뇌졸중 환자에 대한 연례보고(2003년) (Yearly Report on CVA Patients)

  • Objectives : This study was prepared for investigating the clinical features of stroke patients. Methods : We retrospectively reviewed the medical records of 169 patients who were admitted to Dept. of Internal Medicine, College of Oriental Medicine, Kyung-Won University with a diagnosis of stroke from Jan. 1, 2003 to Dec. 31, 2003. Results : Ischemic stroke(include TIA, 85.2%) was more common than hemorrhagic stroke(14.8%). The incidence in male was 46.7%, in female was 53.3% and the most prevalent age group is over-sixties. Cerebral infarction was most frequently noticed in MCA territory and lacunar-inf., hemorrhage in putamen. Hypertention, the most preceding diseases, followed by diabetes mellitus. The rate of recurrence was 30.18%. Cerebral infarction and cerebral hemorrhage was much occurred in acting. The most patients visited the hospital after 5 days. The most common symptoms in admission time were motor weakness and speech disorder. The complication was mostly urinary tract infection. Physical treatment from onset, in cerebral infarction was 9.14 days and in cerebral hemorrhage was 18.33 days. Conclusions : Our study on CVA patients was similar to previous studies from 1994 to 2002. In most cases, western and oriental treatment and medicine were given synthetically.

  • Object : The aim of this study was to examine the differences of symptom improvement between different diagnosis classification groups in acute cerebral infarction patients. Methods : We studied inpatients within a month after the onset of cerebral infarction who were admitted at Kyunghee University Oriental Medical Center from May 2011 to October 2012. We compared the improvement of Motricity Index and Scandinavian Stroke Scale score between Fire-heat group(n=20), Yin deficiency group(n=31), Dampness-phlegm group(n=30), and Qi deficiency group(n=13). Results : Yin deficiency group patients with cerebral infarction showed the most improvement in MI and SSS scores, and patients in Fire-heat group showed the poorest improvement in MI and SSS scores. There was a significant difference between the two groups, but there were no significant differences between all four diagnosis classification groups. Conclusions : This study provides evidence that diagnosis classification could be considered as an important factor in predicting the prognosis of acute cerebral infarction.

  • 1. Objectives The purpose of this research is to find out the Constitutional characters of Stroke inpatients. 2. Methods Stroke inpatients admitted into KyungHee Oriental Medical Center from January 2000 to April 2004, were divided into 2 group, cerebral infarction 493 inpatients and cerebral hemorrhage 83 inpatients. This research investigated the distribution of Sasasng Constitution, the gender, the age decade, the period of admission, the elapse of the time after onset, the course of admission, the frequency of Sasang Constitutional Herb-medicine, the past history of hypertension and diabetes mellitus, the obesity, the prevalence rate of hypertension and diabetes mellitus, EKG abnormality and hyperlipidemia. 3. Results & Conclusions The Sasang Constitutional distribution of cerebral infarction 493 inpatients was 0.4% of Taeyangin, Soyangin 34.1%, Taeurnin 56.4%, Soeumin 9.1%. The Sasang Constitutional distribution of cerebral hemorrhage 83 inpatients was Taeyangin 1.2%, Soyangin 47.9%, Taeurnin 43.4%, Soeumin 9.6%. Stroke incidence rate of Taeumin and Soyangin inpatients was high. Sasang Constitution had significant relations with the prevalence of the obesity and hypertension in cerebral infarction inpatients, the prevalence of obesity, diabetes mellitus and ECG abnormality in cerebral hemorrhage inpatients.

뇌졸중후 진전의 임상적, 전기생리학적 특성 (Clinical and Electrophysiological Characteristics of Post-stroke Tremor)

  • Background : Tremor is uncommon manifestation of stroke. Therefore a few cases have been reported until now. There is still uncertainty about the characteristics of post-stroke tremor. Furthermore the pathogenesis and responsible structures of post-stroke tremor are not precisely known. We have recently experienced 34 cases of post-stroke tremor for the past 6 years. We analysed the clinical features and electrophysiologic findings of post-stroke tremor to evaluate the general characteristics and to analogize the possible pathogenetic mechanisms of post-stroke tremor. Methods : The clinical characteristics of post-stroke tremor were summarized in according to the onset time, involved body parts, types, tremor frequencies, neuroradiologic findings, and associated symptoms. The tremor frequencies were recorded by using a gyroscope. The spectral analysis of tremor frequencies were done automatically with Motus I soft ware. Results : Tremor onset were remarkably varied. Some patients showed a tremor appearing at the onset of a stroke and other patients showed delayed-onset tremor 10 years after a stroke. Tremor frequencies were also much varied. The range of hand tremor frequencies were from 1.5 to 12 Hz. Lesions were found in 31 cases(infarction 27, hemorrhage 4) on neuroimaging. In the cases of cerebral infarctions, 7 cases showed multiple small vessel diseases and 20 cases showed cerebral vessel lesions. The most commonly involved cerebral vessel lesion was the middle cerebral artery territory Several different clinical patterns of post-stroke tremor were identified. Conclusions : There are some evidences from the data summarized here to suggest that several pathogenetic mechanisms including central oscillators could be involved for the development of tremors and that tremor generating neural circuits could be more complex than previously suggested neural circuits.

Coffee Consumption as a Risk Factor of Ischemic Cerebral Infarction in Koreans

  • Background and Purpose : To prevent ischemic cerebral infarction, it is very important to reduce risk factors which might cause stroke. However, the relationship of coffee consumption with ischemic cerebral infarction still remains unclear. The purpose of this study was to investigate the effects of coffee consumption on the risk of ischemic cerebral infarction in Koreans. Methods : A case-control study was conducted from April 1, 2001 to July 31, 2004. Cases (n=435) of first incident ischemic cerebral infarction were enrolled and were mostly matched by age to stroke-free hospital controls (n=407). All subjects were interviewed, examined and had anthropometric measurements by using an organized questionnaire. The coffee consumption was classified by the average frequency of intake, being none, 1 cup/day, 2-4 cups/day, more than 5 cups/day. Odds ratios (ORs) of ischemic cerebral infarction were proved multivariate analysis after adjustment for demographic factors, diet factors, and vascular risk factors. Results : When adjusted for sex, age, and other factors, coffee consumption and stroke do not have a significant association. (${\leq}$ cup/day OR=1.035, 95% CI=0.880-2.756; 2-4cups/day OR=1.452, 95% CI=0.864-2.440; ${\geq}$ 5 cups/day OR=1.557, 95% CI=0.705-3.435) Conclusions : In this study, we conclude that coffee consumption is not an important risk factor of ischemic cerebral infarction in Koreans. Prospective and cohort study on the relation between coffee consumption and the possibility of inducing ischemic cerebral infarctions in Koreans will be required in the future.

  • Objectives : We studied the aspects of headache in the patients with stroke. We compared character of headache on stroke with functional one. Methods : The subject of this study was based on 63 patients with headache who were admitted to the oriental hospital after stroke. We analyzed patients into sex & age, month, severity of headache on stroke type & lesion, site & character of headache. associated symptoms & signs on headache state.Results : The age of headache with stroke is higher than general headache. There was significant differences between general headache and headache with stroke. The cerebral hemorrhage is severer than cerebral infaction in th pain of headache. The most frequently appeared site of headache is temporal area and associated symptoms is vertigo & nausea.Conclusions : The headache of stroke patient is different form functional headache. because of cerebral vascular accident influence on inducing headache. Therefore we should study different methods between the headache of stroke patient and the others in medical treatment.

  • Stroke is a disease which results in impairment of body functions and affects everyday life. The ability of cerebral neurons to become reorganized and restore function after damage is called plasticity. Motor impairment typically appears contralateral to the affected cerebral hemisphere in patients with cerebral lesions. The authors report a case of a patient with hemiplegia ipsilateral to affected cerebral hemisphere, along with its conjectured mechanism.

Clinical Analysis of Symptoms and Oriental Medical Prescriptions According to Elapsed Time of Stroke in Oriental Medical Hospital Inpatients

  • Objectives : This study was intended to understand characteristics of symptoms, oriental medicine prescription and laboratory test results according to elapsed time of stroke. Methods : Through the medical records of 205 stroke inpatients in the oriental medical hospital in the year 2010, we investigated manifested symptoms, administered oriental medicine prescription and clinical pathological examination results. Collected items were classified to depend on stroke types, cerebral infarction and hemorrhage. We analyzed association between manifested symptoms, the oriental medicine prescription, and laboratory test results of stroke patients and elapsed time. Chi-square tests were performed to determine the significance level of association. Results : All symptoms, prescriptions and laboratory test results in cerebral infarction patients were associated with elapsed time. Especially, symptoms, prescriptions and pathological examination results showed very high statistical significance with elapsed time (a symptom; chi-square(df)=164.3(22), p<0.001, prescription; chi-square(df)=93.5(22), p<0.001, and pathological examination results; chi-square(df)=164.3(22), p<0.0004). But in the case of cerebral hemorrhage, there was not statistical significance. Conclusions : The elapsed time of stroke may be an essential requisite in catching symptoms and prescribing for stroke patients in oriental medical treatment.

Factors Affecting Delayed Hospital Arrival Times in Acute Ischemic Stroke Patients

  • The purpose of this study was to investigate the factors of hospital arrival delays of acute ischemic stroke patients. The study subjects were 126 cerebral infarction patients G Metropolitan City university hospital emergency center. General characteristics, disease-related characteristics and stroke-related were collected by self-reported questionnaires. Hospital arrival times by subjects' characteristics were tested by $x^2$ test and logistic regression analysis. Of 126 cerebral infarction patients, Their average hours taken to move to a hospital was 12.7 hours with the fastest case being 0.5 hour and the most delayed case being 127.8 hours. 61.1%(77 persons) of the stroke patients under this experiment said to have taken 3 hours or less. In logistic regression analyses, Coming to the hospital directly without passing through other hospitals was found to have higher probability of arriving less than 3 hours(${\beta}$=2.960, p=.009), And if LAPSS was tested positive, such cases are more likely to arrive within 3 hours(${\beta}$=2.219, p=.049). For acute ischemic stroke and caregivers need training to be conducted promptly admitted to hospitals for education and treatment hospital stroke screening will help to improve the treatment of stroke patients

우황청심원(牛黃淸心元)의 단독투여 및 연속투여에 따른 정상인의 뇌혈관반응도의 비교연구 (A Comparative Study about Cerebrovascular Reactivity from a Single Medication and Continuous Medication on Healthy Subjects)

  • Objectives: Uwhangchungsim-won (DC) has been used in various medical fields such as stroke, hypertension, atherosclerosis, autonomic imbalance and mental instability, etc. The aim of this study was to evaluate the effect of UC on cerebral hemodynamics and estimate the appropriate dose of UC. Methods: We studied changes of hyperventilation-induced cerebrovascular reactivity and mean blood flow velocity of middle cerebral arteries (MCA) using transcranial Doppler. We observed the changes of mean blood pressure, pulse rate and expiratory CO2 using S/5 Compact Anesthesia Monitor from 10 healthy young volunteers who were administered UC twice a day in the 1 st section and then once a day in the 2nd section. Results: Mean blood pressure tended to decrease at 1 hour and pulse rate tended to decrease at 2 hours after second administration. After 2 hours, mean blood pressure rose to state before administration, but pulse rate maintained from 2 hours to 4 hours. The changes were not statistically significant. Cerebral blood flow velocity in middle cerebral artery was not statistically significant after second administration. Cerebrovascular reactivity increased from 2 hours to 4 hours after second administration. Conclusions: This study provides that administration of UC twice a day is more effective on hyperventilation-induced cerebrovascular reactivity than administration of UC once a day.
