We introduce an implementation of plug-ins on PLUTO. These plug-ins discriminate inflammatory nodules from other types of nodules in chest X-ray CT images. The PLUTO is a common platform for computer-aided diagnosis systems on Microsoft Windows series and it is easy to add new functions as plug-ins. We coded two plug-ins. One of the them calculates features based on medical knowledge. The other plug-in calculates parameters to classify the type of nodules, and it also classifies nodules into inflammatory nodules and others using SVM. These plug-ins are coded using MIST library which is produced at Nagoya University, Japan. In our previous study, the MIST library was parallelized, so that we can utilize a number of CPUs to calculate features and SVM learning/classifying depending on the amount of computation. Using these plug-ins, it became easy to extract features to discriminate inflammatory nodules from other types of nodules and to change parameters for feature extraction and SVM learning/classifying with GUI interface. The accuracy of the classifying result is 100% with 78 solid nodules which contains 43 inflammatory nodules and 35 other type of nodules.

Objective: To extract pulmonary and cardiovascular metrics from chest CTs of patients with coronavirus disease 2019 (COVID-19) using a fully automated deep learning-based approach and assess their potential to predict patient management. Materials and Methods: All initial chest CTs of patients who tested positive for severe acute respiratory syndrome coronavirus 2 at our emergency department between March 25 and April 25, 2020, were identified (n = 120). Three patient management groups were defined: group 1 (outpatient), group 2 (general ward), and group 3 (intensive care unit [ICU]). Multiple pulmonary and cardiovascular metrics were extracted from the chest CT images using deep learning. Additionally, six laboratory findings indicating inflammation and cellular damage were considered. Differences in CT metrics, laboratory findings, and demographics between the patient management groups were assessed. The potential of these parameters to predict patients' needs for intensive care (yes/no) was analyzed using logistic regression and receiver operating characteristic curves. Internal and external validity were assessed using 109 independent chest CT scans. Results: While demographic parameters alone (sex and age) were not sufficient to predict ICU management status, both CT metrics alone (including both pulmonary and cardiovascular metrics; area under the curve [AUC] = 0.88; 95% confidence interval [CI] = 0.79-0.97) and laboratory findings alone (C-reactive protein, lactate dehydrogenase, white blood cell count, and albumin; AUC = 0.86; 95% CI = 0.77-0.94) were good classifiers. Excellent performance was achieved by a combination of demographic parameters, CT metrics, and laboratory findings (AUC = 0.91; 95% CI = 0.85-0.98). Application of a model that combined both pulmonary CT metrics and demographic parameters on a dataset from another hospital indicated its external validity (AUC = 0.77; 95% CI = 0.66-0.88). Conclusion: Chest CT of patients with COVID-19 contains valuable information that can be accessed using automated image analysis. These metrics are useful for the prediction of patient management.

In this paper, the algorithm that can automatically segment the lung, the airway and the pulmonary vessels in a chest CT was proposed. The proposed method is progressed in three steps. In the first step, the lung and the airway are segmented by the region growing law through the optimal threshold and three-dimensional labeling. In the second, from the start point to the first carina of the airway is segmented by the deduction operation, and the next airway of the bifurcations are segmented by applying a variable threshold technique. In the third step, the left/right lungs are divided by the restoration process for the lung, and the outside of lungs for abnormal is checked by applying the advanced rolling ball algorithm, and if abnormal is found, that part is removed, and it is restored to the normal lungs by connecting the outside of the lung in the form of second-order polynomial. Finally, pulmonary vessels are segmented by applying the three-dimensional connected component labeling method and three-dimensional region growing method. As the results of simulation, it could be confirmed that the pulmonary vascular is accurately divided without loss of tissue around lung.
In the diagnosis of lung cancer, the tumor size is measured by the longest diameter of the tumor in the entire slice of the CT. In order to accurately estimate the size of the tumor, it is better to measure the volume, but there are some limitations in calculating the volume in the clinic. In this study, we propose an algorithm to segment lung cancer by applying a custom loss function that combines focal loss and dice loss to a U-Net model that shows high performance in segmentation problems in chest CT images. The combination of values of the various parameters in custom loss function was compared to the results of the model learned. The purposed loss function showed F1 score of 88.77%, precision of 87.31%, recall of 90.30% and average precision of 0.827 at α=0.25, γ=4, β=0.7. The performance of the proposed custom loss function showed good performance in lung cancer segmentation.

Recently, digital tomosynthesis system (DTS) has been developed to reduce overlap using conventional X-ray and to overcome high patient dose problem using computed tomography (CT). The purpose of this study was to develop image reconstruction algorithm and to evaluate image characteristics and dose with chest digital tomosynthesis (CDT) system. Image reconstruction was used for filtered back-projection (FBP) methods and system geometry was constructed ${\pm}10^{\circ}$, ${\pm}15^{\circ}$, ${\pm}20^{\circ}$, and ${\pm}30^{\circ}$ angular range for acquiring phantom images. Image characteristics carried out root mean square error (RMSE) and signal difference-to-noise ratio (SDNR), and dose is evaluated effective dose with ${\pm}20^{\circ}$ angular range. According to the results, the phantom image with slice thickness filter has superb RMSE and SDNR, and effective dose was 0.166 mSv. In conclusion, we demonstrated usefulness of developed CDT image reconstruction algorithm and we constructed CDT basic output data with measuring effective dose.
Following the appearance of the latest medical equipment with improved function, the importance of image analysis which enables effective image processing and analysis consistent with the hardware performance is on the rise. As well as, ongoing study is being done on the 2D medical image processing and 3D reconstruction. This paper segments chest CT images into each stage and finally shows 3D reconstruction of each segmented result. Among various image segmentation methods, Region Growing and apply sharpening and Gamma Controller as for image improvement for effective segmentation, image segmentation in order of bronchus and lung, bronchus, lung. Human organs image of segmented is use VTK(Visualization Toolkit) to make 3D reconstruction, two and three-dimensional medical image processing and analysis for lesions diagnosis are able to utilized.
Purpose: The purpose of this study has attempted to evaluate and compare the image evaluation and exposure dose by respectively applying Filtered Back Projection(FBP), the existing test method, and Adaptive Statistical Iterative Reconstruction(ASIR) with different values of tube voltage during the Low Dose Computed Tomography(LDCT). Materials and Methods: With the image reconstruction method as basis, Chest Phantom was utilized with the FBP and ASIR set at 10%, 20% respectively, and the change of Tube Voltage (100kVp, 120kVp). For image evaluation, Back ground noise, Signal to Noise ratio(SNR) and Contrast to Noise ratio(CNR) were measured, and, for dose evaluation, CTDIvol and DLP were measured respectively. The statistical analysis was tested with SPSS(ver. 22.0), followed by ANOVA Test conducted after normality test and homogeneity test. (p<0.05). Results: In terms of image evaluation, there was no outstanding difference in Ascending Aorta(AA) SNR and Infraspinatus Muscle(IM) SNR with the different values of ASIR application(p<0.05), but a significant difference with the different amount of tube voltage(p>0.05). Also, there wasn't noticeable change in CNR with ASIR and different amount of Tube Voltage (p<0.05). However, in terms of dose evaluation, CTDIvol and DLP showed contrasting results(p<0.05). In terms of CTDIvol, the measured values with the same tube voltage of 120kVp were 2.6mGy with No-ASIR and 2.17mGy with 20%-ASIR respectively, decreased by 0.43mGy, and the values with 100kVp were 1.61mGy with No-ASIR and 1.34mGy with 20%-ASIR, decreased by 0.27mGy. In terms of DLP, the measured values with 120kVp were $103.21mGy{\cdot}cm$ with No-ASIR and $85.94mGy{\cdot}cm$ with 20%-ASIR, decreased by $17.27mGy{\cdot}cm$(about 16.7%), and the values with 100kVp were $63.84mGy{\cdot}cm$ with No-ASIR and $53.25mGy{\cdot}cm$ with 20%-ASIR, a decrease by $10.62mGy{\cdot}cm$(about 16.7%). Conclusion: At lower tube voltage, the rate of dose significantly decreased, but the negative effects on image evaluation was shown due to the increase of noise. For the future, through the result of the experiment, it is considered that the method above would be recommended for follow-up patients or those who get health checkup as long as there is no interference on the process of diagnosis due to the characteristics of Low Dose examination.
We propose an automatic segmentation method for identifying pulmonary structures using gray-level information of chest CT images. Our method consists of following five steps. First, to segment pulmonary structures based on the difference of gray-level value, we select the threshold using optimal thresholding. Second, we separate the thorax from the background air and then the lungs and airways from the thorax by applying the inverse operation of 2D region growing in chest CT images. To eliminate non-pulmonary structures which has similar intensities with the lungs, we use 3D connected component labeling. Third, we segment the trachea and left and right mainstem bronchi using 3D branch-based region growing in chest CT images. Fourth, we can obtain accurate lung boundaries by subtracting the result of third step from the result of second step. Finally, we select the threshold in accordance with histogram analysis and then segment radio-dense pulmonary vessels by applying gray-level thresholding to the result of the second step. To evaluate the accuracy of proposed method, we make a visual inspection of segmentation result of lungs, airways and pulmonary vessels. We compare the result of the conventional region growing with the result of proposed 3D branch-based region growing. Experimental results show that our proposed method extracts lung boundaries, airways, and pulmonary vessels automatically and accurately.
Objective: To investigate the image quality of ultralow-dose CT (ULDCT) of the chest reconstructed using a cycle-consistent generative adversarial network (CycleGAN)-based deep learning method in the evaluation of pulmonary tuberculosis. Materials and Methods: Between June 2019 and November 2019, 103 patients (mean age, 40.8 ± 13.6 years; 61 men and 42 women) with pulmonary tuberculosis were prospectively enrolled to undergo standard-dose CT (120 kVp with automated exposure control), followed immediately by ULDCT (80 kVp and 10 mAs). The images of the two successive scans were used to train the CycleGAN framework for image-to-image translation. The denoising efficacy of the CycleGAN algorithm was compared with that of hybrid and model-based iterative reconstruction. Repeated-measures analysis of variance and Wilcoxon signed-rank test were performed to compare the objective measurements and the subjective image quality scores, respectively. Results: With the optimized CycleGAN denoising model, using the ULDCT images as input, the peak signal-to-noise ratio and structural similarity index improved by 2.0 dB and 0.21, respectively. The CycleGAN-generated denoised ULDCT images typically provided satisfactory image quality for optimal visibility of anatomic structures and pathological findings, with a lower level of image noise (mean ± standard deviation [SD], 19.5 ± 3.0 Hounsfield unit [HU]) than that of the hybrid (66.3 ± 10.5 HU, p < 0.001) and a similar noise level to model-based iterative reconstruction (19.6 ± 2.6 HU, p > 0.908). The CycleGAN-generated images showed the highest contrast-to-noise ratios for the pulmonary lesions, followed by the model-based and hybrid iterative reconstruction. The mean effective radiation dose of ULDCT was 0.12 mSv with a mean 93.9% reduction compared to standard-dose CT. Conclusion: The optimized CycleGAN technique may allow the synthesis of diagnostically acceptable images from ULDCT of the chest for the evaluation of pulmonary tuberculosis.
In this paper, we propose a hybrid approach for segmenting the lungs efficiently and automatically in chest CT images. The proposed method consists of the following three steps. first, lungs and airways are extracted by two- and three-dimensional automatic seeded region growing and connected component labeling in low-resolution. Second, trachea and large airways are delineated from the lungs by two-dimensional morphological operations, and the left and right lungs are identified by connected component labeling in low-resolution. Third, smooth and accurate lung region borders are obtained by refinement based on image subtraction. In experiments, we evaluate our method in aspects of accuracy and efficiency using 10 chest CT images obtained from 5 patients. To evaluate the accuracy, we Present results comparing our automatic method to manually traced borders from radiologists. Experimental results show that proposed method which use connected component labeling in low-resolution reduce processing time by 31.4 seconds and maximum memory usage by 196.75 MB on average. Our method extracts lung surfaces efficiently and automatically without additional processing like hole-filling.
