선천성 근성 사경에 대한 새로운 촉진 기법을 이용한 소아 통합 도수치료적용 후 경추각도의 변화와 머리각도 변화: 단일사례연구 (Effect of Pediatric Integrative Manual Therapy, a Novel Mobilization with Facilitation Movement Technique, on Congenital Muscular Torticollis after Cervical Rotation and Head angle: A Case Report)

  • Background: Congenital muscular torticollis results in reduced head mobility, such as cervical rotation, due to the abnormal size and contraction of the sternocleidomastoid muscle. Korea Pediatric integrative manual therapy and stretching are recommended to improve head rotation upper cervical spine mobility. Therefore, in this study, the effect of the new PIMT was investigated. Methods: The patient is a 3.5 month-old diagnosed with congenital muscular torticollis (CMT). Due to the limitation of head rotation and cervical spine rotation and flexion mobility, the child visited a rehabilitation center and after diagnosis, Pediatric integrative manual therapy (PIMT) treatment was performed five times a week for a total of 15 weeks. The child's head rotation and flexion limitation and plagiocephaly were evaluated. Results: In conclusion, this study shows that compared to other treatments, PIMT approach is a more effective treatment for improving head rotation and cervical limitation for range of motion in CMT infants. Conclusion: PIMT approach was effective in improving cervical rotation and Head lateral flexion mobility and plagiocephaly in CMT patients.

Cervical Cord Decompression Using Extended Anterior Cervical Foraminotomy Technique

  • Objective : At present, gold-standard technique of cervical cord decompression is surgical decompression and fusion. But, many complications related cervical fusion have been reported. We adopted an extended anterior cervical foraminotomy (EACF) technique to decompress the anterolateral portion of cervical cord and report clinical results and effectiveness of this procedure. Methods : Fifty-three patients were operated consecutively using EACF from 2008 to 2013. All of them were operated by a single surgeon via the unilateral approach. Twenty-two patients who exhibited radicular and/or myelopathic symptoms were enrolled in this study. All of them showed cervical cord compression in their preoperative magnetic resonance scan images. Results : In surgical outcomes, 14 patients (64%) were classified as excellent and six (27%), as good. The mean difference of cervical cord anterior-posterior diameter after surgery was 0.92 mm (p<0.01) and transverse area was $9.77mm^2$ (p<0.01). The dynamic radiological study showed that the average post-operative translation (retrolisthesis) was 0.36 mm and the disc height loss at the operated level was 0.81 mm. The change in the Cobb angle decreased to 3.46, and showed slight kyphosis. The average vertebral body resection rate was 11.47%. No procedure-related complications occurred. Only one patient who had two-level decompression needed anterior fusion at one level as a secondary surgery due to postoperative instability. Conclusions : Cervical cord decompression was successfully performed using EACF technique. This procedure will be an alternative surgical option for treating cord compressing lesions. Long-term follow-up and a further study in larger series will be needed.

Bilateral Internal Superior Laryngeal Nerve Palsy of Traumatic Cervical Injury Patient Who Presented as Loss of Cough Reflex after Anterior Cervical Discectomy with Fusion

  • Injury to the bilateral internal branch of superior laryngeal nerve (ibSLN) brings on an impairment of the laryngeal cough reflex that could potentially result in aspiration pneumonia and other respiratory illnesses. We describe a patient with traumatic cervical injury who underwent bilateral ibSLN palsy after anterior cervical discectomy with fusion (ACDF). An 75-year-old man visited with cervical spine fracture and he underwent ACDF through a right side approach. During the post-operative days, he complained of high pitched tone defect, and occasional coughing during meals. With a suspicion of SLN injury and for the work up for the cause of aspiration, we performed several studies. According to the study results, he was diagnosed as right SLN and left ibSLN palsy. We managed him for protecting from silent aspiration. Swallowing study was repeated and no evidence of aspiration was found. The patient was discharged with incomplete recovery of a high pitched tone and improved state of neurologic status. The SLN is an important structure; therefore, spine surgeons need to be concerned and be cautious about SLN injury during high cervical neck dissection, especially around the level of C3-C4 and a suspicious condition of a contralateral nerve injury.

경부 경막외 차단 시 약물의 용량에 따른 분포 양상 비교 (A Comparison of the Spread Level of the Cervical Epidural Block in Terms of Volume)

  • Background: Cervical epidural injection, performed via the interlaminar approach, represents a useful interventional pain management procedure indicated in patients with a cervical herniated disk. Due to thedecreased epidural space in the cervical region, cervical epidural injections may result in potentially serious complications, especially during a large volume injection. Methods: Thirty-four patients with neck pain due to a cervical herniated disk that were referred to the pain clinic for cervical epidural steroid injection were randomized into two groups. One group received a cervical epidural injection of 4 ml drug and the other group received 2 ml drug. The injected mixture included triamcinolon, ropivacaine and omnipaque. Spread levels of the drug after injection were estimated with the use of C-arm fluoroscopy. Results: Spread levels to the cephalad for patients in the two groups were $4.88{\pm}0.78segments$ and $4.53{\pm}0.49segments$, respectively. Spread levels to the caudad for patients in the two groups were $4.59{\pm}0.93segments$ and $4.47{\pm}0.51segments$, respectively. The results showed no significant difference in the spread level between the two groups. Conclusions: Use of a small volume of drug (2 ml) can provide a sufficient spread level of the injected drug that is desirable for patients with a cervical herniated disk.

건상검진상 발견된 Killian-Jamieson Diverticulum 1예 (A Case of Killian-Jamieson Diverticulum in the Esophagus)

  • A Killian-Jamieson diverticulum (KJD) is an unfamillar and unusual cervical esophageal diverticulum. This diverticulum originates on the anterolateral aspect of the esophagus through the Killian-Jamieson's area that is formed between cricopharyngeal muscle and the lateral to longitudinal esophageal muscle. Recently, we experienced a patient who was found outpouching lesion on lateral side of left esophagus on the duodenoscopy. Then, a barium esophagography performed and in left lateral position demonstrated a left-sided diverticulum with a frontal projection, highly suggestive of a KJD. There are two ways of surgical approach to manage the KJD. First is external approach, another one is endoscopic approach. In common, external approach has been recommended for the treatment of KJD because of concern of nerve injury. We present a case of KJD that underwent external approach and sternocleidomastoid muscle flap in the management of KJD.

후종격동에 발생한 갑상선종 1예 (A Case of Posterior Mediastinal Goiter)

  • Intrathoracic goiters can be classified anterior and posterior mediastinal goiter with its locations. Most intrathoracic goiters are retrosternally situated in the anterior mediastinal compartment. Posterior mediastinal goiters are rare, but might present a difficult diagnostic and surgical problem. Although thyroid goiters are nearly always amenable to a cervical approach, posterior mediastinal goiters may require a combined cervicothoracic approach with sternotomy or thoracothomy. We herein describe a case of posterior mediastinal goiter which was excised only by cervical approach. The relevant literature is briefly reviewed.

Acute Myelopathy Caused by a Cervical Synovial Cyst

  • Synovial cysts of the cervical spine, although they occur infrequently, may cause acute radiculopathy or myelopathy. Here, we report a case of a cervical synovial cyst presenting as acute myelopathy after manual stretching. A 68-year-old man presented with gait disturbance, decreased touch senses, and increased sensitivity to pain below T12 level. These symptoms developed after manual stretching 3 days prior. Computed tomography scanning and magnetic resonance imaging revealed a 1-cm, small multilocular cystic lesion in the spinal canal with cord compression at the C7-T1 level. We performed a left partial laminectomy of C7 and T1 using a posterior approach and completely removed the cystic mass. Histological examination of the resected mass revealed fibrous tissue fragments with amorphous materials and granulation tissue compatible with a synovial cyst. The patient's symptoms resolved after surgery. We describe a case of acute myelopathy caused by a cervical synovial cyst that was treated by surgical excision. Although cervical synovial cysts are often associated with degenerative facet joints, clinicians should be aware of the possibility that these cysts can cause acute neurologic symptoms.

고유수용성촉진법을 이용한 호흡운동이 경수 손상환자의 호흡기능에 미치는 영향 : 증례보고 (Using the PNF Approach to Improve Respiratory Function in Patients with Cervical Spinal Cord Injuries)

  • Purpose: The study aimed to examine the influence of PNF direct and indirect breathing treatments for patients with cervical spinal cord injuries who had breathing problems. Methods: For each cervical spinal cord patient, force vital capacity (FVC), peak expiratory flow, maximum phonation time (MPT), rib cage width, and VAS were measured pre-intervention and four weeks after post-intervention. The indirect method and the direct method were used for interventions. We treated patients with the indirect method using scapular anterior depression pattern, bilateral extensor pattern with rhythmic initiation, and a combination of isotonic. We treated patients with the direct method, applying pressure on the sternum and using rhythmic initiation (hold relax and stretch reflex) for the rib cage. Training occurred for 50 minutes a day and three days per week for four weeks. Results: FVC, MPT, peak expiratory flow, and rib cage width were increased and decreased at the VAS point for rolling after treatment. Conclusion: Patients with cervical spinal cord injuries who had breathing problems felt uncomfortable when they had conversations on a couch. We found that PNF direct and indirect treatments improved rib cage width and breathing functions of patients with cervical spinal cord injuries.

경부 경막외강내 카테테르의 X-선상 위치 및 조영제의 확산 (The Radiological Location of the Catheters in Cervical Epidural Space and the Spread of Radiopaque Dye)

  • Continuous epidural block can be useful in the management of acute and chronic pain. For the most effective analgesia, it is important to localize the tip of epidural catheter and the spread of radiopaque dye. Epidural catheterization was performed in 12 patients on the sitting position. Catheters were advanced by 10 cm cephalad in the cervical epidural space by median approach and radiopaque dye 3 ml was injected through the catheters. The position of cervical epidural catheters and the spread of dye was confirmed by radiography. The course of epidural catheter were: coiled 3/12 (25%), loop 2/12 (16.7%), straight 2/12 (16.7%). In 8 cases, the tip of epidural catheters were located within one vertebral segment from the level of insertion site. Radiopaque dye spreaded average 3.68 vertebral segment to cranially and 1.67 vertebral segment to caudally from the insertion site.

원인근 테이핑 요법이 경추부 관절운동과 동통에 미치는 영향 (Effects of taping technique applied to muscles causing pain on cervical movement and pain)

  • Pain is the most common symptom that brings a patient to the hospital. Repetitious stress and sprain injury result in various pains, and so we tried to improve cervical movement and release from pain by using taping technique of actual agonists and postural muscles in addition to psychological relaxation. The 4 patient with neck problems were applied Arikawa taping approaches. The flexor or extensor patterns were determined by Arikawa method at first. if the symptoms and patterns were similar, the taping was attached same point. After taping on major muscles of causing neck pain levator scapula, scalenus medius, sternocleidomastoid, splenius capitis, semispinalis capitis. - we found neck pain released and cervical ROM increased. In conclusion, we determined cervical movement related to rotation of splenius capitis, extention of semispinalis capitis, levator scapula, flexion and rotation of scalenus medius.
