유리 장 이식편을 이용한 식도 재건 (Free jejunal graft for replacement of cervical esophagus)

  • Reconstruction of the pharynx and cervical esophagus is a difficult surgical problem. A successful case of reconstruction of cervical esophagus by free jejunal graft upon a 23-year-old female who had stricture in the pharynx and cervical esophagus after ingestion of hydrochloric acid is presented. This procedure was done after the primary traditional reconstruction with right sided colon had resulted in restenosis due to necrosis of the cervical portion of the graft. A proximal jejunal segment, about 12 cm in length was isolated for free graft preserving its vascular arcade. Both superior thyroid artery and vein were anastomosed to the graft vessels in end to end by continuous suture of 8-0 monophil. Nylon. The postoperative course was uneventful. The patient has been followed for 4 months after operation and she can eat every kind of food without dysphagia. We think free jejunal graft offers an excellent and safe method of reconstructing cervical esophagus and pharynx with definitive advantages over other traditional techniques.

  • In recent era, the incidence of combined injury of cervical trachea and esophagus by penetrating wound has been increased considerably. If initial operative repair is unsatisfied, devastating complications and even death may be considered. A 5 years old boy and 67 years old female were admitted to our department for deep cervical stab wounds. On exploration, cervical trachea and esophagus was nearly total transected anteroposteriorly. Emergent reconstructive surgery for trachea and esophagus had done. Postoperative course was uneventful without any complications.

  • Despite of technical advances in surgery & other therapeutic modalities five-year survival rates in patients with carcinoma of hypopharynx have remained low. Many techniques have been used to create a structure capable of allowing the passage of food and fluids in an attempt to maintain the anatomy and physiology of the upper digestive system. The development of microsurgical techniques and the concept of mucocutaneous unit has brought about important changes in the reconstruction of cervical esophagus following tumor resection. The one-stage procedure using microvascular anastomosis of free jejunal graft provides physiologic reconstruction of cervical esophagus and has a low morbidity rate as well as a short recuperation time. With free jejunal graft, there is marked improvement in the quality of life and numerous advantages over the previous methods of reconstruction. Reconstruction of esophageal defect after resection of carcinomas of the hypopharynx, and cervical esophagus has traditionally been carried out with deltopectoral, or musculocutaneous skin-lined flaps. A second approach is to reconstruct the defect with the colon or stomach. A more ideal mettled is to repair these defects with mucosa-lined flaps. The authors experienced 35 cases of reconstruction of cervical esophagus after resection of carcinoma of the hypopharynx with free jejunal autograft and one case of secondary repair with radial forearm free flap after failure of initial free Jejunal autograft. Postoperative results were satisfactory in most patients and two patients expired in 8 days postoperatively because of carotid blow out by chronic inflammation.

부갑상선 종양으로 오인된 경부 식도 과립세포종 1례 (A case of granular cell tumor of the cervical esophagus misdiagnosed as parathyroid tumor)

  • Granular cell tumor is an uncommon neoplasm that can occur everywhere in the human body. Granular cell tumor of the cervical esophagus is rare. Histopathologically, granular cell tumor consists of large polygonal cells with small dark nuclei and abundant, fine, granular eosinophilic cytoplasm that show positive immunohistochemical staining using S-100 protein. Surgical excision is the treatment of choice for granular cell tumor. Recurrence is rare, but inadequate resection of granular cell tumor may cause local recurrence. We have experienced one case of granular cell tumor of the cervical esophagus that was misdiagnosed with parathyroid tumor. Therefore, we report it with the literature review.

  • Reconstruction for hypopharynx and cervical esophagus after wide resection of extensive hypopharynx and larynx cancers have been used various methods including myocutaneous flaps, gastric pull up, md jejunal or large bowel free graft. Recently, the authors had experienced hypopharynx and larynx cancers with extension to the posterior hypopharyngeal wall and thoracic esophagus. Of course, these reconstructive methods should be selected in accordance with the patient's age, physical status, extend of prim.:W lesion, and defect after on block resection, however, gastric pull up was performed in hypopharynx cancer which had skipped lesion in the thoracic esophagus and jejunal free grafts were performed in case 2, 3 which had a extended lesions to the posterior pharyngeal wall. Some complications were noted, which were successfully stabilized by conservative managements. The gastric pull up and jejunal free graft were considered suitable methods for reconstrunction of hypopharynx and cervical esophagus, however, further studies are necessary about it.

둔상에 의한 경부 식도 파열 (Cervical Esophageal Perforation after Blunt Trauma)

  • Esophageal perforation due to blunt trauma is rare. A 67-year-old male presented to Konyang University Hospital with painful neck swelling. His neck was injured by blunt trauma at work. Esophageal perforation was detected by neck CT and esophagography. We performed primary repair of cervical esophagus through the Lt. neck approach. The postoperative course was uneventful and the patient was discharged at postoperative day 15.

  • An esophageal perforation following anterior cervical fusion is rare. Early development of an esophageal perforation after anterior cervical fusion is usually due to iatrogenic injury from retraction, injury associated with the original traumatic incident, improperly placed instruments or a bone graft. A 31-year-old man had a cervical dislocation and spinal cord injury because of severe cervical trauma after a traffic accident. He was quadriplegic and had no feeling below T4 dermatome. Anterior decompression of the cervical spine and anterior fusion with mesh with autobone were performed. An esophagocutaneous fistula occurred 7 days after anterior cervical surgery. A second anterior surgery was done because of pus drainage. The mesh was changed with an iliac bone graft, and the esophagocutaneous fistula site was primary repaired, but pus continued to drain. Conservative treatment, which consisted of wound drainage and intravenous administration of antibiotics, was tried, but was unsuccessful. After all, we removed the plate and screws, but did not removed the iliac bone graft, We closed the esophageal fistula, and transposed the sternocleidomastoid muscle flap to the interspace between the esophagus and the cervical spine. The wound to the esophagus was well repaired. In conclusion, precautionary measures are needed to avoid the complication, and adequate treatment is necessary to resolve those complications when they occur.

양성식도협착에 대한 결장을 이용한 식도성형술 (The Use of Colonic Conduit in the Management of Benign Esophageal Stricture)

  • Between 1967 and 1980, a total of 99 patients with a benign stricture of esophagus, resulting from a chemical burn, underwent a reconstructive procedure in which various segments of colon were used to bridge the gap between the cervical esophagus and the stomach. There were 42 males and 57 females and most were in their twenties and thirties. The most frequent site of the stricture was upper 1/3 of the thoracic esophagus [48.5%] and the next most common site was the low cervical esophagus [23.2%]. In 89 cases, the right colon with or without the terminal ileum was used as the conduit in an isoperistaltic manner and in 10, the left colon was used in an antiperistaltic position, because the right colon was not suitable as the conduit. There was a higher incidence of regurgitation [90% vs 0%], leakage at cervical anastomosis [80% vs 27%] and stenosis at anastomotic site [70% vs 15%] in an antiperistaltic left colon anastomosis, as compared to isoperistaltic right colon anastomosis. This was felt to be due to the orad peristaltic motion of the transplanted colon which acted as a functional obstruction distal to the esophagocolic suture line, resulting in breakdown of the anastomosis, leakage and eventual stenosis at the site of anastomosis. In conclusion, colon is useful and effective conduit as an esophageal substitute. Either the right or the left colon can be used for this purpose, provided that it is placed in an isoperistaltic position to minimize some of the complications listed above.

동전에 의한 경부식도천공: 1례보고 (Coin Perforation of the Esophagus)

  • Esophageal perforation by a ingested coins is a rare but usually life threatening complication, particularly if there is a delay in their removal. We report a case of esophageal perforation which occurred in a 4-year-old child after ingestion of a coin. The coin was removed through a left cervical esophagostomy approach. A gastrograffin study of the esophagus, performed four days after surgery, revealed the fistula had reopened. The wound was reexplored and the fistula was closed again with some debridement. In spite of the procedure, there had been continuous drainage of the saliva through the cervical fistula of esophagus. However within three weeks of conservative management, aided by a local drainage and hyperalimentation, the fistula of the esophagus was closed spontaneously.

다발성 원발성 식도종양 1례 보 (Multicentric Primary Tumor of the Esophagus)

  • A 51 year-old male was admitted with the chief complaints of swallowing difficulty and pain on neck and upper chest for 2 months prior to admission: He was taken biopsy under esophagoscopy, and the result was squamous cell carcinoma. Preoperatively we studied the esophagogram and chest CT. On these, we found the main appreciable mass at midesophagus easily. But, we missed the upper cervical unexpected mass. So we performed the esophagectomy and lymph node dissection from upper clavicle level to the esophagogastric junction by thoracic approach as wide as possible, and cervical esophagostomy and feeding gastrostomy also. At that evening we reviewed the studied films in detail, and we found another mass lesion at C4-C6 level of cervical esophagus. We performed the 2nd operation e.g. cervical esophagectomy on next morning without hesitation. Between these two masses, there was almostly normally looking skip area grossly. The squamous cell carcinoma of the esophagus could be multicentric in character and may have skip area. But, we heard little reports until now. The two masses could be different in origin or be same probably by submucosal spreading. Anyway, it was an alarming case to the surgeon not to neglect the another possible lesions in squamous cell type. Postoperative course was uneventful, he took G-tube feeding with no problems on 7th postoperative day. Now he took the postoperative irradiation at out patient department.
