THE EFFECT OF SANDWICH TECHNIQUE USING FLOWABLE COMPOSITE RESIN BASE ON THE MICROLEAKAGEIN CLASS II CAVITIES OF MOLAR (구치부 제2급 와동에서 고흐름성 복합레진을 이용한 적층 충전술식의 미세누출에 미치는 영향)

  • The aim of this study was to evaluate the marginal adaptation of direct class II sandwich restoration with packable composites(P-60), resin modified glass ionomer cement(Fuji-II LC), flowable compomer(Dyract Flow), flowable composites(Filtek Flow) in comparison with total bond restorations. In addition, for sandwich restorations, influence of different sandwich techniques was also evaluated. Large butt-joint box typed class II cavites with cervical margins 1mm below the cemento-enamel junction were cut into 70 extracted human molars. The cavities(7 groups, n=10) were filled using a closed/open sandwich restoration or total bond restoration technique with materials according to the manufacturer's recommandation using the single-component bonding agent for each system. Teeth were thermocycled 500 times between 5$^{\circ}C$ and 55$^{\circ}C$ with 30-second dwell time. The teeth were then coated with nail polish 1mm short of the restoration, placed in a 2% methylene blue for 24 hours, and sectioned with diamond wheel. Sections were examined with a stereoscope to determine the extent of microleakage. Dentine /Cementum margins were analyzed for microleakage on scale of 0(no leakage) to 4(entire axial wall) and interface between materials, on scale of 0(no leakage) to 3(axial wall). Results were evaluated with Kruskal Wallis Test, corrected for ties, to determine whether there were statistically significant differences among the seven groups. Pairs of groups were analyzed using the Student-Newman-Keuls Method and Dunn s Method. The results were as follows : 1. All groups showed some micoleakage in cervical portion. But there were no microleakage in interface between materials. 2. Closed sandwich restorations with Fuji-II LC and Filtek Flow had significantly lower leakage rating than total restorations with only P-60. However, open sandwich restorations with Dyract Flow showed significantly higher (P<0.05) 3. Closed sandwich restorations had significantly lower leakage rating than total restorations. However open sandwich restoration s showed significantly higher (P<0.05). 4. Sandwich restorations with Fuji-II LC were iou$.$or leakage than only P-60. Filtek Flow, Dyract Flow. But there were no statistically differences among the materials. From the results above, it could be concluded, closed sandwich restorations was effective in reducing microleakage of class II restorations. The best results showing the least microleakage were for the closed sandwich technique with Fuji-II LC and Filtek Flow.

FINITE ELEMENT STRESS ANALYSIS OF CLASS V COMPOSITE RESIN RESTORATION SUBJECTED TO CAVITY FORMS AND PLACEMENT METHODS (와동 형태와 충전 방법에 따른 Class V 복합 레진 수복치의 유한요소법적 응력 분석)

  • Most of cervical abrasion and erosion lesions show gingival margin where the cavosurface angle is on cementum or dentin. Composite resin restoration of cervical lesion shrink toward enamel margin due to polymerization contraction. This shrinkage has clinical problem such as microleakage and secondary caries. Several methods to diminish contraction stress of composite resin restoration, such as modifying cavity form and building up restorations in several increments have been attempted. The purpose of this study was to compare polymerization contraction stress of composite resin in Class V cavity subjected to cavity forms and placement methods. In this study, finite element model of 5 types of Class V cavity was developed on computer tomogram of maxillary central incisor. The types are : 1) Box cavity 2) Box cavity with incisal bevel 3) V shape cavity 4) V shape cavity with incisal bevel 5) Saucer shape cavity. The placement methods are 1) Incisal first oblique incremental curing 2) Bulk curing. An FEM based program for light activated polymerization is not available. For simulation of curing dynamics, time dependent transient thermal conduction analysis was conducted on each cavity and each placement method. For simulation of polymerization shrinkage, thermal stress analysis was performed with each cavity and each placement method. The time-temperature dependent volume shrinkage rate, elastic modulus, and Poisson's ratio were determined in thermal conduction data. The results were as follows : 1. With all five Class V cavifies, the highest Von Mises stress at the composite-tooth interface occurred at gingival margin. 2. With box cavity, V shape cavity and saucer cavity, Von Mises stress at gingival margin of V shape cavity was lower than the others. And that of box cavity was lower than that of saucer cavity. 3. Preparing bevel at incisal cavosurface margin decreased the rate of stress development in early polymerization stage. 4. Preparing bevel at incisal cavosurface margin of V shape cavity increased the Von Mises stress at gingival margin, but decreased at incisal margin. 5. At incisal margin, stress development by bulk curing method was rapid at early stage. Stress development by first increment of incremental curing method was also rapid but lower than that by bulk curing method, however after second increment curing final stress was the same for two placement methods. 6. At gingival margin, stress development by incremental curing method was suddenly rapid at early stage of second increment curing, but final stress was the same for two placement methods.

Histologic changes of tooth and periodontal tissues applying to contraction & intrusion force for the maxillary four incisors of dogs (성견 상악 4절치의 Contraction과 압하시 치아 및 주위 조직의 조직학적 변화에 관한 실험 연구)

  • The Purposes of this study were to investigate the initial tissue changes on the teeth and surrounding tissues under contraction and intrusive force by contraction UTA. A control and experimental dogs, 10-months in age, were studied. Contraction and intrusive force(60gm) were applied at upper four incisors by contraction UTA. Experimental dogs were sacrificed at 2 weeks and 4 weeks after force application, respectively. In this study, 2 experimental groups were designed by the duration of force applied(E1, E2). The specimens were taken around the upper first and second incisor in each groups and were prepared for the H-E and MT stain for light microscopic observation. From the results of the study, the following conclusions may be drown. : 1. In control group, the periodontal ligament width was constant from apical third to cervical third of the root and periodontal fiber arrangement was horizontal or oblique in cervical third, oblique in middle third and apical third. In alveolar bond, smooth appearance was shown with osteoblast. 2. In experimental group 1, in proportion to force was concentrated at labial middle third and apical third of root of the upper first and second incisors, root of these tooth tipped labially and intruded at a time. 3. In experimental group 2, periodontal ligament width and arrangement was similar to control and observed strong calcified response at the labial middle third of root. But, alveolar bond resorption and cementum resorption were as before seen at labial middle third and apical third of root that force was concentrated.

EARLY HISTO(PATHO)LOGIC CHANGES AND RECOVERY OF TOOTH AND PERIODONTAL TISSUE IN INTRUSION OF PREMOLAR ON YOUNG ADULT DOGS (유성견의 소구치 압하시 초기 치아 및 치주 조직의 조직학적 변화와 재생에 관한 연구)

  • The purpose of this study was to investigate the initial tissue change, to repair on the teeth & surrounding tissue under the intrusive orthodontic forces by use of elastic chain, through the microscopic findings. For this study, three young adult mongrel dogs were used, and were divied into three group : the control group was deliveried only casting crown and the experimental group 1 was equipped with energy chain during 1 week and experimental 2 group was deliveried using energy chain during 1 week and 3 weeks observation. All experimental groups and control groups were sacrificed to make the samples for microscopic findings on premolar teeth. All samples were examed and compared the histologic changes through the microscopic with H-E stain. The obtained results were as follows. 1. In hematoxylin-eosin stain of the control group, the periodontal ligament was constant width from apical third to cervical third of the root, and the periodontal fiber arrangement was horizontal or oblique in cervical third, oblique in middle and apical third of the root. 2. In Masson Trichrome stain of the control group, osteoblast and osteoclast appeared in cervical third of root, and bone resorption and new bone formation was observed in middle and apical third of the root. 3. In experimental 1, osteoclasts were increased highly, and hyperemia of blood vessels and new bone formation and bone resorption by reversal line in apical third of the root were seen. PDL width was increased apprarently from crest to apex of the root and more in apical third. 4. In experimental 2, osteoclasts and hyperemia of blood vessels were more increased than control material in apical third of the root. PDL width was increased more than control group in root apex, and was seen less than experimental 1. PDL arrangement was similar to experimental 1 and was mixed only in root apex. Therefore, in premolar intrusion of the young adult dog, there were increased osteoclast, hyperemia and dilation of blood vessel, resorption of alveolar bone and cementum and different arrangement of PDL in initial tissue change. There was not observed complete repair after remove intrusive force.

EFFECT OF BISCOVER ON THE MARGINAL MICROLEAKAGE OF COMPOSITE RESIN RESTORATION (복합레진 수복물의 변연 미세누출에 관한 BiscoverTM 전색제의 효과)

  • The purpose of this study was to compare the effect on marginal leakage of a resin surface sealant (Biscover) applied before or after polymerization of composite resin to unsealed composite restorations. Thirty Class V cavities with the occlusal margin in enamel and cervical margin in dentin or cementum were prepared on the buccal surfaces of sound extracted molars and restored with a microfilled light-cured composite resin (Micronew). Restorations were randomly assigned into one of three equal groups (n = 10): a control group - no surface sealing, group 1 - applied Biscover after polymerization of the composite resin. and group 2 - applied Biscover before polymerization of the composite resin. Specimens were thermocycled, immersed in a $20\%$ methylene blue solution for 4 hoots, sectioned longitudinally, and analyzed for leakage at the occlusal and gingival margins. The results of this study were as follows 1. In sealed group, group 2 showed higher microleakage than group 1 at both occlusal and gingival margins. but there was no significant difference between two groups (p > 0.05). 2. Unsealed control group showed a little higher microleakage than sealed group at occlusal margins, and a little Higher or similar microleakage than sealed group at gingival margins (p > 0.05) 3. Control group and group 2 showed significantly less microleakage at the occlusal margins, but group 1 showed no significantly difference between microleakage at the occlusal and gingival margins.

Changes in midpalatal suture area and adjacent periodontal tissues of individual tooth following rapid palatal expansion in young adult dogs ; Histomorphologic and immunohistochemical study (유성견 급속 구개확장시 정중구개봉합부 및 치아주위 조직 변화에 관한 조직형태학적 및 면역조직화학적 연구)

  • The purpose of this study was to determine the proliferative activity of the osteoblasts and fibroblasts in the midpalatal area and to investigate the adjacent periodontal tissues of individual tooth following rapid expansion of the palate. Ten young adult dogs, aged approximately ten months, were used in the experiment. The experimental design was consisted of 1 week expansion group(Group E1, 3 dogs), 2 week expansion group(Group E2, 3 dogs), 2 week expansion and 2 week retention group(Group E3, 3 dogs), and control group(Group C, 1 dog). For each group, expansion screw was activated one time per day(1/4 turn;$90^{\circ}$) following Hyrax-screw application. The experimental animals in each group were sacrificed at 1, 2 and 4 weeks following palatal expansion. Maxillary tissue blocks were obtained and prepared ior the histomorphologic and immunohistochemical studies. Light mcroscope, polarizing microscope, and soft X-ray apparatus were used in this study, and following results were obtained. 1. In polarizing microscopic study, the expansion groups(E1 & E2) showed blue color representing bone resorption and new bone formation in midpalatal suture area. E3 groups skewed less blue color compared to the E1 and E2 group. But yellow color increased by calcification in the E3 groups. 2. Immunohistochemical study revealed that positive responses of the osteoblasts to PCNA and undifferentiated fibroblasts to EGF in E1 group were somewhat increased. Positive response to PCNA and EGF were increased in fibroblasts and the osteoblasts forming new bone in E2 group. In E3 group, the positive response cell concentrated the periphery of edge of palatal process in both PCNA and EGF. 3. Throughout the expansion period(E1 & E2), light microscopic study showed the edges of the extensive resorption and new palatal processes, indicating bone remodeling within the suture. E3 group exhibited less remodeling of midpalatal suture area. E2 group and E3 group showed cementum formation and resorption at the apex of 3rd premolar and 1st molar E3 group exhibited extensive hyalinized zone on the cervical portion of buccal side of 1st molar. 4. Soft X-ray analysis of E1 group showed hypomineralized defect and microfractures in various parts of the suture areas when compared with control animals. There was no significant difference in the degree of mineralization in the midpalatal suture region between the C and E3 groups. Tooth axis showed tipping of 3rd premolar and 1st molar in the E2 group and E3 group. Based upon these experimental results, it is concluded that the undifferentiated mesenchymal cells always presented in midpalatal suture area following RPE. Differentiated osteoblasts and fibroblasts possess proliferating cellular activity until the 2 week retention period. The posterior teeth are tend to tip buccally as RPE force applied. Retention group exhibited irreversible response with severe hyalinized zone on the buccal surface of the first molar.
