흉곽출구 증후군[TOS];치험 1례 (Thoracic Outlet Syndrome - One Case Report -)

  • Thoracic outlet syndrome is very rare thoracic surgical neurovascular disorder which is subject to compression by bones and muscular structures impinging upon the subclavian artery, vein k brachial plexus. The operative therapy is applied to remove the anatomical abnormal structures which leads to the compression to develop symptoms. We have operated one patient with thoracic outlet syndrome by excision of cervical ribs & first ribs with using transaxillary & posterior parascapular approaches in the thoracic surgical department, Yonsei University College of medicine, The post-operative courses has been uneventful for 7 months to now.

  • Rupture of a bronchus is an unusual result of nonpenetrating trauma to the chest. This case was a 19 year old male who was a worker in the mine. The patient had sustained a compression chest injury without external wound or rib fracture. At five days after trauma, he was suffered from dyspnea and the symptom was progressively increased. Two weeks after the chest trauma, a diagnosis of left main bronchial obstruction due to traumatic bronchial rupture was made by means of bronchoscopy and bronchography. He was operated upon. The left lung was completely atelectatic and the left main bronchus was found to be transected at the level of 3cm from carina. End-to-end anastomosis of the bronchus was performed and the left lung was aerated very well. postoperative course was uneventful.

  • Spinal epidural lipomatosis (SEL) is an overgrowth of the normally encapsulated adipose tissue in the epidural space around the spinal cord in the thoracic and lumbar spine causing compression of the neural components. Idiopathic SEL in non-obese patients is exceptional. Idiopathic SEL can result in thoracic myelopathy and lumbar radiculopathy. A thoracic radiculopathy due to idiopathic SEL has not been reported yet. We report a case of idiopathic SEL with intractable chest pain and paresthesia. We suggest that idiopathic SEL should be considered as a cause of chest pain.

  • A 36 years old male patient underwent en bloc excision of the primary chondrosarcoma arising from 8th, 9th and 10th ribs of the chest wall. The tumor was 11.5 x 7.5 x 6.5 cm in size and was 320 gram in weight. After en bloc excision of the tumor, the defect of the chest wall was reconstructed with pedicle muscle flap and using compression dressing for 2 weeks, there happened no respiratory problem. And so we made a case report and review of literature.

2010년 심폐소생술 가이드라인에서 2인 구조자의 팀워크 중심의 격려방법에 따른 가슴압박 질 비교 (A Comparison of Chest Compression Quality According to 2 Rescuers' Teamwork-Oriented Encouragement Method in Cardiopulmonary Resuscitation(CPR) Guidelines for 2010)

  • 본 연구는 2010년 심폐소생술 가이드라인에서 2인 구조자의 팀워크 중심에서 격려방법에 따른 가슴압박 질 차이를 알아보고자 한다. 대상자는 심폐소생술 교육과정을 이수한 응급구조과 재학생을 무작위 추출하였다. 집단은 1회 격려집단(33명)과 연속적 격려집단(33명)으로 구분하였다. 측정 기간은 2012년 4월 30일부터 5월 7일까지 진행되었다. 수집된 자료는 SPSS WIN program(version 12.0)으로 분석하였다. 연구결과 가슴압박 정확도(회, %)는 1회 격려집단(118.72회, 78.54%)이 연속적 격려집단(95.12회, 62.96%) 보다 정확도 질이 높았다(p<.05), 주관적 집중력 인식은 1회 격려집단(3.90점)이 연속적 격려집단(3.12점) 보다 집중력에 도움이 된다는 인식이 높았고(p<.05), 주관적 피로도 완화 인식도 1회 격려집단(3.96점)이 연속적 격려집단(3.30)보다 피로도 완화에 도움이 된다는 인식이 높았다(p<.05).

구급차 내에서의 심폐소생술 방법에 관한 연구 (A Study on the Cardiopulmonary Resuscitation Methods in Ambulance)

  • 이 연구는 소방에 근무하는 응급구조사에 의해 시행되는 구급차 내 심폐소생술 방법에 관한 연구이다. 연구 대상은 경기도 관할 780명으로 2012년 보수교육 종료 후 설문을 통하여 수집하였다. 일반적 특성, 가슴압박 방법(한 손 가슴압박 VS 두 손 가슴압박), 심폐소생술 방법(표준심폐소생술 VS 가슴압박소생술) 등에 대하여 Chi-square test, t-test. ANOVA로 분석하였다. 한 손을 이용한 가슴압박 14.0%, 두 손을 이용한 가슴압박 86.0%를 보였고, 표준심폐소생술(가슴압박 VS 인공호흡)은 28.3%, 가슴압박소생술 71.7%를 보였다. 고품질의 심폐소생술을 위해서는 심정지 환자의 구급 출동만이라도 운전자 포함 3명 이상의 응급구조사가 출동할 수 있도록 탄력적인 인력 운영 방안이 필요하며, 역량 강화를 위한 심폐소생술의 정기적인 교육이 필요하다.

  • Objective: Performing high quality cardiopulmonary resuscitation (CPR) is important for improving the survival rate with a good neurological outcome and fewer complications. The retention of accurate CPR knowledge is essential for providing high quality CPR. This study examined the effects of chest compression only CPR training on the retention of correct CPR knowledge. Methods: In December 2016, an interview survey to target the study population was conducted by trained interviewers, using a structured questionnaire. The respondents' general characteristics, status of CPR education, and knowledge and willingness regarding CPR were investigated. Pearson's chi-square tests and multivariate logistic regression analyses were used to determine which education-related factors affected the correct skill knowledge of performing CPR. Results: Among the respondents, there are 80 persons (17.4%) who answered correctly in the questions regarding the skills of performing CPR. The respondents who had a willingness to perform CPR to family and strangers were 90.2% and 44.9% respectively. Through multivariable analysis, the factors related to correct skill knowledge in performing CPR in the didactic with practice group were people who had undergone CPR training within 2 years (odds ratio [OR], 2.293; 95% confidence interval [CI], 1.311-4.009), and person who had undergone chest compression only CPR training (OR, 2.044; 95% CI, 1.033-4.042). Conclusion: Chest compression only type of CPR training and the experience of CPR education within 2 years were associated with accurate skill knowledge of performing CPR.

이송 중 구조자용 벨트(EMT-belt) 착용에 대한 가슴압박 성공률 비교 (Comparison of success rates of chest compressions performed with and without a rescuer's belt during transfer)

  • Purpose: The aim of this study was to provide basic data to improve the survival rate of pre-hospital cardiac arrest patients. This study suggests a more effective method of performing effective chest compressions for a cardiac arrest patient in a moving ambulance. Methods: To compare the differences between gender and license (qualification), SPSS 18.0 (Windows) was used. Independent and paired t-tests were used for differences between before and after wearing a rescuer's belt. Results: The success rate of chest compressions according to gender was higher in males ($68{\pm}21.91%$) than in females ($25.04{\pm}16.88%$). There was no difference according to license ($44.70{\pm}26.63$ for paramedic, $45.05{\pm}19.25$ for nurse). However, the depth (mm) and the success rate (%) were improved during the evaluation of chest compressions when wearing the rescuer's belt (depth: $46.95{\pm}6.49$ vs. $49.55{\pm}6.05$, success rate: $44.80{\pm}24.66$ vs. $57.39{\pm}26.823$). Conclusion: Wearing a rescuer's belt in an ambulance during patient transport can result in deeper and more accurate chest compressions; therefore, it is expected to be effective in recovering the circulation of patient with cardiac arrest.

  • The purpose of this study is to investigate the effect of high quality CPR using the 4 types of rescue device equipment and chest compressions energy measurement in pre-hospital settings. So, we used the mode to insert load cell in ALS Skill master Manikin to develop CFMM(Compression Force Measurement Manikin) on main stretcher, CPR board, long spine board, scoop stretcher and floor. And, our research team could know that the main stretcher needed average force of 32.55 (${\pm}1.01$) kg, CPR board of 27.23 (${\pm}1.08$) kg, long spine board of 27.13 (${\pm}1.18$) kg, Scoop Stretcher of 27.38 (${\pm}1.05$) kg and Floor of 27.24 (${\pm}0.93$) kg. CPR board must be necessary in the case of CPR on main stretcher in a moving ambulance. But if the condition of patient's back surface is the removable stretcher and the long spine plate, the patient doesn't have to be spent time to use a CPR board. Furthermore, this research suggests to consider that how to take advantage of the education to students for the equipment to check in real time the energy(kg) requirement of chest compressions.

  • Cardiopulmonary resuscitation (CPR) is a series of life-saving actions that improve the chances of survival, following cardiac arrest. Successful resuscitation, following cardiac arrest, requires an integrated set of coordinated actions represented by the links in the Chain of Survival. The links include the following: immediate recognition of cardiac arrest and activation of the emergency response system, early CPR with an emphasis on chest compressions, rapid defibrillation, effective advanced life support, and integrated post-cardiac arrest care. The newest development in the CPR guideline is a change in the basic life support sequence of steps from "A-B-C" (Airway, Breathing, Chest compressions) to "C-A-B" (Chest compressions, Airway, Breathing) for adults. Also, "Hands-Only (compression only) CPR" is emphasized for the untrained lay rescuer. On the basis of the strength of the available evidence, there was unanimous support for continuous emphasis on high-quality CPR with compressions of adequate rate and depth, which allows for complete chest recoil, minimizing interruptions in chest compressions and avoiding excessive ventilation. High-quality CPR is the cornerstone of a system of care that can optimize outcomes beyond return of spontaneous circulation (ROSC). There is an increased emphasis on physiologic monitoring to optimize CPR quality, and to detect ROSC. A comprehensive, structured, integrated, multidisciplinary system of care should be implemented in a consistent manner for the treatment of post-cardiac arrest care patients. The return to a prior quality and functional state of health is the ultimate goal of a resuscitation system of care.
