Train collision and derailment are types of accident that happen with low probability of occurrence but could lead to disastrous consequences including multiple life losses. Risk assessment of the accidents are typically performed per their hazardous events, which are defined as events that cause accidents. This study classifies the train collision and derailment based on the relevant hazardous event, and investigates the causes related to the hazardous events. Finally, the relation of the causes, hazardous events, and the accidents are defined.
Ship is being operated under a highly dynamic environments and many factors are related with ship's collision and those factors are interacting. So, the analysis on ship's collision causes are very important to prepare countermeasures which will ensure the safe navigation. This study analysed the ship's collision data over the past 10 years(1991-2000), which is compiled by Korea Marine Accidents Inquiry Agency. The analysis confirmed that ‘ship's collision' is occurred most frequently and the cause is closely related with human factor. The main purpose of this study is to propose risk control measures of ship's collision. For this, the structure of human factor is analysed by the questionnaire methodology. Marine experts were surveyed based on major elements that were extracted from the human factor affecting to ship's collision. FSM has been widely adopted in modeling a dynamic system which is composed of human factors. Then, the structure analysis on the causes of ship's collision are performed using FSM. This structure model could be used in understanding and verifying the procedure of real ship's collision. Furthermore it could be used as the model to prevent ship's collision and reduce marine accidents.
본 연구의 목적은 과거 12년(2010~2021년)간 발생한 상선의 충돌사고 668건을 조사하여 충돌의 원인요인을 조사하고 이를 통계적으로 분석하여 항해사의 인적과실 예방 충돌회피(HEPCA) 모델을 제안하는 것이다. 중앙해양안전심판원의 통계연보 및 충돌사건 재결서를 조사하여 상선의 충돌 원인요인 데이터를 수집하고 통계분석 도구인 SPSS를 이용하여 빈도분석을 수행하였다. 1단계 분석으로 상선 충돌사고 668건을 대상으로 충돌원인을 분석하였고, 2단계 분석에서는 식별된 최대빈도 원인요인을 세부적으로 분석하였다. 분석결과, 충돌원인은 항해사의 인적과실이 98 %인 것으로 식별되었으며, 빈도 높은 요인 순서는 경계소홀 > 항행법규위반 > 조선 부적절 순이었다. 경계소홀의 원인 요인은 주로 상대선 초인 후 지속감시 소홀이었으며 상대선박의 존재를 인식하지 못한 원인은 주로 당직시간에 다른 작업을 한 요인이었다. 분석결과를 적용하여 인적과실 예방을 위한 HEPCA 모델을 제안하였고, 이를 재결서의 충돌사건에 적용해보았다. 본 연구결과는 해기사 교육기관 및 실무에서 항해사의 인적과실로 발생하는 충돌사고를 방지하기 위한 교육 자료로 활용이 가능할 것으로 기대된다.
Train collision and derailment are types of accident with low probability of occurrence, but they could lead to disastrous consequences including loss of lives and properties. The development of the risk assessment model has been called upon to predict and assess the risk for a long time. Nevertheless, the risk assessment model is recently introduced to the railway system in Korea. The classification of the hazardous events and causes is the commencement of the risk assessment model. In previous researches related to the classification, the hazardous events and causes were classified by centering the results. That classification was simple, but might not show the root cause of the hazardous events. This study has classified the train collision and derailment based on the relevant hazardous event including faults of the train related the accidents, and investigates the causes related to the hazardous events. For the risk assessment model, FTA (fault tree analysis) and ETA (event tree analysis) methods are introduced to assess the risk.
According to Korea Maritime Safety Tribunal, there have been 14,100 maritime accidents from 2017 to 2021. Among those accidents, ship collision accidents have been up to 1,275 cases. But in the accidents relating to small ships like a fishing vessel, analyzing the causes of the accidents would depend on statements of the persons related because there is often no navigational data. But those statements can be incorrect and give rise to disagreements between them so that it causes conflicts with each other during the trial. So a replay system of ship collision accident (RESCA) has been developed in order to reproduce the ship collision accident by using AIS, V-PASS or radar data. But the reproduced trajectory of ship collision accident is needed to be verified because it can be unreasonable physically. So a method to verify the reproduced trajectory and collect the physical data on ship collision accident is newly designed by using free running test. In the RESCA, the accident is reproduced using the navigational data from the trajectory for a ship and measured from free running model test for the other ship at the same time. Through free running test, the behavior of the model ship is transformed from model scale into real scale. In order to check into the accuracy of the new method, free running model tests by using RESCA are carried out on the actual ship collision accidents.
지금까지 IMO를 비롯한 해운산업분야에서는 해상의 인명 재산, 해양환경보호에 항상 큰 관심을 가지고 해양사고예방을 위한 많은 노력들이 견주되어 왔다. 하지만 이러한 노력에도 불구하고 크고 작은 해양사고가 지속적으로 발생하고 있는 것이 오늘날의 현실이다. 한편, 선박충돌사고는 수많은 원인이 서로 복잡하게 상호작용을 하고 있어서 사고예방대책마련에 어려움이 많다 따라서, 선박충돌사고의 정량적인 분석을 위해서는 이들 상호작용요소간의 관계를 시스템적으로 파악하고 분석하는 것이 선행되어야 한다. 본 연구에서는 먼저, 지난 10년(1991-2000)간 국내에서 발생한 선박충돌사고에 대한 위험성을 분석하였고, 또한 사고발생에 가장 큰 영향을 미치는 위해요소(Hazard)인 인적요소(Human Factor)에 대해서 전문가집단의 의견을 수렴하여 FSM기법을 이용하여 인적 위해요소를 계층화한 후 각 요소 상호간의 관련성을 분석하였다. 그 결과로써 인적요소에 의한 선박충돌사고의 발생과정과 각 계층에 속한 요소가 사고에 미치는 영향력을 규명하고, 각 요소간 상호관계를 파악하여 사고예방대책마련을 위한 우선순위를 결정할 수 있는 선박충돌사고의 인적요소 구조그래프를 제시하였다.
The proportion of collision in the total marine accidents is high. The main causes of collisions are navigation rule violation, safety speed violation, neglected watch-keeping and improper collision avoidance action. There are two main ways of avoiding collision situations during maritime navigation: the method of altering course and reducing ship's speed. The purpose of this study is to analyze the result of the collision avoidance action of the reserve officer in case of encountering a multiple number of ships using the ship handling simulator. Full-mission ship handling simulator was used to experiment the situation scenarios that encountered multiple ships. After the experiment, the questionnaire about the experiment was investigated. A total of 50 subjects were participated in the experiment. Experimental results showed that the number of the experimenters who used the engine was 11 and the number of the experimenters who did not use the engine was 39. In the case of using the engine, there were 0 collision accident, 1 grounding accident, and 10 no accidents. However, when the engine was not used, there were 28 collision accidents, 2 grounding accidents, and 9 no accidents. The causes of these results can be found in the survey results. 74 % of the non used engine participants said they were hesitate to use the engine. As can be seen from these results, the reserve officer are hesitant to use the engine and need a way to get correct of it. Maritime course subject can emphasize the importance of using ship's engines and case study also can be it. So, It is considered that various case study scenario will need to developed by various tools in the future.
According to the statistics of KMST (2010~2019), 68.5% of marine casualties have been occurred in fishing boats in the last 10 years. Although the incidence rate of annual average per ship is 10.4% for merchant ships and 2.3% for fishing boats, it is necessary to first reduce the marine casualties of fishing boats in order to reduce the occurrence of the whole marine casualty, considering that the number of registered fishing boats are 86.5% of the total. In this study, the top 10 fishing types with high occurring marine casualties in the past 10 years were selected using the statistics of KMST. Then, the causes of the marine casualties, focusing on the collision, were analyzed and counterplan were suggested. The biggest marine casualty that the top 10 fishing types have caused in the past 10 years was a collision, accounting for 71.7% of the total, and the rest were less than 10.0% each. Also, it was found that the biggest environment and cause of the collision were due to human factors such as poor look out or negligence of duty. Eventually, it was concluded that a reduction in the human factors for the causes basically, could lower the incidence of the marine casualties. Therefore, if the continuous and systematic correcting education of the attitude toward the ship operating of seafarers could conducted with the focus on the main human factors, the whole marine casualties will be expected to decrease gradually.
The most frequent occurring and the serious marine casualties in fishing vessels are the collision in statistics from Korea Maritime Safety Tribunal (2008-2015). Collisions are is given a great deal of weight in all marine casualties, and the main cause of collisions is the negligence of watching. From this point of view, this study deals with the collision and its immediate cause, the negligence of watching which are main factors on the indirect causes. Basic analyzing data was gathered from the questionnaire made by experts of sea service part, and analyzed by using the fault tree analysis (FTA). From the result of the study, the occurrent probability of the negligence of watching in the collision due to the indirect causes occupied 64.9%, and its probability caused by the man factors was less than the other factors; i.e. the media factors and the management factors. For the reduction of the negligence of watching in the collision from this study, it needs an effort to remove not only the man factors, but also the media factors and management factors.
선박충돌사고는 많은 원인이 서로 복잡하게 상호작용을 하여 발생하고 있으며, 특히 인적요인에 의한 충돌사고가 가장 큰 비중을 차지하고 있다. 이러한 선박충돌사고 원인분석은 선박의 안전 운항상의 측면에서 매우 중요하다고 할 수 있다. 따라서 본 연구의 목적은 시스템다이내믹스법을 이용하여 선박충돌사고 인적요인 모델을 구축하고, 선박충돌사고를 감소시키기 위한 가장 효과적인 대책을 수립하기 위한 정책요소론 제시하고자 한다. 본 연구의 수행을 위해 ]nM법에 의한 충돌사고원인의 구조분석을 인과지도상의 정량적, 정성적, 피드백 루프로 변환하였다. 그리고 표준시뮬레이션모델과 8가지 정책시뮬레이션모델을 20년간(1993-2012) 시뮬레이션을 수행하였다.
