Empirical antibiotics for recurrent urinary tract infections in children

  • Objectives: The purpose of this study was to compare antibiotic resistance patterns between first urinary tract infection (UTI) and recurrent UTI groups and to obtain information regarding empirical antibiotic selection for treating recurrent UTI. Methods: We retrospectively reviewed 148 children treated for UTIs from January 2009 to June 2016. The patients were divided into two groups: first UTI (N = 148) and recurrent UTI (17 patients and 20 episodes). Results: In both groups, Escherichia coli was the most frequent causative organism, accounting for 89.9% and 75.0% in the first and recurrent UTI groups, respectively. When E. coli or Klebsiella pneumoniae was the causative organism, extended-spectrum ${\beta}-lactamase$ (ESBL)-producing organisms were more frequent in the recurrent UTI group (17.6%) than in the first UTI group (14.0%); however, this difference was not statistically significant (P = 0.684). Cefotaxime was the most frequently used first-line empirical antibiotic in both groups. In the first UTI and recurrent UTI groups, 7.4% and 15.0% of patients were treated with intravenous antibiotics as definitive therapy, respectively (P = 0.250). Fifteen out of 17 patients having a second UTI had different causative organisms or antibiotic susceptibility patterns compared to their previous episode. Conclusions: Escherichia coli was the most frequent causative organism in the recurrent UTI group. There were no differences in the proportion of ESBL-producing organisms between the first UTI and recurrent UTI groups. Therefore, when a UTI recurs in children, the antibiotics effective on the most common causative organism might be administered as empirical antibiotics.

  • A total of 2,053 quarter milk samples of 518 dairy cows from 46 herds were examined for mastitis and results obtained were as follows: 1. A total of 428 quarters(20.8%) of 271 dairy cows(52.3%) from 41 herds(89%) were found to be infected with mastitis. 2. It was found that 71 quarters(3.5%) of 41 cows(7.9%) from 21 herds(45.6%) were clinical mastitis and 357 quarters(17.3%) of 230 cows(44.4%) from 20 herds (43.4%) were subclinical mastitis. 3. The main causative organisms of the mastitis were Staphylococcus aureus(26%), Streptococcus uberis(16%), and Streptococcus agalactiae(10%). 4. The majority of causative organisms were very resistant to colistin(85.9%), penicillin(67%), and streptomycin(39%). Most of the other drugs were highly effective as inhibitor for the most causative organisms in vitro. However sulfisoxazole was moderatively effective against them.

  • It is the most important to know the causative organisms and the most effective antibiotics in the cases of chronic suppurative otitis media not only for the treatment of the cases of O.M.P.C. but also for postoperative control of middle ear surgery. In the authors' opinion, there are various organisms in the middle ear depending on the country, hygine, and area, etc., and many new antibiotics have recently been developed. To the otologists, it is helpful to know the causative organisms of O.M.P.C. and to compare with other authors' results about the sensitivity of antibiotics to the organisms. Thus the authors have performed bacteriological studies and its sensitivity test to the causative organisms in the cases of O.M.P.C. and obtained the following results. According to the our studies, in the gram-negative species, Proteus species (16.9%) was the most sensitive to Ampicillin 81.8% and Pseudomonas(18.5%) was the most sensitive to Colimycin 91.7%, and in the gram positive group, Staphylococcus Aureus (29.2%) was sensitive to Erythromycin 100% and Orbenin 94.7%.

Clinical review of four patients of acute purulent pericarditis (급성 화농성 심낭염의 외과적 고찰)

  • Four patients of acute purulent pericarditis were seen at the Jeonbug National University Hospital between January, 1979 and December 1983. All patients were proven to have acute purulent pericarditis by clinical evaluation, bacterial study, and biopsy. Three patients were male and one female. The three patients were in pediatric age. The primary focuses were meningitis, Rt. coxitis, lobar pneumonia, and gastro-enteritis. The causative organisms were staphylococcus aureus in two patients. In the other two patients, organisms were not cultured from the pericardial pus. The reason why the causative organisms were not cultured in pericardial pus was probably due to massive antimicrobial treatment before pus collection. All patients were treated with systemic antibiotic therapy and pericardiostomy with normal saline irrigation. The three patients were treated without specific complication. The one patient developed the chronic constrictive pericarditis. In this patient, pericardiectomy was performed.

  • A total of 364 quarter milk samples of 91 dairy cattle from 4 area around Jeonju in Jeonbug area were examined for infection rate and causative abets of mastitis by the Laboratory Procedures used in the Connecticut Mastitis-Control Program. The results obtained were as follows: 1. One hundred and thirty quarters (35.7%) from 67 cows (73.6%) were found to be infected with mastitis. It was found that 5 (1.37%) of the infected quarters were clinical mastitis and all of the rest were subclinical mastitis (125 quarters). 2. The main causative agents were found to be Staphylococcus aureus (46 quarters), Streptococcus agalactiae (49 quarters), streptococcus disgalactiae (18 quarters), Steptococcus uberis (13 quarters). Corynebacteria, Coliform organisms, Pseudomonas were also occasionally found to be causative organisms. 3. Having examined the number of infection Quarter per head of mastitis dairy cattle samplings, major findings were that the average number of infection quarter to total dairy cattle examined was 1.4 quarters, and that average number of infection quarter to the infected cattle was 1.9 quarters.

Urinary Tract Infection Related to The Indwelling Cathete with Closed Drainage (도뇨환자의 유치기간별 요로감염상태에 관한 연구)

  • This study attempted to identify important problems of the factors related to the indwelling catheter with closed drainage and to enhance recognization of Urinary Tract Infection management to protect patients with indwelling catheter from UTI. We selected the 58 patients with indwelling catheter with closed drainage from md-surg. intensive care units, and general neurological and neurosurgical ward, in one hospital in Pusan, Korea from May 30 to October 15, 1993. Patients were examined by urinary analysis before inserting catheter and then by urine cultures three times. The obtained data was analized using an SPSS /PC$^{+}$ The statistical analyses employed here was the frequency, percentage and x$^2$-tests. 1. Analyses for Indwelling Catheter induced UTI and infection rates were found to be presented in patients related to the factors of being on antibiotic drugs (x$^2$=14.105, p=.000) and the duration of the indwelling catheter(x$^2$=58.0, p=.000), both of which were found to affect the UTI. 2. UTI rates according to the duration of the in-dwelling catheter were high 8.6% for the 2nd day, 27.6 for the 4th day, and 36.2% for the 7th day, and as the duration for the indwelling catheter was longer, UTI rates were higher. 3. The UTI rates according to the duration of the indwelling catheter related re factors, on the 7th day, were high-52.0% for women, 37.0% for more than 50 years in ages, 30.0% for patients under sur-gery, 33.3% for altered mental states outsides the alert, 76.7% for patients not given antibiotic drugs, 36.4% for less than 3500cc of fluids intakes, it was found that these affected to increase the UTI. 4. Causative organisms of UTI were found ; E.Coli appears the most than all other isolated organisms in women and Klebsiella and Pseudomonas in men. For antibiotic susceptibility of causative organisms, all causative organisms were found to be susceptible to Sul -cefoperazone, Trim-prim, and unasyn and Pseudomonas mostly net susceptible.e.

  • A total of 820 dairy cattle were examined formastitis by California mastitis test method and 181 dairy cattle were examined bacteriologically according to the method of Connecticut mastitis control program. The results obtained were as followings: 1. Milk samples from 1.231(38.1%) of 3,225 quarters and 568(69.3%) of 820 dairy cattle were positive for mastitis by California mastitis test method. 2. Among 181 cattle examined bacteriologically by the method of Connecticut mastitis control program 159(22.9%) quarters of 102(56.4%) cattle were infected. Clinical mastitis was found in 5 cattle. 3. Important causative organisms were found to de Staphylococcus aureus(44.8%), Streptococcus agalactiae(21.5%), and non-str, agalactiae Streptococcus(26.4%). Corynebacterium ssp., Pseudomonas spp., Coliforms and Yeasts were also occasionally found to be causative organisms. 4. Strains of Staphylococcus aureus isolated from bovine udders were tested for sensitivity to antibiotics. The rates of resistant strain were 13.7%, 11.8%, 8.8%, 5.9%, 100%, 4.9% and 4.9% for penicillin G, streptomycin, terramycin, erythromycin, chloramphenicol, ampicillin and kanamycin, respectively.

  • A total of 2,065 quarter milk samples of 534 dair cows from 35 herds were examined for mastis and the results obtained were as follows: 1. 654 quarters (31.6%) of 301 dairy cows (56.3%) from 34 herds (97.1%) were infected with mastitis. It was found that 8 cows (1.5%) showed clinical mastitis and other cows were subclinical. 2. Low infection rate of mastis was shown in first delivery cattle and higher infection rate in more deliverable of cattle. 3. The main causative organisms of the matitis were Staphylococcus aureus (19.4%), Streptococcus agalactiae (11.3%) and Streptococcus dysgalatiae (5%). 4. The majority of causative organisms were very resistant to colistin and highly sensitive to tetracycline. Especially Streptococcus strains were highly sensitive to leukomycin, and Staphylococcus aureus were highly sensitive to tetracycline and the other bacteria were highly sensitive to tetracycline and kanamycin.

Antimicrobial Susceptibility of Causative Agents of Mastitis Isolated from Mammary Glandular Tissues of Slaughtered Holstein Cows (도태 유우의 유선조직에서 분리된 세균의 항균제 감수성)

  • Causative agents of mastitis were isolated from glandular tissues of 101 culled Holstein cow udders and tested with antimicrobials by standardized disc diffusion method. Pathogens most commonly isolated were coagulase-negative staphylococci (43.9%) and streptococci (21.4%). Most of the udders (88.5%) showed mixed infections with more than two species of bacteria. Antimicrobial sensitivity tests revealed that 90.0% and 84.5% of the isolated organisms were susceptible to amoxicillin and gentamicin, while most organisms were resistant to erythromycin. Coliforms were resistant to ampicillin, cephalothin, cefazolin, erythromycin, kanamycin, neomycin, penicillin and tetracycline and streptococci to erythromycin, kanamycin, neomycin, trimethoprim/sulfamethoxazole and tetracycline.

Etiologies of Bacterial Pneumonia with Implications for Therapy (세균성 폐렴의 원인균과 그 치료)

  • Background: Although we gain new knowledge, the problem of pneumonia will not be eliminated. We should understand who is at risk, why these people develop this problem, what causes the pneumonia, and how to manage and prevent respiratory infection. To clarify the alterations of the etiologies of bacterial pneumonia we analysed the recent causative organisms and evaluated the therapeutic implications. Methods: A retrospective four-year study of bacterial pneumonia was conducted in Soon Chun Hyang University Hospital. 190 episodes of bacterial pneumonia was investigated. Results: 1) The causative organisms were isolated in 173 cases on the sputum culture: 154 cases (89%) were gram negative bacilli and 19 cases(11%) were gram positive cocci. The major organisms were Pseudomonas species 49 cases(28%), Klebsiella pneumoniae 29 cases(17%), Enterobacter species 25 cases(14%), and Acinetobacter species 20 cases(12%) in decreasing order. Pseudomonas species(13 cases, 34%) were frequently found in nosocomial pneumonia. 2) The causative organisms were isolated in 16 cases on the blood culture: 7 cases(43%) were gram negative bacilli and 9 cases(57%) were gram positive cocci. The major organisms were Staphylococcus aureus(6 cases, 38%), Pseudomonas species(3 cases, 19%) in decreasing order. 3) In the susceptibility test of causative organisms to antimicrobial drugs, Pseudomonas was susceptible to amikacin, ciprofloxacin, aztreonam, ceftazidime(more than 50%) and resistant to piperacillin, gentamicin, carbenicillin(more than 60%). Klebsiella was susceptible to chloramphenicol, gentamicin, cefotetan(more than 70%) and resistant to carbenicillin, ampicillin(more than 70%). Staphylococcus was susceptible to methicillin(64%), and Streptococcus pneumoniae was susceptible to oxacillin(94%). 4) The response rate after antibiotics therapy was 81% and the mortality rate was 19%. Conclusion: As considering the changes of causative organisms and antibiotic resistance, it behooves us to exercise caution in dispending antibiotics in order to maximize their continued efficacy and to do appropiate antibiotics therapy based on cultures and susceptibility test.
