Frontometaphyseal dysplasia is a rare genetic syndrome first described by Gorlin and Cohen in 1969. This disease affects the skeletal system and connective tissue, and could be characterized by hyperostosis of the skull, prominence of supraorbital ridges, additional skeletal and extraskeletal abnormalities. It is believed that the condition is an X-linked dominant trait with severe manifextations in males and extreme variability in females.We described a case of 15-year-old boy manifested a pronounced supraorbital ridge associated with exorbitism. He also had bilateral progressive hearing deficit, thoracic spine scoliosis, chest wall deformity, bilateral maxillary sinusitis and both 5th finger arachnodactyly.The patient underwent a fronto-temporo-orbital cranioplasty through a coronal incision. The frontal bone including supraorbital region, orbital lateral rim and temporal bone were extensively contoured with burr. And then, burring of the medial aspect of lateral orbital wall was made to increase both orbital volume for correction of exorbitism. Postoperative results show well corrected prominent supraorbital ridge, hyperostosis of frontotemporal bone and exorbitism. The patient was satisfied with the improved appearance. No recurrence occurred during the 6 months of follow-up period. We report this case as it shows esthetically good result without any complication.
A 10-year-old Korean native female Jin-do dog revealed two growing subcutaneous masses measuring 7.0 ${\times}$ 7.0 ${\times}$ 2.5 cm and 5.0 ${\times}$ 4.0 ${\times}$ 2.0 cm in the left shoulder and lower part of chest, respectively. Grossly tumor masses were well-circumscribed and exhibited ulcerated surface and purulent exudate on the cut surface. Histologically tumor masses were characterized by cystic structures some of which were fused together. Although the cells of cyst wall differentiated to those similar to infundibulum, isthmus, inferior segment of hair follicles, respectively, the most cyst wall consisted of cells similar to infundibulum The luminal content of cysts depended on the cells of cysts; infundibulum-like cyst contained lamellar keratin, isthmus-like cyst was amorphous keratin, and inferior segment-like cyst had shadow cells. From these results, these tumor masses were diagnosed as multiple trichoepithelioma. To our knowledge, this is the first report of canine multiple trichoepthelioma in a dog in Korea.
Recently, due to the increased use of medical radiation, the radiation exposure of radiation workers should be considered as well as medical exposure of patients. And it is recommended to close the door during radiography. however, In this study, when the door was inevitably opened for radiography, the proposed method was to install the shield as a method of reducing the exposure dose. And its efficiency was analyzed. In simple chest radiography, the measurement point was changed according to the measurement location. Dose rate were measured 10 times for each condition using a dosimeter. And the average value was derived. Using this, the change of dose according to the opening and closing of the door and the installation of the shield was analyzed. Using this, we compared and analyzed the dose change according to the door opening and closing and the installation of the shield, and significance was verified through the SPSS ver. 24. Depending on whether the door was opened or closed, 11,215.35%, 159.0%, 101.9% increased in front of the door in the consol room, behind the wall and behind the lead glass. Depending on the installing of the shield, the 49.2%, 29.6%, 19.9%, 30.6% decrease in front of the door in the examination and consol room, behind the wall and lead glass. In addition, statistical analysis was showed that there were significant differences in both the results according to whether the door was opened or closed and shielding(p<.05). Close the door during radiography. However, when the door should be opened, it was confirmed that the dose rate were reduced by installing the shield. Therefore, to optimize radiation protection, it is recommended to install shields when opening the door.
This is a report on four cases of successful surgical correction of coarctation of the aorta [COA] in Department of the Thoracic & Cardiovascular Surgery, Hanyang University Hospital. The first case was a postductal type of coarctation of the aorta associated with Patent ductus arteriosus [PDA], Persistent left superior vena cava [LSVC] and richly developed collateral circulation. Blood pressure was measured to be hypertensive at the arm, but hypotensive at the legs. The coarctation of the aorta was corrected with following procedure: Partial resection of the aortic wall with diaphragmatic structure lust above and below the coarctating line of the aorta, and then the defect of the aortic wall was closed by lateral aortographic suture. PDA was closed by ligation procedure. The second case a preductal type of coarctation of the aorta associated with PDA, LSVC, ventricular septal defect [VSD] and poorly developed collateral circulation. Normal blood pressure was measured at the arm, but hypotension was observed at the legs. Correction of coarctation of the aorta was performed under the establishment of tube bypass because of poor collateral circulation. After resection of coarctating short segment, end to end anastomosis was performed without any tension. PDA was closed by division procedure. Simple suture closure of VSD was performed by open heart surgery two weeks after correction of COA. The third case was a long segment COA without any other anomaly. Blood pressure was measured to be hypertensive at the arm, but hypotensive at the legs. Vascular prosthesis was performed using Teflon graft tube after resecting coarctating long segment [6.5 cm] of the aorta. The fourth case was a long segment COA associated with aortic insufficiency and richly developed collateral circulation. Normal blood pressure was measured at the arm, but hypotension was observed at the legs. Vascular prosthesis was performed using Teflon graft tube after resecting coarctating long segment [6.0 cm] of the aorta. Both blood pressure and peripheral pulse on the arm and the legs returned to normal postoperatively in all patients.
The myocardial temperature was monitored in 19 patients of open heart surgery using the Shiley myocardial temperature probe in Department of Thoracic and Cardiovascular Surgery, Yeungnam University Hospital. The myocardial temperature were measured time-wise: initial and 15 minutes following the infusion of the cardioplegic solution into the aortic root and upon reinfusion, which is 30 minutes apart from the initial. The measurements were made in the anterior wall of the right ventricle, the posterior wall of the left ventricle and the interventricular septum. Immediately after initial infusion, the temperature of the right ventricle[10.7 $\pm$4.3oC] was the lowest and that of the left ventricle[12.9$\pm$3.9 oC] the highest in the mean values among all 19 patients. However, no significant differences were noted among the different regions. At 15 minutes after infusion, the lowest temperature was in the right ventricle[17.5$\pm$5.1 oC], followed by interventricular septum[17.9$\pm$2.9 oC], and left ventricle[21.4$\pm$2.5 oC]. At 30 minutes after infusion, the lowest temperature was measured in the interventricular septum[13.6$\pm$2.7 oC ], followed by right ventricle[13.8$\pm$4.0 oC] and left ventricle[14.5$\pm$4.5 oC ]. Evaluating changes of myocardial temperature according to postinfusion time, the temperature at 15 minutes after infusion showed significant increase as compared with that immediately after the infusion in all three regions[p<0.05] and the temperature at 30 minutes after infusion showed significant decrease as compared with that at 15 minutes after the infusion in the left ventricle and the interventricular septum[p<0.05]. The left ventricle was rewarmed most rapidly and its temperature the highest in the mean values, Accordingly, the maintenance of optimal hypothermia of the left ventricle indicated a very careful factor in myocardial protection.
Ischemic ventricular rupture is one of the most fatal complications following myocardial infarction, and this requires prompt diagnosis and operation. A 75-year-old female was admitted to the ER in a semicomatous mentality with cyanotic extremities. Cardiac echography was carried out in the ER, and a $1.5{\sim}2\;cm$ thickness of effusion in the pericardium was seen. Because the patient's heart had declined to 35 times per min, an emergency operation was started while giving cardiac massage. After observing a 1 cm rupture on the right ventricular wall and a necrotic hemorrhagic scar with a rupture on the left ventricular apical wall, repair of the ruptured areas with a large Satinsky clamp was carried out to control bleeding without cardiopulmonary bypass. On the 28th day after surgery, she was discharged home with a minimal degree of dyspnea.
Using an experimental model of pulmonary hypertension, the effects of anticonstrictive drugs on the development of pulmonary vascular remodeling and right ventricular hypertrophy were studied. Male Sprague-Dawley rats weighing 200~250 gm were used. For the experimental model of pulmonary hypertension, a group of animal was given by a subcutaneous injection of monocrotaline on a dose of 20mg, 40mg, or 60mg per kg of body weight. After 4 weeks of injection, all animals were sacrificed. Another group of animal was given by a subcutaneous injection of monocrotaline in a dose of 40 mg per kg of body weight. The animals were sacrificed, in which they were kept alive for 1, 2, 3 and 4 weeks, respectively. For the effects of anticonstrictive drugs on the development of pulmonary vascular remodeling and right ventricular hypertrophy, the animals treated with monocrotaline were given daily by an intraperitoneal injection of phenoxybenzamine in a dose of 1.3mg/kg of body weight, and were given propranolol via their drinking water at a concentration of 400mg/liter. The animals were sacrificed after 4 weeks of administration. The hearts and lungs were examined histopathologically and morphometrically. The results obtained were summarized as follows: 1. The rats treated with monocrotaline showed an interstitial pneumonitis, medial thickening of the pulmonary small arteries and hypertrophy of the right ventricular wall. 2. The medial thickening of the pulmonary arteries in rats treated with monocrotaline was due to muscular hypertrophy and hyperplasia, and the right ventricular hypertrophy was due to hypertrophy of cardiac muscles. Both medial thickening of the pulmonary arteries and hypertrophy of right ventricular wall were more marked with time and with dose. 3. The daily intraperitoneal injection of phenoxybenzamine suppressed significantly the percentage medial thickness of pulmonary small arteries and the index of right ventricular hypertrophy in rats given a single subcutaneous injection of monocrotaline, but propranolol has shown no protective effect on the development of medial thickening of pulmonary arteries and right ventricular hypertrophy in treated with monocrotaline. The results described above suggested that monocrotaline is an alkaloid selectively inducing pulmonary hypertension and that a-adrenergic receptor is responsible for the pathogenesis of monocrotaline induced pulmonary hypertension in rat.
We present a case of 58-year-old (tamale with dilated cardiomyopathy(DCMP) in whom we performed left ventricular(LV) remodeling surgery(Batista operation) to reduce the left ventricle diameter and improve left ventricular unction. The patient was admitted September 1996 with heart failure NYHA class IV. There was severe orthopnea and peripheral edema. 2-D echocardiography(Echo) showed DCMP with the ejection fraction(EF) I5%, LV end diastolic dimension(LVEDD) 80mm, mitral regurgitation(MR) grade IV, tricuspid regurgitation ('m) grade ll. Preoperative cardiac output(CO) was 1.5/L/min and cardiac index(Cl) was 1.0 L/min/m2. We proceeded with LV remodeling surgery by resection a part of LV lateral wall between both papillary muscle, from the mitral annulus to the LV apex. Size of resected LV wall was 90 $\times$ 100 $\times$ 15 mm. At the mean time, mitral valve and tricuspid valve were repaired. Postoperative 2-D Echo showed the EF 37%, LVEDD 50 mna, trivial MR, no TR. CO was 3.SL/min and Cl was 2.3 L/min/m2. Her fuctional NYHA class was 1.

Background: For the surgical management of anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), there have been various techniques that reduce the tension and kinking of the coronary artery during reimplantation to the aorta. The aim of this study is to describe the results of our modified technique of coronary reimplantation for the treatment of ALCAPA. Methods: Between October 2003 and February 2011, seven patients underwent coronary reimplantation with the modified technique (tubing formation with the sinus wall of the pulmonary artery and trapdoor formation at the site of implantation in the aorta). The median follow-up duration was 52 months (range, 4 to 72 months). Clinical outcomes and serial echocardiographic data were reviewed. Results: There was no mortality. One patient had a small amount of cerebral hemorrhage postoperatively and improved without any sequelae. Another patient had left diaphragm palsy and underwent diaphragm plication. Follow-up echocardiogram showed that all patients had normal ventricular function without chamber enlargement. Conclusion: Our modified technique (tubing formation with the sinus wall of the pulmonary artery and trapdoor formation at the site of implantation in the aorta) demonstrated successful clinical outcomes. We conclude that this surgical technique can be a potential alternative for the treatment of ALCAPA.
Aortobronchial fistula (ABF) induced by an infected pseudoaneurysm of the thoracic aorta is a life-threatening condition. As surgical treatment is associated with significant mortality and morbidity, thoracic endovascular aneurysm repair (TEVAR) may be an alternative for the treatment of ABF. However, the long-term durability of this intervention is largely unknown and the recurrence of ABF is a potential complication. We experienced a case of recurrent ABF after stent grafting as an early procedure for an infected pseudoaneurysm of the thoracic aorta. Remnant ABF, bronchial and/or aortic wall erosion, vasa vasorum connected with ABF, and recurrent local inflammation of the thin aortic wall around ABF might cause recurrent hemoptysis. As a result, we suggest that TEVAR should be considered as a bridge therapy for the initial treatment of ABF resulting from an infected pseudoaneurysm, and that several options, such as second-stage surgery, should be considered to prevent the recurrence of ABF.
