As the use of digital radiographic system has been expanded, there are some concerns an increase about in patient of radiation dose. Therefore, International Electro-technical Commission (IEC) has been proposed a standard foe exposure index (EI). In this study, the EI was measured on human chest model using computed radiography (CR). Radiation quality used RQA5 of IEC62494-1. After acquiring the chest anterior posterior image (Chest AP) by using the phantom, the EI was obtained by applying the system response. In this study, we have analyzed the images with the detector size (Full filed ROI) and the optimized image (Fit filed ROI). The EI increased proportionally with radiation dose increase. Due to the discrete increase in pixel value, the EI showed an exponential increase. The discrete increase in noise equivalent quanta (NEQ) resulted in a discrete increase in the EI. The EI of the two images used in this study increased with increasing NEQ but showed different increments. For the measurement of the EI, IEC standards must be followed. The EI should be used as an index to evaluate the image quality for quality control of X-ray image rather than as an indicator of exposure dose. When calculating the EI, the system response should be applied depending on whether or not the grid is used. The size of the field should be obtained by including only the necessary parts.
This study was to develop a portable digital radiography (PDR) system with a function measuring the X-ray source-with-detector angle (SDA) and to evaluate the imaging performance for the diagnosis of chest imaging. The SDA device consisted of an Arduino, an accelerometer and gyro sensor, and a Bluetooth module. According to different angle degrees, five anatomical landmarks on chest images were assessed using a 5-point scale. Mean signal-to-noise ratio and contrast-to-noise ratio were 182.47 and 141.43. Spatial resolution (10% MTF) and entrance surface dose were 3.17 lp/mm ($157{\mu}m$) and 0.266mGy. The angle values of SDA device were not significant difference as compared to those of the digital angle meter. In chest imaging, SNR and CNR values were not significantly different according to different angle degrees (repeated-measures ANOVA, p>0.05). The visibility scores of the border of heart, 5th rib and scapula showed significant differences according to different angles (rmANOVA, p<0.05), whereas the scores of the clavicle and 1st rib were not significant. It is noticeable that the increase in SDA degree was consistent with the increase of visibility score. Our PDR with SDA device would be useful to be applicable to clinical radiography setting according to the standard radiography guideline at various fields.
본 연구는 자동노출제어 흉부 방사선 검사 시 신체 기능 일부를 대체 보조하거나 의약품 등을 주입하는 인체 이식형 의료기기가 흉부 영상의 선량과 화질에 영향을 미치는지 알아보고자 하였다. 제조회사와 모델이 다른 3대의 디지털 X선 발생장치와 인체모형 팬텀을 사용하여 흉부 검사와 동일한 방법으로 위치잡이를 선정 후 선행 연구에서 선량의 변화가 관찰된 인공심장박동기(Pacemaker), 심장 재동기화 치료기(CRT), 케모포트(Chemoport) 3개의 HIMD(Human Implantable Medical Device)를 상단 이온전리조센서에 부착한 후 Monte Carlo 방법론 기반의 프로그램 PCXMC 2.0을 사용하여 실험에서 도출된 DAP(Dose Area Product) 값을 입력하여 유효선량을 측정하였다. 또한 흉부 영상의 화질 평가를 하기 위해 가슴 부위에 관심 영역 3곳과 잡음영역 1곳을 설정하고 신호대잡음비(SNR; Signal to Noise Ratio), 대조도대잡음비(CNR; Contrast to Noise Ratio)를 측정하였다. 연구 결과는 유효선량의 유의미한 차이를 보여주었으며 AEC 적용과 미적용 그룹을 비교하였을 때 Pacemaker와 CRT는 유의한 차이가 있었다. (p<0.05) AEC 적용 시 Pacemaker에서 37%, CRT에서 52% 유효선량이 증가하였다. Chemoport는 유효선량의 10% 차이는 있었지만, 유의미한 차이를 보이지 않았다. (p>0.05) 영상 품질 평가에서는 모든 HIMD 삽입과 AEC 적용 유무에 따른 SNR, CNR의 유의미한 차이를 보이지 않았다. (p>0.05) 최종 결론은 AEC 적용 후 HIMD가 삽입된 환자의 흉부 X-ray 검사 시 유효선량이 증가하였으며 AEC 적용 유무에 따른 흉부 영상의 품질 차이는 없음을 알 수 있었다. 이는 HIMD가 삽입된 환자의 흉부 검사 시 AEC 미사용이 환자의 선량을 낮추는 타당성을 확인한 것이며 과피폭을 주의하고 피폭 저감화를 위한 다양한 방법을 모색하여야 한다.
Objectives: The purpose of this study is to find the effect of Keumsuyukgunjeongamibang and aromatherapy on a child diagnosed with mycoplasma pneumonia. Methods: The child had been treated with herbal medicine and aromatherapy for 8 days. In order to examine the improvement, chest X-ray and blood test were performed. Results: Various symptoms, such as, coughing, rhinorrhea, dyspnea, was disappeared. The series of 4 X-ray images and blood test proved that the patient has improved. Conclusions: The result of this study shows that administration of Keumsuyukgunjeongamibang and aromatherapy are effective in treatment of mycoplasma pneumonia.
Spontaneous hemopneumothorax is a rare disease, and it can cause life threatening condition. It is characterized by the accumulation of more than 400 mL of blood and air in the pleural cavity without any other apparent causes. A previously healthy 22-year-old female patient presented with acute chest pain and dyspnea. Chest X-ray and computed tomography revealed a massive hemopneumothorax in the left hemithorax. The images showed a completely collapsed left lung with right-sided tracheal deviation, several pleural adhesion bands, and fluid collection with air-fluid level. We emergently performed a closed thoracostomy, and then 560 mL of fresh bloods were initially drained. We considered an emergent video-assisted thoracoscopic surgery for pulmonary wedge resection and bleeding control because of the massive hemothorax. However, the patient's vital signs were stabilized after blood transfusion and supportive cares for re-expansion pulmonary edema. The patient discharged from the hospital on 11th in-hospital day after removal of the chest tube, and there had not been any recurrence of the pneumothorax for 10 months. We suggest that treatment strategy should be decided upon individually based on the patient's condition and clinical course of the disease.
Background: Blunt thoracic trauma in children has a high morbidity and mortality. In this study, we assessed the significance of the injury pattern, mechanism and initial status in emergency department on severity and prognosis in pediatric blunt thoracic trauma patients. Method: We retrospectively reviewed medical records and chest X-ray and CT images of 111 pediatric blunt thoracic trauma patients from October 2000 to June 2005. Data recorded age, gender, season, injury mechanism, injury pattern, associated injury, length of hospital stay and cause of death. Result: Of all 111 patients, 68 patients were injured by motor vehicle accidents, 30 were falls, 5 were motorcycle accidents, 3 were sports accidents and 5 were miscellaneous. In thoracic trauma, single injury of lung contusion were 35 patients and 32 patients had multiple thoracic injuries. Hospital stay in school age group were longer than preschool age group. The causes of death were brain injury in 9, respiratory distress in 4, and hypovolemic shock in 2 patients. Emergently transfused and mechanically ventilated patients had higher mortality rates than other patients. Patients required emergency operation and patients with multiple thoracic injuries had higher mortality rates. Conclusion: In this study, patients with combined injury, emergency transfusion, mechanical ventilation, emergency operation, multiple injuries in chest X-ray had higher mortality rates. Therefore in these pediatric blunt thoracic trauma patients, accurate initial diagnosis and proper management is required.
The purpose of this study is to improve the classification accuracy compared to the existing InceptionV3 model by proposing a new model modified with the fully connected hierarchical structure of InceptionV3, which showed excellent performance in medical image classification. The data used for model training were trained after data augmentation on a total of 1026 chest X-ray images of patients diagnosed with normal heart and Cardiomegaly at Kyungpook National University Hospital. As a result of the experiment, the learning classification accuracy and loss of the InceptionV3 model were 99.57% and 1.42, and the accuracy and loss of the proposed model were 99.81% and 0.92. As a result of the classification performance evaluation for precision, recall, and F1 score of Inception V3, the precision of the normal heart was 78%, the recall rate was 100%, and the F1 score was 88. The classification accuracy for Cardiomegaly was 100%, the recall rate was 78%, and the F1 score was 88. On the other hand, in the case of the proposed model, the accuracy for a normal heart was 100%, the recall rate was 92%, and the F1 score was 96. The classification accuracy for Cardiomegaly was 95%, the recall rate was 100%, and the F1 score was 97. If the chest X-ray image for normal heart and Cardiomegaly can be classified using the model proposed based on the study results, better classification will be possible and the reliability of classification performance will gradually increase.

Objective: This study presents a preliminary report on the chest radiographic and computed tomography (CT) findings of the 2019 novel coronavirus disease (COVID-19) pneumonia in Korea. Materials and Methods: As part of a multi-institutional collaboration coordinated by the Korean Society of Thoracic Radiology, we collected nine patients with COVID-19 infections who had undergone chest radiography and CT scans. We analyzed the radiographic and CT findings of COVID-19 pneumonia at baseline. Fisher's exact test was used to compare CT findings depending on the shape of pulmonary lesions. Results: Three of the nine patients (33.3%) had parenchymal abnormalities detected by chest radiography, and most of the abnormalities were peripheral consolidations. Chest CT images showed bilateral involvement in eight of the nine patients, and a unilobar reversed halo sign in the other patient. In total, 77 pulmonary lesions were found, including patchy lesions (39%), large confluent lesions (13%), and small nodular lesions (48%). The peripheral and posterior lung fields were involved in 78% and 67% of the lesions, respectively. The lesions were typically ill-defined and were composed of mixed ground-glass opacities and consolidation or pure ground-glass opacities. Patchy to confluent lesions were primarily distributed in the lower lobes (p = 0.040) and along the pleura (p < 0.001), whereas nodular lesions were primarily distributed along the bronchovascular bundles (p = 0.006). Conclusion: COVID-19 pneumonia in Korea primarily manifested as pure to mixed ground-glass opacities with a patchy to confluent or nodular shape in the bilateral peripheral posterior lungs. A considerable proportion of patients with COVID-19 pneumonia had normal chest radiographs.
최근 디지털 단층영상합성 시스템 (digital tomosynthesis system, DTS)은 일반 X-ray의 영상 중첩현상과 전산화단층촬영장치 (computed tomography, CT)의 높은 선량의 문제점을 해결하기 위하여 개발되었다. 본 연구의 목적은 흉부 촬영용 디지털 단층영상합성 시스템 (chest digital tomosynthesis, CDT)의 재구성 알고리즘 개발 및 화질과 선량 평가를 수행하는 것이다. 영상의 재구성은 필터 후 역투영 (filtered back-projection, FBP)을 모델링하였고, 팬텀 영상을 획득하기 위한 X-선과 검출기 사이의 각도를 ${\pm}10^{\circ}$, ${\pm}15^{\circ}$, ${\pm}20^{\circ}$, 그리고 ${\pm}30^{\circ}$로 구성하였다. 영상의 화질 평가는 평균 제곱근 편차 (root mean square error, RMSE)와 신호대 잡음 변화율 (signal difference-to-noise ratio, SDNR)로 수행하였고, 선량 평가는 ${\pm}20^{\circ}$의 범위에서 유효선량으로 수행하였다. 결과적으로, 모든 각도에서 Slice thickness 필터를 적용한 팬텀영상이 가장 우수한 RMSE와 SDNR 결과를 나타내었고, 최종 유효 선량은 0.166 mSv로 측정되었다. 결론적으로, 개발한 CDT 재구성 알고리즘의 유용성을 증명하였고, 최종 유효 선량을 측정하여 CDT의 기초 실험 데이터를 구축할 수 있었다.
본 연구는 엑스선 흉부촬영에서 격자를 이용하여 촬영법과 Air Gap Technique을 이용한 촬영법에서 얻은 영상을 평가하기 위하여 대학병원 영상의학과에서 10년 이상 근무한 방사선사 선생님 5인에게 주관적 평가 ROC와 객관적 평가 SNR를 시행하였다. 분석한 결과는 Cronbach Alpha 값이 0.714로 유의하게 높았다. Air Gap Technique 촬영법에서 팬톰과 상수용체 간격을 20 cm 띄우고 관전압 100 kVp, 관전류와 촬영시간 8 mAs로 설정하고 촬영한 영상을 ROC 평가에서 18점, SNR 평가에서 6.149로 가장 높은 점수를 얻었다. 또한 격자를 이용하여 촬영법에서는 팬톰과 상수용체 간격을 15 cm 띄우고 관전압 110 kVp, 관전류와 촬영시간을 8 mAs에서 촬영 시 ROC 평가점수가 19점과 객관적평가인 신호대잡음비(SNR)가 6,622으로 가장 높은 점수를 받았다. 이에 격자를 이용하여 나타나는 격자의 cut-off 현상으로 인하여 재촬영뿐만 아니라 판독지연, 환자의 피폭선량 증가, 기계적 수명단축 등의 단점을 벗어나는 Air Gap Technique 촬영법을 활용한다면, 중환자실 및 응급환자를 위한 이동형 엑스선 촬영장비를 사용할 경우를 포함하여 흉부 촬영 시에 많은 도움이 되리라 사료된다.
