두가지 두부 방사선 사진 중첩 방법의 비교 - Nasion-Sella line 중첩과 Anterior cranial base 중첩 (Comparision of two cephalometric superimposition methods - Nasion-sella line method and Anterior cranial base method)

  • 성장 또는 치료로 인한 악안면골의 변화는 두부 방사선을 중첩 함으로써 판단할 수 있다. 중첩에 대한 평가는 중첩방법에 따라 변화되므로 좀더 정확한 중첩 방법을 선택하는 것은 치료 효과에 대한 분석이나 성장 변화에 대한 평가를 정확히하는 선결 요건이 될것이다. 3년 정도의 비교적 짧은 성장기간을 가진 11세 전후의 여자 환자 19명을 대상으로, 가장 널리쓰이는 중첩 방법인 Nasion-Sells line중첩과 Bjork가 주장하는 Anteirior cranial base (ACB)중첩을 비교하여 다음과 같은 결론을 얻었다. 1 Nasion-Sells line중첩과 ACB중첩은 중첩오차(registration error)의 크기가 거의 같아서 reliability에 있어서 큰차이가 없는 것으로 나타났다 2. 검사자간 차이(interobserver difference)에 있어서 Nasion-Sells line중첩은 7개의 측정 항목 모두에서 두 검사자간에 차이가 없었으나 ACB중첩은 7개의 측정 항목중 4개의 측정 항목에서 두 검사자간 유의성 있는 차이가 나타났다. 3. 중첩방법의 차이 (intermethod difference)에 있어서 Nasion-Sella line중첩으로 측정한 계 측치와 ACB 중첩으로 측정한 계측치가 유의성있게 차이가 나지 않았다.

A reliable method for evaluating upper molar distalization: Superimposition of three-dimensional digital models

  • Objective: The aim of this study was to evaluate the reliability of measurements obtained after the superimposition of three-dimensional (3D) digital models by comparing them with those obtained from lateral cephalometric radiographs and photocopies of plaster models for the evaluation of upper molar distalization. Methods: Data were collected from plaster models and lateral cephalometric radiographs of 20 Class II patients whose maxillary first molars were distalized with an intraoral distalizer. The posterior movements of the maxillary first molars were evaluated using lateral cephalometric radiographs (group CP), photocopies of plaster models (group PH), and digitized 3D models (group TD). Additionally, distalization and expansion of the other teeth and the degrees of molar rotation were measured in group PH and group TD and compared between the two groups. Results: No significant difference was observed regarding the amount of molar distalization among the three groups. A comparison of the aforementioned parameters between group PH and group TD did not reveal any significant difference. Conclusions: 3D digital models are reliable to assess the results of upper molar distalization and can be considered a valid alternative to conventional measurement methods.

Validation of three-dimensional digital model superimpositions based on palatal structures in patients with maximum anterior tooth retraction following premolar extraction

  • Objective: This study aimed to evaluate the superimposition accuracy of digital modes for measuring tooth movement in patients requiring anterior retraction after premolar extraction based on the proposed reference regions. Methods: Forty patients treated with bilateral maxillary first premolar extraction were divided into two groups: moderate retraction (< 7.0 mm) and maximum retraction (≥ 7.0 mm). Central incisor displacement was measured using cephalometric superimpositions and three-dimensional (3D) digital superimpositions with the 3rd or 4th ruga as the reference point. The Wilcoxon signed-rank test and linear regression analyses were performed to test the significance of the differences and relationships between the two measurement techniques. Results: In the moderate retraction group, the central incisor anteroposterior displacement values did not differ significantly between 3D digital and cephalometric superimpositions. However, in the maximum-retraction group, significant differences were observed between the anteroposterior displacement evaluated by the 3rd ruga superimposition and cephalometric methods (p < 0.05). Conclusions: This study demonstrated that 3D digital superimpositions were clinically as reliable as cephalometric superimpositions in assessing tooth movements in patients requiring moderate retraction. However, the reference point should be carefully examined in patients who require maximum retraction.

Total intrusion and distalization of the maxillary arch to improve smile esthetics

  • This case report illustrates the successful treatment of a patient with skeletal Class II malocclusion and an unesthetic smile involving excessive gingival display and large buccal corridors. By applying dual buccal interradicular miniscrews, total intrusion of the maxillary dentition along with distalization was induced to improve both the occlusion and smile esthetics. In addition to the conventional cephalometric superimposition, three-dimensional superimposition was performed and evaluated to validate the treatment outcome.

  • The author has studied each landmark for successful interpretation in the radiograph of the head that have the complex anatomic structures, using panoramic radiograph, postero-anterior cephalometric radiograph, lateral cephalometric radiograph, Waters' radiograph of the skull. The anatomic structures of the human dry skull attached by radiopaque materials were taken radiographs and analysed comparatively. The results were as follows: 1. The overall anatomic structures of the mandible showed sharp images in the panoramic radiograph than other radiographs with relatively less distortion, superimposition, blurring of the image. 2. The anatomic structures were situated on sagital plane of the skull showed blurred images in panoramic radiograph than other radiographs. 3. The anatomic structures which were situated on the basal portion of the skull showed blurred and secondary images in the panoramic radiograph than other radiographs. 4. In the panoramic radiograph, the lower 3rd portion of the orbit appeared to be superimposed with the superior portion of the maxillary sinus and the medial and lateral surface of the nasal cavity showed extensively superimposition of the orbit and the maxillary sinus, which images showed blurring. 5. The inferior surface and posterior surface of maxillary sinus showed to be good image in the panoramic radiograph than other radiographs. 6. In the panoramic radiograph, line of maxillary bone between lateral pterygoid plate, line of maxillary bone between zygomatic bone showed distinct image with another structures.

Validity of palatal superimposition of 3-dimensional digital models in cases treated with rapid maxillary expansion and maxillary protraction headgear

  • Objective: The purpose of this study was to evaluate the validity of the 3-dimensional (3D) superimposition method of digital models in patients who received treatment with rapid maxillary expansion (RME) and maxillary protraction headgear. Methods: The material consisted of pre- and post-treatment maxillary dental casts and lateral cephalograms of 30 patients, who underwent RME and maxillary protraction headgear treatment. Digital models were superimposed using the palate as a reference area. The movement of the maxillary central incisor and the first molar was measured on superimposed cephalograms and 3D digital models. To determine whether any difference existed between the 2 measuring techniques, intra-class correlation (ICC) and Bland-Altman plots were analyzed. Results: The measurements on the 3D digital models and cephalograms showed a very high correlation in the antero-posterior direction (ICC, 0.956 for central incisor and 0.941 for first molar) and a moderate correlation in the vertical direction (ICC, 0.748 for central incisor and 0.717 for first molar). Conclusions: The 3D model superimposition method using the palate as a reference area is as clinically reliable for assessing antero-posterior tooth movement as cephalometric superimposition, even in cases treated with orthopedic appliances, such as RME and maxillary protraction headgear.

삼차원 전산화단층촬영사진과 측모두부 방사선규격사진의 계측자에 따른 계측오차에 대한 비교분석 (Comparison of the observer reliability of cranial anatomic landmarks based on cephalometric radiograph and three-dimensional computed tomography scans)

  • Introduction: Accurate diagnosis and treatment planning are very important for orthognathic surgery. A small error in diagnosis can cause postoperative functional and esthetic problems. Pre-existing 2-dimensional (D) chephalogram analysis has a high likelihood of error due to its intrinsic and extrinsic problems. A cephalogram can also be inaccurate due to the limited anatomic points, superimposition of the image, and the considerable time and effort required. Recently, an improvement in technology and popularization of computed tomography (CT) provides patients with 3-D computer based cephalometric analysis, which complements traditional analysis in many ways. However, the results are affected by the experience and the subject of the investigator. Materials and Methods: The effects of the sources human error in 2-D cephalogram analysis and 3-D computerized tomography cephalometric analysis were compared using Simplant CMF program. From 2008 Jan to 2009 June, patients who had undergone CT, cephalo AP, lat were investigated. Results: 1. In the 3 D and 2 D images, 10 out of 93 variables (10.4%) and 11 out 44 variables (25%), respectively, showed a significant difference. 2. Landmarks that showed a significant difference in the 2 D image were the points frequently superimposed anatomically. 3. Go Po Orb landmarks, which showed a significant difference in the 3 D images, were found to be the artificial points for analysis in the 2 D image, and in the current definition, these points cannot be used for reproducibility in the 3 D image. Conclusion: Generally, 3-D CT images provide more precise identification of the traditional cephalometric landmark. Greater variability of certain landmarks in the mediolateral direction is probably related to the inadequate definition of the landmarks in the third dimension.

측모두부방사선사진을 이용한 상악 중첩 방법의 비교연구 (ARATIVE STUDY OF MAXILLARY SUPERIMPOSITION METHODS ON A LATERAL R NTGENOGRAPHIC CEPHALOMETRY)

  • This study was carried out to compare the amount of the maxillary bone remodeling and tooth displacement in each three maxillary superimposition methods, Ricketts, Best-fit, Structural method. Forty cases of the lateral cephalometric radiographs from 27 boys and 13 girls who had been treated to correct anterior cross-bite were selected for the study. The initial radiographs were taken at about 8-year-old and the second radiographs were taken in about 3.3 years later. Followings were the results: 1. With the Structural method, backward movement was shown in PNS, while forward movement was observed in ANS and point A. With the Ricketts method, however, all structures were shown significant backward movement comparing with Structural method(P<0.05). With the Best-fit method, the amount of horizontal movement was similar to that of the Structural method(P>0.05). 2. The palate seemed to be moved downward with Structural method, but there was no measured downward remodeling on nasal floor with Ricketts and Best-fit method(P<0.05). 3. Comparing with Structural method, Ricketts and Best-fit method significantly underestimated the eruption of the teeth by 20% to 30% (P<0.05). 4. The Structural method showed the anteroinferior rotation (43%) and posteroinferior rotation(57%) of the palatal plane, while the Best-fit method showed mostly anterosuperior rotation(87%), but no change was found in the Ricketts method. 5. With the Structural method, there was a statistically significant correlation between the amount of the rotation of the palatal plane and that of N-S line(r=0.86). 6. The measured angles of the long axis of the incisors and molars showed no significant difference in each 3 methods(P>0.05).

임상교정 교육용 멀티미디어 데이터베이스 웹서버 구축에 관한 연구 (The establishment of orthodontic web server multimedia database system for continuing education)

  • 본 연구는 인터넷상의 가상공간에 다양한 자료로 구성된 환자 증례를 통해, 교정을 배우고자 하는 임상의나 학생들의 교육에 필요한 멀티미디어 데이터베이스를 구축하기 위한 것이다. 이러한 시스템을 구현하기 위해서는 이미지 자료의 처리, computer network를 통한 빠른 정보처리 기능, 진단 분석법의 개발, 데이터베이스의 구축 기술, 데이터베이스와 인터넷과의 연계, 증례를 통한 교육방법론과 같은 기술적 문제점들이 있다. 진단과 치료계획을 위한 분석은 모델 분석과 두부방사선 사진분석으로 나누어 제공하였다. 모델분석법은 arch length discrepancy와 Bolton tooth ratio를 환자 정보란에 제공하였다. 두부방사선 사진분석은 초진란에 제공하였다. Cephalometric analysis ver 2.0과 Power Ceph pro 3.3.5를 이용하여 Tweed, Steiner, Jarabak분석법 등을 제공하였으나, 본문에서는 주로 Kim's analysis와 몇 가지 계측치를 추가하여 설명하였고, 나머지는 tracing란에서 제시하였다. 또한 치료 종료 후나 보정란에서 치료 전,후의 두부방사선 사진을 중첩해서 보여줌으로써 치료에 의한 효과를 직접 볼 수 있도록 하였다.

3차원 CT자료에서 선정된 계측점을 정중시상면으로 투사한 영상과 두부계측방사선사진상의 계측정의 위치 비교 (Comparison of landmark position between conventional cephalometric radiography and CT scans projected to midsagittal plane)

  • 본 연구는 두부계측방사선사진에서 선정한 계측점과, 3차원 CT영상에서 계측점을 선정하고 이를 정중시상면으로 투영하였을때 두 계측점 사이의 위치적 연관성에 대해 알아보고자 시행하였다. III급 부정교합을 주소로 서울대학교 치과병원에 내원한 환자 20명을 대상으로 술 전에 CT와 두부방사선사진을 촬영하였다. CT자료에서 계측점을 선정하고, 정중시상면을 기준으로 투사영상을 얻은 후에 이것을 110%로 확대하였다. 전두면과 후두골의 외연을 기준으로 두부방사선사진 투사도와 CT자료의 정중시상면 투사영상을 중첩하고, FH평면을 기준으로 공통 좌표계를 설정하였다. 이 좌표계를 기준으로 얻은 계측점 좌표값 차이의 평균과 표준편차를 구하고 paired t test를 시행하였다. X축은 $-0.14{\pm}0.65$에서 $-2.12{\pm}2.89\;mm$, Y축은 $0.34{\pm}0.78$에서 $-2.36{\pm}2.55\;mm$ ($6.79{\pm}3.04\;mm$)의 범위를 보였으며, 20개의 계측점 중 X축은 9개에서, Y축은 7개에서 통계적으로 유의한 차이가 없는 것으로 나타났다. 이러한 오차는 촬영자세에 따라 악골의 위치가 변화한 경우, 골단부에 위치함으로써 주변구조물에 가려진 경우, 해부학적 구조물의 중첩에 따른 식별오차, 계측점의 정의가 다른 경우 발생할 수 있다.
