PURPOSE: This study compared the immediate effect of a passive mobilization of upper cervical spine (Kaltenborn's joint mobilization) and an active upper cervical stretching (Olaf's Auto-stretching) on patients with neck pain and ROM. METHODS: Twenty-three subjects were randomized selected in the passive group (Kaltenborn's joint mobilization) included twelve subjects and the active group (Olaf's Auto-stretching) included eleven subjects. VAS (Visual Analogue Scale) was measured before and after neck rotation performance. DUALER IQ PRO (JTECH Medical, U.S.A.) was used to measure the neck ROM. Mean value of double measurement was used before performance and after performance. SPSS version 18 was used to compare values independent t-test and paired t-test were used to compare pain and ROM. RESULTS: There are significant difference in the pain and the ROM in both of two group (p<.05). But there are no significant difference pain and ROM between two groups. CONCLUSION: Both of the passive mobilization of upper cervical spine and the active upper cervical stretching are effected on symptom improvement of patients with neck pain reduction and ROM increasing. Especially active upper cervical stretching is more economical, because it has similar effects with the passive mobilization, help to maintain the treatment effect of therapist by themselves and can help to save medical expenses of patients.
Purpose: Cervical pain is caused mainly by a static position, lasting work, bad habits and stress, and is accompanied by pain in the upper trapezius. Traditionally, heat and traction, exercise, mobilization, manipulation have been used to treat cervical pain. This study examined the effect of stretching and manipulation (high-velocity low-amplitude: HVLA) on the pain and ROM in women with chronic cervical and upper shoulder pain. Methods: Fifty-two women diagnosed with chronic cervical and upper shoulder pain were enrolled in this study. Among them, 26 patients (experiment group) were managed by passive stretching and manipulation, and another 26 patients (control group) were treated with physical therapy intervention (hat pack: HP, transcutaneous electrical nerve stimulation: TENS, ultra sound: US). Each group made use of a Visual Analogue Scale (VAS) and the highly reliable (flextion: 0.92, extention: 0.99) cervical range of motion (CROM) to compare the possible changes in pain and ROM in the two groups after treatment. Results: In the experimental group, the pain decreased and the ROM has increased in all directions. In the control group, the pain decreased but the ROM was not changed in all directions. Conclusion: According to the results, passive stretching and manipulation is effective for increasing the ROM and decreasing the level of pain. Moreover, physical therapy intervention (HP, TENS, US) is effective for immediately decreasing the pain but has little effect on the ROM.

Purpose : The purpose of this study was to ascertain the effects of air-pressure cervical traction on the tension headache in the adults. Method : All subjects were treated with air-pressure cervical traction while 15 minutes a day, 3 times per week, for 4 weeks. The effects of air-pressure cervical traction were evaluated by HIT-6(headache impact test), VAS, and flexion, extension, left right flexion, left right rotation of cervical ROM. The measured data were analyzed by using repeated ANOVA. Especially in order to investigate the effect of air-pressure cervical traction. The comparison with among three measurement time. Results : The results of this study were as follows; 1) Prior and two weeks measurement did not show significant differences. The head forward posture than before the experiment, each experimental group was significantly increased(p<.05). 2) Prior and two weeks measurement did not show significant differences. The experimental group than ever cervical ROM of the movable range increased significantly(p<.05). 3) The experimental group was significantly lower than before the VAS(p<.05). 4) HIT score of the experimental group was significantly lower than before(p<.05). Conclusion : These findings suggest that air-pressure cervical traction had effect on the tension headache in the adults.

Objective: The aim of this study was to compare sling exercise group to McKenzie exercise group in patients with chronic neck pain. Design: Two group pretest-posttest design. Methods: Twenty subjects who have chronic neck pain were randomly divided into sling exercise group (n=10) and McKenzie exercise (n=10). Sling exercise group (n=10) received sling exercise for 30 minutes per day, twice a week over a 4 week period. And the other group were exercised McKenzie exercise (n=8) for 30 minutes per day, twice a week over a 4 week period. Neck disability index (NDI), Visual analog scale (VAS), algometer, digital manual muscle tester (MMT) and cervical muscle strength and cervical range of motion (ROM) are closely measured to identify the effect of sling exercise and McKenzie exercise. Results: For NDI, VAS, algometer on both trapezius, both rotation of cervical muscle strength, both lateral flexion of cervical muscle strength, cervical extension of ROM and both lateral flexion of ROM were significantly increased after intervention in sling exercise group (p<0.05), For VAS, algometer on both trapezius, left (Lt.) rotation of cervical muscle strength, Lt. lateral flexion of cervical muscle strength, cervical flexion and extension of ROM and Lt. lateral flexion of ROM were significantly increased after than before intervention in McKenzie exercise group (p<0.05). Conclusions: These study outcomes clearly support the notion that sling and McKenzie exercise improved pain, Muscle strength and ROM of patients with chronic neck pain. These results suggest that sling and McKenzie exercise program is suitable for chronic neck pain.
Objectives: The purpose of this study was to determine whether the cranial vertebral angle (CVA) and the range of motion (ROM) was different between participants with a forward head posture (FHP), with or without pain. Methods: Forty-four participants who had FHP participated in this study. The FHP was assessed digitally by measuring a lateral view the CVA for each subject. A cervical ROM device measured the cervical ROM. The volunteers were allocated to either, with pain (n = 22), or without pain (n = 22) groups, and pain was evaluated using the Numeric Pain Rating Scale. Results: The FHP in the pain group showed a significant difference in the CVA, and the cervical ROM in both flexion and extension, compared with those in the FHP without pain group (p < 0.05). Logistic regression analysis indicated that the occurrence of cervical area pain was higher amongst subjects who had a decreased CVA and flexion motion. Conclusion: This study suggested that decreased CVA and cervical flexion range, were predictive factors for the occurrence of pain in the cervical region.

Background: This study examined the effects of the muscle energy technique and cervical traction after applying conservative physical therapy in patients with cervical radiculopathy. Methods: Patients were randomly divided into two groups muscle energy technique (8 subjects) and cervical traction (8 subjects). Each group performed their exercise 70 minutes per day, three times week for four weeks. Pain intensity was measured with a visual analogue scale (VAS). Function was measured with the neck disability index (NDI). Cervical range of motion (ROM) was measured with a cervical range of motion (CROM) goniometer. Results: After four weeks of therapy, VAS (p<.05) and NDI (p<.05) significantly decreased, and ROM significantly increased in both groups (p<.05). There were also significant differences between the two groups for these three measures (p<.05). Conclusion: The muscle energy technique and cervical traction are more effective than cervical traction alone in reducing VAS and NDI and increasing ROM in patients with cervical radiculopathy.

The purpose of this study was to investigate the effects of occipital bone stimulation by cervical stabilizing exercise on the muscle tone, stiffness, ROM, and cervical lordosis in patient with forward head posture(FHP). This study was a case study of a single patient with forward head posture. This study used a ABA' design, A and A' were the baseline phases and B was the intervention phase. The intervention was occipital bone stimulation by cervical stabilization exercise. It was administered once daily for 7 days. The therapist kept hands together, and placed the two index fingers under the subject's occipital bone. The subject performed the chin-in exercise with a maximum isometric contraction for 20 sec.The exercise was implemented by performing the movements 10 times as a set and repeating the set three times. The muscle tone was not significantly changed after intervention. However, the stiffness was decreased and lasted the effect lasted without intervention. The cervical flexion angle was increased, but the cervical extension angle was not significantly changed after the intervention. The left and right lateral flexion angles were increased and the effect lasted without any intervention. However, the left and right rotation angles were significantly changed after the intervention. Cervical lordosis increased not from $37^{\circ}$ to $41^{\circ}$ after the intervention. These results suggest that occipital bone stimulation by cervical stabilizing exercise had a positive effect on cervical stiffness, flexion and lateral flexion ROM, and lordosis in a patient with forward head posture.
