Secondary Neurulation Defects-1 : Retained Medullary Cord

  • Retained medullary cord (RMC) is a relatively recent term. Pang et al. newly defined the RMC as a late arrest of secondary neurulation leaving a non-functional vestigial portion at the tip of the conus medullaris. RMC, which belongs to the category of closed spinal dysraphism, is a cord-like structure that is elongated from the conus toward the cul-de-sac. Because intraoperative electrophysiological confirmation of a non-functional conus is essential for the diagnosis of RMC, only a tentative or an assumptive diagnosis is possible before surgery or in cases of limited surgical exposure. We suggest the term 'possible RMC' for these cases. An RMC may cause tethered cord syndrome and thus requires surgery. This article reviews the literature to elucidate the pathoembryogenesis, clinical significance and treatment of RMCs.

  • When faults occur in the power equipment that is used a great deal in industrial sites, it may lead to fatal accidents that cause losses both economically and in manpower. In this paper, the effect of particle impact on the partial discharge of $SF_6$ gas was measured by simulating a partial discharge following the type of fault found in the GIS using $SF_6$ insulating gas as the insulating material. A spectrum analysis was performed on the radiate electromagnetic waves emitted upon partial discharge by using the UHF insulation diagnosis method. This subject of this study is insulation diagnosis by the measurement of radiate electromagnetic waves when particles exist in the GIS and power equipment insulated with $SF_6$ gas.

Congenital Aortic Stenosis: Report of 2 Cases (선천성 대동맥 판막 협착증 치험 2예)

  • Congenital aortic stenosis, a relatively uncommon congenital heart disease, may cause heart failure and may be fatal. In recent years, increased clinical awareness and improved diagnostic and operative technique has made accurate diagnosis and successful treatment possible. Recently we experienced 2 cases of congenital aortic stenosis, and which was corrected surgically. The first case was 9 years old boy, and second case was 16 years male. Preoperative diagnosis was entertained by angiography and cardiac catheterization in both cases. In each case, aortic valve opening was widened by incision along the fused commissure between the combined left and right coronary cusp on one side, and the noncoronary cusp on the other side. Post-op. pressure gradient between the aorta and left ventricle markedly reduced, in the first case, 50mmHg, and in the 2nd case, 55mmHg.Both patients discharged with good results 2 weeks after open heart surgery.

  • Drug-induced liver injury (DILI), including herbal and dietary supplement hepatotoxicity, is often passed lightly; however, it can lead to the requirement of a liver transplant or may even cause death because of liver failure. Recently, the American College of Gastroenterology, Chinese Society of Hepatology and European Association for the Study of the Liver guidelines for the diagnosis and treatment of DILI have been established, and they will be helpful for guiding clinical treatment decisions. Roussel Uclaf Causality Assessment Method scoring is the most commonly used method to diagnose DILI; however, it has some limitations, such as poor validity and reproducibility. Recently, studies on new biomarkers have been actively carried out, which will help diagnose DILI and predict the prognosis of DILI. It is expected that the development of new therapies such as autophagy inducers and various other technologies of the fourth industrial revolution will be applicable to DILI research.

  • Prostate cancer is more common in men over the age of 65 years. There are 15% cases with positive family history of prostate cancer Worldwide. Prostate cancer is the second leading cause of death among the U.S. men. Prostate cancer incidence is strongly related to age with the highest rates in older man. Globally millions of people are suffering from this disease. This study aims to provide awareness about prostate cancer as well as an updated knowledge about the epidemiology, etiology, diagnosis and treatment of prostate cancer.

  • Neuromyelitis optica (NMO) is an inflammatory demyelinating disease of the central nervous system associated with longitudinally extensive myelitis and optic neuritis. It is characterized by relapses that lead to blindness and paralysis sequelaes. But, this is rare disease; therefore high clinical suspicion for a correct diagnosis and proper examinations are not easy. However, early diagnosis is essential to prevent sequelae. We report the case of NMO with headache. A 30-year male patient who suffered headache visited our pain clinic because of aggravated pain despite treatment. The cause of the pain was revealed as NMO by more detailed previous history and examination.

  • This paper deals with an application of an expert system to transmission line fault diagnosis using alarm information line possible solution can be obtained even in case that the cause of alarms is due to relays, circuit breakers or alarm systems. The expert system diagnoses not only any possible fault element, but also normal or abnormal misoperations. Also, this system can give any possible answers only when the sum of appropriate error indices assigned to false operation of devices is less than the appropriate criterion specified in advance. This paper is written in Official Projection System-Version 5 (OPS-5) which is one of the AI languages.

Left Atrial Myxoma: Report of 2 Cases (좌심방내 점액종 치험 2예)

  • Left atrial myxoma, an unusual clinical entity, may cause severe and progressive cardiac disease mimicking mitral valvular disease. In recent years, increased clinical awareness and improved diagnostic techniques has led to a higher incidence of correct preoperative diagnosis. Recently we experienced 2 cases of left atrial myxoma, which were removed successfully under the cardiopulmonary by pass. The first case was 45 years old woman and the second was 23 years old female. Preoperative definite diagnosis was entertained by angiocardiography and echocardiography in both cases. In the first case, tumor was removed with left atriotomy and atrial septectomy was done with additional right atriotomy. In the second case, tumor and atrial septum were removed en bloc through the right atrium. Both patients were discharged with good results 2 weeks postoperatively.

Transposition of the Great Arteries (TGA) -Report of An Autopsy Case- (대혈관전위증 부검 1례 보고)

  • Transposition of the great arteries is one of the commonest forms of severe congenital heart disease and produces severe cyanosis threatening survival from the day of birth. Anatomical anomalies which the aorta arises from the infundibulum of the right ventricle and the pulmonary artery arises from the outflow tract of the left ventricle make the deranged circulation. Survival is possible only if additional anomalies are present which allow mixing of the pulmonary and systemic circulations. Preoperative diagnosis as TGA was taken on the 15 day old female via the preoperative examination and the right cordioangiography. As palliative treatment for cyanosis, Blalock-Hanlon operation was performed in this patient. The results were good as 54 mmHg changed from 27 mmHg of $PO_2$ in aorta, but sudden cardiac arrest was developed in postoperative 12 hours. In order to confirm the cause of death and the cardiac anomalies, autopsy was performed on the date of death. The diagnosis of the autopsy showed; [1] Transposition of the Great Arteries. [2] Patent Ductus Arteriosus. [3] Patent Foramen Ovale. [4] Ventricular Septal Defect, 2 Muscular Type. [5] Double Ureter, Right. [6] Artificial Atrial Septal Defect. [7] Total Collapse of the left lung and Intraparenchymal hemorrhage of right lung.

  • The R.C Building will be superannuated as time passes. This program is generated by propagation of cracks. In order to manage such cracks, time and efforts, expense, etc. are required. In this study, glass sensors were embedding in a model beam and column and leakage of fluorescence and adhesive material was investigated. Further, currents in glass pipe were observed to find the leakage of liquid in glass pipes. Progressive cracks generated by cause the fracture of glass pipes. Therefore, the liquid become to flow and electric current stops, and the cracked part of the member can be found easily. Moreover, the adhesive delays progressive cracking system that responds in air, and the life of a structure can be made to extend. The purpose of this research is to develop of low price sensors that can perform of self-diagnosis in addition to ability of concrete repair concrete to damage.
