rise. According to the evaluation, improvements of mobility were conspicuous especially in parts of body like neck, breast, waist, back, and arms. When evaluating objective satisfaction, we measured a angle difference in a prescribed standard-grid on a photograph that picked up while the subject performed each motion in front of 10 cm-interval grid. As a result, the motion that had angle difference above 5 degrees were arm stretching and standing on single leg holding at forward or at his side. Bending necks and waist, crouching also had angle difference of over 5 degrees. The improvements are discussed from the viewpoint that the increased ease in parts of armscye circumference and crotch in new flight suit pattern.

Effect of Thoracic Joint Mobilization and Breathing Exercise on The Thickness of The Diaphragm, Expansion of The Chest, Respiratory Function, and Endurance in Chronic Stroke Patients

  • Objective: This study was performed to investigate the effects of thoracic joint mobilization and breathing exercises on diaphragmatic thickness, chest expansion, respiratory function, and endurance in patients with chronic stroke. Design: Randomized controlled trial Methods: The study included 24 chronic stroke patients who were randomly divided into two groups. The experimental group (12 people) performed 15 minutes of thoracic joint mobility exercises and 15 minutes of breathing exercises, three times a week for 6 weeks, 30 minutes each time. The control group (12 people) received 15 minutes of conservative physical therapy and 15 minutes of breathing exercises, 3 times a week for 6 weeks, 30 minutes per session, the same as the experimental group. The experimental and control groups performed the same breathing exercises. To assess training effectiveness, changes in diaphragm thickness, chest expansion, respiratory function, and endurance were measured. Results: As a result, the experimental group exhibited significant improvements in diaphragm thickness, chest expansion, and respiratory function. The endurance mode also displayed significant enhancement (p<0.05), a finding consistent with the control group. However, the experimental group displayed more substantial improvements in non-affected diaphragm thickness and thoracic expansion compared to the control group (p<0.05). Conclusions: Drawing from these findings, breathing exercise which combine thoracic mobilization, will be actively utilized in addition to physical therapy interventions in clinical trials as an effective intervention method.

메카트로닉스 산업용 일반 작업복 개발 - 춘하 및 동절기 작업복 평가를 중심으로 - (The Development of Work Clothes for the Mechatronics Industry through Evaluating Spring-Summer and Winter Suits' Clothing Performance)

  • To develop the work clothes for the employees in the mechatronics industry in South Korea, a questionnaire survey on many aspects of the work clothes such as type, color, detailed design and function preference of the subject employees has been carried out in the study. The results throughout the survey highlighted certain functions such as extensibility, thermal and air permeability of importance for the work clothing performance. To improve the discomfort caused by some work motions (i.e. squatting down, ascending/descending the stairs, raising arms), more allowances were given to the measurements (i.e. body rise + 2cm and crotch depth + 1cm for squatting down pose; thigh circumference + 1cm and knee circumference + 1cm for ascending/ descending the stairs pose; chest circumference + 5~6cm and jumper length to the level of hip circumference line for raising arms pose) for work clothes patternmaking. The evaluation of the two types of spring/summer and winter work clothes' clothing comfort and wearer mobility suggested certain points to improve the clothing performance (e.g. ventilation slits on the back pleats, arm pits and collar band of the spring/summer suit using net material and lining for the winter suit using quilted thermal materials) and wearer mobility (e.g. pants hem buckles for the easy work motion), which also enabled to fulfill the workers' work clothes design preference.

교통약자를 고려한 보도의 경사도와 높이 결정을 위한 실험연구 (Experimental Study on the Determination of Slope and Height of Curbs Considering the VRUs)

  • PURPOSES : As the population of the mobility handicapped, who are classified as the disabled, the elderly, pregnant women, children, etc., has increased, the voices for guaranteeing their rights have been increasing as well. Thus, the design manuals for roads and sidewalks for the mobility handicapped were developed by the local government, such as the Ministry of Land, Transport, and Tourism, in Seoul City. However, according to the 2013 survey results of the Seoul Metropolitan City, the mobility handicapped still feel uncomfortable with the sidewalks, and particularly request for the improvement of the step and slope of the sidewalk curb. Therefore, in this study, we conducted an empirical experimental study to determine the slope of the sidewalk curb and height of the steps considering the mobility handicapped and analyzed whether there is a statistically significant difference. METHODS : The methodology of this study is an empirical experimental one. In the study, five non-disabled people, 10 wheelchair users, and 10 eye patch and stick users walked about 2-3 min on the sidewalk plates of the sloped type (0%, 5%, 6.3%, 8.3%) and stepped type (0 cm, 1 cm, 3 cm, 6 cm), and their human physiological responses, such as the skin temperature, volume of perspiration on forehead and chest, and heart rate, were measured and recorded. After combining the data, we conducted a nonparametric test, ANOVA, or t-test to determine whether there was a statistically significant difference according to each slope and step type. RESULTS : It was found that for the non-disabled, there was no significant difference in human physiological responses according to the slope and steps of the sidewalk. It can be said that the non-disabled do not feel much physiological discomfort while walking. In the case of the sloped sidewalk plate, the heart rate of the wheel chair users increased when the slope was 6.3%. In the case of the eye patch and stick users, the volume of perspiration on the chest increased at a slope of 5.0%. In general, it is judged that a sidewalk with a slope that is less than 5% does not cause a change in the physiological response. In the case of a stepped sidewalk plate, when 0 cm, 1 cm, and 3 cm were compared for wheelchair users, the amount of forehead perspiration increased from 1 cm. Meanwhile, in the case of the eye patch and stick users, when 0 cm and 6 cm were compared, the amount of perspiration on the forehead and chest as well as the heart rate all increased at 6 cm. Taken together, in the case of wheelchair users, a difference was shown when the height of the step of the sidewalk plate was 1 cm, suggesting that installing it at 0 cm does not cause any physiological discomfort. Moreover, in the case of the eye patch and stick users, when comparing only 0 cm and 6 cm, 0 cm was considered to be suitable, as there was a difference in physiological response at 6 cm. CONCLUSIONS : In this study, we set the human physiological responses such as chest skin temperature, amount of perspiration, and heart rate as evaluation items, and our study was considered to be a meaningful experiment that targeted wheelchair users as well as eye patch and stick users. The validity of the evaluation items was confirmed, as the results of human physiological responses were significant. As for the sidewalk design, according to the experiment result, it is considered that differential application should be implemented according to the type of mobility handicap, rather than uniformly applying a sidewalk step of 2 cm and sidewalk slope of 1/25, which are the current legal standards.

뇌성마비 아동에서 노력성 폐활량에 영향을 미치는 요인 분석에 관한 연구 (Predictive Factors Affected to Forced Vital Capacity in Children with Cerebral Palsy)

  • Purpose: Children with cerebral palsy generally have a high incidence of respiratory problem, resulted from poor coughing, airway clearance problem, respiratory muscle weakness, kyphoscoliosis and so forth. The purpose of this study is to investigate the possible factors that can be affected to forced vital capacity (FVC) in children with cerebral palsy. Methods: Total thirty six children with diplegic and hemiplegic cerebral palsy were recruited in this study. They were evaluated by general demographic data (i.e., age, gender, body mass index (BMI)) and variables related to respiratory functions (i.e., chest mobility, waist mobility, maximal phonation time, and maximum inspiratory/expiratory pressure (MIP/MEP)). The correlation between forced vital capacity and the rested variables were analyzed, and multiple regression with stepwise method was conducted to predict respiratory function, in terms of FVC as the dependent variable, and demographic and other respiratory variables as the independent variable. Results: FVC showed a significant correlation with waist mobility (r=0.59, p<0.01), maximal phonation time (r=0.48, p<0.05), MIP (r=0.73, p<0.01), and MEP (r=0.60, p<0.01). In addition, the multiple regression analysis model indicated that FVC could be predicted by the assessment of each waist mobility and MIP. Conclusion: These finding suggest that respiratory function is related to body size and respiratory muscle strength, and that BMI, waist mobility, and MIP can be predictable factors to affected respiratory function in term of FVC.

흉벽 재건에서 합성 물질의 피복을 위한 부분 근피판 조합의 효용성 (Usefulness of Partial Muscle Flaps and Combination Method for Coverage of Prosthetic Material in Chest Wall Reconstruction)

  • Purpose: Reconstruction of chest wall has always been a challenging problem. Muscle flaps for chest wall reconstruction have been helpful in controling infection, filling dead space and covering the prosthetic material in this challenge. However, when we use muscle flaps, functional and cosmetic donor site morbidities could occur. The authors applied and revised various partial muscle flaps and combination use of them to cover the prosthetic material for the chest wall reconstruction and evaluated the usefulness of partial muscle flaps. Methods: This study included 7 patients who underwent chest wall reconstruction using partial muscle flap to cover prosthetic material from 2004 to 2008. The pectoralis major muscle was used in anterior 2/3 parts of it leaving lateral 1/3 parts of it. The anterior 2/3 parts of the pectoralis major muscle were used while lateral 1/3 parts were left. In case of the rectus abdominis muscle flap, we used upper half of it, or we dissected it around its origin and then advanced to cover the site. The latissimus dorsi muscle flap was elevated with lateral portion of it along the descending branch of the thoracodorsal artery. If single partial muscle flap could not cover whole prosthetic material, it would be covered with combination of various partial muscle flaps adjacent to the coverage site. Results: Flap coverage of the prosthetic material and chest wall reconstructions were successfully done. There occurred no immediate and delayed post operative complications such as surgical site infection, seroma, deformity of donor site and functional impairment. Conclusion: When we use the muscle flaps to cover prosthetic material for chest wall reconstruction, use of the partial muscle flaps could be a good way to reduce donor site morbidity. Combination of multiple partial flaps could be a valuable and good alternative way to overcome the disadvantages of partial muscle flaps such as limitation of volume and size as well as flap mobility.

COVID-19 감염병 대응 의료진용 개인보호복의 동작성 및 생리적 부담 평가를 위해 개발된 모의 작업 프로토콜의 타당도 (Validity of a Simulated Practical Performance Test to Evaluate the Mobility and Physiological Burden of COVID-19 Healthcare Workers Wearing Personal Protective Equipment)

  • This study evaluated the validity of a newly developed mobility protocol examining the comfort functions and requirements of personal protective equipment (PPE) for COVID-19 healthcare workers. Eight males (age: 24.7 ± 3.0 y, height: 173.4 ± 2.3 cm, and body weight 69.9 ± 3.7 kg) participated in the following three PPE conditions: (1) Plastic gown ensemble, (2) Level D ensemble, and (3) Powered air purifying respirator (PAPR) ensemble. The mobility protocol consisted of 10 different tasks in addition to donning and doffing. The 10 tasks were repeated twice at an air temperature of 25oC with 74% RH. The results showed significant differences among the three PPE conditions in mean skin temperature, local skin temperatures (the forehead, thigh, calf, and foot), clothing microclimate (the chest and back), thermal sensation, thermal comfort, and humidity sensation, while there were no significant differences in heart rate or total sweat rate. At rest, the subjects felt less warm and more comfortable in the PAPR than in the Level D condition (P<0.05). However, subjective perceptions in the PAPR and Level D conditions became similar as the tasks progressed and mean skin and leg temperature became greater for the PAPR than the Level D condition (P<0.05). An interview was conducted just after completing the mobility test protocol, and suggestions for improving each PPE item were obtained. To sum up, the mobility test protocol was valid for evaluating the comfort functions of PPE for healthcare workers and obtaining requirements for improving the mobility of each PPE item.

건강한 성인에게 가슴가동운동과 스트레칭운동이 최대들숨압 및 최대날숨압에 미치는 효과 (The Effects of Thoracic Mobilizing and Stretching Exercise on Maximal Inspiratory Pressure and Maximal Expiratory Pressure in Healthy Adults)

  • Purpose: The purpose of this study was to investigate the effect of chest mobilization and stretching exercises on maximal inspiratory pressure and maximal expiratory pressure in healthy adults who use computers for extended periods of time each day due to coronavirus disease 2019. Methods: Twenty-five healthy adults in their 20s and without respiratory disease (15 female, 10 male) took part in this study. Two types of thoracic mobilizing exercises using a Theraband and three types of stretching exercises using a foam roller were performed. Maximum inspiratory pressure and maximum expiratory pressure were measured three times each before and after the interventions. In terms of statistical methods, the maximum inspiratory pressure due to chest mobility and stretching was compared with the maximum expiratory pressure using parametric paired t-test and non-parametric Wilcoxon signed-rank test. Results: Maximum inspiratory pressure (p = .012) and maximum expiratory pressure (p = .006) showed significant differences before and after chest mobilization exercise and stretching among the participants. Conclusion: The results of this study suggest that chest mobilization and stretching exercises are effective exercise methods for improving maximal inspiratory and expiratory pressure. They suggest that these exercises can prevent respiratory muscle weakness and improve aerobic fitness in healthy people as well as those in need of cardiorespiratory physiotherapy.

한국철도공사 작업복에 관한 연구 (제1보) -동복 상의를 중심으로- (A Study on the Working Clothes for Korea Railroad -Focused on Winter Upper Garment-)

  • The purpose of this study was to develop the functionally improved winter work clothing for the Korean railroad workers. Based on the questionnaires, the sample clothing for the Korean railroad workers was produced. The design, materials, and patterns were modified to increase mobility and comfort of workers. Water-repellent treatment was added to the material in order to decrease the dirt and stains while working. Also the reflective strips were added to chest and back of the upper garment in order to increase visibility and safety of the workers. In terms of the design and pattern underarm of the sleeve and side panel of the bodice were put in together and action pleats at the back were inserted in order to decrease the pullback of the armpit and scapular area and increase mobility of the arm movement. Also the patterns of the elbows areas were modified to increase mobility of joint. The prototype was evaluated an objective assessment and subjective assessment to compare to the existing working uniform. Assessment group was consisted of 5 subject groups and 11 expert groups and evaluated external appearance and adaptability to the movements. According to the results, the prototype was evaluated better than the existing working uniform in appearance and adaptability except height of collar. A field test was conducted to compare the prototype and the existing working uniform. The field tests were performed by 6 workers in the Korean railroad. According to the results, the prototype was improved in terms of adaptability and comfort.
