액와접근법을 통한 제 1 늑골 절제술로 치료한 흉곽출구 증후군 - 증례보고 - (Transaxillary Approach for First Rib Resection to Relieve Thoracic Outlet Syndrome - A Case Report -)

  • The authors report a case of thoracic outlet syndrome in left side. Thoracic outlet syndrome is a collective term in which symptoms and signs are due to bony and soft tissue compression of the neurovascular bundle at the thoracic outlet. It is a rare case in neurosurgical field. So we had experienced one case of thoracic outlet syndrome which was improved by transaxillary approach for resection of first rib. The clinical features, diagnostic test, radiological findings, and operative technique are presented with review of literatures.

  • Recently, the Image compression methods in cooperation with Human Visual System characteristics are being investigated lively. In this study, we propose a new block coding method using i) non-linearity of HVS in the noticeable differences, ii) spatial easting effects, and iii) HVS sensitivity curve according to spatial frequencies. For vector quantization, the third scheme was dole effective than existing ones. We have applied this method on chest images and the results have shown better subjective qualities.

심부정맥 혈전증과 폐전색증을 동반한 매독성 대동맥류 치험 1례 (Syphilitio Abdominal Aortic Aneurysm Associated with Deep Vein Thrombosis and Pulmonary Embolism)

  • A rare syphilitic abdominal aortic aneurysm associated with pulmonary embolism and deep vein throbosis is reported. We have experienced a huge infrarenal syphilitic abdominal aortic aneurysm which caused venous compression at left common iliac vein. A 29 year-old female was admitted via emergency room due to several episodes of hemoptysis. Clinical evaluation for this patient revealed a couple of small ill-defined masss densities on the both lung field and abominal aortic anuerysm. Emergency wedge resections of left upper and lower lobes were performed because of a massive hemoptysis. Pathologic diagnosis showed pulmonary infarction. 20 days after thoracotomy an elective operation on abdominal aortic anuerysm was successfully carried out and post operative pathology finding showed syphilitic aortitis.

종격동 해면상 혈관종: 1례 보고 (Cavernous Hemangioma of Mediastinum: A Case Report)

  • The cavernous hemangioma often occur on the skin and mucosal surfaces of the body, but are also found in many viscera, particularly the liver, spleen, pancreas and occasionally in the brain. But it is rarely encountered in the mediastinum, but when found occur predominantly in the anterior mediastinum. It can occur in any age and have no characteristic symptoms or roentgenographic findings including angiocardiography. Inspite of its histologic benignancy, it may be locally invasive and can result in rib erosion or adjacent structural compression. Usually, surgical exploration is not only the sole means of assuring a diagnosis and the only treatment. Recently, we experienced one case of cavernous hemangioma in the anterior mediastinum, which was removed surgically, being proved to be cavernous hemangioma on histologic examination. Related literatures were reviewed.

  • Successful emergency pulmonary embolectomy with the cardiopulmonary bypass was performed in a 41 \ulcorneryear old male who suffered massive pulmonary embolism after longterm bed rest due to the injury of left knee. Temporary cardiopulmonary bypass provided 120 minutes of circulatory support while complete removal of bilateral pulmonary emboli accomplished using Forgarty catheter and Gall stone forceps. Also, manual compression of the lungs was necessary to remove distal branching emboli. The patient had smooth and uneventful hospital course without complications and discharged from hospital taking coumadine on the 13th day after the operation.

  • Manual or mechanical compression followed by 4 to 8 hours of bed rest is still the standard technique for accessing site management of the femoral arterial puncture site. But these methods are often uncomfortable and delay hospital discharge. Recently, a number of new devices to achieve hemostasis have been developed. These devices uses collagen to facilitate local hemostasis. But many complications associated with the use these devices have been reported internationally. We present a case of successful treatment of Rt. femoral and Rt. popliteal arteries thrombosis caused by Angioseal.

종격동에 발생한 악성변환 다발성 신경섬유종증 1례 (Von Reklinghausen's Disease Associated with Neurofibrosarcoma in Mediastinum)

  • Neurofibromatosis [Von Recklinghausen`s disease] is a rare Mendelian dominant disease, which shows multiple generalized symptoms and signs at various sites [Ex Skin, Bone, Nerve, Endocrine, Mediastinum rarely Lung, etc.]. We experienced one case of neurofibromatosis which has typical skin lesions [cafe-au-lait, multiple nodules, axillary freckling] with neurofibrosarcoma [malignant change from mediastinal lesion]. Patient was admitted our department because of recently developed severe dyspnea which was probably due to main tracheal compression by mediastinal neurofibrosarcoma. After successful removal of mediastinal mass dyspnea disappeared completely. Patient`s postoperative course was uneventful, and the patient was discharged 14 days after operation.

갑상선 종양에 의한 기도협착 치험 -1례 보고- (Circumferential Resection and Direct End to End Anastomosis of Tracheal Stenosis Invaded by Thyroid Carcinoma.)

  • The patient was 22-year old male who had been suffered from labored breathing. Computerized tomography, tracheoscopy, tracheogram disclosed tracheal obstruction by external mass compression on Tl level, which ranged 4.Oem in the length and approximately 4mm in diameter on tracheogram. Under the local anesthesia, tracheostomy was done to prevent intraoperative airway obstruction. And general anesthesia, low collar incision 8z extended median sternotomy was made and nearly total thyroidectomy was performed. After the circumferential resection of the obstructed segment approximately 4cm in length[7 tracheal rings], direct end-to-end anastomosis of trachea was performed. Postoperatively, the patient`s neck was maintained in flexion state to reduce tension of anastomotic site. Postoperatively. medical therapy[Comthyroid k Calcium lactate] and radiotherapy were done. At present, 2-months after operation, he lives well.

  • Postpneumonectomy syndrome (PPS) is a rare late complication of pneumonectomy. It occurs more often in children than in adults, and is characterized by respiratory failure resulting from bronchial compression caused by severe mediastinal shift. Various methods have been used to treat PPS, including aortopexy and the insertion of plastic balls, silastic implants, and saline-filled breast prostheses. We describe two cases of PPS corrected with tissue expanders after right pneumonectomy in patients with esophageal atresia.

  • Insertion of an epicardial pacemaker is a useful treatment for pediatric patients with an abnormal heart rhythm. However, there are limitations and concerns when implanting epicardial pacemakers in infants and neonates due to their small body size. We report a patient who experienced rare complications after implantation of a permanent pacemaker.
