Malignancy in Patients With Inborn Errors of Immunity Beyond Infectious Complication: Single Center Experience for 30 Years

  • 목적: 선천면역장애 환자들은 감염에 취약할 뿐만 아니라, 면역이 정상인 사람들에 비해 암 발생률도 높은 것으로 알려져 있다. 본 연구는 단일 기관에서 추적 중인 선천면역장애 환자들에서의 암 발생을 조사하여 보고자 하였다. 방법: 1994년 11월부터 2023년 9월까지 삼성서울병원에서 선천면역장애 진단 하에 추적하는 환자를 대상으로 후향적으로 의무기록을 리뷰하였다. 선천면역장애 환자 중에서 암으로 진단된 환자를 확인하였다. 결과: 총 194명의 선천면역장애 환자 중, 7명(3.6%)의 환자에서 암이 진단되었다. 5명의 환자가 림프종으로 진단받았으며 그 중 4명의 환자는 Epstein-Barr 바이러스 연관 림프종이었다. 나머지 암은 위암과 다발 골수종이었다. 암 진단 당시 나이는 중앙값 18세 (범위, 1세–75세)였다. 암이 발생한 환자들의 면역결핍 질환은 X-linked lymphoproliferative disorder-1 (XLP-1) 3명, activated phosphoinositide 3-kinase delta disease (APDS) 2명, cytotoxic T-lymphocyte antigen 4 (CTLA-4) haplo-insufficiency 2명이었다. 개별 질환별로 분석하였을 때, XLP-1 환자의 75.0%, APDS 환자의 40.0%, CTLA-4 환자의 50.0%에서 암이 발생하였다. XLP-1 환자는 APDS 및 CTLA-4 haplo-insufficiency 환자에 비해 더 이른 나이에 암이 발생하였다 (중앙연령 5세, P<0.001). 한 명은 조혈모세포 이식 치료 중 사망하였다. 결론: 국내 단일 기관에서 진료받는 선천면역장애 환자들의 3.6%에서 암이 발생하였다. 선천면역장애 환자들을 진료하는 의료진들은 이들 환자에서 감염이나 염증 등의 문제외에도 암 발생의 가능성, 특히 Epstein-Barr 바이러스 감염과 연관된 암의 비중이 높은 것에 대한 인식을 갖는 것이 중요하다.

섬쑥부쟁이 에탄올 추출물이 대식세포와 예쁜꼬마선충에서의 항염증 및 항산화 효과 (Ethanol extract of Aster glehni exhibits anti-inflammatory and anti-oxidant effects in RAW 264.7 cells and Caenorhabditis elegans)

  • 본 연구는 울릉 섬쑥부쟁이의 생리활성을 평가하여 기능성 농식품 소재로서의 자료 제공을 목적으로 하였다. 대식세포와 선충을 활용하여 울릉 섬쑥부쟁이의 항산화 및 항염증 효과를 평가하였다. 총폴리페놀 함량과 총플라보노이드 함량은 AG와 DAG의 에탄올(70%, 100%) 용매가 열수보다 추출수율이 높았으며, 추출 조건별 항산화 활성 분석도 열수보다는 에탄올 추출물에서 가장 높았다. AG의 경우 100% 에탄올 추출물(4.50%)이, DAG의 경우 70% 에탄올 추출물(4.19%)로 수율이 가장 높았다. 항산화 성분의 추출수율과 라디컬소거 활성 결과와 식품소재 활용을 고려하여 70% 에탄올을 최종 용매로 선정하였다. 시료의 효과평가를 위해 세포독성이 없는 100 ㎍/mL를 최대 농도로 설정하여 대식세포 RAW 264.7에서의 항염증 활성을 평가하였다. LPS로 염증을 유도하고 AG와 DAG 에탄올 추출물 처리군은 LPS 단독 처리군(양성 대조군)과 비교하여 NO의 생성과 iNOS의 발현이 유의적으로 감소하였다. 선충을 이용한 AG와 DAG 에탄올 추출물의 항산화 활성 분석 결과, juglone 처리로 인한 산화적 스트레스 조건하에서 선충의 생존율 연장에는 영향을 미치지 못했다. 선충에 LPS를 처리하여 염증 스트레스 조건하에서 AG와 DAG 에탄올 추출물이 LPS 단독 처리군에 비해 24시간 후 생존율이 증가하였고, 두 시료 모두 생존율 개선 효과가 농도 의존적으로 보였다. 염증과 산화의 스트레스 조건 없이 선충의 평균 수명 기간인 20일 동안 AG와 DAG 추출물을 처리하여 항노화 효과를 평가한 결과 AG와 DAG 에탄올 추출물을 처리한 선충의 생존율 중앙값(10일)은 CON보다 높았으며, DAG 에탄올 추출물은 농도 의존적으로 생존율을 높이는 경향을 보였으나, AG 에탄올 추출물은 그렇지 않았다. AG와 DAG의 20일 생존율은 CON보다 낮거나 유사(DAG 50)하였다. 본 연구의 결과, 울릉 섬쑥부쟁이 에탄올 추출물은 in vitro에서 항산화 활성을 보였고, 염증이 유도된 대식세포에서 iNOS 유전자 발현을 억제하여 NO의 생성량이 감소하였다고 사료된다. 또한, 염증 스트레스를 유도한 선충의 생존율 개선 효과를 보였다. 이 결과는 울릉 섬쑥부쟁이의 건강기능식품 개발 시 기초자료로 활용하고, 지역 농산물의 소비 활성화에 도움이 될 수 있을 것으로 기대된다.

식물 성장 촉진 활성을 가진 Bacillus amyloliquefaciens Y10에 의한 가금 우모의 분해 및 생산된 우모 분해산물의 생리활성 (Degradation of Poultry Feathers by Bacillus amyloliquefaciens Y10 With Plant Growth-promoting Activity and Biological Activity of Feather Hydrolyzates)

  • 가금류 우모는 환경오염 물질이자 병원성 세균의 저장소로 간주되는 축산폐기물이다. 따라서 우모 폐기물을 관리하기 위한 지속 가능하고, 환경친화적인 방법을 개발하는 것은 매우 중요하다. 본 연구는 폐기된 닭의 우모로부터 분리된 Y10 균주를 동정하고, 특성화하기 위하여 수행되었다. Y10 균주는 표현형 및 16S rRNA 유전자 분석을 통해 Bacillus amyloliquefaciens에 속하는 것으로 확인되었다. B. amyloliquefaciens Y10은 곰팡이 세포성분을 분해하는 효소(cellulase, lipase, protease and pectinase), siderophore, 암모니아 및 IAA 생성과 같은 식물 성장 촉진 활성을 나타내었다. 나아가 Y10 균주는 일부 식물병원성 곰팡이의 균사체 생육을 억제할 수 있었다. 기본배지에 탄소원으로 sucrose 0.1%, 질소원으로 casein 0.05%를 첨가한 후, 배지의 pH를 10으로 조정하여 35℃에서 배양했을 때, 실험균주에 의한 우모 분해율은 기본배지 대비 약 2배 증가되었으며, 배양 4일에 우모는 완전히 분해되었다. 또한 실험균주는 개선된 조건에서 오리 우모, 양모, 사람의 손발톱과 같은 다양한 케라틴 기질을 분해할 수 있었다. Y3 균주를 이용하여 조제된 우모 분해산물은 DPPH 라디칼 소거능(EC50 = 0.38 mg/ml)과 SOD 유사활성(EC50 = 183.7 mg/ml)과 같은 항산화능이 있음을 확인하였다. 이 결과는 실험균주는 케라틴 폐기물의 미생물학적 처리뿐만 아니라 농축산 산업에 적용할 수 있는 생물 비료, 생물 농약 및 사료첨가제 개발의 잠재적인 후보가 될 수 있을 시사한다.

복령 균핵의 에탄올 추출물에서 분리한 화합물의 조골세포 분화 촉진 및 파골세포 생성 억제 효과 (Effects of Compounds Isolated from an Ethanol Extract of the Sclerotium of Wolfiporia hoelen on Osteoblast Differentiation and Osteoclast Formation)

  • 복령(Wolfiporia hoelen (Fr.) Y.C.Dai & V. Papp)은 아시아 및 유럽 등 많은 나라에서 약용 및 식용으로 사용하고 있는 중요한 약재이다. 복령의 주요 성분들의 약리 활성이 많은 연구를 통해 밝혀지고 있으며, 골대사에도 영향을 주는 것으로 나타났다. 이에 본 연구는 복령 균핵 50% 에탄올 추출물이 골질환 예방 및 천연 치료제의 소재로 활용 가능성이 있는지 알아보고자 하였다. 복령 균핵의 에탄올 추출물을 계통분획법에 따라 n-헥산, 디클로로메탄, 에틸아세테이트순으로 분획하여 C2C12세포에 처리하였으며, 디클로로메탄 분획물에서 C2C12 세포의 ALP 분화 활성이 대조구보다 약 29%가 증가함을 알 수 있었다. 활성을 보인 디클로로메탄 분획물로부터 총 4개의 화합물을 분리하였으며, 기기분석 결과와 문헌을 참고하여 3α-dehydrotrametenolic acid, ergosterol, pachymic acid와 dehydrotumulosic acid로 동정할 수 있었다. 골대사 효능이 있는 지표물질을 찾기 위하여 4개의 화합물을 C2C12 세포의 ALP 분화 활성과 BMMs의 RANKL 유도 억제 활성을 농도별(1, 3, 10, 30, 100 μM)로 평가하였으며, 세포독성 검정도 동일한 농도에서 수행하였다. 그 결과 분리된 화합물 중 dehydrotumulosic acid의 3 μM 농도에서 ALP 활성이 160%로 나타났으며, BMP-2 대조구보다 약 24% 유의미하게 증가하였다. 또한 BMMs의 RANKL 유도 억제 활성은 dehydrotumulosic acid를 10-100 μM 농도로 처리할 때 대조구보다 약 15-86%로 유의미하게 감소하였으며, 10 μM 농도에서 나타난 RANKL억제 활성은 세포독성과 다른 패턴으로 나타나는 것을 확인하였다. 본 연구결과를 토대로 복령의 균핵 에탄올 추출물이 골모세포 분화를 증진시키거나 억제할 수 있는 가능성을 확인하였으며, 복령의 디클로로메탄 분획물로부터 분리 정제된 dehydrotumulosic acid가 골다공증 예방 및 치료에 도움을 줄 수 있을 것으로 사료된다.

Field Studios of In-situ Aerobic Cometabolism of Chlorinated Aliphatic Hydrocarbons

  • Results will be presented from two field studies that evaluated the in-situ treatment of chlorinated aliphatic hydrocarbons (CAHs) using aerobic cometabolism. In the first study, a cometabolic air sparging (CAS) demonstration was conducted at McClellan Air Force Base (AFB), California, to treat chlorinated aliphatic hydrocarbons (CAHs) in groundwater using propane as the cometabolic substrate. A propane-biostimulated zone was sparged with a propane/air mixture and a control zone was sparged with air alone. Propane-utilizers were effectively stimulated in the saturated zone with repeated intermediate sparging of propane and air. Propane delivery, however, was not uniform, with propane mainly observed in down-gradient observation wells. Trichloroethene (TCE), cis-1, 2-dichloroethene (c-DCE), and dissolved oxygen (DO) concentration levels decreased in proportion with propane usage, with c-DCE decreasing more rapidly than TCE. The more rapid removal of c-DCE indicated biotransformation and not just physical removal by stripping. Propane utilization rates and rates of CAH removal slowed after three to four months of repeated propane additions, which coincided with tile depletion of nitrogen (as nitrate). Ammonia was then added to the propane/air mixture as a nitrogen source. After a six-month period between propane additions, rapid propane-utilization was observed. Nitrate was present due to groundwater flow into the treatment zone and/or by the oxidation of tile previously injected ammonia. In the propane-stimulated zone, c-DCE concentrations decreased below tile detection limit (1 $\mu$g/L), and TCE concentrations ranged from less than 5 $\mu$g/L to 30 $\mu$g/L, representing removals of 90 to 97%. In the air sparged control zone, TCE was removed at only two monitoring locations nearest the sparge-well, to concentrations of 15 $\mu$g/L and 60 $\mu$g/L. The responses indicate that stripping as well as biological treatment were responsible for the removal of contaminants in the biostimulated zone, with biostimulation enhancing removals to lower contaminant levels. As part of that study bacterial population shifts that occurred in the groundwater during CAS and air sparging control were evaluated by length heterogeneity polymerase chain reaction (LH-PCR) fragment analysis. The results showed that an organism(5) that had a fragment size of 385 base pairs (385 bp) was positively correlated with propane removal rates. The 385 bp fragment consisted of up to 83% of the total fragments in the analysis when propane removal rates peaked. A 16S rRNA clone library made from the bacteria sampled in propane sparged groundwater included clones of a TM7 division bacterium that had a 385bp LH-PCR fragment; no other bacterial species with this fragment size were detected. Both propane removal rates and the 385bp LH-PCR fragment decreased as nitrate levels in the groundwater decreased. In the second study the potential for bioaugmentation of a butane culture was evaluated in a series of field tests conducted at the Moffett Field Air Station in California. A butane-utilizing mixed culture that was effective in transforming 1, 1-dichloroethene (1, 1-DCE), 1, 1, 1-trichloroethane (1, 1, 1-TCA), and 1, 1-dichloroethane (1, 1-DCA) was added to the saturated zone at the test site. This mixture of contaminants was evaluated since they are often present as together as the result of 1, 1, 1-TCA contamination and the abiotic and biotic transformation of 1, 1, 1-TCA to 1, 1-DCE and 1, 1-DCA. Model simulations were performed prior to the initiation of the field study. The simulations were performed with a transport code that included processes for in-situ cometabolism, including microbial growth and decay, substrate and oxygen utilization, and the cometabolism of dual contaminants (1, 1-DCE and 1, 1, 1-TCA). Based on the results of detailed kinetic studies with the culture, cometabolic transformation kinetics were incorporated that butane mixed-inhibition on 1, 1-DCE and 1, 1, 1-TCA transformation, and competitive inhibition of 1, 1-DCE and 1, 1, 1-TCA on butane utilization. A transformation capacity term was also included in the model formation that results in cell loss due to contaminant transformation. Parameters for the model simulations were determined independently in kinetic studies with the butane-utilizing culture and through batch microcosm tests with groundwater and aquifer solids from the field test zone with the butane-utilizing culture added. In microcosm tests, the model simulated well the repetitive utilization of butane and cometabolism of 1.1, 1-TCA and 1, 1-DCE, as well as the transformation of 1, 1-DCE as it was repeatedly transformed at increased aqueous concentrations. Model simulations were then performed under the transport conditions of the field test to explore the effects of the bioaugmentation dose and the response of the system to tile biostimulation with alternating pulses of dissolved butane and oxygen in the presence of 1, 1-DCE (50 $\mu$g/L) and 1, 1, 1-TCA (250 $\mu$g/L). A uniform aquifer bioaugmentation dose of 0.5 mg/L of cells resulted in complete utilization of the butane 2-meters downgradient of the injection well within 200-hrs of bioaugmentation and butane addition. 1, 1-DCE was much more rapidly transformed than 1, 1, 1-TCA, and efficient 1, 1, 1-TCA removal occurred only after 1, 1-DCE and butane were decreased in concentration. The simulations demonstrated the strong inhibition of both 1, 1-DCE and butane on 1, 1, 1-TCA transformation, and the more rapid 1, 1-DCE transformation kinetics. Results of tile field demonstration indicated that bioaugmentation was successfully implemented; however it was difficult to maintain effective treatment for long periods of time (50 days or more). The demonstration showed that the bioaugmented experimental leg effectively transformed 1, 1-DCE and 1, 1-DCA, and was somewhat effective in transforming 1, 1, 1-TCA. The indigenous experimental leg treated in the same way as the bioaugmented leg was much less effective in treating the contaminant mixture. The best operating performance was achieved in the bioaugmented leg with about over 90%, 80%, 60 % removal for 1, 1-DCE, 1, 1-DCA, and 1, 1, 1-TCA, respectively. Molecular methods were used to track and enumerate the bioaugmented culture in the test zone. Real Time PCR analysis was used to on enumerate the bioaugmented culture. The results show higher numbers of the bioaugmented microorganisms were present in the treatment zone groundwater when the contaminants were being effective transformed. A decrease in these numbers was associated with a reduction in treatment performance. The results of the field tests indicated that although bioaugmentation can be successfully implemented, competition for the growth substrate (butane) by the indigenous microorganisms likely lead to the decrease in long-term performance.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500,550,570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500,550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100,300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. On this base, we composed of the radial pulse diagnosis system in korean traditional medicine. The system composed of a radial pulse wave detection system and a radial pulse diagnosis system. With a detection system, we detected Inyoung and Cheongu radial pulse wave and processed it. Then, we have got the characteristic parameters of radial pulse wave and also quantified that according to the method of Inyoung-Cheongu Comparison Radial Pulse Diagnosis. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area.
