The Status of Treatment of the Physically Handicapped in a Large City (일부 도시 재가 지체장애인의 치료실태)

  • To find out the actual status of treatment of physically handicapped who were stayed of home, 320 physically handicapped persons were selected among total 6,264 physically handicapped registered in Taejon city, surveyed from January 1st to March 30, 1997. Following are the results ana lysed of collected data from 201 samped persons. 1. For the cause of physically handicapped, 36.3% was congenital, 26.9% was cerebralvascular acciednt(CVA). and 14.9% was infectious diseases and others in that orders. The must frequest cause of below 20 years age group was congenital cause, but that of $20{\sim}30$ years age group and $40{\sim}50$ years age group were accident and CVA repectively. 30.0% of respondents caused by infection and others earned household living expenses. Household living expenses were higher among the respondents caused by congenital cause but those were lowest in the accidental couse group. Handicap durations were longer in the accidental cause group and infections and others group than the congenital cause group and CVA group. 2. 54.7% of studied handicapped were under treatment. But 36.3% of them were stopped treatment, and 8.9 % of them answered not treated. Most handicapped, coused by congenital and CAV, were under treatment, but most of them caused by accidental and others were stopped treatment. 3. For the medical facilities, 54.5% of them utilized welfare facility, but 25.5% utilized general hospital and 20.0% of them utilized oriental medical hospital and health centers. Congenital cause group frequently selected welfare facility for the treatment facility. But accidental cause group frequently seleced general hospitals and CVA gruop frequently selected oriental medical hospital and others. The medical cost of welfare facility, oriental medical hospitals and others were lower than that of general hospitals. 4. The proportions of under treatment were higher among yonger age higher monthly living expenses group shorter handicapped duration groups, congenital cause group and handicap grade II group. But that of stop treatment were higher among congenital cause group, infectious and others cause group, and handicap grade I & then III group. 5. For the feelings satisfacion about medical treatment, only 35.3% of under treated group responed satisfation. 55.5% of them responded so-so, but 9.1 % responed dissatisfaction. The proportion of satisfaction was higher among shorter handicap duration group.

Vascular Plant Species in the Southern Sejong (세종시 남부일대의 관속식물상 연구)

  • The flora of Sejong were summarized as 531 taxa including 104 families, 315 genera, 462 species, 3 subspecies, 59 varieties and 7 form. The rare plants were 8 taxa including Nepeta cataria L. and Aristolochia ontorta Bunge. The Korea endemic plants were 9 taxa including Populus tomentiglandulosa T. B. Le and Clematis trichotoma Nakai.. The specific plants were 42 taxa including Phytolacca esculenta Van Houtte(V), Corydalis decumbens (Thunb.) Pers(IV), Poncirus trifoliata Raf(III), Pseudostellaria coreana (Nakai) Ohwi(II), Cyrtomium fortunei J.Sm.(I). The naturalized plants were 49 taxa including Sicyos angulatus L, Ambrosia trifida L, Physalis angulata L. Panicum dichotomiflorum Michx., Galinsoga parviflora Cav, and Erigeron philadelphicus L. Especially, Sicyos angulatus L., Phytolacca americana L. and Bidens frondosa L. have been spread throughout the country are required to manage. The naturalized index(NI) and urbanized index(UI) were 8.7% and 15.3%. Useful plants incuding Edible was 199 taxa, Pasture was 193 taxa, Medicinal was 178 taxa, Ornamental was 82 taxa, Timber was 25 taxa, Fiber was 18 taxa, Stain was 9 taxa. Useful plants will be important materials for plant design in recreational and healing forests.

  • A 5-year-old, intact female Shih-Tzu dog was presented with 1 year history of icterus, ascites and anorexia. The serum biochemistry revealed elevated liver enzyme levels. Microhepatica and decreased serosal detail were detected in abdominal radiography. Abdominal ultrasonographic findings included irregular liver margins, multifocal hypoechoic nodules in the liver parenchyma, and ascites. Computed tomography (CT) showed multifocal hypodense nodules with ring-like contrast enhancement. Cytologic and histopathologic examination by liver core biopsy revealed fibrosis. Cirrhosis was diagnosed based on above results. This report focuses on the imaging characteristics of ultrasonography and CT for liver cirrhosis in a dog.

Damage and vibrations of nuclear power plant buildings subjected to aircraft crash part I: Model test

  • Investigations of large commercial aircraft impact effect on nuclear power plant (NPP) buildings have been drawing extensive attentions, particularly after the 9/11 event, and this paper aims to experimentally assess the damage and vibrations of NPP buildings subjected to aircraft crash. In present Part I, two shots of reduce-scaled model test of aircraft impacting on NPP building were carried out. Firstly, the 1:15 aircraft model (weighs 135 kg) and RC NPP model (weighs about 70 t) are designed and prepared. Then, based on the large rocket sled loading test platform, the aircraft models were accelerated to impact perpendicularly on the two sides of NPP model, i.e., containment and auxiliary buildings, with a velocity of about 170 m/s. The strain-time histories of rebars within the impact area and acceleration-time histories of each floor of NPP model are derived from the pre-arranged twenty-one strain gauges and twenty tri-axial accelerometers, and the whole impact processes were recorded by three high-speed cameras. The local penetration and perforation failure modes occurred respectively in the collision scenarios of containment and auxiliary buildings, and some suggestions for the NPP design are given. The maximum acceleration in the 1:15 scaled tests is 1785.73 g, and thus the corresponding maximum resultant acceleration in a prototype impact might be about 119 g, which poses a potential threat to the nuclear equipment. Furthermore, it was found that the nonlinear decrease of vibrations along the height was well reflected by the variations of both the maximum resultant vibrations and Cumulative Absolute Velocity (CAV). The present experimental work on the damage and dynamic responses of NPP structure under aircraft impact is firstly presented, which could provide a benchmark basis for further safety assessments of prototype NPP structure as well as inner systems and components against aircraft crash.

The effects according to the timing of thoracic radiotherapy in limited stage small cell lung cancer (제한병기 소세포폐암에서 흉부 방사선 치료의 도입 시기에 따른 치료 효과의 비교 분석)

  • Background: Combination chemotherapy is now considered to be the cornerstone of small cell lung cancer (SCLC). management but the optimal management of limited SCLC is not well defined. The role of thoracic radiotherapy (TRT) is less well established. Recent meta-analyses reports revealed that TRT combined with chemotherapy produce "good" local control and prolonged survival. But other reports that survival was not changed. The liming, dose, volume and fractionation for TRT with the combined chemotherapy of SCLC remains unsettled. In this study, we analyzed the effects according to the timing of thoracic radiotherapy in limited SCLC. Method: All fifty one patients received cytoxan, adriamycin and vincristine(CAV) alternating with etoposide and cisplatin(VPP) every 3 weeks for 6 cycles were randomized prospectively into two groups: concurrent and sequential. 27 patients received 4500cGy in 30 fractions(twice daily 150cGy fractional dose) over 3 weeks 10 the primary site concurrent with the first cycle of VPP(concurrent gorup). 24 patients received 4000 to 5000cGy over 5 or 6 weeks after completion of sixth cycles of chemotherapy(sequential group). Results: 1. Response rates and response duration : Response rates were not significantly different between two groups(p=0.13). But response duration was superior in the concurrent group(p=0.03). 2. Survival duration was nor different between two groups(p=0.33). 3. Local control rate was superior in the concurrent group(p=0.00). 4. Side effects and toxicities: Hematologic toxicities, especially leukopenia, infection and frequency of radiation esophagitis were higher in the concurrent group (p=0.00, 0.03, 0.03). Conclusion: The concurrent use of TRT with chemotherapy failed to improve the survival of limited stage SCLC patients compared with the sequential use of TRT but response duration and local control rate were superior in the concurrent group. Frequency of radiation esophagitis, life threatening hematologic toxicities and infection were more frequent in the concurrent group than sequential group. So, the selection of an optimal schedule of chemotherapy combined with TRT that would lead to a major increase in survival with minimal toxicity is remained to be validated in large scale study in the future.

Once vs. Twice Daily Thoracic Irradiation in Limited Stage Small Cell Lung Cancer (국한성 병기 소세포폐암의 방사선치료시 분할 조사방식에 따른 치료성적)

  • Purpose : A retrospective study was conducted comparing single daily fraction (SDF) thoracic radiotherapy (TRT) with twice daily (BID) TRT to determine the potential benefit of BID TRT in limited-stage small cell lung cancer (SCLC). Endpoints of the study were response. survival, pattern of failure, and acute toxicity. Materials and Methods : Between November 1989 to December 1996, 78 patients with histologically proven limited-stage SCLC were treated at the Department of Therapeutic Radiology, Chungnam National University Hospital. Of these, 9 were irradiated for palliative intent, and 1 had recurrent disease. Remaining 68 patients were enrolled in this study. There were 26 patients with a median age of 58 years, and 22 (85$\%$) ECOG performance score of less than 1 in SDF TRT. There were 42 patients with a median age of 57 years, and 36 (86$\%$) ECOG performance score of less than 1 in BID TRT By radiation fractionation regimen, there were 26 in SDF TRT and 42 in BID TRT. SDF TRT consisted of 180 cGy, 5 days a week. BID TRT consisted of 150 cGy BID, 5 days a week in 13 of 42 and 120 cGy BID, in 29 of 42. And the twice daily fractions were separated by at least 4 hours. Total radiotherapy doses were between 5040 and 6940 cGy (median, 5040 cGy) in SDF TRT and was between 4320 and 5100 cGy (median, 4560 cGy) in BID TRT. Prophylactic cranial irradiation (PCI) was recommended for patients who achieved a CR. The recommended PCI dose was 2500 cGy/10 fractions. Chemotherapy consisted of CAV (cytoxan 1000 mg/$m^2$, adriamycin 40 mg/$m^2$, vincristine 1 mg/$m^2$) alternating with VPP (cisplatin 60 mg/$m^2$, etoposide 100 mg/$m^2$) every 3 weeks in 25 (96$\%$) of SDF TRT and in 40 (95$\%$) of BID TRT. Median cycle of chemotherapy was six in both group. Timing for chemotherapy was sequential in 23 of SDF TRT and in 3 BID TRT, and concurrent in 3 of SDF TRT and in 39 of BID TRT Follow-up ranged from 2 to 99 months (median, 14 months) in both groups. Results : Of the 26 SDF TRT, 9 (35$\%$) achieved a complete response (CR) and 14 (54$\%$) experienced a partial response (PR). Of the 42 BID TRT, 18 (43$\%$) achieved a CR and 23 (55$\%$) experienced a PR. There was no significant response difference between the two arms (p=0.119). Overall median and 2-year survival were 15 months and 26.8$\%$, respectively. The 2-year survivals were 26.9$\%$ and 28$\%$ in both arm, respectively (p=0.51). The 2-rear survivals were 35$\%$ in CR and 24.2$\%$ in PR, respectively. The grade 2 to 3 esophageal toxicities and grade 2 to 4 neutropenias were more common in BID TRT (p=0.028 0.003). There was no difference in locoregional and distant metastasis between the two arms (p=0 125 and 0.335, respectively). The most common site of distant metastasis was the brain. Conclusion : The median survival and 2-year survival were 17 months and 20.9$\%$ in SDF TRT with sequential chemotherapy, and 15 months and 28$\%$ in BID TRT with concurrent chemotherapy, respectively. We did not observe a substantial improvement of long-term survival in the BID TRT with concurrent chemotherapy compared with standard schedules of SDF TRT with sequential chemotherapy. The grade 2 to 3 esophageal toxicities and glade 2 to 4 neutropenias were more common in BID TRT with concurrent chemotherapy. Although the acute toxicities were more common in BID TRT with concurrent chemotherapy than SDF TRT with sequential chemotherapy, a concurrent chemotherapy and twice daily TRT was feasible. However further patient accrual and long-term follow up are needed to determine the potential benefits of BID TRT in limited-stage SCLC.

The Spin-Rotation Interaction of the Proton and the Fluorine Nucleus in the Tetrahedral Spherical Top Molecules

  • The spin-rotation constants of the proton and tile fluorine nucleus in C $H_4$, Si $H_4$, Ge $H_4$, C $F_4$, Si $F_4$ and Ge $F_4$ were determined experimentally by the molecular beam magnetic resonance method. From the Hamiltonian and the high field approximation, the quantized energy level is given by the following equation. W $m_{I}$ $m_{J}$=- $g_{I}$ $m_{I}$H- $g_{J}$ $m_{J}$H- $C_{av}$ $m_{I}$ $m_{J}$, where $c_{av}$ is one third of the trace of the C tensor. In the nuclear resonance experiment, the proton and the fluorine nuclear resonance curves consist of many unresolved lines given by v=- $g_{J}$H- $C_{av}$ $m_{I}$, and a Gaussian approximation is made to correlate $c_{av}$ to the experimentally obtained half-width of the resonance curve. In the rotational resonance experiment, the five resonance peaks as predicted by v=- $g_{I}$H- $c_{av}$ $m_{I}$, $m_{I}$=0, $\pm$1 and $\pm$2, were all observed. The magnitude of car was determined by measuring the frequency distance between two adjacent peaks. The sign of $c_{av}$ was determined by the side peak suppression technique. The technique is described, and the sign and magnitude of the spin-rotation constant cav are summarized as following: for C $H_4$ -10.3$\pm$0.4tHz(from the rotational resonance), for SiH +3.71$\pm$0.08kHz(from the nuclear resonance), for Ge $H_4$+3.79$\pm$0.13kHz(from the nuclear resonance), for C $F_4$, -6.81$\pm$0.08kHz(from the rotational resonance), for Si $F_4$, -2.46$\pm$0.06kHz(from the rotational resonance), and finally for Ge $F_4$-1.84$\pm$0.04kHz(from the rotational resonance).onal resonance).esonance).

  • In order to investigate histopathologically the nature of varietal resistance and infection process of the rice bast fungus, Pyricularia oryzae. this experiment was undertaken by using the resistant cultivar Tongil and susceptible cultivars Norin No.6 and Jinheung in 1973 to 1974. 1) It was found that appressorium of the fungus forms not only at 4 hours after incubation under the favorable conditions but also peak of appressorial formation is at 48 hours treatment. Physical stimulus known to be a definite factor for appressorial formation. The optimum temperature range for appressorial infection was at $24^{\circ}C$ to $28^{\circ}C$, and pH was between 4.8 to 8.0 with 6.8 as the optimum. 2) Although percent of appressorial formation on the leaves of resistant Tongil and susceptiible Norin No.6 were only slightly different, there was a remarkable difference between resistant and susceptible cultivars with regard to percent of hyphal infection and index for hyphal extension. Index of hyphal extension was 1.6-2.7 in Tonyil. while in susceptible cultivar was 3.4-6.6. The rate of discoloration of infected cells, a indication of hypersensitivity, was greater in the resistant than in susceptible cultvar. 3) Therefore, it could be concluded that resistance of Tongil cultivar may be attributable to a higher degree of mechanical barriers as well as a higher level of antifungal substance accumulation.

  • Erwinia carotovora subsp. carotovora, Pseudomonas viridiflava and Pseudomonas marginalis pv. marginalis were tested for their pathogenicity to 35 kinds of domestic flowering plants. Among them, the following domestic flowering plants showed clear symptoms. 1. Erwinia carotovora subsp. carotovora : (Needle inoculation). Development of the Information Delivery System for the Home Nursing Service (가정간호사업 운용을 위한 정보전달체계 개발 I (가정간호 데이터베이스 구축과 뇌졸중 환자의 가정간호 전산개발))

  • The purpose of the study was to development an information delivery system for the home nursing service, to demonstrate and to evaluate the efficiency of it. The period of research conduct was from September 1996 to August 31, 1997. At the 1st stage to achieve the purpose, Firstly Assessment tool for the patients with cerebral vascular disease who have the first priority of HNS among the patients with various health problems at home was developed through literature review. Secondly, after identification of patient nursing problem by the home care nurse with the assessment tool, the patient's classification system developed by Park (1988) that was 128 nursing activities under 6 categories was used to identify the home care nurse's activities of the patient with CAV at home. The research team had several workshops with 5 clinical nurse experts to refine it. At last 110 nursing activities under 11 categories for the patients with CVA were derived. At the second stage, algorithms were developed to connect 110 nursing activities with the patient nursing problems identified by assessment tool. The computerizing process of the algorithms is as follows: These algorithms are realized with the computer program by use of the software engineering technique. The development is made by the prototyping method, which is the requirement analysis of the software specifications. The basic features of the usability, compatibility, adaptability and maintainability are taken into consideration. Particular emphasis is given to the efficient construction of the database. To enhance the database efficiency and to establish the structural cohesion, the data field is categorized with the weight of relevance to the particular disease. This approach permits the easy adaptability when numerous diseases are applied in the future. In paralleled with this, the expandability and maintainability is stressed through out the program development, which leads to the modular concept. However since the disease to be applied is increased in number as the project progress and since they are interrelated and coupled each other, the expand ability as well as maintainability should be considered with a big priority. Furthermore, since the system is to be synthesized with other medical systems in the future, these properties are very important. The prototype developed in this project is to be evaluated through the stage of system testing. There are various evaluation metrics such as cohesion, coupling and adaptability so on. But unfortunately, direct measurement of these metrics are very difficult, and accordingly, analytical and quantitative evaluations are almost impossible. Therefore, instead of the analytical evaluation, the experimental evaluation is to be applied through the test run by various users. This system testing will provide the viewpoint analysis of the user's level, and the detail and additional requirement specifications arising from user's real situation will be feedback into the system modeling. Also. the degree of freedom of the input and output will be improved, and the hardware limitation will be investigated. Upon the refining, the prototype system will be used as a design template. and will be used to develop the more extensive system. In detail. the relevant modules will be developed for the various diseases, and the module will be integrated by the macroscopic design process focusing on the inter modularity, generality of the database. and compatibility with other systems. The Home care Evaluation System is comprised of three main modules of : (1) General information on a patient, (2) General health status of a patient, and (3) Cerebrovascular disease patient. The general health status module has five sub modules of physical measurement, vitality, nursing, pharmaceutical description and emotional/cognition ability. The CVA patient module is divided into ten sub modules such as subjective sense, consciousness, memory and language pattern so on. The typical sub modules are described in appendix 3.
