치료적 마사지와 맥켄지 운동이 만성 경부통 환자의 통증과 관절가동범위에 미치는 영향: 단일사례연구 (The Effect of Therapeutic Massage and Mckenzie Exercise on Pain and Range of Motion in Chronic Neck Pain Patient: a Case Study)

  • Purpose : The purpose of this study was to examine the effect of therapeutic massage and Mckenzie exercise on pain and range of motion in chronic neck pain patient: a case study. Method : A Twenty-year old female subject with chronic neck pain was selected, she received therapeutic massage and Mckenzie exercise for 30 minutes a day, three days a week for five weeks. Pain was measured visual analog scale(VAS), neck disability index(NDI) and the Copenhagen neck functional disability scale (CNFDS). Range of motion was measured using a goniometer(shoulder joint) and the CROM Deluxe(cervical range of motion instrument). Result : VAS, NDI and CNFDS were decreased 3 score, 4 score and 6 score(respectively) than before the training. The range of motion was increased in shoulder joint and neck movement than before the training. Conclusion : The massage and Mckenzie exercise increased range of motion in shoulder joint and neck movement, and decreased neck pain in chronic neck pain patient. Additional research on therapeutic massage and Mckenzie exercise for improving pain and range of motion is need.

마사지 및 정적 스트레칭이 20대 정상 성인의 경부 관절가동범위에 미치는 효과 (The Effects of Massage and Static Stretching on Cervical Range of Motion in Their 20s of Normal Adult)

  • 본 연구의 목적은 20대의 정상 성인에서 경부의 관절가동범위와 마사지 및 정적 스트레칭이 관절가동범위에 미치는 영향을 측정하는 것이다. 근골격계와 신경계의 질환이 없는 100명(마사지그룹=50, 스트레칭그룹=50)을 대상으로 실시하였다. 마사지와 정적 스트레칭은 흉쇄유돌근, 사각근, 승모근, 반극근, 판상근, 후두하근, 다열근과 회선근에 적용되었다. 두 그룹은 주 3회의 중재를 받았다. 중재 시간은 10분이었다. 마사지그룹은 경찰법, 유날법 및 스트라이핑 마사지가 사용되었고, 스트레칭그룹은 정적 스트레칭이 사용되었다. 경부의 가동범위 측정도구는 경부의 8가지 동작(후두하 굽힘과 폄, 경부의 굽힘과 폄, 왼쪽 옆굽힘과 오른쪽 옆굽힘, 왼쪽 돌림과 오른쪽 돌림)을 분석하기 위하여 사용되었다. 통계학적 분석결과는 다음과 같다. 첫째, 정상적인 경부의 관절가동범위는 남자에서 후두하굽힘과 폄이 $2.39^{\circ}$38.36^{\circ}$, 경부의 굽힘과 폄이 $54.11^{\circ}$69.39^{\circ}$, 왼쪽과 오른쪽의 옆굽힘이 $43.50^{\circ}$$41.28^{\circ}$, 왼쪽과 오른쪽의 돌림이 $66.39^{\circ}$$65.94^{\circ}$로 나타났고, 여자에서는 후두하 굽힘과 폄이 $5.14^{\circ}$$36.47^{\circ}$, 경부의 굽힘과 폄이 $55.92^{\circ}$$71.22^{\circ}$, 왼쪽과 오른쪽의 옆굽힘이 $43.34^{\circ}$$41.06^{\circ}$, 왼쪽과 오른쪽의 돌림이 $69.38^{\circ}$$68.63^{\circ}$로 나타났다. 둘째, 후두하 굽힘, 왼쪽 돌림과 오른쪽 돌림에서 여성이 남성보다 더 높은 관절가동범위를 보였다(p<0.05). 셋째, 마사지군과 스트레칭군은 치료 후에 모든 항목에서 관절가동범위의 증가를 보였지만 두 그룹간의 비교에서는 유의한 차이를 보이지 않았다(p<0.05). 위의 결과는 마사지와 정적 스트레칭이 경부 근육의 신장 및 이완을 통해 가동범위를 증가시키는 적당한 방법이라는 것을 제시한다. 그리고 경부의 가동범위를 조사하는 연구의 기초로 제공될 수 있을 것이다.

안와 골절로 유발된 복시 1례에 대한 한방치료 증례보고 (A Case Report of Diplopia after an Orbital Wall Fracture Treated with Korean Medicine Treatment)

  • Objectives : The purpose of this report is to introduce a case of diplopia caused by the fracture of the left orbital wall and to suggest Korean Medicine treatment including electroacupuncture as a possible method of conservative treatment. Methods : A 44-year-old male with diplopia, restricted ocular motility and facial paresthesia after the fracture of the left orbital wall was treated with Korean Medicine treatment from June 15, 2016 to July 15, 2016. Improvements of symptoms were measured by the diplopia questionnaire, cervical range of motion (CROM) diplopia examination, goniometer diplopia examination, subjective diplopia field and visual analogue scale (VAS). Results : During 4 weeks of treatment, the patient showed consistent improvement in the diplopia questionnaire score, range of diplopia and VAS of facial paresthesia. Conclusion : According to the results, Korean Medicine treatment would be a possible conservative treatment for diplopia due to a traumatic accident. Further studies are needed regarding possible long-term effects.

두부자세와 경추형태에 따른 근활성의 변화에 관한 연구 (Changes of the Electromyographic Activity by Head Posture and Cervical Spine Shape)

  • This study was performed to investigate the effects of changes of head posture and cervical spine shape on the mandibular resting or clenching electromyographic(EMG) activity in anterior temporalis(TA), masseter(MM), sternocleidomastoid muscle(SCM) and trapezius insertion(TI). 30 patients with Temporomandibular Disorders(TMDs) participated in this study. EMG activity($\mu$V) at rest and clenching was observed in four head postures, namely natural head posture(NHP), forward head posture(FHP), upward head posture(UHP), and downward head posture(DHP). For taking in upward or downward head posture head was inclined 10$^{\circ}$ upward or downward and CROM$^\textregistered$(cervical-range-of motion, Performance attainment Inc., USA) was used to maintain same posture during the procedure, and BioEMG$^\textregistered$ (Bioelectromyograph, Bioresearch Inc., USA) was used to record EMG activity in the above four muscles at eight locations on both sides. The recorded EMG activity($\mu\textrm{V}$) were compared and analyzed by cervical spine shape such as the head position from plum line, cervical curvature, and cervical inclination. Head position from plum line was measured in vertical plate calibrated with cm scale, comical curvature by radius was measured with adjustable curved ruler, and cervical inclination by cervical vertebrae tangent(CVT)was measured in lateral cephalograph. The results obtained were as follows : 1. Mean value of head position from plum line, cervical curvature, and cervical inclination were 4.8cm, 26.7cm, and 86.6$^{\circ}$, respectively, And There were no correlationship among these items. 2. For resting EMG activity by head posture, the value in anterior temporalis was higher at FHP than at DHP, the value in masseter was higher at FHP than at NHP, and DHP, the value in sternocleidomastoid muscle was higher at UHP than at NHP, and the value in trapezius insertion was higher at FHP and DHP than, NHP and UHP. The clenching EMG activity, however, did not show any difference by head posture. 3. Comparison of resting and clenching EMG activity between higher and lower groups by head position from plum line, cervical curvature, and cervical inclination did not show any significant difference. From this result, the author concluded that the cervical spine shape had not significantly affected to EMG activity in usual patients with TMDs.

The Effects of Posture on Neck Flexion Angle While Using a Smartphone according to Duration

  • PURPOSE: The purpose of this study was to examine changes in the neck flexion angle according to posture while using a smartphone and the duration of smartphone usage. METHODS: The subjects in this study were 16 healthy young students in their 20s. The subjects wore a cervical range of motion instrument. They were instructed to use a smartphone while standing, sitting on a chair, and sitting on the floor. In all postures, they could use the smartphone and use their arms freely while keeping their back connected to the wall or the back of the chair. When sitting on the floor, they assumed a cross-legged position. The neck flexion angle was measured at zero, three, six, and nine minutes for each posture. RESULTS: Neck flexion is affected by the posture while using a smartphone (p<.05). Neck flexion in the standing position is larger than that in the sitting on the floor position. Neck flexion was affected by smartphone usage duration (p<.05). In general, as usage time increases, the neck flexion angle increases as well. CONCLUSION: This study suggests that using smartphone in the standing position and for a short period of time is a method to reduce the neck flexion angle.

경부통증을 호소하는 고등학생에게 적용한 밸런스 테이핑 요법의 효과 (Effect of Balance Taping Therapy on Neck Pain in High School Students)

  • Purpose: The purpose of this study was to examine the effect of balance taping therapy on neck pain in high school students. Methods: The study employed a randomized control group pretest-posttest design with four-time repeated measures. Data were collected from 62 high school students with neck pain. The experimental group (n=31) took balance taping therapy for six days with appropriate position and stretching education while the control group (n=31) applied patches including non-steroidal anti-inflammatory drugs (NSAIDS) for six days with appropriate position and stretching education. Neck pain, cervical range of motion (CROM) and neck disability were measured at pretest, day one, day three and day six which was the posttest day. Results: For the experimental group, the neck pain was significantly improved on all three days (F=16.82, p<.001), and extension and right lateral flexion of the CROMs were significantly improved over time compared to the control group (F=3.85, p =.011; F=2.71, p=.047, respectively). Neck disability was also improved in the experimental group compared to the control group (F=8.64, p<.001). Conclusion: The balance taping therapy was an efficient intervention for high school students with neck pain. Nurses could apply non-pharmacological interventions such as balance taping therapy without pharmacological side effects.

만성 목-어깨 통증이 있는 여성 성인에게 시청각 매체를 활용한 탄력밴드 저항운동이 통증, 고유수용성 감각과 운동기능에 미치는 영향 (Effects of Elastic Band-Resistive Exercise using Audio-visual Medium on Pain, Proprioceptive Sense, and Motor Function in Adult Females with Chronic Neck and Shoulder Pain)

  • Background: This study aimed to investigate the effect of elastic band-resistive exercise using audio-visual medium on pain, proprioception, and motor function in adults with chronic neck and shoulder pain. Design: One group pretest-posttest follow-up experimental design. Method: Twenty adult women with neck and shoulder pain voluntarily participated in this study. Elastic band-resistive exercise using audio-visual medium including cervical flexion and extension, shoulder external rotation, and scapular retraction-protraction motions was conducted 5 times a week for 3 weeks. The Numerical Rating Scale, pressure threshold tool, CROM goniometer, and Image J software were used to assess subjective pain level, tenderness threshold (pain), joint position sense error (proprioception), joint range of motion, and postural alignment (motor function), respectively. Result:: The pain intensity and threshold and joint position sense error showed significant decreases after the intervention, whereas the joint range of motion angle revealed significant increases. The postural alignment including forward head posture and rounded shoulder revealed significant improvements after the intervention. Conclusions: Therefore, we suggest that elastic band-resistive exercise through audio-visual medium would be helpful in preventing and managing pain and physical dysfunction in individuals with chronic neck and shoulder pain, and then it would support the development of health management-related online education content.

서 있는 자세와 앉은 자세에서 두부자세의 변화 (Changes of Head Posture in Standing and Sitting Posture)

  • This study was performed to investigate the changes of head posture according to natural standing or sitting posture. Twenty seven healthy dental students without any signs and symptoms of temporomandibular disorders participated in this study. Cervical resting posture (CRP) of the head in sagittal plane was measured by Cervical-Range-of-Motion $^\textregistered$(CROM, U.S.A.) and lateral cephalograph was taken in natural posture. The items related to angle in cephalograph were the angles of cranial and cervical inclination to true vertical line(VER/NSL, VER/AML), the angles of cervical inclination to nasion-sella line(CVT/NSL, OPT/NSL), the angles of comical inclination to horizontal line(CVT/HOR, OPT/HOR), the angle of cervical lordosis(CVT/OPT). The items related to line measurement were the distance from subocciput to Cl(Dl), Cl to C2(D2), C2 to C3(D3), C3 to C4(D4), the upper(PNS to posterior pharyngeal wall) and the lower(tongue base to posterior pharyngeal wall) pharyngeal space, the distance from nation to mention(Na-Me), and the radius of comical curvature from the first comical vertebra(Cl ) to the fifth cervical vertebra(C5). The data were analyzed with SAS/STAT program. The obtained results were as follows : 1. Most items related to angular measurement showed significant difference between in standing and sitting posture. The angles of CRP, CVT/NSL, OPT/NSL, CVT/HOR, OPT/HOR, and CVT/OPT were high in sitting posture, but the angles of VER/NSL, VER/NSL were low in sitting posture. 2. In vertebral distance, only the distance between C3 and C4 was differed by the posture, which decreased in sitting posture. In sitting posture, the distance from nasion to menton(Na-Me) was longer, but the radius was shorter than in standing posture. 3. Correlationship in angular measurements was almost same in both postures. Ceervical resting posture(CRP) was correlated with VER/NSL, VER\ulcornerNSL was correlated with CRP, CVT/NSL, and OPT/NSL, VER/AML was correlated with CVT/HOR, OPT/HOR, CVT/OPT, and the angle of cervical lordosis(CVT/OPT) was correlated with the radius. 4. Correlationship in linear measurement was observed only in among D3, D4, and radius. And the Na-Me was not correlated with any other items. From this results, The author concluded that the head posture in sitting was mote backward extended than in standing.

바이오피드백을 이용한 심부목굽힘근운동이 목 질환에 미치는 영향: 메타분석 (Effects of Biofeedback Based Deep Neck Flexion Exercise on Neck Pain: Meta-analysis)

  • Previous studies have reported that deep neck flexor (DNF) exercise can improve neck problems, including neck pain, forward head posture, and headache, by targeting the deep and superficial muscles of the neck. Despite the prevailing opinion across studies, the benefits of DNF can vary according to the type of neck problems and the outcome measures adopted, ranging from positive outcomes to non-significant benefits. A meta-analysis was conducted in this study to assess conclusive evidence of the impact of DNF exercise on individuals with neck problems. We used PUBMED, MEDLINE, NDSL, EMBASE, and Web of Science to search for primary studies and the key terms used in these searches were "forward head posture (FHP)," "biofeedback," "pressure biofeedback unit," "stabilizer," "headache," and "neck pain." Twenty-four eligible studies were included in this meta-analysis and were coded according to the type of neck problems and outcome measures described, such as pain, endurance, involvement of neck muscle, craniovertebral angle (CVA), neck disability index (NDI), cervical range of motion (CROM), radiographs of the neck, posture, strength, endurance, and headache disability index. The overall effect size of the DNF exercise was 0.489. The effect sizes of the neck problems were 0.556 (neck pain), -1.278 (FHP), 0.176 (headache), and 1.850 (mix). The effect sizes of outcome measures were 1.045 (pain), 0.966 (endurance), 0.894 (deep neck flexor), 0.608 (superficial neck flexor), 0.487 (CVA), 0.409 (NDI), and 0.252 (CROM). According to the results of this study, DNF exercise can effectively reduce neck pain. Thus, DNF exercise is highly recommend as an effective exercise method for individuals suffering from neck pain.

두부자세에 따른 두경부 근활성 및 교합접촉양태의 변화 (Effects of Head Posture on Resting EMG Activity of Craniocervical Muscles and on Occlusal Contacts)

  • This study was performed to investigate influence of the changes of head posture on resting electromyographic (EMG) activity in anterior temporalis, masseter, sternocleidomastoid muscle and trapezius, and on status of occlusal contacts. For this study twenty-nine patients with temporomandibular disorders(TMD) and thirty dental students without any masticatory symptoms were selected as patients group and control group, respectively. EMG activity($\mu$V) at rest was observed in four kind of head postures such as natural or normal head posture(NHP), forward head posture(FHP), upward head posture(UHP), downward head posture(DHP), and in NHP and FHP, EMG activity with flat occlusal splint was also checked. BioEMG$^\textregistered$(Bioelectromyograph, Bioresearch Inc., USA) was used to record EMG activity in the above four muscles with eight locations on both sides. The author used T-Scan$^\textregistered$(Tekscan Co., USA) system to investigate the changes of oclusal contats on clenching in the four head postures about number, force, time(duration) and total left-right statistis(TLR, occlusal stability crossing left-right dental arch on clenching). For taking in upward or downward head posture, head was inclined $10^{\circ}$ upward or downward and CROM$^\textregistered$ (cervical-range-of motion, Performance attainment Inc., USA) was used to maintain same posture during the procedure. The results obtained were as follows : 1. For resting EMG activity, anterior temporalis did not show any difference by change of head posture, but masseter and sternocleidomastoid muscle showed higher value of EMG activity in FHP and UHP, and trapezius showed higher value of EMG activity in FHP and DHP. 2. EMG activity of trapezius was higher than that of any other muscles in NHP, FHP, and DHP, but in UHP, the activity was the lowest reversely. 3. Patients group showed higher EMG activity than control group did in all the muscles in NHP. And significant difference between the two groups were also observed in anterior temporalis in FHP, in sternocleidomastoid muscle in UHP, and in sterno-cleidomastoid muscle and trapezius in DHP with higher activity in patients group. 4. There was no change of EMG activity in NHp with splint, but EMG activity in anterior temporalis and masseter was decreased in FHP with splint. 5. In general, status of occlusal contacts was not changed with head posture in all subjects, and difference between patients group and control group was only noted for number and force of tooth contact in UHP and DHP with more value in control group. 6. Correlationship between EMG activity and number ad force of tooth contacts was shown negatively with regard to masseter in NHP, and trapezius in UHP and DHP.
