X선 조사가 닭 유정란 부화과정에 미치는 영향 (Experimental study for the irradiation effect of x-ray on the hatching process of fertilized eggs)

  • 방사선 감수성이 높은 시기의 닭 유정란에 방사선치료용 선형가속장치를 이용하여 고 에너지 X선을 조사하고, 부화과정 및 생존율, 외형적 기형, 혈액세포의 변화를 관찰하여 방사선 조사가 유정란 부화과정에 미치는 영향에 대하여 연구 하고자 하였다. 방사선 감수성이 가장 높을 것으로 예상 되는 5일째에 고 에너지 10 MV-X선을 5개의 유정란을 한 군으로 설정한 실험군에 각각 5, 7.5, 10 Gy를 조사하고 부화기에 입란 하였다. 매일 검란을 실시하여 20일간의 부화과정을 관찰하고 부화 예정 1일 전에는 방사선촬영을 실시하여 부화상태와 전체 골격 형성을 확인하였다. 부화 된 병아리는 방사선촬영과 혈액분석을 실시하였다. 부화율은 대조군과 실험군에서 각각 80, 46.7%이고, 조사선량 별 부화률은 대조군과 5, 7.5, 10 Gy 에서 각각 80, 100, 40, 0% 로 조사선량이 증가 함에 따라 부화율은 감소하였다. 기형발생률은 대조군과 실험군에서 각각 0, 60% 이다. 조사선량 별 기형 발생률은 5, 7.5, 10 Gy 에서 각각 20, 80, 80% 이다. 미부화란의 인공 파각 후 좌측날개, 안구돌출, 발, 목뼈 기형이 나타났으며, 조사선량이 증가함에 따라 기형발생률이 증가 하였다. 백혈구 평균치는 대조군, 5, 7.5 Gy에서 각각 87.64, 100.76, 81.42 ($10^3/{\mu}L$) 으로 나타났다. 고에너지 X선에 의한 직, 간접적 손상인 부리 이상, 눈 결손, 계태사, 혈액성분의 변화를 확인 하였다. 특히 조사선량이 높을수록 부화율과 백혈구의 평균수치가 감소하였으며, 기형의 발생률과 계태사가 증가 하였다. 기형 발생의 원인은 염색체 이상으로 추정된다. 향후 고 에너지 X선 조사에 따른 기형의 원인을 알아내기 위한 지속적인 유전학적 연구가 필요할 것이다.

한국 성인의 두경부자세와 두개안면형태에 관한 두부 X-선 계측학적 연구 (STUDY OF CRANIOCERVICAL POSTURE AND CRANIOFACIAL MORPHOLOGY IN KOREAN YOUNG ADULTS)

  • 본 연구는 한국성인의 두경부자세와 두개안면형태를 두개내 및 두개외 기준선에 대하여 알아보고 그 상관성을 평가하여 향후 진단시 보조적 자료로 사용하고자하여 시행되었다. 양호한 안모와 1급 구치관계를 지닌 성인 남, 녀 각 25명씩 총 50명에 대하여 natural head position 측모 두부 X-선 계측사진을 촬영하여 투사도를 그린후 posterior nasal spine을 지나는 true vertical line과 sella를 지나는 true horizontal line을 기준으로하여 자세변수 및 형태변수를 계측, 계산하였다. SPSS통계처리 프로그램을 이용하여 남녀평균, 표준편차를 구하고 student t-test로 유의차를 검정한후 자세변수와 형태변수간의 상관관계를 보았으며, 또한 자세변수를 이용하여 두경부자세의 재현성을 평가하여 다음과같은 결과를 얻었다. 1. 한국성인에서의 두경부자세변수와 두개안면형태변수의 평균과 표준편차를 얻었다. 2. 한국성인은 OPT/CVT간 각에 있어서 $3.55{\pm}2.58^{\circ}$의 경추 전방만곡을 보였다. 3. 자세변수와 형태변수의 상관관계에서는 두경부자세와 안면의 수직적인 비율, 안면돌출도, 하악골의 회전이 높은 상관성을 보였다. 4. 두경부자세와 상하악간 관계, 전후방적 악골비율의 상관관계는 미약하였다. 5. Natural head position에서의 head positioning error는 $1.65^{\circ}$로 SN line의 개인간 변이도 $3.31^{\circ}$ 보다 작았다. 다른 자세변수도 head positioning error가 각 변수들의 개인간 변이도보다 작았다.

전방머리자세(Forward Head Posture)의 정도와 척추 만곡 변형의 상관관계 (The Study on Correlation between the Forward Head Posture and Spinal Alignment)

  • Objectives This study was designed to investigate the correlation between the forward head posture and the spinal alignment. Methods We examined the whole spine x-rays of the 144 student sample. We measured the Craniovertebral angle (CVA), Cervical angle (CA), Thoracic kyphotic angle (TKA), lumbar lordosis angle (LLA) and Ferguson's angle (FA) of the students. We then analyzed the relationship between these angles. Results CVA had correlation with CA, but it was weak. There was significant correlation between CVA and TKA. There were no significant correlation among CVA, LLA and FA. Conclusions According to above results, there is a negative relationship between the CVA and the TKA - in that higher CVAs yielded lower TKAs. But CVA had no significant correlation with LLA or FA.

다제내성 결핵에 의한 횡단척수염 1예 (A case of Transverse Myelitis due to Multidrug-Resistant Tuberculosis)

  • 폐결핵 치료중 흉부엑스선상 악화 및 갑자기 발생한 하지 마비와 감각이상으로 자기공명영상 촬영후 급성 횡단척수염 진단 및 객담 검사상 다제내성 결핵균 검출로 2차 결핵약제와 스테로이드 병합치료를 시행하여 부분적으로 호전을 보였던 증례이다. 급성 횡단척수염은 매우 드문 질환이며 균주의 직접 침범이나 면역학적 기전으로 발생하나 후자가 더 가능성 있는 기전으로 생각되어지고 있다. 아직도 결핵 및 다제내성 결핵의 유병률이 높은 국내 상황에서 드물게 결핵이 원인으로 추정된 급성 횡단척수염의 증례를 보고하는 바이다.

Feasibility of Bilateral Crossing C7 Intralaminar Screws : A Cadaveric Study

  • Objective : When the pedicle screw insertion technique is failed or not applicable, C7 intralaminar screw insertion method has been used as an alternative or salvage fixation method recently. However, profound understanding of anatomy is required for safe application of the bilaterally crossing laminar screw at C7 in clinic. In this cadaveric study, we evaluated the anatomic feasibility of the bilateral crossing intralaminar screw insertion and especially focused on determination of proper screw entry point. Methods : The C7 vertebrae from 18 adult specimens were studied. Morphometric measurements of the mid-laminar height, the minimum laminar thickness, the maximal screw length, and spino-laminar angle were performed and cross-sectioned vertically at the screw entry point (spino-laminar junction). The sectioned surface was equally divided into 3 parts and maximal thickness and surface area of the parts were measured. All measurements were obtained bilaterally. Results : The mean mid-laminar height was 13.7 mm, mean minimal laminar thickness was 6.6 mm, mean maximal screw length was 24.6 mm, and mean spinolaminar angle was $50.8{\pm}4.7^{\circ}$. Based on the measured laminar thickness, the feasibility of 3.5 mm diameter intralaminar screw application was 83.3% (30 sides laminae out of total 36) when assuming a tolerance of 1 mm on each side. Cross-sectional measurement results showed that the mean maximal thickness of upper, middle, and lower thirds was 5.0 mm, 7.5 mm, and 7.3 mm, respectively, and mean surface area for each part was $21.2mm^2$, $46.8mm^2$, and $34.7mm^2$, respectively. Fourteen (38.9%) sides of laminae would be feasible for 3.5 mm intralaminar screw insertion when upper thirds of C7 spino-laminar junction is the screw entry point. In case of middle and lower thirds of C7 spino-laminar junction, 32 (88.9%) and 28 (77.8%) sides of laminae were feasible for 3.5 mm screw insertion, respectively. Conclusion : The vertical cross-sectioned area of middle thirds at C7 spinolaminar junction was the largest area and 3.5 mm screw can be accommodated with 77.8 % of feasibility when lower thirds were the screw entry point. Thus, selection of middle and lower thirds for each side of screw entry point in spino-laminar junction would be the safest way to place bilateral crossing laminar screw within the entire lamina. This anatomic study result will help surgeons to place the screw safely and accurately.

다발성 외상환자에서 혈관계 접근을 통해 치료한 쇄골하동맥 손상 2례 (Treatment of Subclavian Artery Injury in Multiple Trauma Patients by Using an Endovascular Approach: Two Cases)

  • Introduction: Surgical treatment of subclavian artery (SA) injury is challenging because approaching the lesion directly and clamping the proximal artery is difficult. This can be overcome by using an endovascular technique. Case 1: A 37-year-old male was drawn into the concrete mixer truck. He had a right SA injury with multiple traumatic injuries: an open fracture of the right leg with posterior tibial artery (PTA) injury, a right hemothorax, and fractures of the clavicle, scapula, ribs, cervical spine and nasal bone. The injury severity score (ISS) was 27. Computed tomography (CT) showed a 30-mm-length thrombotic occlusion in the right SA, which was 15 mm distal to the vertebral artery (VA). A self-expandable stent($8mm{\times}40mm$ in size) was deployed through the right femoral artery while preserving VA flow, and the radial pulse was palpable after deployment. Other operations were performed sequentially. He had a viable right arm during a 13-month follow-up period. Case 2: A 25-year-old male was admitted to our hospital due to a motorcycle accident. The ISS was 34 because of a hemothorax and open fractures of the mandible and the left hand. Intraoperative angiography was done through a right femoral artery puncture. Contrast extravasation of the SA was detected just outside the left rib cage. After balloon catheter had been inflated just proximal to the bleeding site, direct surgical exploration was performed through infraclavicular skin incision. The transected SA was identified, and an interposition graft was performed using a saphenous vein graft. Other operations were performed sequentially. He had a viable left arm during a 15-month follow-up period. Conclusion: The challenge of repairing an SA injury can be overcome by using an endovascular approach.

척수종양의 방사선 치료 (External Beam Radiotherapy for Primary Spinal Cord Tumors)

  • 1969년부터 1983년까지 버지니아 의대부속병원에서 방사선 치료를 받은 34예의 원발성 척수암중, 조직검사 또는 아절제술을 시행한 후 수술후 방사선 요법으로 치료받은 32예 (Astrocytoma 16, ependymoma 16)에 대한 치료실적을 보고하고자 한다. 29예에서는 $45\~55Gy$를 5내지 6주간에 조사 받았으며 나머지 3예에서는 40Gy이하의 선량을 조사받았다. 50Gy 이상 조사된 6예는 모두 척수가 조사야에 포함되었다. 본 연구의 최저 추적 기간은 5년이었다. 방사선 치료 실패 후 재수술로 치료된 3예를 포함하여, 모든 예에서 생존율은 5년과 10년에서 각각 $73\%\;와\;50\%$이었다. 각각의 투병생존율은 $60\%\;와\;32\%$이었다. 45Gy 이상의 선량을 받은 29예에서는 5년 무병생존이 $63\%$이었다. 13예에서 치료실패를 경험했으며 치료실패는 조사야에서만 관찰되었다. Ependymoma가 astrocytoma보다 통계적으로 유의하게 무병생존율이 높았다 $(45\%\;:\; 89\%(p<0.05))$, 원발부위에 있어 경추가 포함된 예에서 그렇지 않은 예보다 생존율이 저하되었다. 원발부위의 cauda equina포함여부는 예후에 영향이 없었다. 최고 20년간 추적결과 방사선 치료에 의한 신경성 합병증은 관찰되지 않았다. 본 연구는 원발성 척수암의 치료에 있어서 방사선 치료가 안전하고 효과적임을 확인하였다.

다분절 경추 유합 및 내고정 수술결과 (Multi-Level Anterior Interbody Fusion with Internal Fixation in Cervical Spine)

  • Objective : The purpose of the present study was to examine neurologic changes, fusion rate and degree of kyphosis from the surgical results of those patients who underwent multi-level anterior interbody fusion and internal fixation. Methods : Among 63 cases of the patients who received multi-level anterior interbody fusion and internal fixation in 5 years between 1995 to 1999 at the neurosurgery department, we performed a retrospective study in 52 cases that could be followed up with dynamic view imaging ; the results were compared and analyzed. The analysis was based on the results of history taking, physical findings and radiologic findings, and Odom criteria were used to classify those cases with neurologic changes. Results : Among those 52 cases in whom the follow-up was possible for at least a year and dynamic view imaging was possible, bone fusion was seen in 93% of the trauma cases and 95% in the non-trauma cases and overall bone fusion was observed in 94% of the cases. Bone fusion was seen in 93% of the autobone cases, 95% of the allobone cases, and 94% of the Mesh cases. Radiologic changes were observed by comparing the lateral view after surgery ; kyphosis was seen in 53% of the autobone cases, in 70% of the allobone cases, and in 35% of Mesh cases ; in 45% and 60% of the non-trauma cases and trauma cases, respectively ; and in 55% of the 2 level fusion cases and 46% of the 3 level fusion cases. Neurologic changes classified according to Odom criteria showed excellent result in 48% of all the cases, good in 23%, fair in 4%, and poor in 25%. Conclusion : Even those cases with multi-level fusion, a high fusion rate could be obtained by performing anterior interbody fusion and internal fixation ; those cases with kyphosis were related more with the presence or absence of posterior compartment injury rather than the fusion level ; and those trauma cases showed not much difference in the fusion rate compared with non-trauma cases but had a high possibility of kyphosis.

Relief of Chronic Posterior Neck Pain Depending on the Type of Forest Therapy: Comparison of the Therapeutic Effect of Forest Bathing Alone Versus Forest Bathing With Exercise

  • Objective To compare the pain-reducing effect of forest bathing alone versus forest bathing in combination with stretching and strengthening exercises in patients with chronic posterior neck pain. Methods Sixty-four subjects with posterior neck pain that had lasted more than 3 months were enrolled. They were randomly divided into a forest bathing alone (FBA) group and a forest bathing with exercise (FBE) group; each group included 32 subjects. All subjects from both groups walked every morning in the forest for about 2 hours for 5 days. In the afternoon, the FBE group did a stretching and strengthening exercise for about 4 hours; the FBA group had free time in the woods. Visual analog scale (VAS) on one day, VAS over the previous week, neck disability index (NDI), EuroQol 5D-3L VAS (EQ VAS) and index (EQ index), McGill pain questionnaire (MPQ), the number of trigger points in the posterior neck region (TRPs), and the range of motion of the cervical spine were evaluated on the first and last day of the program and compared between the two groups. Results The number of TRPs were significantly reduced in the FBE group compared with the FBA group (p=0.013). However, the other scales showed no significant difference between the two groups. Conclusion When patients with chronic posterior neck pain underwent a short-term forest bathing (less than 7 days) program, FBE was more effective in the reduction of the number of TRPs than FBA. However, all other pain measurement scales we evaluated showed no statistically significant difference between the two protocols.
