완벽주의와 시험불안이 시험기간 중 여고생의 월경양상 변화에 미치는 영향 (Impact of Perfectionism and Testing Anxiety on the Menstrual Cycle during Test Evaluations among High School Girls)

  • Purpose: Dysmenorrhea is one of the most common gynecologic complaints of young women. This study investigated the changes in the menstrual cycle during test evaluations according to perfectionism and testing anxiety among high school girls in order to provide information for care of dysmenorrhea. Method: Data collection was done from July 14 to 18, 2008 by self administered questionnaires with 300 high school girls. Result: The subjects experienced amenorrhea(35.3%), changes in the menstrual cycle(66.3%), changes in menstrual duration(50.0%), changes in amount of menstruation(47.7%), and increases in menstrual pain during testing evaluations. There were significant differences in changes of menstrual patterns, physical symptoms, and menstrual pain during test evaluations between a high perfectionist group and low perfectionist group. Also, there were significant differences in changes of menstrual patterns, physical symptoms, and menstrual pain during test evaluations between a high testing anxiety group and low testing anxiety group. Perfectionism and testing anxiety showed a positive relationship with changes in menstrual patterns, physical symptoms, and menstrual pain. Conclusion: As perfectionism and testing anxiety seem to contribute to changes in the menstrual cycle during test evaluations, school health-care providers should screen routinely for dysmenorrhea and give more attention to health education on this topic to girls.

여성 호르몬의 변화가 치은 섬유아세포와 치주인대세포의 교원질 분해 효소의 활성에 미치는 영향 (In Vitro Effects of Female Sex Hormones on Collagenase Activity of Gingival Fibroblast and Periodontal Ligament Fibroblast)

  • Many factors may affect periodontal changes during the physiologic conditions of woman(e.g. puberty, menstrual cycle, pregnancy, menopause). Recently many research has focused on the immunological changes of host, but the exact mechanism is not clear. Collagen is a major constituent of periodontium, and collagenase specifically digests the collagen and plays a role in destruction of periodontal tissue. So, I suppose that it participates with the cytokines in the inflammation of gingiva and vascular response during the changes of female sex hormones. Because there are some evidences of the existence of the receptors of estrogen and progesterone in the gingiva, it may be a target tissue of female sex hormones. In this experiment, gingival fibroblast and periodontal ligament cell were cultured in the presence of various concentrations of estrogen or progesterone corresponding to the menstrual cycle and pregnancy. Collagenase activity of the supernatant of culture media was determined by Spectrophotometric collagenase assay. The enzyme activity was calculated by the % decrease of the coated collagen. 1. The estrogen at both concentrations had no effect on the activity of collagenase of the gingival fibroblast. 2. The progesterone had some effect on the collagenase activity of the gingival fibroblast at low and high concentration of menstrual cycle, and elevated the enzyme activity at all range of pregnancy concentrations. 3. In periodontal ligament cells, estrogen elevated the enzyme activity at the early pregnancy concentration and progesterone elevated at the concentration just before menstruation. In this experiment, pregesterone elevated the collagenase activity of gingival fibroblast and periodontal ligament cells. But the mechanism of the up-regulation of the enzyme activity was not confirmed. The more experiments of direct effect of progesterone on gingival at the molecular level(e.g. northern blot analysis) can reveal the exact mechanism.

2019년도 충청남도 한방 난임치료 지원사업에 대한 결과 보고 (A Study Reporting the Korean Medicine Treatment Infertility Support Program in Chungcheongnam-do in 2019)

  • Objectives: The purpose of this study is to report the results of Korean medicine treatment infertility support program in Chungcheongnam-do in 2019. Methods: Participants of Korean medicine treatment infertility support program were provided with Korean herbal medicine, acupuncture and moxibustion for 3 months and followed up for 3 months after the treatments. A total of 115 women out of 125 applicants completed the treatment, and 106 participants' data was collected. The data containing pregnancy rates, characteristics of subjects, treatments, and satisfaction of the program were analyzed. Results: After the treatment, 18 out of 115 subjects became pregnant and had a pregnancy rate of 15.7%. There were no significant differences in factors such as age, body mass index (BMI), social history, infertility period, and history of infertility treatment between pregnant and non-pregnant groups except smoking. Some of the subjects reported positive changes in the menstrual conditions such as cycle, volume and menstrual pain. More than 80% of the subjects answered that they were satisfied with the program in the survey questionnaire. Conclusions: Based on this report, the clinical data were obtained from the infertility treatment program. This study can be used as basic data to help establish infertility support programs and subsequent policies.

항암화학요법을 받은 유방암판자의 무월경 발생과 갱년기 증상 관련요인 (Factors of Occurrence of Amenorrhea and Climacteric Symptoms in Breast Cancer Patients Underwent Chemotherapy)

  • Purpose: This study aimed to investigate the factors of occurrence of amenorrhea and the severity of climacteric symptoms in breast cancer patients who underwent chemotherapy. Method: Women diagnosed with breast cancer without metastasis or recurrence, had surgery followed by chemotherapy, and had menses at the time of surgery were recruited from S hospital located in Seoul. A total of 99 women aged 31 thru 55 years participated and filled out a structured questionnaire including the Functional Assessment Cancer Therapy-Breast plus Endocrine Symptom when they visited the clinic for follow-up. Result: In 88 women amenorrhea occurred within an average of 2 months since beginning chemotherapy, and menstruation was resumed in only 11 women. About 98% of women aged over 40 experienced a cessation in menses thus age was an apparent factor of amenorrhea (Exp(B)=.76, p<.05). Presence of chronic disease (${\beta}=.25$, p<.05) and body weight change (${\beta}=.30$, p<.01) were significant factors influencing the severity of climacteric symptoms. Conclusion: Nurses need to have clinical evidences of menstrual changes due to breast cancer treatment. Information about premature menopause and climacteric symptoms should be provided according to women's health conditions so that they cope better during their survival.

DNA Array Analysis of Changes in Gene Expression Profile in DHEA-induced PCO

  • Under normal conditions, women produce a single dominant follicle that participates in a single ovuation each menstrual cycle. But Polycystic ovary syndrome(PCOS) conditions, folliculogenesis does not proceed normally. This condition leads to the accumlation of large numbers of small graffian follicles in which the theca interstitial cells (TIC) produce abnormally large amounts of androgen. PCOS is probably the most common endocrine disorder, affecting women of reprodutive age with 5-10% prevalence estimate. Chronic anovulation, hyperandrogenism, hirsutism, obesity, infertility and polycystic ovaries are clinical hallmarks of women with PCOS. Its etiology remains unknown. To investigate the gene expression pattern of ovary in PCO-induced rat, we used cDNA expression analysis. Total RNA was extracted from the ovary of PCO-induced rat and reverse-transcribed in the presence of[$\alpha$$^{32}$P]-dATP Which were hybridized to Atlas$^{TM}$ Rat Toxicology 1.2 array (Clontech) representing approximately 1176 rat genes. We compared gene expression between ovary of pco-induced immature female rats and control. Differential gene expression profiles were revealed (LIFR-alpha, ADRA1A, Heat shock 90-kDa protein A, PDGFRA). Reverse transcription-polymerase chain reaction(RT-PCR) was used to validate the relative expression pattern obtained by the cDNA array. The precise relationship between the altered expression of genes and PCO is a matter of further investigation. This study was supported by Korea Science and Engineering Foundation(KOSEF)

자궁내막증 환자의 자궁내막내 성호르몬 수용체, Integrins, Cyclooxygenase의 발현과 변이 양상 (Expression Pattern of Progesterone Receptor, Integrin, Cyclooxygenase (COX) in Human Endometrium of Patients with Endometriosis)

  • Objectives: To develop a new immunohistochemical marker system for supplementation of the Noyes histological classification of the endometrium in women of child bearing age with regular menstrual cycles, and to employ this system to evaluate pathologic factors involved in endometriosis, and thus to ascertain if it is useful in diagnosis. Materials and Methods: Endometrial biopsies were sampled from the posterior fundus of 41 (24 proliferative phases, 17 secretory phases) women with regular menstrual cycles (28-32 days), and each sample was immunohistochemically stained according to Noyes et al (1975) for determination of expression for estrogen receptor (ER), progesterone receptor (PR), integrin ${\alpha}_1$, ${\alpha}_4$, ${\beta}_3$, COX-1 and COX-2. Then, the PR, integrin ${\beta}_3$ and COX-2 which were clearly expressed in the luteal phase was with endometrial samples were obtained from 20 cases of normal patients (group 1) and 25 cases with endometriosis (group 2) after confirming the day of ovulation by sex steroid level measurements 7-8 days after ovulation Results: In the regular menstruation group the expression of ER showed a tendency to be increased in the proliferative phase and decreased in the secretory phase, and was the highest in the proliferative phase. However, PR in the stromal cells showed no change in the entire menstrual cycle while in the epithelial cells, PR reached a peak in the late proliferative phase and was almost absent in the secretory phase. Integrin (${\alpha}_1$, ${\alpha}_4$, and ${\beta}_3$ expression in the epithelial cells was absent in the proliferative phase but ${\alpha}_1$ was strongly expressed starting from the early secretory phase into the entire secretory phase. ${\alpha}_4$ was expressed strongly in the early and mid secretory phases and disappeared in the late proliferative phase, while ${\beta}_3$ appeared after the mid secretory phase and continued to be expressed until the late secretory phase. Expression in the stromal cells was weak overall and did not show any cyclic pattern. COX-1 expression was shown as a cyclic pattern in the stromal and epithelial cells and was particularly strongly expressed in the mid secretory phase of epithelial cells, and in the mid secretory and menstruation phase of stromal cells. In the endometrial epithelial cells there was strong expression during the entire cycle with stronger expression in the secretory phase compared to the prolferative phase. COX-2 was clearly expressed in the late proliferative, early and mid secretory phases in the stromal cells. No expression was observed in the proliferative phase of the epithelial cells, but which began to appear in the early secretory phase reaching a significant pattern from the mid secretory phase onwards. There was almost no expression in the stromal cells. In the cases with endometriosis showing normal endometrial maturation according to the Noyes classification, PR expression was increased while Integrin-${\beta}_3$의 expression was significantly decreased compared to the normal group. Also, COX-2 expression was slightly decreased in the stromal cells of patients with endometriosis while it was significantly increased in the stromal cells. Conclusion: Immunohistochemical markers can supplement the original Noyes classification of histological endometrial dating and therefore ascertain existing pathologic conditions. Particularly for patients with endometriosis with normally mature endometrial cells, changes in COX-2 and integrin expression patterns may assist in elucidating pathophysiologic mechanisms and therefore aid in the diagnosis of abnormal implantation conditions, and consequently determine a treatment modality.
