항암단을 투여 받은 림프절 외 변연부 B세포 림프종 환자에 대한 증례보고 (A Case Report on Extranodal Marginal Zone B Cell of Mucosa-associated Lymphoid Tissue (MALT) Type Lymphoma Treated with Hangam-dan)

  • Objective : The aim of this case report is to drive further studies evaluating the effectiveness of Korean oriental medicine on extranodal marginal zone B cells of MALT (mucosa-associated lymphoid tissue) type lymphoma. Methods : This case report is about a patient who was diagnosed with extranodal marginal zone B cell of MALT type lymphoma, who refused chemotherapy and preferred to be treated with Korean oriental medicine. Neck, chest abdomen & pelvis with enhanced computed tomography (CT) and positron emission tomography computed tomography (PET-CT) were performed to evaluate the anticancer effect of Hangam-dan (HAD). Results : The patient diagnosed with MALT showed partial response after receiving 17 months of Korean oriental medical treatment. Neck, chest, abdomen & pelvic CT and PET-CT show decrease in size. Conclusion : This case report shows a possibility that Korean oriental treatment could offer potential benefits for patients with extranodal marginal zone B cell of MALT type lymphoma.

Silicone Granuloma Mimicking a Lymphatic Metastasis in a Lung Cancer Patient: A Case Report

  • Silicone granulomas are rare, benign lesions that may occur after breast augmentation. Occasionally, a careful differential diagnosis is necessary because lymphadenopathy or malignancy is suspected based on an imaging study. A 56-year-old woman who visited the hospital due to a lung nodule in the left upper lobe (LUL) underwent a staging work-up with the suspicion of lung cancer. Positron emission tomography/computed tomography and chest computed tomography revealed the LUL nodule and a lesion in the left internal mammary chain (IMC), suggesting lymphadenopathy. Diagnostic wedge resection was performed, followed by curative surgery. The final biopsy result confirmed that the LUL nodule was pathologic stage IB adenocarcinoma; unexpectedly, the lesion in the left IMC was a silicone granuloma.

폐아세포종 -수술치험 1례- (Pulmonary Blastoma -A Case Report-)

  • We experienced a very rare case of pulmonary blastoma in a 29 year old female. She complained of left chest pain and dyspnea for 1 month. The characteristic feature of this tumor is it`s biphasic pattern consisting of a spindle cell stroma containing glandular structures. A serial check of simple chest X-ray and computed tomography revealed a growing huge lung mass occupying whole left thorax. We successfully removed the upper lobe of left lung with a huge tumor mass. Pathologic study revealed this tumor as pulmonary blastoma. We report a case with review of literature.

  • Calcified right ventricular thrombi are very rare.One case of large clacified thrombus in right ventricle being seen in a 65-year old man, is presented. He had dyspnea and generalized edema.The chest film showed a large calcified mass in cardiac shadow especially in lateral view.On echocardiography and chest computed tomography, large calcified mass density occupied nearly entire right ventricular outflow tract.The mass removal was performed through right ventriculotomny.The mass was whitish, stony hard, measured 4 cm x 4.5 cm x 8 cm, tightly attached to right ventricular infundibular septum and ventriculoinfundibular fold.

  • The Coronavirus disease 2019 (COVID-19) has rapidly spread across the world and caused a pandemic. It can be transmitted by an infected person or an asymptomatic carrier and is a highly contagious disease. Prevention and early identification of COVID-19 are important to minimize the transmission of COVID-19. Chest computed tomography (CT) has a high sensitivity for detecting COVID-19, but relatively low specificity. Therefore, chest CT may be difficult to distinguish COVID-19 findings from those of other infectious (notably viral types of pneumonia) or noninfectious disease. Pulmonary contusion has also a lot of similarities on chest CT with COVID-19 pneumonia. We present trauma patients with pulmonary contusion whose CT scans showed findings similar to those of COVID-19, and we report our experience in the management of trauma patients during the COVID-19 pandemic.

흉부 둔상환자에서 흉부전산화단층촬영이 진단과 치료에 미치는 영향 (The Effect of a Chest CT Scan on the Treatment and Diagnosis of Major Blunt Chest Trauma)

  • 배경: 흉부 둔상은 전체 흉부 손상의 90%정도를 차지하며 외상과 관련된 사망률의 20%를 유발시킨다. 흉분 둔상에 의한 손상 후 이환율과 사망률의 주요 원인은 발견되지 못한 손상이 남아있기 때문이다. 그리하여 흉부전산화단층 촬영은 외상환자의 진단적 검사에서 매우 자주 사용되어져 왔다. 그러나 흉부 컴퓨터 촬영은 가격이 비싸고, 방사선 노출을 증가시킬 수 있다. 이번 연구를 통하여 흉부둔상 환자에서 흉부단순촬영과 비교하여 흉부전산화단층촬영을 통하여 얼마나 더 많은 정보를 얻을 수 있는지 조사해 보고 그리고 흉부 컴퓨터 촬영의 진단과 치료에 있어서의 역할을 알아보고자 한다. 대상 및 방법: 2006년 11월부터 2007년 7월까지 응급실로 내원한 환자 100명을 대상으로 하였다. 이 중 74명의 자동차 관련사고 환자와 26명의 낙상 사고 환자가 있었으며 흉부엑스선과 흉부전산화단층 촬영을 동시에 시행한 환자를 전체 응급실 환자 중에 선택하였다. 자료는 차트를 통하여 혈역학적 소견, 중재적 치료 여부, 손상의 중증도(RTS)와 종류를 조사하였으며, 초기 응급실 내원환자 중 흉부단순촬영과 흉부전산화단층촬영을 시행한 환자를 대상으로 발견되지 못한 병적 소견을 각각 질환별로 분석하였다. 결과: 100예의 환자 중 흉부엑스선 검사상 하나 이상의 병적소견을 보인 환자가 79예였으며 21예의 환자에서는 흉부엑스선 검사상 정상 소견을 보였으며, 이 21예의 환자 중 17예에서 흉부전산화단층촬영상 이상소견이 발견되었다. 흉부엑스선 검사상 발견하지 못한 소견으로는 기흉, 혈흉, 폐좌상, 흉골 골절 등이 있었으며 이러한 병적 소견의 진단은 흉부전산화단층촬영이 흉부엑스선 검사보다 통계적으로 유의하게 우수한 것으로 나타났다. 하지만 치료에 있어서는 흉부전산화촬영 시행 후 발견된 병변으로 흉관삽입술등의 치료를 시행한 환자는 31명에 불과했고 흉관삽입술, 개흉술 등의 흉부외과적인 치료 없이 집중관찰을 위하여 입원한 환자가 42명이었으며, 흉부엑스선촬영과 환자의 이학적 소견으로 진단되어 치료를 시행한 환자가 27명이었다. 결론: 흉부전산화단층촬영은 진단에 있어서 흉부엑스선 촬영보다 통계적으로 의미 있게 진단에 도움이 되는 것을 알 수 있었으나 발견되지 못한 병변이 증가할지라도 치료의 변화와 방법의 변화가 있는 경우는 매우 소수에 불과했다. 그리하여 응급실에서의 흉부전산화단층촬영의 오남용을 막기 위하여 선별적인 흉부전산화단층 촬영을 고려해야 할 것이다.

Are There Any Additional Benefits to Performing Positron Emission Tomography/Computed Tomography Scans and Brain Magnetic Resonance Imaging on Patients with Ground-Glass Nodules Prior to Surgery?

  • Background: A ground-glass nodule (GGN) represents early-stage lung adenocarcinoma. However, there is still no consensus for preoperative staging of GGNs. Therefore, we evaluated the need for the routine use of positron emission tomography/computed tomography (PET)/computed tomography (CT) scans and brain magnetic resonance imaging (MRI) during staging. Methods: A retrospective analysis was undertaken in 72 patients with 74 GGNs of less than 3 cm in diameter, which were confirmed via surgery as malignancy, at the Samsung Medical Center between May 2010 and December 2011. Results: The median age of the patients was 59 years. The median GGN diameter was 18 mm. Pure and part-solid GGNs were identified in 35 (47.3%) and 39 (52.7%) cases, respectively. No mediastinal or distant metastasis was observed in these patients. In preoperative staging, all of the 74 GGNs were categorized as stage IA via chest CT scans. Additional PET/CT scans and brain MRIs classified 71 GGNs as stage IA, one as stage IIIA, and two as stage IV. However, surgery and additional diagnostic work-ups for abnormal findings from PET/CT scans classified 70 GGNs as stage IA, three as stage IB, and one as stage IIA. The chest CT scans did not differ from the combined modality of PET/CT scans and brain MRIs for the determination of the overall stage (94.6% vs. 90.5%; kappa value, 0.712). Conclusion: PET/CT scans in combination with brain MRIs have no additional benefit for the staging of patients with GGN lung adenocarcinoma before surgery.

  • SPM은 기저질환이나 외상의 과거력 없이 종격동에 기포가 생기는 질환으로 주로 젊은 연령에서 발생하며, 재발은 세계적으로 소수의 증례보고 위주로 되어 있다. 저자들은 특별한 유발 요인 없는 젊은 여자환자에서 27개월 간격으로 재발한 SPM를 경험하였고, 이는 국내에 보고된 바 없으며, 문헌고찰과 함께 보고하는 바이다.

Evolution of Interferon-Gamma Release Assay Results and Submillisievert Chest CT Findings among Close Contacts of Active Pulmonary Tuberculosis Patients

  • Background: Latent tuberculosis (TB) infection among TB contacts is diagnosed using plain chest radiography and interferon-gamma release assays (IGRAs). However, plain chest radiographs often miss active TB, and the results of IGRA could fluctuate over time. The purpose of this study was to elucidate changes in the results of the serial IGRAs and in the findings of the serial submillisievert chest computed tomography (CT) scans among the close contacts of active pulmonary TB patients. Methods: Patients age 20 or older with active pulmonary TB and their close contacts were invited to participate in this study. Two types of IGRA (QuantiFERON-TB Gold In-Tube assay [QFT-GIT] and the T-SPOT.TB test [T-SPOT]) and submillisievert chest CT scanning were performed at baseline and at 3 and 12 months after enrollment. Results: In total, 19 close contacts participated in this study. One was diagnosed with active pulmonary TB and was excluded from further analysis. At baseline, four of 18 contacts (22.2%) showed positive results for QFT-GIT and T-SPOT; there were no discordant results. During the follow-up, transient and permanent positive or negative conversions and discordant results between the two types of IGRAs were observed in some patients. Among the 17 contacts who underwent submillisievert chest CT scanning, calcified nodules were identified in seven (41.2%), noncalcified nodules in 14 (82.4%), and bronchiectasis in four (23.5%). Some nodules disappeared over time. Conclusion: The results of the QFT-GIT and T-SPOT assays and the CT images may change during 1 year of observation of close contacts of the active TB patients.

A Modified Length-Based Grading Method for Assessing Coronary Artery Calcium Severity on Non-Electrocardiogram-Gated Chest Computed Tomography: A Multiple-Observer Study

  • Objective: To validate a simplified ordinal scoring method, referred to as modified length-based grading, for assessing coronary artery calcium (CAC) severity on non-electrocardiogram (ECG)-gated chest computed tomography (CT). Materials and Methods: This retrospective study enrolled 120 patients (mean age ± standard deviation [SD], 63.1 ± 14.5 years; male, 64) who underwent both non-ECG-gated chest CT and ECG-gated cardiac CT between January 2011 and December 2021. Six radiologists independently assessed CAC severity on chest CT using two scoring methods (visual assessment and modified length-based grading) and categorized the results as none, mild, moderate, or severe. The CAC category on cardiac CT assessed using the Agatston score was used as the reference standard. Agreement among the six observers for CAC category classification was assessed using Fleiss kappa statistics. Agreement between CAC categories on chest CT obtained using either method and the Agatston score categories on cardiac CT was assessed using Cohen's kappa. The time taken to evaluate CAC grading was compared between the observers and two grading methods. Results: For differentiation of the four CAC categories, interobserver agreement was moderate for visual assessment (Fleiss kappa, 0.553 [95% confidence interval {CI}: 0.496-0.610]) and good for modified length-based grading (Fleiss kappa, 0.695 [95% CI: 0.636-0.754]). The modified length-based grading demonstrated better agreement with the reference standard categorization with cardiac CT than visual assessment (Cohen's kappa, 0.565 [95% CI: 0.511-0.619 for visual assessment vs. 0.695 [95% CI: 0.638-0.752] for modified length-based grading). The overall time for evaluating CAC grading was slightly shorter in visual assessment (mean ± SD, 41.8 ± 38.9 s) than in modified length-based grading (43.5 ± 33.2 s) (P < 0.001). Conclusion: The modified length-based grading worked well for evaluating CAC on non-ECG-gated chest CT with better interobserver agreement and agreement with cardiac CT than visual assessment.
